BackgroundThe cognitive level of post-stroke aphasia patients is generally lower than non-aphasia patients, and cognitive impairment affects the outcome of stroke. However, for different types of PSA, what kind of cognitive assessment methods to choose is not completely clear. We investigated the Montreal Cognitive Assessment, the Mini-Mental State Examination, and the Non-language-based Cognitive Assessment to observe the evaluation effect of CI in patients with fluent aphasia and non-fluent aphasia.Methods92 stroke patients were included in this study. Demographic and clinical data of (...) the stroke group were documented. The language and cognition were evaluated by Western Aphasia Battery, MoCA, MMSE, and NLCA. PSA were divided into FA and NFA according to the Chinese aphasia fluency characteristic scale. Pearson’s product–moment correlation coefficient test and multiple linear regression analysis were performed to explore the relationship between the sub-items of WAB and cognitive scores. The classification rate of CI was tested by Pearson’s Chi-square test or Fisher’s exact test.ResultsThe scores of aphasia quotient, MoCA, MMSE, and NLCA in NFA were lower than FA. AQ was positively correlated with MoCA, MMSE, and NLCA scores. Stepwise multiple linear regression analysis suggested that naming explained 70.7% of variance of MoCA and 79.9% of variance of MMSE; comprehension explained 46.7% of variance of NLCA. In the same type of PSA, there was no significant difference in the classification rate. The classification rate of CI in NFA by MoCA and MMSE was higher than that in FA. There was no significant difference in the classification rate of CI between FA and NFA by NLCA.ConclusionMoCA, MMSE, and NLCA can be applied in FA. NLCA is recommended for NFA. (shrink)

One recurring criticism of the best interests standard concerns its vagueness, and thus the inadequate guidance it offers to care providers. The lack of an agreed definition of ‘best interests’, together with the fact that several suggested considerations adopted in legislation or professional guidelines for doctors do not obviously apply across different groups of persons, result in decisions being made in murky waters. In response, bioethicists have attempted to specify the best interests standard, to reduce the indeterminacy surrounding medical decisions. (...) In this paper, we discuss the bioethicists’ response in relation to the state's possible role in clarifying the best interests standard. We identify and characterise two clarificatory strategies employed by bioethicists —elaborative and enumerative—and argue that the state should adopt the latter. Beyond the practical difficulties of the former strategy, a state adoption of it would inevitably be prejudicial in a pluralistic society. Given the gravity of best interests decisions, and the delicate task of respecting citizens with different understandings of best interests, only the enumerative strategy is viable. We argue that this does not commit the state to silence in providing guidance to and supporting healthcare providers, nor does it facilitate the abuse of the vulnerable. Finally, we address two methodological worries about adopting this approach at the state level. The adoption of the enumerative strategy is not defeatist in attitude, nor does it eventually collapse into (a form of) the elaborative strategy. (shrink)

Background Supporting physicians in Intensive Care Units s as they face dying patients at unprecedented levels due to the COVID-19 pandemic is critical. Amidst a dearth of such data and guided by evidence that nurses in ICUs experience personal, professional and existential issues in similar conditions, a systematic scoping review is proposed to evaluate prevailing accounts of physicians facing dying patients in ICUs through the lens of Personhood. Such data would enhance understanding and guide the provision of better support for (...) ICU physicians. Methods An SSR adopts the Systematic Evidenced Based Approach to map prevailing accounts of caring for dying patients in ICUs. To enhance the transparency and reproducibility of this process, concurrent and independent use of tabulated summaries, thematic analysis and directed content analysis is adopted. Results Eight thousand three hundred fifty-eight abstracts were reviewed from four databases, 474 full-text articles were evaluated, 58 articles were included, and the Split Approach revealed six categories/themes centered around the Innate, Individual, Relational and Societal Rings of Personhood, conflicts in providing end of life care and coping mechanisms employed. Conclusion This SSR suggests that caring for dying patients in ICU impacts how physicians view their personhood. To resolve conflicts within individual concepts of personhood, physicians use prioritization, reframing and rely on accessible, personalized support from colleagues to steer coping strategies. An adapted form of the Ring Theory of Personhood is proposed to direct timely personalized, appropriate and holistic support. (shrink)

It is evident, in the face of the COVID-19 pandemic that has physicians confronting death and dying at unprecedented levels along with growing data suggesting that physicians who care for dying patients face complex emotional, psychological and behavioural effects, that there is a need for their better understanding and the implementation of supportive measures. Taking into account data positing that effects of caring for dying patients may impact a physician’s concept of personhood, or “what makes you, ‘you’”, we adopt Radha (...) Krishna’s Ring Theory of Personhood (RToP) to scrutinise the experiences of physicians working in intensive care units (ICU) using a fictional scenario that was inspired by real events. The impact of death and dying, its catalysts, internal constituents, external factors, dyssynchrony, and buffers, specific to ICU physicians, were identified and explored. Such a framework allows for ramifications to be considered holistically and facilitates the curation of strategies for conflict resolution. This evaluation of the RToP acknowledges the experience and wide-ranging effects it has on ICU physicians. As such, our findings provide insight into their specific needs and highlight the importance of support on a personal and organisational level. Although further research needs to be conducted, the RToP could serve as the basis for a longitudinal assessment tool supported by the use of portfolios or mentorship due to their provision of personalised, appropriate, specific, timely, accessible and long-term support. BackgroundThe COVID-19 pandemic has changed the healthcare landscape drastically. Stricken by sharp surges in morbidity and mortality with resource and manpower shortages confounding their efforts, the medical community has witnessed high rates of burnout and post-traumatic stress amongst themselves. Whilst the prevailing literature has offered glimpses into their professional war, no review thus far has collated the deeply personal reflections of physicians and ascertained how their self-concept, self-esteem and perceived self-worth has altered during this crisis. Without adequate intervention, this may (...) have profound effects on their mental and physical health, personal relationships and professional efficacy.MethodsWith mentions of the coronavirus pervading social media by the millions, this paper set out to collate and thematically analyse social media posts containing first-person physician reflections on how COVID-19 affected their lives and their coping mechanisms. A consistent search strategy was employed and a PRISMA flowchart was used to map out the inclusion/exclusion criteria.ResultsA total of 590 social media posts were screened, 511 evaluated, and 108 included for analysis. Salient themes identified include Disruptions to Personal Psycho-Emotional State, Disruptions to Professional Care Delivery, Concern for Family, Response from Institution, Response from Society and Coping Mechanisms.ConclusionIt is evident that the distress experienced by physicians during this time has been manifold, multi-faceted and dominantly negative. Self-concepts were distorted with weakened self-esteem and perceived self-worth observed. The Ring Theory of Personhood (RToP) was adopted to explain COVID-19’s impact on physician personhood as it considers existential, individual, relational and social concepts of the self. These entwined self-concepts serve as ‘compensatory’ to one another, with coping mechanisms buffering and fortifying the physician’s overall personhood. With healthcare institutions playing a vital role in providing timely and targeted support, it was further proposed that a comprehensive assessment tool based on the RToP could be developed to detect at-risk physicians and evaluate the presence and effectiveness of established support structures. Tasan Chong Yagyong (1762-1836) is one of the most creative thinkers Korea has ever produced, one of the country's first Christians, and a leading scholar in Confucian philosophy. Born in a staunchly Neo-Confucian society, in his early twenties he encountered writings by Catholic missionaries in China and was fascinated. However, when he later learned that the Catholic Church condemned the Confucian practice of placing a spirit tablet on a family altar to honor past generations, he left the small Catholic community (...) he had helped found and ostensibly returned to the Neo-Confucian fold. Nevertheless, the Christian ideas he studied in his youth influenced his thinking for the rest of his life, stimulating him to look at Neo-Confucianism with a critical eye and suggest new solutions to problems Confucian scholars had been addressing for centuries. A Korean Confucian's Advice on How to Be Moral is an annotated translation of Tasan's commentaries on the Confucian classic Zhongyong (usually translated as The Doctrine of the Mean) in which he applies both Confucianism and Christianity to the question of how to best develop a moral character. Written as a dialogue with King Chongjo, (r. 1776-1800) these texts reveal how Tasan interpreted his Confucian tradition, particularly its understanding of how human beings could cultivate morality, while the king's questions illustrate the mainstream Neo-Confucianism Tasan was reacting against. Tasan challenged the non-theistic standard, insisting that living a moral life is not easy and that we need to be motivated to exert the effort necessary to overcome our selfish tendencies. He had abandoned his faith by the time he wrote these commentaries but, influenced by Catholic works and determined to find a more effective way to live a moral life than non-theistic Neo-Confucianism provided, Tasan constructed a Confucian philosophy of moral improvement centered on belief in God. This translation, helpfully annotated for context and analysis, is an exploration of early Korean engagement with the West and a powerful guide to all those interested in Confucianism, Christianity, and morality. 사람들의 행동은 대부분 어떠한 심리과정을 거친 후에 이루어진다고 할 수 있다. 도덕적 행동은 이러한 심리적 과정이 더욱 필요하다고 생각 되며, 가치 판단이 가장 대표적인 심리작용이라고 할 수 있다. 심리작용의 요소로 감정과 사고, 그리고 동기를 들 수 있다. 이러한 세 가지 요소에 초점을 맞춰, 정약용의 맹자에 대한 윤리관을 살펴보고자 한다. 맹자가 말하고 있는 도덕적 행동은 인간의 본성에서 비롯된다. 인간은 어떠한 상황에서도 도덕적 감정, 즉 사단을 느낄 수 있으며, 이러한 감정을 토대로 도덕적 행위를 결정할 수 있다는 것이다. 그리고 마음에서 어떠한 방향으로 행위를 (...) 결정하면, 이러한 행위를 끌고 갈 수 있는 기운, 즉 실천 에너지가 생기게 된다는 것이다. 정약용은 주희와는 달리 性을 嗜好로 보았다. 性이란 善을 좋아하고 惡을 싫어한다는 것이다. 이러한 性은 좋아하는 것을 추구하려고 하고 싫어하는 것을 회피하려고 하는 욕구라고 할 수 있다. 이러한 욕구가 실제로 드러나는 것이 사단이라는 감정이 된다. 사단은 善을 추구하려는 욕구를 지닌 개별적인 감정이라고 하겠다. 또 思란 ‘大體章’에서와 같이 미미할지라도 道心에서 나온 사단을 자각하고, 이를 기준으로 지각한 외부 대상이 大體에 이로운가를 판단하는 것이다. 정약용은 또 의지란 道心과 人心 모두에서 비롯될 수 있는 욕구로 보았다. 정약용의 견해대로『맹자』를 보게 되면, 도덕행동의 도출과정에서 개인의 도덕적 욕구가 주도적으로 관여한다고 할 수 있다. 이러한 점을 볼 때, 정약용은 형이상학에서 비롯된 권위에 도덕적 근거를 두는 윤리체계보다는, 개인이 자연스러운 도덕적 근거를 내재하고 있으며 이를 통하여 능동적으로 도덕적 주체가 될 수 있다고 하는『맹자』의 윤리관을 보다 더 잘 살린다고 할 수 있다. 또 이를 통하여『맹자』에서 윤리적 자아가 더욱 부각된다고 생각한다. 'Jiwoohak' in Confucian Analects is described as one stage of the six-process of life written by Confucius after he recalls his own life. Since Confucius is suggested as a incarnation of sage and also as a being of ideal living, many Confucian scholars have paid attention to this direct description of Confucius's life. Nevertheless, the Confucian scholars show different opinions on a detailed interpretation, especially on the interpretation on 'Isoon(耳順)'. Zhu Xi interprets 'Isoon' as a stage of getting to know (...) without thinking because of that one's Ji(知) reached the most. He thinks that the three-process of 'Boolhok(不惑)' - 'Jicheonmyeong(知天命)' - 'Isoon' is an intensive course of 'Ji'. But, this Zhu Xi's interpretation is rejected by some of the later scholars such as Dasan Chong Yak-Yong, Ito Jinsai and Jiao Xun etc. These later scholars interpret that 'Isoon' is a stage of being not offensive when someone hears of something against the process of nature or slander on oneself. And they regard Zhu Xi's interpretation as a mistranslation by his impractical attempt to sanctify Confucius. However, Zhu Xi's interpretation is based on his Theory of Mind-Nature and the six-process is applied not to a sage but to an ordinary human. Therefore, to raise a question on Zhu Xi's interpretation means to disagree the sanctification of human designated by Zhu Xi and Dasan Chong Yak-Yong and Ito Jinsai stand on that position. Then, Zhu Xi's interpretation on ‘Jiwoohak’ should be read in the context of his Mind-Nature Theory not to misread it. In addition, to analyze his interpretation on the six-process of life can help Zhu Xi's Theory of Mind-Nature and understanding of human. This paper focuses on that the three-process of 'Boolhok' - 'Jicheonmyeong' - 'Isoon' is an intensive course of 'Ji'. Zhu Xi accounts that 'Boolhok' is to know in terms of Sa(事, affair), 'Jicheonmyeong' is to know in terms of Li(理, principle) and 'Isoon' is to be well versed in terms of both Sa and Li. The progress of 'Sa - Li - Li and Sa' can be explained as 'phenomenalistic knowing - substantialistic knowing - knowing the unit of phenomenon and substance' as Avatamsaka shows. The development of this three-process means not an increase or extend of the knowledge but a leap and qualitative transformation of a subject and object. (shrink)

Evolutionary game theory and public goods games offer an important framework to understand cooperation during pandemics. From this perspective, the COVID-19 situation can be conceptualized as a dilemma where people who neglect safety precautions act as free riders, because they get to enjoy the benefits of decreased health risk from others’ compliance with policies despite not contributing to or even undermining public safety themselves. At the same time, humans appear to carry a suite of evolved psychological mechanisms aimed at curbing (...) free riding in order to ensure the continued provision of public goods, which can be leveraged to develop more effective measures to promote compliance with regulations. We also highlight factors beyond free riding that reduce compliance rates, such as the emergence of conspiratorial thinking, which seriously undermine the effectiveness of measures to suppress free riding. Together, the current paper outlines the social dynamics that occur in public goods dilemmas involving the spread of infectious disease, highlights the utility and limits of evolutionary game-theoretic approaches for COVID-19 management, and suggests novel directions based on emerging challenges to cooperation. This study aims to examine key attributes affecting Airbnb users' satisfaction and dissatisfaction through the analysis of online reviews. A corpus that comprises 59,766 Airbnb reviews form 27,980 listings located in 12 different cities is analyzed by using both Latent Dirichlet Allocation and supervised LDA approach. Unlike previous LDA based Airbnb studies, this study examines positive and negative Airbnb reviews separately, and results reveal the heterogeneity of satisfaction and dissatisfaction attributes in Airbnb accommodation. In particular, the emergence of the topic (...) “guest conflicts” in this study leads to a new direction in future sharing economy accommodation research, which is to study the interactions of different guests in a highly shared environment. The results of topic distribution analysis show that in different types of Airbnb properties, Airbnb users attach different importance to the same service attributes. The topic correlation analysis reveals that home like experience and help from the host are associated with Airbnb users' revisit intention. We determine attributes that have the strongest predictive power to Airbnb users' satisfaction and dissatisfaction through the sLDA analysis, which provides valuable managerial insights into priority setting when developing strategies to increase Airbnb users' satisfaction. Methodologically, this study contributes by illustrating how to employ novel approaches to transform social media data into useful knowledge about customer satisfaction, and the findings can provide valuable managerial implications for Airbnb practitioners. Objectives: Athlete burnout is a crucial concern affecting the development and athletic performance of young weightlifters. To reduce or relieve the prevalence of athlete burnout, this study examined the relationship across social support, sports motivation, mental toughness, and athlete burnout in weightlifters.Methods: A total of 315 weightlifters aged 17–28 years old from Sichuan, Chongqing, and Shanxi in China participated in this survey. The Perceived Available Support in Sport Questionnaire, Sports Motivation Questionnaire, Sports Mental Toughness Questionnaire, and Athlete Burnout Questionnaire were (...) used in this study. SPSS Statistics 19.0, AMOS 21.0, and PROCESS 3.0 macro were used to analyze the collected data.Results: The results indicated that weightlifters’ social support could negatively significantly affect athlete burnout [beta = −0.398; 95% confidence interval : −0.3699, −0.2184; P < 0.05) via mental toughness and sports motivation. The mediation analysis revealed that they had partial mediating effect, including three paths: First, social support had a direct effect on athlete burnout ; second, sport mental toughness had a mediating effect on athlete burnout ; and finally, sports motivation had a mediating effect on athlete burnout.Conclusion: The findings revealed that social support could inhibit or prevent athlete burnout via mental toughness and sports motivation; thus, to decrease or relieve the prevalence of burnout in weightlifters, it is an important solution to enhance their social support. ZusammenfassungDie öffentliche Theologie geht von der Kritik an der Privatisierung der Theologie aus und richtet sich auf das aktive Teilnehmen an den öffentlichen Diskursen der Gesellschaft für das gemeinschaftliche Wohl. Der vorliegende Aufsatz fasst den Ausgangs- sowie Zielpunkt der öffentlichen Theologie unter dem Begriff Entprivatisierungsthese zusammen. Während öffentliche Theologie auf die Überwindung von Säkularisierung und Individualisierung zielt, die unter dem Begriff von Privatisierung mitbegriffen sind, sind hier die positiven öffentlichen Bedeutungen von Säkularisierung und Individualisierung verdeutlicht. Ausgehend von Harnacks privatisierter theologischer (...) Auffassung vom Evangelium und seiner Teilnahme an politischen und sozialen Fragen wird herausgestellt, dass die öffentliche Dimension der Theologie nicht in der Exklusion von Säkularisierung und Individualisierung, sondern in ihrer positiven Aufnahme zurückzugewinnen ist. This paper belongs to the research on the limit of the first incompleteness theorem. Effectively inseparable (EI) theories can be viewed as an effective version of essentially undecidable (EU) theories, and EI is stronger than EU. We examine this question: Are there minimal effectively inseparable theories with respect to interpretability? We propose tEI, the theory version of EI. We first prove that there are no minimal tEI theories with respect to interpretability (i.e., for any tEI theory T, we can effectively (...) find a theory which is tEI and strictly weaker than T with respect to interpretability). By a theorem due to Pour-EI, we have that tEI is equivalent with EI. Thus, there are no minimal EI theories with respect to interpretability. Also, we prove that there are no minimal finitely axiomatizable EI theories with respect to interpretability. (shrink)
