Controlled donation after circulatory determination of death (cDCDD) is an important strategy for increasing the pool of eligible organ donors.

1 Evidence-based policy requires researchers to provide the answers to ecological questions that are of interest to policy makers. To find out what those questions are in the UK, representatives from 28 organizations involved in policy, together with scientists from 10 academic institutions, were asked to generate a list of questions from their organizations. 2 During a 2-day workshop the initial list of 1003 questions generated from consulting at least 654 policy makers and academics was used as a basis for (...) generating a short list of 100 questions of significant policy relevance. Short-listing was decided on the basis of the preferences of the representatives from the policy-led organizations. 3 The areas covered included most major issues of environmental concern in the UK, including agriculture, marine fisheries, climate change, ecosystem function and land management. 4 The most striking outcome was the preference for general questions rather than narrow ones. The reason is that policy is driven by broad issues rather than specific ones. In contrast, scientists are frequently best equipped to answer specific questions. This means that it may be necessary to extract the underpinning specific question before researchers can proceed. 5 Synthesis and applications. Greater communication between policy makers and scientists is required in order to ensure that applied ecologists are dealing with issues in a way that can feed into policy. It is particularly important that applied ecologists emphasize the generic value of their work wherever possible. (shrink)

This paper explores the cognitive developments underlying conventionalized social phenomena such as language and ownership. What do children make of the claims that, 'This is mine' or 'That is called "water"?' Understanding these features of social reality involves appreciating status as a system of normative prescriptions. Research on children's theories of intentional agency suggests important constraints on the development of status systems. Key insights are that prescriptions affect behavior only via representations, and that the norms involved in prescriptions are distinct (...) from statements of preferences. The issue of the protection of communities in clinical research first arose 10 years ago in studies conducted in technologically developing countries by scientists from technologically developed nations. The question was, which ethical standards ought to apply, those of the Western investigators or local standards?

Over the last quarter of a century, English medical law has taken an increasingly firm stand against medical paternalism. This is exemplified by cases such as Bolitho v City and Hackney Health Authority, Chester v Afshar, and Montgomery v Lanarkshire Health Board. In relation to decision-making on behalf of incapacitous adults, the actuating principle of the Mental Capacity Act 2005 is respect for patient autonomy. The only lawful acts in relation to an incapacitous person are acts which are in the (...) best interests of that person. The 2005 Act requires a holistic assessment of best interests. Best interests are wider than ‘medical best interests’. The 2018 judgment of the Supreme Court in An NHS Trust v Y from patients in prolonged disorders of consciousness ) risks reviving medical paternalism. The judgment, in its uncritical endorsement of guidelines from various medical organisations, may lend inappropriate authority to medical judgments of best interests and silence or render impotent non-medical contributions to the debate about best interests—so frustrating the 2005 Act. To minimise these dangers, a system of meditation should be instituted whenever it is proposed to withdraw life-sustaining CANH from patients with PDOC, and there needs to be a guarantee of access to the courts for families, carers and others who wish to challenge medical conclusions about withdrawal. This essay argues that Knafo and Teschke fundamentally misread Brenner’s original contribution to the transition debate. They equate his rejection of trans-historical or trans-modal laws of motion with the notion that social-property relations do not have strong rules of reproduction that structure the actions of agents and give rise to ‘developmental patterns’ specific to each form of social labour. Knafo and Teschke’s critique of Brenner’s analysis of capitalist expansion and crisis is also theoretically and empirically questionable.

BackgroundEarly in the COVID-19 pandemic, the urgent need to discover effective therapies for COVID-19 prompted questions about the ethical problem of randomization along with its widely accepted solution: equipoise. In this scoping review, uses of equipoise in discussions of randomized controlled trials of COVID-19 therapies are evaluated to answer three questions. First, how has equipoise been applied to COVID-19 research? Second, has equipoise been employed accurately? And third, do concerns about equipoise pose a barrier to the ethical conduct of COVID-19 (...) RCTs?MethodsGoogle Scholar and Pubmed were searched for articles containing substantial discussion about equipoise and COVID-19 RCTs. 347 article titles were screened, 91 full text articles were assessed, and 48 articles were included. Uses of equipoise were analyzed and abstracted into seven categories.Results and discussionApproximately two-thirds of articles used equipoise in a way that is consistent with the concept. They invoked equipoise to support RCTs of specific therapies, RCTs in general, and the early termination of RCTs after achieving the primary outcome. Approximately one-third of articles used equipoise in a manner that is inconsistent with the concept. These articles argued that physician preference, widespread use of unproven therapies, patient preference, or expectation of therapeutic benefit may undermine equipoise and render RCTs unethical. In each case, the purported ethical problem can be resolved by correcting the use of equipoise.ConclusionsOur findings highlight the continued relevance of equipoise as it supports the conduct of well-conceived RCTs and provides moral guidance to physicians and researchers as they search for effective therapies for COVID-19. David Elder-Vass's “For Emergence: refining Archer's account of social structure,” is the latest of a number of papers which together constitute a family quarrel in the cognitive space After Postmodernism among realist social scientists. In the case under examination here in “Elder-Vass's Move and Giddens's Call”, the concern is the structure and agency problem in the social sciences. The debate continuing in Elder-Vass's paper represents the proponents of the resurrection of Durkheim's social realism under the auspices of Bhaskar's Transcendental Realism; (...) the debate that continues here as a response to the Elder-Vass paper, opposes the latter's attempt to advance the argument for social realism under the canopy of Harre's Immanental realism. The theme of this response is this: Elder-Vass fails to realize the promise of reconciling social structural realism and social relational realism. (shrink)

Population ethics contains several principles that avoid the repugnant conclusion. These rules rank all possible alternatives, leaving no room for moral ambiguity. Building on a suggestion of Parfit, this paper characterizes principles that provide incomplete but ethically attractive rankings of alternatives with different population sizes. All of them rank same-number alternatives with generalized utilitarianism.

The question a judge has to ask in deciding whether or not life-sustaining treatment should be withdrawn is whether the continued treatment is lawful. It will be lawful if it is in the patient’s best interests. Identifying this question gives no guidance about how to approach the assessment of best interests. It merely identifies the judge’s job. The presumption in favour of the maintenance of life is part of the job that follows the identification of the question.The presumption is best (...) regarded as a presumption of law. It has long been recognised as part of the way in which the English law discharges its obligations under Article 2 of the European Convention on Human Rights (the right to life). But even if it is a ‘mere’ evidential presumption it cannot, on the facts of most cases involving applications for the withdrawal of life-sustaining treatment from patients in prolonged disorders of consciousness, be rebutted. In the mid-1950s, a young professor at Princeton named Charles Gillispie began teaching Humanities 304, one of the first undergraduate courses offered anywhere in the world on the history of science. From Galileo's analysis of motion to theories of evolution and relativity, Gillispie introduces key concepts, individuals, and themes. The Edge of Objectivity arose out of this course. It must (...) have been a lively class. The Edge of Objectivity is pointed, opinionated, and selective. Even at six hundred pages, the book is, as the title suggests, an essay. Gillispie is unafraid to rate Mendel higher than Darwin, Maxwell above Faraday. Full of wry turns of phrase, the book effectively captures people and places. And throughout the book, Gillispie pushes an argument. He views science as the progressive development of more objective, detached, mathematical ways of viewing the world, and he orchestrates his characters and ideas around this theme. This edition of Charles Coulston Gillispie's landmark book introduces a new generation of readers to his provocative and enlightening account of the advancement of scientific thought over the course of four centuries. Since the original publication of The Edge of Objectivity, historians of science have focused increasingly on the social context of science rather than its internal dynamics, and they have frequently viewed science more as a threatening instance of power than as an accumulation of knowledge. In the twenty-first century, Stephen Hawking proclaimed the death of philosophy. Only science can address philosophy’s perennial questions about human values. The essay first examines Nietzsche’s nineteenth century view to the contrary that philosophy alone can create values. A critique of Nietzsche’s contention that philosophy rather than science is competent to judge values follows. The essay then analyzes Edward O. Wilson’s claim that his scientific research provides empirically-based answers to philosophy’s questions about human values. Wilson’s bold new hypothesis about the (...) ‘social conquest of the earth’ challenges Nietzsche’s vision of philosophy’s mission. Confronting both Nietzsche and Wilson, the essay then considers three theoretical proposals for a consilience of philosophy, science, engineering and technology. The conclusion presents a working African model of consilience that addresses the existential problem of poverty in the Global South. (shrink)

Do high-grade assessments have a use in marking episodes in mundane conversation? Inspection suggests that closing sequences in telephone conversations, when they include such embedded actions as making arrangements, have a slot which can be filled by a turn-initial high-grade assessment. I suggest that the high-grade assessment makes a special display of resuming a closing which had been suspended. I make a link between marked resumption in such mundane closings and more institutional agenda-marking, and speculate that using a resumptive high-grade (...) assessment might display a claim to `ownership' of the closedown sequence.
