The use of the Quality Adjusted Life-Year (QALY) as a measure of the benefit obtained from health care expenditure has been attacked on the ground that it gives a lower value to preserving the lives of people with a permanent disability or illness than to preserving the lives of those who are healthy and not disabled. The reason for this is that the quality of life of those with illness or disability is ranked, on the QALY scale, below that of (...) someone without a disability or illness. Hence we can, other things being equal, gain more QALYs by saving the lives of those without a permanent disability or illness than by saving the lives of those who are disadvantaged in these ways. But to do so puts these disadvantaged people under a kind of double jeopardy. Not only do they suffer from the disability or illness, but because of it, a low priority is given to forms of health care that can preserve their lives. This, so the objection runs, is unjust or unfair. This article assesses this objection to the use of QALYs as a basis for allocating health care resources. It seeks to determine what is sound in the double jeopardy objection, and then to show that the defender of QALYs has an adequate response to it. (shrink)

This paper explores two aspects of Hume's skeptical crisis in the conclusion to _Treatise<D> Book I: his involved personal experience of the crisis, and his detached naturalistic reflection on it. I discuss several distinct states of mind reported in the text, ranging from extreme skepticism that rejects all belief, to natural dogmatism that rejects all reflection, to mitigated skepticism that tries to reconcile reflection and belief. I argue against interpretations according to which Hume's skepticism supports his naturalism, and I suggest (...) that Hume's extreme skepticism ultimately depends on a predisposition to think of the crucial elements of human cognition as "trivial", as epistemically disabling rather than enabling. More than a billion people now live on less than the purchasing-power equivalent, in their own country, of what can be bought in the United States for $1. In the year 2000, Americans made private donations for foreign aid of all kinds totaling about $4 per person, or roughly $20 per family. Through their government, they gave an additional $10 per person, or $50 per family. That makes a total of $70 per family.

The home secretary is debating whether to allow the American animal rights activist Dr Jerry Vlasak into Britain after it was reported that he had said that killing five to 15 vivisectors could save millions of non-human lives. Now that the United States is again considering going to war, it is timely to reassess the last war fought by the Bush Administration. Was the war in Afghanistan a just war? If not, our scrutiny of present moves towards another US-initiated war will need to be that much more strict.

As the war goes on, the casualties inevitably rise: American and British combatants, Australian and British journalists, Iraqi combatants, and Iraqi civilians are being killed. How many lives is it justifiable to sacrifice to protect our security, and to free the Iraqi people from Saddam Hussein's dictatorship?

I have never tried to develop a system in the sense that, say, Whitehead and Santayana did, yet I think of myself as a systematic philosopher. This is exciting medical researchers because it means that, at least in theory, the cells from an early embryo could eliminate the need for organ transplants entirely, cure leukaemia, enable people with diabetes to manufacture insulin, treat Parkinson's and Alzheimer's disease, and repair the nerve systems of quadriplegics. Though these prospects are still far from realisation, results achieved by Oliver Brustle at the University of Bonn Medical Centre have brought them a step closer. John Rawls's A Theory of Justice was published in December 1971 and has already established itself as a landmark. No other philosophical work, in our time or before, has, to my knowledge, excited so much attention in so short a time and in such varied circles. Clearly the book answers to a set of needs that have just recently surfaced, and it was published at just the right time to benefit from changing directions in philosophy and other areas of intellectual (...) life brought on by enormous and accelerating changes in social conditions and concerns, by the disaster of Vietnam, and by the sense that something serious and sufficient must be done to counter the increasing fragmentation of knowledge and the divorce of science from society. Rawls gives one the sense of putting it all together; of seeing things clearly and seeing them whole; of joining together again economics, jurisprudence, and social and moral theory; and of putting philosophy to work once again in the service of society and humanity. In particular, he gives one the sense that there can be a science of justice, of morality, and of values. (shrink)

In earlier publications, we had argued that Paul Ramsey is inconsistent because he simultaneously asserts that (i) 'all our days and years are of equal worth' and (ii) 'that it is permissible to refrain from prolonging the lives of some dying patients'. Thomas Long has suggested that we have not shown that Paul Ramsey is inconsistent. Ramsey and we, he holds, start from incommensurable metaphysical views: for Ramsey, the dying process has religious significance--God is calling his servant home. While it (...) is normally a good thing to keep a patient alive, it would, for Ramsey, show deafness to God's call to keep a dying patient alive. It is true we do not share Paul Ramsey's religious views. It is, however, not necessary to rely on any particular metaphysical views to refute Ramsey's position. For Ramsey's view to be internally consistent, Ramsey would have to be able to distinguish between dying and non-dying patients. We examine some of Ramsey's examples and show that his practical judgements do not allow us to draw this distinction. This means that, contra Long, we hold fast to our charge that Ramsey's view is inconsistent. (shrink)

The very title of Sidgwick’s great work is fascinating: the methods of ethics. We hear much—and persons a hundred years ago heard much—of the methods of science. But we hear very little of the methods of ethics. Is ethics a science? No, and Sidgwick never thought that it was. But he did think that the methods, or something of the spirit, of scientific investigation could be imported into ethical studies, with results which, though they would not necessarily be dramatic and (...) would certainly not be edifying, would nevertheless be worthwhile. Sidgwick himself characterized his work as “essentially an attempt to introduce precision of thought into a subject usually treated in a too loose and popular way”. “I have thought that the predominance in the minds of moralists of a desire to edify has impeded the real progress of ethical science; and that this would be benefited by an application to it of the same disinterested curiosity to which we chiefly owe the great discoveries of physics”. Contemporary perspectives on conciousness provide us with a powerful metaphor for the corporate planning process; although organisations ultimately differ, in systems terms, from organisms. Like consciousness, planning has survival value and confers operational advantages.

Many utilitarians accept Bentham's view that to argue for the principle of utility is as ‘impossible as it is needless'. They take utilitarianism as a first principle which one either accepts or does not. They do, of course, defend utilitarianism against objections, and make objections to other ethical positions; but the principle of utility itself, they hold, must stand on its own merits. In this article we use a different approach. We introduce a principle, which we call ‘Weak Majority Preference', (...) which we believe likely to be accepted by many who do not consider themselves utilitarians. We then show that from this principle it is possible to derive the general principle of utility. (shrink)
