Surprisingly, the 1977 "Russian flu" H1N1 pandemic influenza virus was genetically indistinguishable from strains that had circulated decades earlier but had gone extinct in 1957. This essay puts forward the most plausible chronology to explain the reemergence of the 1977 H1N1 pandemic virus: (1) in January–February 1976, a self-limited small outbreak of a swine H1N1 influenza virus occurred among Army personnel at Fort Dix, New Jersey; (2) in March 1976, the US launched a nationwide H1N1 swine influenza vaccine program; (3) (...) other countries then also launched their own H1N1 R&D efforts; (4) a new H1N1 outbreak, genetically unrelated to the Fort Dix swine virus but indistinguishable from previously extinct H1N1 viruses, was detected early in 1977 in China; (5) the leading Chinese influenza virologist later disclosed that the Chinese military had conducted large H1N1 vaccine R&D studies in 1976. It is likely that the resurrected H1N1 influenza viruses were laboratory-stored strains that were unfrozen and studied as part of the emergency response to a perceived epidemic threat, and that accidentally escaped. The fear of a _possible_ H1N1 pandemic was the critical factor that gave rise to the _actual_ H1N1 pandemic, resulting in an avoidable "self-fulfilling prophecy pandemic.". (shrink)

Health research increasingly relies on organized collections of health data and biological samples. There are many types of sample and data collections that are used for health research, though these are collected for many purposes, not all of which are health-related. These collections exist under different jurisdictional and regulatory arrangements and include: 1) Population biobanks, cohort studies, and genome databases 2) Clinical and public health data 3) Direct-to-consumer genetic testing 4) Social media 5) Fitness trackers, health apps, and biometric data (...) sensors Population biobanks, cohort studies, and genome databases Clinical and public health data Direct-to-consumer genetic testing Social media Fitness trackers, health apps, and biometric data sensors Ethical, legal, and social challenges of such collections are well recognized, but there has been limited attention to the broader societal implications of the existence of these collections. Although health research conducted using these collections is broadly recognized as beneficent, secondary uses of these data and samples may be controversial. We examine both documented and hypothetical scenarios of secondary uses of health data and samples. In particular, we focus on the use of health data for purposes of: Forensic investigations Civil lawsuits Identification of victims of mass casualty events Denial of entry for border security and immigration Making health resource rationing decisions Facilitating human rights abuses in autocratic regimes Forensic investigations Civil lawsuits Identification of victims of mass casualty events Denial of entry for border security and immigration Making health resource rationing decisions Facilitating human rights abuses in autocratic regimes Current safeguards relating to the use of health data and samples include research ethics oversight and privacy laws. These safeguards have a strong focus on informed consent and anonymization, which are aimed at the protection of the individual research subject. They are not intended to address broader societal implications of health data and sample collections. As such, existing arrangements are insufficient to protect against subversion of health databases for non-sanctioned secondary uses, or to provide guidance for reasonable but controversial secondary uses. We are concerned that existing debate in the scholarly literature and beyond has not sufficiently recognized the secondary data uses we outline in this paper. Our main purpose, therefore, is to raise awareness of the potential for unforeseen and unintended consequences, in particular negative consequences, of the increased availability and development of health data collections for research, by providing a comprehensive review of documented and hypothetical non-health research uses of such data. Martha Nussbaum and Seyla Benhabib have raised the question of how the Western subject might engage with the non-Western other in a non-imperialistic fashion. However, both of these feminist thinkers propose a universalist framework, consistent with Donald Davidson’s conclusions regarding the translatability of ”conceptual schemes”. Drawing upon the thought of G.W.F. Hegel and Walter Benjamin, I argue that the historically constituted subject that emerges in the wake of the Enlightenment affords an account of subjectivity that recasts the meaning of (...) rationality and thus the way in which translation should be understood. I shall argue, with Benjamin, that a linguistic conceptual scheme--in short, a world—needs translation in order to remain vital, and that genuine translation not only translates the other but transforms the self who seeks to translate. KelleyThe Journal of the History of Ideas, in collaboration with other institutions, including the Universities of Bucharest and Budapest and the Soros Foundation, recently sponsored the second in a series of international conferences being planned on topics in current intellectual history. (The first, “Interrogating Tradition,” was held at Rutgers University, 13–16 November 1997.) The Romanian conference, which was held in the Elisabeta Palace (...) in Bucharest (27–31 May 1998), was devoted to “Culture and the Politics of National Identity in Modern Romania.” Over forty scholars, mainly from Romania but some also from the U.S., France, Germany, and Hungary, gathered to discuss a variety of questions about Romanian society, past, present, and speculative future; and they were joined by at least as many visitors, especially students, from the local area and beyond.Topics discussed included politics, language, economics, historiography, religion, education, philosophy, ideologies, intelligentsia, particular intellectuals, minorities, and women (though not Gypsies or gays). Under this riot of rubrics a wide range of questions were posed in interdisciplinary fashion, with frequent, sometimes impassioned interventions. What is—was, might have been, can be—Romania? Who is a Romanian (and who not)? What is Romania’s relation to the West and the East? (And what are the “West” and the “East”?) To modernity, to modernism, to modernization? What, more generally, is the role of history in answering such questions? Or is history perhaps (as Karl Krause remarked of psychoanalysis) the disease for which it claims to be the cure?Keith Hitchens’s keynote address suggested that rehashing old essentialist debates might not be productive at millennium’s end, but the conference itself could not avoid some excursions down the paths of memory and myth. For historians, Western historians anyway, Romania seems to have too many pasts. Located at the juncture of four previous empires, Romania is a mosaic, or perhaps a pandemonium, of ethnic, religious, economic, and ideological groups that can only imagine, or invent, a nationality. Geographically (and in many ways culturally) remote from the West, Romania for a long time (as Sorin Antohi remarked) lacked both a public sphere and a civil society; and suffering “a Counter-Reformation without a Reformation” (Alexandru Dutu’s phrase), it has not enjoyed a tradition of religious or ethnic toleration. Nor have either the Germans or the Jews (as Glass said) found their own unity within the changing borders of the modern Romanian state. Not until the nineteenth century did Romania develop, out of its Boyar background (as Siupiur argued), an intellectual elite that could create a modern national history and help turn “peasants into Romanians” (Antohi’s phrase); and even then this elite seemed more attracted to conservative [End Page 735] (Herder, Savigny, Burke) than to liberal views of autochthonism. Many features of Western modernity, such as Newtonian science, reached Romania late, and indeed there was a retrograde movement against liberalism under the aegis by orthodox religion. In any case this intelligentsia has been as fragmented and multiform as the Romanian past itself. True, some Romanian scholars like to posit a mainstream tradition, “excesses aside,” as one speaker put it—but how, asked another, can historians put “excesses” aside from the devastating perspective of this century?Political and cultural self-examination was the dominant theme of this conference. Nationality, said Pippidi, depends on three factors, viz., name, language, religion, and territory. The Romanian name dates from the tenth century, indicating a separation of the Roman from the Greek tradition in Wallachia; and from the seventeenth century the idea of a mainly Roman provenance was given privilege. As a language, “Romanian” was spoken in the late Middle Ages, but the first written document is dated 1521. If patriotism can thus be seen as an incentive or as an instigation even in such a recondite science as epistemology, how much more readily can it be seen to perform such functions in other studies more immediately or inextricably bound up with communal human life? I pass over instances that occur to me—for instance, the Victorian Jesuit, Father Hopkins, declaring that every good poem written by an Englishman was a blow struck for England--and profit instead, if I may, by (...) the presence among us of Edward Said. I do not know, and it is none of my business to know, what passport Said presents at the international frontier. But it is surely common knowledge among us that he has deep and feelingful and intimate allegiances to the state of Lebanon. Who of us has failed to connect this with his books Orientalism and The Question of Palestine? The point is that, having made this connection, none of us thinks the worse of Said. On the contrary, we recognize that he has a special stake in such topics and therefore speaks on them with a special authority. Unless I am mistaken, that stake and that authority are, in a perhaps extended sense, patriotic. And whatever our speculative objections to the idea and the principle of patriotism, in practice we recognize it and we honour it.What I am questioning, it will now be plain, is the principle of "disinterest." "The disinterested pursuit of knowledge"—it is what in our distinct disciplines all of us have paid lip-service to, and perhaps more than lip-service. But when we come right down to it, is it what we believe? The honest patriot declares an interest; and if we are wise, we take note of the declaration, making allowances and reserving doubts accordingly. But what are we to make of the scholar who declares no interest, who claims implicitly to be truly disinterested. Can we believe him? And if we cannot, what guidance do we have as to what reservations to make, what doubts to entertain? I am of one mind with my Marxist colleagues who, from a political position very far from mine, warn us to be especially suspicious of the scholar who claims to have no axe to grind. We, all of us, have axes to grind; the crucial distinction is between those who know this about themselves and those who don't.Let me make myself clear. When I urge that the terms "patriotism" and "patriotic" be reinstated in our discourse, and particularly in those forms of our discourse that may be called "interpretation," I do not imply that patriotism is a nobler, a more elevated instigation than sundry others, mostly ideological, of which we are more aware. The point is precisely that of these others we are aware because we share a vocabulary which acknowledges them, whereas "patriotic" has been banished from our vocabulary, and so the reality which the word represents is left out of our calculations. Let me admit for the sake of argument what I do not in fact believe-- that patriotism is a concept and a sentiment so besmirched by the unholy uses made of it that, if mankind is to survive, patriotism will have to be eradicated. Even if that were the case, it remains true that patriotic interest and incitement are very far from having been eradicated from the world that we in fact inhabit, and try to interpret, here and now; and if we try to work within a vocabulary that pretends otherwise, we condemn ourselves to producing interpretations that are drastically partial and perhaps disastrously misleading. The point is not whether patriotism is a good thing or a bad thing but simply that it is; it exists, as powerful factor which we all in our hearts acknowledge even as our vocabulary refuses to. And when we speak in this context of "the world," we certainly include in that world ourselves, who offer to interpret it. Every one of our interpretations is coloured by the fact that we, the several interpreters, are British or American, French or Italian or Russian or whatever. If we think otherwise, we deceive ourselves; and yet where, in any of our currently acceptable vocabularies, determined as all of them are by the glib rationalism of the Enlightenment, do we find that momentous fact about ourselves acknowledged? Where is it acknowledged, for instance, in the vocabulary of feminism that "woman," as conceived by an American writing about Italians, cannot help but be significantly different from "woman" as conceived by an Italian looking at Americans? Or again, an Italian woman may well, we must suppose, be an Italian patriot; but where, in the current vocabulary of feminists, is that dimension of her "woman-ness" allowed for? Let it be acknowledged only so as to be deplored; but let it in any event be acknowledged. At the moment, it isn't.Donald A. Davie, the distinguished poet, is Andrew W. This paper suggests a new principle for confirmation theory which is called the principle of explanatory surplus. This principle is shown to be non-Bayesian in character, and to lead to a treatment of simplicity in science. Two cases of the principle of explanatory surplus are considered. The first (number of parameters) is illustrated by curve-fitting examples, while the second (number of theoretical assumptions) is illustrated by the examples of Newton's Laws and Adler's Theory of the Inferiority Complex.

Identification of those who have the potential to become knowledgeable, skilled, and compassionate physicians, and determining how best to prepare them for medical education has been an on ongoing challenge since the mid-1800s (Ludmerer 1985). When medical education was almost exclusively proprietary, the primary consideration for admission was having adequate financial resources. The year 2010 marks the hundredth anniversary of the Report on Medical Education in the United States and Canada (1910), written by Abraham Flexner as Bulletin Four of the Carnegie Foundation for the Advancement of Teaching. This report was monumental in helping to define excellence for the next century of medical education. The editors of Perspectives in Biology and Medicine determined that in recognition of the Flexner Report, this year is appropriate to consider emerging trends that are likely to guide (...) and revitalize medical education in the 21st century. A theoretical framework Is presented that distinguishes among three knowledge sources that form the basis for generative performance. The three knowledge sources, termed conceptual, procedural, and utilizational competence, were implemented as a computational model that derives plans for counting procedures. In a previous analysis, Greeno, Riley, and Gelman (1984) developed a characterization of the conceptual competence (implicit understanding of general concepts and principles) associated with the skill of counting and related conceptual competence to various models of performance. In the current (...) work all three knowledge sources are formalized in a computer program (COUNTPLAN) that generates planning nets of counting procedures. The sufficiency of COUNTPLAN's knowledge components is demonstrated through its capacity to generate new plans for counting in novel settings from a core of conceptual competence.
