Self-injurious behavior is associated with diverse psychiatric conditions. Sometimes, SIB is the most dominant symptom, severely restricting the psychosocial functioning and quality of life of the patients and inhibiting appropriate patient care. In severe cases, it can lead to permanent physical injuries or even death. Primary therapy consists of medical treatment and if implementable, behavioral therapy. For patients with severe SIB refractory to conventional therapy, neuromodulation can be considered as a last recourse. In scientific literature, several successful lesioning and deep (...) brain stimulation targets have been described that can indicate a common underlying neuronal pathway. The objectives of this study were to evaluate the short- and long-term clinical outcome of patients with severe, therapy refractory SIB who underwent DBS with diverse underlying psychiatric disorders and to correlate these outcomes with the activated connectivity networks. We retrospectively analyzed 10 patients with SIB who underwent DBS surgery with diverse psychiatric conditions including autism spectrum disorder, organic personality disorder after hypoxic or traumatic brain injury or Tourette syndrome. DBS targets were chosen according to the underlying disorder, patients were either stimulated in the nucleus accumbens, amygdala, posterior hypothalamus, medial thalamus or ventrolateral thalamus. Clinical outcome was measured 6 months after surgery and at long-term follow-up after 10 or more years using the Early Rehabilitation Barthel index and time of restraint. Connectivity patterns were analyzed using normative connectome. Based on previous literature the orbitofrontal cortex, superior frontal gyrus, the anterior cingulate cortex, the amygdala and the hippocampus were chosen as regions of interest. This analysis showed a significant improvement in the functionality of the patients with DBS in the short- and long-term follow-up. Good clinical outcome correlated with higher connectivity to the amygdala and hippocampus. These findings may suggest a common pathway, which can be relevant when planning a surgical procedure in patients with SIB. (shrink)

Definition of the problemFrom an ethical point of view we analyse the ruling of the German Federal Joint Committee (Gemeinsamer Bundesausschuss, G‑BA) of September 2019 to revise the guidelines about the coverage of noninvasive prenatal tests (NIPT) by mandatory health insurance, in order to include them under specified conditions. The decision contains four essential elements: a definition of the aim of NIPT testing (to avoid invasive testing), a criterion of access (test must be “necessary” for the pregnant woman to tackle (...) her individual situation), statements about the process of decision-making (after counselling by a physician and case by case) and a normative contextualization (termination of pregnancy under § 218 a German penal law) which is to be found in its explanations. There are tensions in the proposed model that oscillate about two axes: (1) The suffering that results from a possible birth of a child with trisomy or from not knowing about the genetic condition of the fetus can ultimately only be evaluated subjectively, from the perspective of the pregnant woman. (2) The meaning of an individual decision in the case-by-case model remains unclear because when judging about individual cases general points of view also need to be considered. However, we argue that the model of the G‑BA could be a sociopolitically and ultimately also ethically defensible pragmatic solution, exactly because of its paradoxes and its inherent flexibility. The ecofeminist argument for veganism is powerful. Meat consumption is a deeply gendered act that is closely tied to the systematic objectification of women and nonhuman animals. I worry, however, that presenting veganism as "the" moral ideal might reinforce rather than alleviate the disordered status quo in gendered eating, further disadvantaging women in patriarchal power structures. In this chapter, I advocate a feminist account of ethical eating that treats dietary choices as moral choices insofar as they constitute an integral part (...) of our relationships to ourselves and to others. I believe that we should think of dietary choices in Aristotelian moral terms as a mean relative to us, falling on a continuum between the vice of doing injustice to ourselves, on the one hand, and the vice of doing injustice to others, on the other. On this view, what it is moral to eat for individuals is not a fixed ideal, but rather depends on particulars of our physiological, psychological, economic, cultural, and relational situations. Self-knowledge is a persistent—and paradoxical—theme in medieval mysticism, which portrays our ultimate goal as union with the divine. Union with God is often taken to involve a cognitive and/or volitional merging that requires the loss of a sense of self as distinct from the divine. Yet affective mysticism—which emphasizes the passion of the incarnate Christ and portrays physical and emotional mystical experiences as inherently valuable—was in fact the dominant tradition in the later Middle Ages. An examination of both the affective (...) and apophatic traditions demonstrates that, in addition to constituting a necessary stage on the path toward union with the divine, self-knowledge in medieval mysticism was seen not just as something to be transcended, but (particularly in the works of female mystics) as a means of overcoming alienation from embodied existence. Two central accounts of human cognition emerge over the course of the Middle Ages: the theory of divine illumination and an Aristotelian theory centered on abstraction from sense data. Typically, these two accounts are seen as competing views of the origins of human knowledge; theories of divine illumination focus on God’s direct intervention in our epistemic lives, whereas Aristotelian theories generally claim that our knowledge derives primarily (or even entirely) from sense perception. In this paper, I address an early attempt (...) to reconcile these two accounts—namely, Robert Grosseteste’s commentary on Aristotle’s Posterior Analytics—and I argue that Grosseteste’s efforts to bring Aristotle’s account of human cognition into harmony with a theory of divine illumination proves both philosophically interesting and largely successful. Grosseteste’s claim that God does not directly illumine our intellects in this life opens his theory to the worry that Grosseteste leaves the divine out of his theory of divine illumination in the CPA. Steven Marrone, for example, holds that Grosseteste’s Aristotelian focus in this work causes him to abandon the theory of divine illumination he advocated in the earlier De veritate in favor of a modified theory of “human” illumination. I argue in contrast that Grosseteste does give God a role in human knowledge in the CPA—a role that allows human beings to remain largely responsible for the acquisition of knowledge while still requiring God’s illumination for actual cognition. In short, I conclude that, although human bodies interfere with God’s direct illumination of our intellects, God nevertheless plays a crucial ideogenic role in human cognition by illuminating the objects of our intellection and making them intelligible to us. Orthorexia is a condition in which the subject becomes obsessed with identifying and maintaining the ideal diet, rigidly avoiding foods perceived as unhealthy or harmful. In this paper, I examine widespread cultural factors that provide particularly fertile ground for the development of orthorexia, drawing out social and historical connections between religion and orthorexia (which literally means “righteous eating”), and also addressing how ambiguities in the concept of “health” make it particularly prone to take on quasi-religious significance. I argue that what (...) makes this sort of disordered eating destructive to both men and women is ultimately a common urge to transcend rather than to embrace the realities of embodiment. In sum, I believe that orthorexia is best understood as a manifestation of age-old anxieties about human nitude and mortality—anxieties which current dominant sociocultural forces prime us to experi- ence and express in unhealthy attitudes toward healthy eating. This paper highlights the corrective and complementary role that historically informed philosophy can play in contemporary discussions. What it takes for an experience to count as genuinely mystical has been the source of significant controversy; most current philosophical definitions of ‘mystical experience’ exclude embodied, non-unitive states -- but, in so doing, they exclude the majority of reported mystical experiences. I use a re- examination of the full range of reported medieval mystical experiences (both in the apophatic tradition, which excludes or (...) denigrates embodied states, and in the affective tradition, which treats such states as fully mystical) to demonstrate how a better understanding of the historical medieval mystic tradition can serve as a valuable complement to ongoing philosophical discussions of religious and mystical experience. The aim of this article is to investigate whether choosing the appropriate referring expression requires taking into account the hearer’s perspective, as is predicted under some versions of bidirectional Optimality Theory but is unexpected under other versions. We did this by comparing the results of 25 young and 25 elderly adults on an elicitation task based on eight different picture stories, and a comprehension task based on eight similar written stories. With respect to the elicitation task, we found that elderly (...) adults produce pronouns significantly more often than young adults when referring to the old topic in the presence of a new topic. With respect to the comprehension task, no significant differences were found between elderly and young adults. These results support the hypothesis that speakers optimize bidirectionally and take into account hearers when selecting a referring expression. If the use of a pronoun will lead to an unintended interpretation by the hearer, the speaker will use an unambiguous definite noun phrase instead. Because elderly adults are more limited in their processing capacities, as is indicated by their smaller working memory capacity, as speakers they will not always be able to reason about the hearer’s choices. As a result, they frequently produce non-recoverable pronouns. (shrink)

The reintroduction of Aristotle's Analytics to the Latin West—in particular, the reintroduction of the Posterior Analytics—forever altered the course of medieval epistemological discussions. Although the Analytics fell decidedly from grace in later centuries, the sophisticated account of human cognition developed in the Posterior Analytics appealed so strongly to thirteenth-century European scholars that it became one of the two central theories of knowledge advocated in the later Middle Ages. Robert Grosseteste's 'Commentarius in Posteriorum Analyticorum Libro', written in the 1220s, is most (...) likely the first complete Latin commentary on the Posterior Analytics. As the thirteenth century wore on, Aristotle’s account of human cognition was generally set in opposition to the Augustinian-influenced theory of divine illumination that was de rigueur in the early thirteenth century. Although common consensus holds that this pattern of opposition was set already in Grosseteste’s CPA, I argue that the story of the Posterior Analytics’s early reception is, in fact, quite different. In particular, I maintain that, rather than seeing himself as forced to choose between his earlier theory of divine illumination and the “new” Aristotelian epistemology, Grosseteste is perfectly content to blend and bring together the diverse elements of these systems and to present a consciously synthetic rather than adversarial picture of these differing accounts of human cognition. What makes someone the same person over time? There are (at least) two ways of understanding this question: A person can be the same in the sense of being very similar to how they used to be (similarity), or they can be the same in the sense of being the same individual (numerical identity). In recent years, several papers have claimed to explore the commonsense notion of numerical identity. However, we suggest here that these researchers have instead been studying similarity. (...) We develop a novel method that uses simple intuitions about objects to illustrate these two notions of “same person”, and then asks which concept applies to instances of personal change. Across 4 studies (N = 2446), we find that these previously documented intuitions are best understood as reflecting judgments about similarity, not identity (Experiments 1 and 2). We then use this method to explore the situations in which participants do perceive a change in numerical identity. We find that when a person's entire brain (Experiments 3 and 4) or soul (Experiment 4) has been replaced with that of another person, the majority of participants judge that numerical identity has changed. However, we also note that a substantial minority of participants denied that identity had changed, opening new questions about the role of the body in intuitive judgments of personal identity. Today, philosophers interested in self-knowledge usually look to the scholastic tradition, where the topic is addressed in a systematic and familiar way. Contemporary conceptions of what medieval figures thought about self-knowledge thus skew toward the epistemological. In so doing, however, they often fail to capture the crucial ethical and theological importance that self-knowledge possesses throughout the Middle Ages. -/- Human beings are not transparent to themselves: in particular, knowing oneself in the way needed for moral progress requires hard and rigorous (...) work. Yet, medieval contemplatives insist, without this work we will never attain our final end. In this paper, I trace the connection drawn in this tradition between self-knowledge, humility, and self-fulfillment, arguing in section 1 that the humility that results from introspection needs to be understood in the context of contemplative expectations for eventual perfection. Self-knowledge is key for developing the relationship with God that leads to mystical union, but (as I show in section 2) in the affective tradition of the 13th-14th centuries, which emphasizes the role of emotion and the body, such union with God tends to restore rather than annihilate us. In fact, I argue in section 3, the outcome of such union even in this life is often knowledge that benefits not only the individual who experiences it but also their broader community. (shrink)

The understanding of locomotion in neurological disorders requires technologies for quantitative gait analysis. Numerous modalities are available today to objectively capture spatiotemporal gait and postural control features. Nevertheless, many obstacles prevent the application of these technologies to their full potential in neurological research and especially clinical practice. These include the required expert knowledge, time for data collection, and missing standards for data analysis and reporting. Here, we provide a technological review of wearable and vision-based portable motion analysis tools that emerged (...) in the last decade with recent applications in neurological disorders such as Parkinson's disease and Multiple Sclerosis. The goal is to enable the reader to understand the available technologies with their individual strengths and limitations in order to make an informed decision for own investigations and clinical applications. We foresee that ongoing developments toward user-friendly automated devices will allow for closed-loop applications, long-term monitoring, and telemedical consulting in real-life environments. The Phaedo and the Symposium are dialogues in which Plato's conception of philosophy is not dissociated of orphic elements and mythical constructions. I propose that the themes of the pre-existence of the soul and the immortality of the soul in the Phaedo and myths told by Aristophanes and Diotima in the Symposium, examined together, provide rich material for understanding the nature and limits of knowledge and philosophy in Plato. I also suggest, before this interpretation, a reading of the "sudden vision (...) of beauty in itself," in Symposium 210e, which takes into account these limits. Development of ontology development tools and ontology-enhanced software applications requires thorough understanding of ontology languages in order to implement them according to their specification. We present a formal specification of the ontologies part of the Web Services Modeling Language WSML documentation as a conceptual model in ORM2. Such an approach abstracts the semantics about ontological constructs, axioms, and properties from their implementation in arbitrary formats, thereby making the model easily understandable and reusable. This formal model in ORM2, which is understandable (...) by both logician and software developer, can be used as any other conceptual model to develop applications, thereby ensuring smooth transition from theory to implementations that are faithful to the theory. As multilevel selection theory has gained greater acceptance over the past quarter-century, scientists and scholars have shown an increased interest in the theory's historical antecedents. Despite this interest, however, the early twentieth century remains largely unexplored. It is generally assumed that biologists thought "naively" about evolutionary dynamics during this era, and that their attempts to explain biological phenomena often lacked sophistication. Now that several recent works have called attention to the complex relationship between biological individuality and the levels of selection, (...) we believe it will prove instructive to revisit these early-twentieth-century biologists and reassess their criteria for biological individuality. Doing so reveals that they constructed a multilevel explanation of evolution that anticipated modern interpretations in several important ways. Though it is certainly true that most of these early biologists failed to recognize natural selection's pervasive agency, it is no less true that one of them, termite expert Alfred Emerson, artfully united the multilevel theory of "emergent evolution" with natural selection in a way that differs but little from the theory of multilevel selection that many scientists and scholars now promote. After reviewing the historical record, we place these early-twentieth-century biologists in their proper historical context, and we compare their interpretation of evolution with modern interpretations. Pp. vi i+ 291. $49.95 Although medieval philosophy generally hasn't received much attention from Anglo-American philosophers in the last few centuries, the philosophy of Thomas Aquinas has long been the exception to that rule. In one (...) sense, then, the collection of essays in Aquinas's Moral Theory fits into a long-standing and rich tradition of critical analysis and thoughtful reconstruction. In another sense, however, the book treads ground that is, if not entirely new, then at least not well worn. Most treatments of Aquinas's ethics discuss it in separation from the rest of his work. Aquinas himself, however, sees his moral theory as intrinsically tied up with other aspects of his philosophical system, such as his metaphysics and his philosophical theology. Aquinas's Moral Theory argues that fully understanding Aquinas's ethics, then, requires attention to the way in which "his interests in moral issues often flow into and inform and shape his work in other areas, just as his moral reflections are shaped and grounded by them" (3).MacDonald and Stump have drawn together a group of ten essays by respected scholars in medieval philosophy—essays written with the explicit intent of examining Aquinas's moral theory both in its own right and in light of Aquinas's broader philosophical system. Not surprisingly, the papers in the book range over a wide number of philosophical and theological topics. Moreover, they explore connections not only within Aquinas's work, but between Aquinas and Aristotle, between Aquinas and his contemporaries, and between Aquinas's thought and problems in contemporary philosophy. Christina Howells gives a clear explanation of many of the key terms of deconstruction - including differance, trace, supplement and logocentrism - and shows how they function in Derrida's writing. She explores his critique of the notion of self-presence through his engagement with Husserl, and (...) his critique of humanist conceptions of the subject through an account of his ambivalent and evolving relationship to the philosophy of Sartre. The question of the relationship between philosophy and literature is examined through an analysis of the texts of the 1970s, and in particular Glas, where Derrida confronts Hegel's totalizing dialectics with the fragmentary and iconoclastic writings of Jean Genet. She argues that deconstruction is a vital stimulus to vigilance in both the ethical and political spheres, contributing significantly to debate on issues such as democracy, the legacy of Marxism, responsibility, and the relationship between law and justice. The development of photodynamic therapy and anti-vascular endothelial growth factor agents have revolutionized the treatment of retinal diseases, transforming the retina subspecialty by ushering in an age of pharmacological treatments for a wide range of diseases, including age-related macular degeneration.

This article contends that environmental organisations vary in type, scale and purpose in ways that help stewards self-sort into the opportunities that align with their individual motivations and environmental concerns. To explore these potential links between personal motivations and environmental organisational attributes, we rely on descriptive and inferential statistics from surveys of two partner environmental organisations: one local scale community based-organisation and one a broader scale environmental non-profit organisation, both located in Central Texas, USA. These partners were selected based on (...) their compatible pro-environmental missions but disparate spatial scales of operation. The survey results (n=377) revealed meaningful between-organisation differences in volunteer environmental stewards, which raise the possibility of a ‘First Law of Environmental Stewardship’ whereby all stewards are related (by their values), but stewards in the same organisation are more related than stewards in different organisations. These findings expose some of the nuances of environmental volunteerism by highlighting connections between personal motivations, geographic scale, and organisation type. (shrink)

The phenomenon and implications of stigma have been recognized across many contexts and in relation to many discrete issues or conditions. The notion of spatial stigma has been developed within stigma literature, although the importance and relevance of spatial stigma for rural places and rural people have been largely neglected. This is the case even within fields of inquiry like public and rural health, which are expansively tasked with addressing the socio-structural drivers of health inequalities. In this paper, we argue (...) that developing a better understanding of rural place stigma is critical for addressing contemporary patterns of spatial injustice and health inequalities affecting rural communities globally. Drawing on international literature and examples from the reported experiences of rurally living Australians and news and other media, we present an analysis highlighting the power in rural place stigma. In doing so, we build a case for the relevance and importance of interrogating rural place stigma, especially in the fields of public and rural health, for changing the conditions within—and the broader positioning of—the rural in the public and political landscapes. The global segmentation of production and distribution has resulted in highly complex global value chains where vertical and horizontal dynamics are equally important in determining working conditions and providing points of leverage for labour. Borrowing notions of multi-level governance, we propose an analytical framework for describing and explaining success and failure of labour agency when attempting to improve working conditions along GVCs. Our starting point is that the high complexity of GVCs and the absence of a global overarching authority have (...) been balanced by the emergence of governance mechanisms of different forms and at different levels. An assessment of these mechanisms, which emphasize process over institutions, reveals how different types of interactions and interdependencies along GVCs impact on labour’s use of power. We argue that, in order to fully situate labour agency within governance debates, any future research agenda needs to be driven by micro-level bottom-up approaches. (shrink)

Background: While conscientious objection is a well-known phenomenon in normative and bioethical literature, there is a lack of evidence to support an understanding of what it is like for nurses to make a conscientious objection in clinical practice including the meaning this holds for them and the nursing profession. Research question: The question guiding this research was: what is the lived experience of conscientious objection for Registered Nurses in Ontario? Research design: Interpretive phenomenological methodology was used to gain an in-depth (...) understanding of what it means to be a nurse making a conscientious objection. Purposive sampling with in-depth interview methods was used to collect and then analyze data through an iterative process. Participants and research context: Eight nurse participants were interviewed from across practice settings in Ontario, Canada. Each participant was interviewed twice over 9 months. Ethical considerations: This study was conducted in accordance with Health Science Research Ethics Board approval and all participants gave consent. Findings: Six themes emerged from data analysis: encountering the problem, knowing oneself, taking a stand, alone and uncertain, caring for others, and perceptions of support. Discussion: This study offers an initial understanding of what it is like to be a nurse making a conscientious objection in clinical practice. Implications for nursing practice, education, policy, and further research are discussed. Conclusion: Addressing ethical issues in nursing practice is complex. The need for education across nursing, healthcare disciplines and socio-political sectors is essential to respond to nurses’ ethical concerns giving rise to objections. Conscience emerged as an informant to nurses’ conscientious objections. The need for morally inclusive environments and addressing challenging ethical questions as well as the concept of conscience are relevant to advancing nursing ethics and ethical nursing practice. Background: Ethical nursing practice is increasingly challenging, and strategies for addressing ethical dilemmas are needed to support nurses’ ethical care provision. Conscientious objection is one such strategy for addressing nurses’ personal, ethical conflicts, at times associated with conscience. Exploring both conscience and conscientious objection provides understanding regarding their implications for ethical nursing practice, research, and education. Research aim: To analyze the concepts of conscience and conscientious objection in the context of nurses. Design: Concept analysis using the method by Walker and (...) Avant. Research context: Data were retrieved from Philosopher’s Index, PubMed, and CINAHL with no date restrictions. Ethical consideration: This analysis was carried out per established, scientific guidelines. Findings: Ethical concepts are integral to nursing ethics, yet little is known about conscientious objection in relation to conscience for nurses. Of note, both concepts are well established in ethics literature, addressed in various nursing codes of ethics and regulatory bodies, but the meaning they hold for nurses and the impact they have on nursing education and practice remain unclear. Discussion and conclusion: This article discusses the relevance of conscience and conscientious objection to ethical nursing practice and proposes a model case to show how they can be appreciated in the context of nurses. Conscientious objection is an option for ethical transparency for nurses but is situated in contentious discussions over its use and has yet to be fully understood for nursing practice. Conscience is an element in need of more exploration in the context of conscientious objection. Further research is warranted to understand how nurses respond to conscience concerns in morally, pluralistic nursing contexts. (shrink)

Academics across widely ranging disciplines all pursue knowledge, but they do so using vastly different methods. Do these academics therefore also have different ideas about when someone possesses knowledge? Recent experimental findings suggest that intuitions about when individuals have knowledge may vary across groups; in particular, the concept of knowledge espoused by the discipline of philosophy may not align with the concept held by laypeople. Across two studies, we investigate the concept of knowledge held by academics across seven disciplines (N (...) = 1,581) and compare these judgments to those of philosophers (N = 204) and laypeople (N = 336). We find that academics and laypeople share a similar concept of knowledge, while philosophers have a substantially different concept. These experiments show that (a) in contrast to philosophers, other academics and laypeople attribute knowledge to others in some “Gettier” situations; (b) academics and laypeople are much less likely to attribute knowledge when reminded of the possibility of error, but philosophers are not affected by this reminder; and (c) non‐philosophy academics are overall more skeptical about knowledge than laypeople or philosophers. These findings suggest that academics across a wide range of disciplines share a similar concept of knowledge, and that this concept aligns closely with the intuitions held by laypeople, and differs considerably from the concept of knowledge described in the philosophical literature, as well as the epistemic intuitions of philosophers themselves.
