Background Cancer patients are at risk of developing blood clots in their veins - venous thromboembolism - which often takes the form of a pulmonary embolism or deep vein thrombosis. The risk increases with advanced disease. Evidence based treatment is low molecular weight heparin by daily subcutaneous injection. The aim of this research is to explore the barriers for doctors in the UK when diagnosing and treating advanced cancer patients with VTE. Method Qualitative, in-depth interview study with 45 doctors. Doctors (...) were from three specialties: oncology, palliative medicine and general practice, with a mixture of senior and junior staff. Framework analysis was used. Results Doctors opinions as to whether LMWH treatment was ethically appropriate for patients who were symptomatic from VTE but at end of life existed on a shifting continuum, largely influenced by patient prognosis. A lack of immediate benefit coupled with the discomfort of a daily injection had influenced some doctors not to prescribe LMWH. The point at which LMWH injections should be stopped in patients at the end of life was ambiguous_._ Some perceived ‘overcaution’ in their own and other clinicians’ treatment of patients_._ Viewpoints were divergent on whether dying of a PE was considered a “good way to go”. The interventionalism and ethos of palliative medicine was discussed. Conclusions Decisions are difficult for doctors to make regarding LMWH treatment for advanced cancer patients with VTE. Treatment for this patient group is bounded to the doctors own moral and ethical frameworks. (shrink)

The Mahabharata, an Indian epic poem, describes a legendary war between two sides of a royal family. The epic’s plot involves numerous moral dilemmas that have intrigued and perplexed scholars of Indian literature. Many of these dilemmas revolve around a character named Krsna. Krsna is a divine incarnation and a self-proclaimed upholder of dharma, a system of social and religious duties central to Hindu ethics. Yet, during the war, Krsna repeatedly encourages his allies to use tactics that violate dharma. In (...) this paper, I try to make sense of Krsna’s actions by analyzing them in terms of categories from Western moral philosophy. I show that Krsna seems to embrace an ethical approach called consequentialism, but that his version of consequentialism differs from Western theories of consequentialism by seeing adherence to dharma as an intrinsic good. In this paper, I offer an explanation of the moral significance of microaggressions, seemingly minor incidents in which someone is demeaned in virtue of an oppressed social identity, often without the full awareness of the perpetrator. I argue for a broadly Kantian account of the wrongs of microaggressions and the moral responsibilities of various actors with respect to these incidents.

Recent theories of negative freedom see it as a value-neutral concept; the definition of freedom should not be in terms of specific moral values. Specifically, preferences or desires do not enter into the definition of freedom. If preferences should so enter then Berlin's problem that a person may enhance their freedom by changing their preferences emerges. This paper demonstrates that such a preference-free conception brings its own counter-intuitive problems. It concludes that these problems might be avoided if the description of (...) the constraints which specify an agent's lack of freedom include the intentions of those who constrain the agents. This paper defends the use of imaginary cases in normative theorizing. Imaginary cases are used as a part of an argument and should be assessed in terms of the role they play within arguments. The paper identifies five ways in which they are used and then uses some of the best examples to bring out how they contribute to debates. While not directly akin to empirical experiments, criticisms of imaginary cases can be represented in terms of the well‐known distinction between (...) internal and external validity in social science experiments. While imaginary cases can suffer from internal and external validity problems, these do not constitute a decisive critique of their use as a method of analysis. Criticisms of imaginary cases should detail the precise form in which they are used, and why any particular use is invalid, internally or externally, in the context of the argument for which the case is presented. Algorithm Decision Making (ADM) systems designed to augment or automate human decision-making have the potential to produce better decisions while also freeing up human time and attention for other pursuits. For this potential to be realised, however, algorithmic decisions must be sufficiently aligned with human goals and interests. We take a Principal-Agent (P-A) approach to the questions of ADM alignment and trust. In a broad sense, ADM is beneficial if and only if human principals can trust algorithmic agents to act (...) faithfully on their behalf. This mirrors the challenge of facilitating P-A relationships among humans, but the peculiar nature of human-machine interaction also raises unique issues. The problem of asymmetric information is omnipresent but takes a different form in the context of ADM. Although the decision-making machinery of an algorithmic agent can in principle be laid bare for all to see, the sheer complexity of ADM systems based on deep learning models prevents straightforward monitoring. We draw on literature from economics and political science to argue that the problem of trust in ADM systems should be addressed at the level of institutions. Although the dyadic relationship between human principals and algorithmic agents is our ultimate concern, cooperation at this level must rest against an institutional environment which allows humans to effectively evaluate and choose among algorithmic alternatives. However, this instability is due not to laissez faire, but to its absence. Comparing the current world financial system to a laissez‐faire benchmark highlights the very significant differences between the two.

While positioning and contextualising the short story 'Green Tea' by Joseph Sheridan Le Fanu (1814-73) in relation to existing Le Fanu scholarship, this article seeks to explore further the textual reflexivity for which it is renowned. Drawing on Foucault’s notion of regimes in the audio and the visual, in particular, through an attention to the interrelationship of the scopic, auditory and textual regimes of ‘Green Tea’, and to the manner in which writing is explicitly figured as both the source of (...) disjunction and the site of interpenetration of the regimes, it is suggested, in conclusion, that a specific understanding of allegory (in a semantic register drawing on work by Walter Benjamin and Paul de Man) can provide a fresh perspective on ‘Green Tea’ as an archive of the regimes of language and light of its time. The use of metaphor and metonymy in social awareness campaigns has been shown as an effective strategy to evoke emotional reactions in the audience and influence behavior change. Using authentic campaign discourse, this study investigated how two multimodal, figurative representations of plastic pollution (plastic pollution is war on nature and plastic pollution is usurpation of marine life) influenced participants’ responses to solutions-oriented and emotions-oriented measures. The data were gathered from two online surveys: Experiment 1 used Likert-scales and free-text responses; these (...) were analyzed using statistical techniques and semantic frame analysis. Experiment 2 was a shorter follow-up study which used Likert-scales. The results suggest that the war frame inspired more negative emotional reactions overall, whereas the usurpation frame inspired more positive emotions. We also find that political orientation significantly moderates people’s sense of empowerment to tackle plastic pollution. The results have implications for future experimental studies seeking to explore the interaction of positive and negative affective states in the context of ecological issues, and for designers of campaigns who seek to use metaphor and metonymy to evoke the desired emotional and behavioral responses. We offer necessary and sufficient conditions – Suitable Reason and Testimonial Honesty – distinguishing manipulative from non-manipulative influence; develop a ‘hypothetical disclosure test’ to measure the degree of manipulation; and provide further criteria to assess and compare the morality of manipulation across cases. Republican freedom is freedom from domination juxtaposed to negative freedom as freedom from interference. Proponents argue that republican freedom is superior since it highlights that individuals lose freedoms even when they are not subject to interference, and claim republican freedom is more ‘resilient’. Republican freedom is trivalent, that is, it includes the idea that someone might be non-free to perform some actions rather than unfree, and in that sense everyone regards republican freedom as different from negative freedom. Trivalence makes republican (...) freedom moralized according to negative libertarians. Beyond that, negative libertarians argue that all the supposed advantages of republican freedom are compatible with those of pure negative-freedom measures. That is, losses and gains of republican freedom are captured in measures of pure negative freedom, and any protection for republican freedom also protects negative freedom, ensuring each is equally resilient. Since republican freedom has no advantages over negative freedom, but has other problems (is moralized and is trivalent), negative freedom is superior. I examine this debate in this article through the ‘coalition problem’ for republican freedom. The coalition problem is that since there is always a coalition of others who could dominate any agent in any sphere, all agents are subject to domination, and hence no one can ever have republican freedom. Pettit’s simple solution to this reductio ad absurdum distinguishes potential from actual coalitions. Individuals are only dominated by actual coalitions, and not by potential ones. The simple solution highlights moralization problems as it demonstrates that domination cannot be purely institutionally defined, but requires consideration of dispositions and expectations about others’ behaviour. I argue that the differences between the ‘free man’ and ‘unfree person’ paradigmatic to republican arguments is best captured not by the difference between domination and interference, but, rather, from familiar distinctions between different types of rights and freedoms. Resilience is a practical matter that might track some of these familiar distinctions. (shrink)

Since Carol Gilligan's analysis of the “Heinz dilemma,” many philosophers working on care have articulated critiques of abstraction and principles in ethics. Their objections to abstraction and principles have not always been systematically set out. In this paper, I try to clarify the debate. I begin by distinguishing several aspects of the care critique. I then consider the strengths of each from a Kantian perspective. I conclude that, although some of these objections point out potential misuses of abstraction and principle, (...) and in doing so, suggest strategies and cautions for their correct and careful use in ethics, they do not present a successful challenge to abstraction or principles as such. In this article, we contribute to sociological literatures on morality, professional and institutional contexts, and morally stigmatized ‘dirty work’ by emphasizing and exploring how they mutually inform one another in lawyers’ work activities. Drawing on interview data with 58 practitioners in the commercial legal industry in Singapore, we analyze how they experience professional and institutional constraints on the expressions of morality in their work. Our findings illustrate how a dominant managerial and economic focus maintains and reproduces a constrained form of (...) morality, limited to instrumental, utilitarian and commercial ends, and subordinated to lucrative client and firm interests. We discuss our findings in terms of the need to research and reform professions in ways that support more rounded and unconstrained moral reflexivity and autonomy in how work is undertaken and valued. This in turn has implications for how organizations and professions might achieve alternative moral institutional orders, and for legal work to avoid the moral and social taints of dirty work. Discourses of interdisciplinary health‐care are becoming more centralised in the context of global healthcare practices, which are increasingly based on multisystem interventions. As with all dominant discourses that are narrated into being, many others have been silenced and decentralised in the process. While questions of the nature and constituents of interdisciplinary practices continue to be debated and rehearsed, this paper focuses on the discourse of interdisciplinary collaboration using psychiatry as an example, with the aim of highlighting competing and alternative discourses. (...) The fundamental premise of this paper is that collaborative relationships form the basis of interdisciplinary practices in psychiatry. Through a critical engagement and a deconstructive reading of the pretext, context and subtexts of interdisciplinarity, we interrogate the concept of interdisciplinary practice within psychiatry. We contend that an important part of understanding and further conceptualising the discourse is through fracturing it. This process is illustrated in the successive stages of our conceptual map of discourse development: establishing, maintaining and developing discourses. An understanding of interdisciplinary practice is not only critical for psychiatry, but also offers important insights into the performance of collaborative failures and indeed successes across nursing and allied health professions. IntroductionAccording to Anselm of Canterbury, God gave human beings two affectiones: the affectio commodi and the affectio iustitiae. For Anselm, these two affectiones are largely equivalent to egoistic motivation and non-egoistic (specifically, moral) motivation: the affectio commodi motivates one to seek one's own advantage (commodum), while the affectio iustitiae motivates one to (...) seek justice (iustitia) for its own sake. Two centuries after Anselm, John Duns Scotus appropriated the terms affectio commodi and affectio iustitiae but made some changes to Anselm's account of the affectiones. Anselm thinks that human beings lost the affectio iustitiae when Adam sinned1 and can regain it only through divine grace,2 but Scotus regards the affectio iustitiae as innate to human beings.3 Also, [End Page 53] Scotus integrates his account of the affectio commodi into the Aristotelian natural teleology commonly accepted at his time, associating the affectio commodi with one's inclination toward one's natural end.4Although Scotus modifies Anselm's account of the affectiones, many interpreters think that he preserves the affectio commodi's association with egoism and the affectio iustitiae's association with non-egoism. I call this interpretation of Scotus's two affectiones "the Standard Reading." On the Standard Reading, the affectio commodi is one's inclination to will things for one's own benefit.5 The Standard Reading's proponents disagree about what exactly the affectio iustitiae is. Some interpret the affectio iustitiae as one's inclination to will or love things because they are good in themselves.6 Others interpret it as one's inclination to love others for their own sake (as opposed to loving them as means to one's own benefit).7 Despite this disagreement, all versions of the Standard Reading agree that the affectio iustitiae enables one to aim at goals other than one's own good.8 [End Page 54]As I argue in this article, some of Scotus's remarks suggest another reading of the two affectiones, one that I call "the Alternative Reading." On this reading, the affectio iustitiae enables one to love both oneself and other beings, and the affectio commodi inclines one to desire things for the benefit of both oneself and other beings that one loves. To be precise, the affectio commodi is an inclination to will what the mind represents as appealing. This inclination, I argue, inclines one to desire what one regards as beneficial to the beings that one loves. Hints of this reading appear in the work of Cruz González-Ayesta,9 but, to my knowledge, no one has drawn special attention to the reading.As I show in this article, both the Standard Reading and the Alternative Reading have textual support, and I regard the support for each as conclusive.10 Therefore, I believe that Scotus accepts both readings. This does not make Scotus inconsistent; I end the article by offering an interpretation that makes both readings true simultaneously. My interpretation is as follows. The affectio commodi is egoistic in that it inclines one to desire what one perceives as beneficial to oneself. The affectio iustitiae is non-egotistic in that it enables one to love a being, whether oneself or something else, for its own sake, as a response to its own objective goodness. But, by loving the being, one comes to see the being's good as part of one's own. Thus, in inclining one to desire things for one's own benefit (as the Standard Reading claims), the affectio commodi inclines one to desire things for the benefit of the beings that one loves (as the Alternative Reading claims) because one perceives their good as part of one's own.To my knowledge, this interpretation of Scotus has not been proposed before. John Hare and Matthew D. Mendham have attributed to Aristotle the view that the self is "elastic": one's own good "stretches" to include the good of one's loved ones. In this paper, I argue for a nuanced, context-sensitive approach to the question of trash talk, based on the Kantian principle of respect for persons and an emphasis on first-person action-guidance. I also suggest that we understand trash talk to have several varieties. On my proposed approach, there is no simple answer to the question of whether trash talk is morally permissible; rather, context-sensitive judgment can help us to determine what we ought to do when the possibility of various forms (...) of trash talk arises. What can philosophy bring to the reading of Beckett? Combining intertextual analysis with a ‘schizoanalytic genealogy’ derived from the authors of L’Anti-Œdipe, Garin Dowd’s Abstract Machines: Samuel Beckett and Philosophy after Deleuze and Guattari offers an innovative response to this much debated question. The author focuses on zones of encounter and thresholds of engagement between Beckett’s writing and a range of philosophers (among them Spinoza, Leibniz and Kant) and philosophical concepts. Beckett’s writing impacts in a variety of ways on Deleuze (...) and Guattari’s thought, and, in particular, resonates with Deleuze’s contributions to the history of philosophy (in books such as Le Pli: Leibniz et le baroque), and his ‘critical and clinical’ approach to literature. Furthermore, the books co-written with Guattari, concerned as they are with the ‘molecularization’ of the discipline of philosophy in the name of ‘thinking otherwise’, reveal themselves in a new light when explored in conjunction with Beckett’s œuvre. With its arresting perspectives on a wide range of Beckett’s works, Abstract Machines will appeal to academics and postgraduate students interested in the philosophical aspects of his writing. Its engagement with alternative contributions to the question of Beckett and philosophy, including that of Alain Badiou, renders it a timely and provocative intervention in contemporary debates on the relationship between literature and philosophy, both within the field of Beckett studies and beyond. If Beckett’s study of Proust has belatedly received the criticisms its author no doubt anticipated, another influential study published a little over thirty years later has, like its predecessor, elicited, among others, the critical response that the author of the Recherche finds himself recruited to the self-serving project of the critic. Gilles Deleuze’s Proust is cast not as the pessimistic Schopenhauerian which Beckett makes of him, but rather, as a force of affirmation in the quasi-Nietzschean register of the ‘powers of (...) the false’. Deleuze would studiously augment his 1964 study with two additions in an effort to improve it. The consequence of this is to render its textual genesis a testament to one of the themes, and for Deleuze the theme of the Recherche, namely, apprenticeship. Moreover, the ‘pedagogy’ to which Proust’s novel subjects the philosopher becomes itself an example of the ‘untimely’ operations of art upon what Deleuze calls the upright or dogmatic image of thought. This third edition of O'Rourke and Boyle's Medical Ethics: Sources of Catholic Teaching is a useful and comprehensive collection of statements published, for the most part, by the central authorities of the Roman Catholic Church, the National Conference of Catholic Bishops in the United States and the bishops' conferences of individual US states on a wide range of issues in the area of medical ethics. ….

Drawing from our interdisciplinary qualitative study of LGBTI conservative Christians and their allies, we name an especially toxic form of shame—what we call sacramental shame—that affects the lives of LGBTI and other conservative Christians. Sacramental shame results from conservative Christianity's allegiance to the doctrine of gender complementarity, which elevates heteronormativity to the level of the sacred and renders those who violate it as not persons, but monsters. In dispensing shame as a sacrament, nonaffirming Christians require constant displays of shame as (...) proof that LGBTI church members love God and belong in the community. Part of what makes this shame so harmful is that parents and pastors often dispense it with sincere expressions of care and affection, compounding the sense that one's capacity to give and receive love is damaged. We foreground LGBTI Christian movements to overcome sacramental shame by cultivating nonhubristic pride, and conclude by discussing briefly their new understandings of love and justice that could have far‐reaching benefits. Brian Barry attacks the `resource account' of power providing a set of definitions through which power should be analysed. While there might be different, equally good, ways of defining power, I argue that the formulations provided by Dowding are superior to those of Barry as they produce fewer anomalies and provide a better foundation for empirical research. Whether or not there is such a thing as moral expertise, and, if so, what constitutes it, is much debated. Empirical expertise bestows epistemic authority over propositional content; that is not the case in moral domains, technical expertise notwithstanding. This article identifies three types of agencies with some authority over decisions in moral matters. It shows that the source of the authority wielded by such agencies, while varying across the three forms identified, is based on empirical and technical knowledge and (...) does not derive solely from any moral expertise of its members. Paper presented at Philippe Garrel, le temps incorporé/Embodied Time Université Paris 8 Nanterre 29 & 30 novembre 2018 This paper proposes a loose typology of space and place in Garrel’s cinema. Its point of departure is the 1969 round table organised by Jacques Rivette which was a supplement to a series of film screenings chosen to illustrate the theme of space in cinema. Among the films chosen was Garrel’s Le lit de la vierge, which, for Rivette, is of interest in (...) the way it exemplifies the cinema of an ‘adventure of place’ rather than an ‘adventure occurring in a place’. The film for Rivette escapes from the “dénoté géographique, topographique”. Although the round table represents a particular juncture it offers a potential line of enquiry regarding the spatialities of Garrel’s subsequent work. Given the crucial role played by memory linked to place, temporality must also be considered. I propose to explore certain examples from Garrel’s body of work through the idea, borrowed from Michael Tawa in his book of this title, of agencies of the frame. Through the possibilities in this conception, a rethinking of the link between character and place/space emerges. Linking the idea of agencies of the frame – which Tawa himself adapts from Simondon – back to Rivette’s early insights, I propose a survey and typology featuring, inter alia, the generic spaces of urban encounter and circulation, specific places of significance to Garrel, types of urban space, screen spaces, abstraction and spatiality and Parisian topographies.
