What exactly is a genetic disease? For a phrase one hears on a daily basis, there has been surprisingly little analysis of the underlying concept. Medical doctors seem perfectly willing to admit that the etiology of disease is typically complex, with a great many factors interacting to bring about a given condition. On such a view, descriptions of diseases like cancer as genetic seem at best highly simplistic, and at worst philosophically indefensible. On the other hand, there is clearly some (...) practical value to be had by classifying diseases according to their predominant cause when this can be accomplished in a theoretically satisfactory manner. The question therefore becomes exactly how one should go about selecting a single causal factor among many to explain the presence of disease. When an attempt to defend such causal selection is made at all, the standard accounts offered (Koch's postulates, Hill's epidemiological criteria, manipulability) are all clearly inadequate. I propose, however, an epidemiological account of disease causation which walks the fine line between practical applicability and theoretical considerations of causal complexity and attempts to compromise between patientcentered and population-centered concepts of disease. The epidemiological account is the most basic framework consistent with our strongly held intuitions about the causal classification of disease, yet it avoids the difficulties encountered by its competitors. In this article we review Western discourse on the relationship between Daoism and anarchist political theory. In particular, we focus on the anarchist reading of Daoism given by Roger Ames, and the more recent contrasting argument against reading Daoism as an anarchism by Alex Feldt. Centering our discussion on the Daodejing 道德經, we argue that, on the one hand, Laozi’s 老子 political theory is less easily reconcilable with anarchist thinking than Ames suggests. On the other hand, we dispute Feldt’s argument (...) that Laozi’s sage-ruler must, of necessity, maintain the capacity for coercive control. Counter to both Ames and Feldt, we suggest that Laozi’s sage-ruler is better framed as a maternal overseer, in contrast to other more paternalistic extrapolations of Daoist thinking, such as that offered in the Hanfeizi 韓非子. In reading Laozi’s thinking as a form of state “maternalism,” we aim to give a more distinctive voice to the nuances of early Daoist political theory. (shrink)

In Knowledge and Its Limits, Timothy Williamson argues that knowledge is a purely mental state, that is, that it is never a complex state or condition comprising mental factors and non-mental, environmental factors. Three of his arguments are evaluated: arguments from (1) the non-analyzability of the concept of knowledge, (2) the “primeness” of knowledge, and (3) the (alleged) inability to satisfactorily specify the “internal” element involved in knowledge. None of these arguments succeeds. Moreover, consideration of the third argument points the (...) way to a cogent argument that knowledge is not a purely mental state. (shrink)

In this article we examine Classical Confucian political thinking through the lens of paternalism. We situate Confucianism amid contemporary models of paternalism to show that Confucianism can be understood as a soft form of paternalism regarding its method. Confucianism stresses cultivation of the people by moral exemplars to guide the people to act in ways that are in their own best interests. This is in contrast to use of law and punishment as a deterrent of unwanted behaviours of the people. (...) We demonstrate that Confucian paternalism does not advocate for a static top-down structure of governance that is incapable of reform, underscoring its non-authoritarian ideal. We do this by stressing the vital upward momentum constituted in general cultivation of the wider population utilizing li. The picture that emerges from an examination of Confucian political thought through the lens of paternalism is what we name “exemplary paternalism.”. (shrink)

Background: Caring for a partner with primary malignant brain tumour can be a dramatic life-changing event. Primary malignant brain tumour is known to give poor life expectancy and severe neurological and cognitive symptoms, such as changed behaviour and personality, which demand greater caring responsibilities from spouses. Aim: The aim of the study is to explore ethical dilemmas spouses experience in the everyday care of a partner in treatment for primary malignant brain tumour. Research design, participants and research context: A phenomenological (...) and hermeneutic qualitative descriptive design was adopted as a method for collecting and analysing data. Ten spouses were interviewed twice using an in-depth, semi-structured interview guide. The interviews took place at the spouses’ homes or at the hospital. Ethical consideration: Ethical matters were considered throughout the research process. Permission from The National Committee on Health Research Ethics and the Danish Data Protection Agency was obtained. Findings: The analysis showed that the spouses perceived daily ethical dilemmas in caring for a partner with primary malignant brain tumour. Their life as well as their partner’s life had changed considerably. The main theme that emerged therefore was ‘oscillating in a changing relationship’. This theme was further elaborated in three subthemes that in more detail demonstrated the dilemmas: ‘doing the right thing in unpredictable daily situations’; ‘torn between patience and guilt’; and ‘living in a time of uncertainty, hope and despair’. Conclusion: Caring for a partner with changed behaviour and personality due to primary malignant brain tumour may involve exhausting ethical caring dilemmas. Spouses’ married life may change to a semi-professional asymmetrical relationship, which is challenged by the oscillation between acting responsibly for their partners’ well-being and caring dilemmas with no answer for what the right thing to do is. Mixed feelings of right and wrong, patience and guilt, hope and despair seem to be spousal companions through their partners’ progressing illness. (shrink)

Despite recognition of the importance of family in health‐care and progress in family theory development, there has been limited transfer of family theory to acute care nursing practice. We argue that this family theory–practice gap results from a persistent lack of conceptual clarity in family nursing and other barriers. Lack of conceptual clarity takes the form of conceptual overlap and semantic inconsistency, as well as the complexity of language found in the family nursing literature. Barriers include practice contexts, relational problems, (...) and knowledge types. Our exploration begins with a brief discussion of the intimate link between nursing theory and practice followed by an overview of some issues associated with the family nursing theory–practice gap. Based on a synthesis of family nursing literature, problems associated with conceptual clarity in family nursing theory are explored. We conclude with recommendations for family nursing research to develop concepts grounded in nursing practice. (shrink)

From unemployment to Brexit to climate change, capitalism is in trouble and ill-prepared to cope with the challenges of the coming decades. How did we get here? While contemporary economists and policymakers tend to ignore the political and social dimensions of capitalism, some of the great economists of the past - Adam Smith, Friedrich List, John Maynard Keynes, Joseph Schumpeter, Karl Polanyi and Albert Hirschman - did not make the same mistake. Leveraging their insights, sociologists John L. Campbell and John (...) A. Hall trace the historical development of capitalism as a social, political, and economic system throughout the twentieth and early twenty-first centuries. They draw comparisons across eras and around the globe to show that there is no inevitable logic of capitalism. Rather, capitalism's performance depends on the strength of nation-states, the social cohesion of capitalist societies, and the stability of the international system - three things that are in short supply today. Australia does not allow adult smokers to buy or use electronic nicotine delivery systems that contain nicotine without a prescription. This paper critically evaluates the empirical and ethical justifications provided for the policy by Federal and State governments, public health advocates and health organisations. These are: that ENDS should only be approved as products for smoking cessation when there is evidence from randomised controlled trials that they are effective; that as a matter of precaution we should not allow the sale (...) of ENDS to smokers as consumer products because we do not know what their long-term effects will be; and that allowing ENDS to be sold as consumer goods will enable the tobacco industry to market ENDS to young people which will also lead to an increase in youth smoking. We show that the arguments and evidence offered in support of all these claims is very weak. We also argue that even if the evidence were stronger, it would not justify denying adult smokers the right to use ENDS either to quit smoking or as a long-term alternative to smoking cigarettes. We outline ENDS policies that would more ethically address the public health concerns that motivated the current policy by allowing adult smokers to access ENDS for smoking cessation or tobacco harm reduction under tight regulations that discourage commercial promotion and adolescent use. Public participation and survivor research in mental health are widely recognized as vital to the field. At the same time, contributions of patient collaborators can present unique challenges to determining authorship. Using an unresolved dispute around research contributions to the American Psychiatric Association’s Psychiatric Services journal, authorship and contribution are addressed. Recommendations are suggested to prevent dilemmas and achieve responsible research credit inclusion, especially among researchers with different backgrounds and asymmetric power relations. Researchers and publishers can prepare proactively for conflict (...) through consensus on authorship criteria, prior agreements around author inclusion, arrangement for third party dispute resolution, transparency in communication and contracts, notification to prospective publications of pending disputes, a contributor-guarantor model of contribution, journal editor “expressions of concern” when authorship disputes go unresolved, and expectation of conflict as generative. This review article surveys recent work in political theory that has brought together biopolitics and the COVID-19 pandemic. Centered on 2021 books by Giorgio Agamben and Benjamin Bratton, the essay outlines prominent visions of “negative” (Agamben) and “positive” (Bratton) biopolitical responses to the pandemic, engages public reactions to these approaches, and reassesses the position of biopolitical thinking in light of these. In doing so, the article recalls the foundations and original interventions of biopolitical theory, calling for a renewed engagement with (...) the perspectives afforded by biopolitics that pushes past the negative/positive binary. Ultimately, the essay gathers together major developments in biopolitical thinking today, counters moves to discard the theoretical approach despite the limitations of recent examples, and repositions biopolitics as an ambivalent tool for political thought and practice going forward. A staff photographer for the Ketchikan Daily News, Hall Anderson counted among his early influences photographers like Robert Frank and Henri Cartier-Bresson, who understood the visual bounty to be found in photographing the candid side of life. For more than twenty-five years, Anderson has brought this perspective to his photographic endeavors, both personal and professional, in the small town of Ketchikan in southeast Alaska. This book addresses the question of what unifies the French philosophical tradition across the twentieth century by exploring these two questions. Kant claimed that the structure of experience mirrored the categories of thought because the cognitive faculties of the subject played a constitutive role in structuring experience. As such, the structure of thought for Kant is fundamentally tied to the nature of metaphysics. In developing his account of what thinking entailed, Kant took judgement, the attribution of a predicate to a (...) subject, to be the paradigm act of thinking. Throughout the nineteenth century, efforts to move beyond Kant's model of thinking as judgement centred on the attempt to augment that model. For instance, Hegel saw Kant's account of judgement as an essentially fixed form of the true nature of thinking, and therefore developed a more processual account of thinking. His account still ultimately operates by putting judgement into motion, however, rather than rejecting the model of thinking as judgement itself. Conversely, philosophers such as Schelling who do not understand thinking solely in terms of judging often retreat into an indeterminate mysticism, thus retaining the idea that anything positive that can be said about thought must take judgement as a model. Plato’s often-quoted statement in the Phaedrus that we should “cut up each kind according to its species along its natural joints” (265e) has become an influential metaphor in discussions of natural kinds and natural properties. In this essay, I investigate the source domain of the metaphor, the joints of the animal body, to determine if these joints are indeed “natural”—meaning that there exists a single, non-disjunctive account of joint-hood applicable to the osteological world. By examining animal joints from the perspective (...) of both the butcher and the biologist, I argue that there is no single natural division of the body into component skeletal parts. I conclude by considering what impact this result should have on our understanding of natural kinds and on the metaphors we use to describe them. (shrink)
