Nurses play a crucial role in reducing health disparities and advancing health equity for individuals and communities. The future nursing workforce relies on their nursing education to prepare them to promote health equity. Nursing educators prepare students through a variety of andragogical learning strategies in the classroom and in clinical experiences and by intentionally updating and revising curricular content to address knowledge and competency gaps. This critical review aimed to determine the extent to which health equity concepts are explicitly present (...) in prelicensure undergraduate nursing curricula globally. Of 434 articles screened, 22 articles describing 20 studies met inclusion criteria. Frequency and quantity of health equity content, concepts and topics, teaching strategies, evaluation strategies, and the overall extent of integration varied widely. Notably, only two articles described overall well‐integrated explicit health equity content, and there was little attention to whether students transfer this learning into practice. A focus on individualism rather than population and community was noted, highlighting the presence of whiteness in nursing. Results from this review confirm that nursing education has room to improve with respect to health equity in the curricula. (shrink)

A collegial procedure refers to the fact that, before making a delicate medical decision, the opinion of the entire care team responsible for the patient is sought, including nurses and orderlies, among others. The Claeys-Leonetti end-of-life law (2016) enshrines this in French law as mandatory when implementing deep and continuous sedation until death (DCSD). The aim of the study was to take stock of the awareness of this aspect of the law among all the healthcare staff of a cancer institute (...) and to identify how collegiality was established for a DCSD decision, depending on the department. We administered an anonymous online questionnaire to all those involved in healthcare (knowledge of the Claeys-Leonetti Act), and then met healthcare workers from the various departments in discussion groups (managers, nursing staff and orderlies) or in individual interviews (doctors). The results show that young healthcare staff (with less than 5 years’ experience) are more familiar with the law, across all professions, and that nurses and orderlies are more likely to assert their rights than doctors. We report on the diversity of the implementation of collegiality for sedation decisions, in terms of formalisation, standardisation and inter-professionalisation. We conclude that the implementation of collegiality in advance of a decision broadens the methods of multi/inter-professional communication and helps to alleviate the suffering of patients, doctors and care givers. We discuss five basic principles of E-Z Reader in terms of their potential for models of eye-movement control in object and scene perception. We identify several obstacles which may hinder the extrapolation of the E-Z Reader principles to nonreading tasks, yet find that sufficient similarities remain to justify using E-Z Reader as a guide for modeling eye-movement control in object and scene perception.

ZusammenfassungIm vorliegenden Beitrag wird vor dem Hintergrund diskursiv verhandelter Topoi über Geflüchtete untersucht, inwiefern Sportlehrkräfte geflüchtete Schüler*innen als ‚Andere‘ wahrnehmen. Angesichts der binären Geschlechterordnung, die sowohl im Sport als auch im Diskurs um Geflüchtete reproduziert wird, liegt der Schwerpunkt hierbei auf der geschlechterbezogenen Konstruktion von ‚Anderssein‘. Dafür werden der Forschungsstand und konstruktivistische und postkoloniale Theoriebezüge skizziert sowie 31 Interviews mit Sportlehrkräften unter einem diskursanalytischen Fokus ausgewertet. Die Befunde legen nahe, dass der Umgang mit geflüchteten Schüler*innen mit dem Normalitätsverständnis der Interviewten (...) und der Institution Schule bricht. Darüber hinaus lassen sich fluchtbezogene Konstruktionsmuster erkennen, die durch die Körperzentrierung des Sportunterrichts besonders aktualisiert und von den Befragten geschlechtstypisierend und kulturell ausgedeutet werden. Die in jüngster Zeit bekannt gewordenen Fälle von sexualisierter Gewalt in pädagogischen Institutionen werden im vorliegenden Beitrag zum Anlass genommen, den Fragen nachzugehen, was bislang über sexualisierte Gewalt im Sport bekannt ist und welche Strukturen und Prozesse gerade in Organisationen des Sports zu sexualisierter Gewalt und ihrer Verdeckung beitragen können. Dazu wird sowohl der Forschungsstand über sexualisierte Gewalt im Sport und in pädagogischen Institutionen herangezogen, als auch ein durch qualitative (...) Mhealth technology is mushrooming world-wide and, in a variety of forms, reaches increasing numbers of users in ever-widening contexts and virtually independent from standard medical evidence assessment. Yet, debate on the broader societal impact including in particular mapping and classification of ethical issues raised has been limited. This article, as part of an ongoing empirically informed ethical research project, provides an overview of ethical issues of mhealth applications with a specific focus on implications on autonomy as a key notion in (...) the debate. A multi-stage model of references to the potential of mhealth use for strengthening some or other form of self-determination will be proposed as a descriptive tool. It illustrates an assumed continuum of enhanced autonomy via mhealth broadly conceived: from patient to user autonomy, to improved health literacy, and finally to the vision of supra-individual empowerment and democratised, participatory health and medicine as a whole. On closer examination, however, these references are frequently ambivalent or vague, perpetuating the at times uncritical use of established autonomy concepts in medical ethics. The article suggests zooming in on the range of autonomy-related aspects against the backdrop of digital innovation and datafied health more generally, and on this basis add to existing frameworks for the ethical evaluation of mhealth more specifically. BackgroundWhereas the majority of bereaved persons recover from their grief without professional assistance, a minority develops pathological grief reactions. Etiological models postulate that dysfunctional cognitions may perpetuate such reactions. The Grief Cognitions Questionnaire assesses thoughts after bereavement in nine interrelated domains. A short form with four domains is often used. However, an evaluation of the psychometric properties of the GCQ-SF and its utility compared to the GCQ is lacking and these instruments have not been validated in German.MethodGerman bereaved persons responded (...) to an online survey containing the GCQ, measures of grief severity, grief rumination, symptoms of depression and anxiety, and optimism and pessimism. 585 participants were included. Item analyses and confirmatory factor analyses were conducted. Correlations between the GCQ and GCQ-SF and grief rumination, optimism and pessimism assessed construct validity. Criterion-related validity was assessed by comparing whether the correlation of the GCQ with grief severity was higher than with anxious and depressive symptoms. Logistic regression and receiver-operator characteristics compared the questionnaires on their ability to predict probable prolonged grief ‘caseness’.ResultsInternal consistencies for both questionnaires were identical and excellent. Confirmatory factor analyses obtained a satisfactory fit for models with nine and four correlated subscales and respective higher-order factor models. The GCQ and the GCQ-SF correlated higher with grief severity than with other measures of psychopathology. The logistic regression showed a significant association between the GCQ-SF and prolonged grief ‘caseness’. Of the remaining subscales of the GCQ, only one subscale contributed to the prediction. The ROC analyses showed nearly identical areas under the curve.ConclusionThe translated GCQ and GCQ-SF demonstrated very good psychometric properties. The correlations with grief severity highlight the questionnaires’ clinical relevance. The questionnaires possessed identical diagnostic specificity and sensitivity. Whenever a timesaving assessment of the most typical grief-specific cognitions is important, the GCQ-SF represents an alternative to the GCQ. The original GCQ may still be superior when a more detailed description of a bereaved person’s cognitions is desirable. (shrink)

Although lie detection procedures have been fundamentally criticized since their inception at the beginning of the 20th century, they are still in use around the world. In addition, they have created some remarkable appeal in the context of counterterrorism policies. Thereby, the links between science and fiction in this topic are quite tight and by no means arbitrary: In the progressive narrative of the lie detection devices, there is a promise of changing society for the better, which is entangled in (...) a fictional narrative provided by many cinematic and literary examples. By drawing on the concept of “diegetic technologies” formulated by David Kirby, we want to highlight the role of science fictional narratives in the historical development and current application of lie detection procedures. We therefore aim to use this conceptual frame to analyze the role of prevention-oriented mind reading procedures, which are developed for or already used in the “war on terror” with reference to their fictional predecessors. One virulent factor has to be highlighted in this context: The fictional engagement with possible new lie detection practices is in itself creating a legitimating ground for future security technologies. Research indicates that family firms often engender a sense of trustworthiness among stakeholders. However, little is known as to whether this trustworthiness is beneficial or detrimental to family firms in the face of an ethical scandal. Ethical transgressions can profoundly undermine stakeholders’ perceptions of a firm’s integrity and benevolence. Our research examines how stakeholders perceive and react to ethical transgressions committed by family firms, as compared to those committed by non-family firms. Drawing upon expectancy violations theory and social identity theory, (...) we theorize that while family firms inherently enjoy a higher degree of trustworthiness, they suffer significantly more in the aftermath of an ethical transgression. Two scenario-based experimental studies support our theorizing, demonstrating that family firms experience a steeper decline in trustworthiness following an ethical transgression than do non-family firms. We uncover the psychological processes behind this finding, revealing that this vulnerability is attributed to heightened stakeholder expectations and pronounced identification with family firms. We empirically show that expectancy violations primarily diminish integrity perceptions, while identity threats degrade benevolence perceptions of family firms. This research broadens the understanding of ethics in family firms, highlighting how their initially perceived trustworthiness may become a double-edged sword during ethical crises. (shrink)

Beauchamp and Childress’ definition of autonomous decision‐making includes the conditions of intentionality, understanding, and non‐control. In genetics, however, a relational conception of autonomy has been increasingly recognized. This article aims to empirically assess aspects of social influence in genetic testing decision‐making and to connect these with principlist and relational theories of autonomy. We interviewed 18 adult genetic counsellees without capacity issues considering predictive genetic testing for cancer predisposition for themselves and two counselling physicians in Switzerland. We conducted a qualitative analysis, (...) building on a grounded theory study about predictive genetic testing decision‐making. We found that some participants agreed to predictive genetic testing predominantly because relatives wanted them to do it, with some even acting contrary to their own convictions. Others, in contrast, based their decision on purely individualistic reasons but expressed difficulties in explaining their decision to their social environment. Healthcare professionals had a critical influence on decision‐making in many cases without being manipulative, as perceived by counsellees. Still, cases of coercion and social pressure occurred within social relationships. In conclusion, predictive genetic testing decision‐making includes relational and individualistic aspects, and both are compatible with autonomous decision‐making. While the principlist and relational notions of autonomy compete on a theoretical level, they are two sides of the same coin when used as analytical lenses for genetic testing decision‐making. Social acceptance of refusal of testing should be improved to mitigate social pressure. Individuals should be encouraged to decide for themselves how much their social environment influences their decision regarding predictive genetic testing. (shrink)

The Liverpool Care Pathway is an interdisciplinary protocol, aiming to ensure that dying patients receive dignified and individualized treatment and care at the end-of-life. LCP was originally developed in 1997 in the United Kingdom from a model of cancer care successfully established in hospices. It has since been introduced in many countries, including Norway. The method was withdrawn in the UK in 2013. This review investigates whether LCP has been adapted and validated for use in nursing homes and for dying (...) people with dementia. This systematic review is based on a systematic literature search of MEDLINE, CINAHL, EMBASE, and Web of Science. The search identified 12 studies, but none describing an evidence-based adaption of LCP to nursing home patients and people with dementia. No studies described the LCP implementation procedure, including strategies for discontinuation of medications, procedures for nutrition and hydration, or the testing of such procedures in nursing homes. No effect studies addressing the assessment and treatment of pain and symptoms that include dying nursing home patients and people with dementia are available. LCP has not been adapted to nursing home patients and people with dementia. Current evidence, i.e. studies investigating the validity and reliability in clinically relevant settings, is too limited for the LCP procedure to be recommended for the population at hand. There is a need to develop good practice in palliative medicine, Advance Care Planning, and disease-specific recommendations for people with dementia. (shrink)

This paper explores two main methods, employed for the analysis of regulatory law, empirical and systems-theoretical approaches. These two approaches are often portrayed in the literature as very different, mainly for two reasons. First, it is contended by some authors that systems-theoretical approaches—in contrast to empirical approaches—do not see a role for individuals in shaping social reality and regulatory law as one aspect of it. This paper, however, claims that systems-theoretical accounts do provide for human agency while empirical accounts have (...) limitations in their analysis of the role of ‘real individuals’ in shaping regulatory law. The paper argues that both empirical and systems-theoretical approaches need to provide a deeper analysis of the notion of ‘the individual’. This requires taking into account how ‘feeling rules’ influence the management of emotions. Secondly, empirical and systems-theoretical approaches are portrayed as distinct because they are considered as employing different methodologies. While systems theory involves speculative grand theory, empirical approaches are based on specific techniques of empirical data collection and analysis. This paper argues, however, that systems-theoretical ideas on how knowledge about the social world can be generated are close to qualitative empirical approaches, which stress the relativity and subjectivity of the meaning systems of the particular actors studied. Numerous studies argue that perceived group deprivation is a risk factor for radicalization and violent extremism. Yet, the vast majority of individuals, who experience such circumstances do not become radicalized. By utilizing models with several interacting risk and protective factors, the present analysis specifies this relationship more concretely. In a large United Kingdom nationally representative survey (n= 1,500), we examine the effects of group-based relative deprivation on violent extremist attitudes and violent extremist intentions, and we test whether this relationship is (...) contingent upon several individual differences in personality. The results show that stronger group-based injustices lead to increased support for and intentions to engage in violent extremism. However, some of the effects are much stronger for individuals who exhibit a stronger need for uniqueness and for status and who demonstrate higher levels of trait entitlement. Conversely, several effects are lessened for those individuals high in trait forgiveness, demonstrating a strong capacity for self-control and for those who are exerting critical as well as open-minded thinking styles, thus constituting buffering protective factors, which dampen the adverse effects of perceived group injustice on violent extremism. The results highlight the importance of considering (a) the interaction between individual dispositions and perceptions of contextual factors (b) the conditional and cumulative effects of various risk and protective factors and (c) the functional role of protective factors when risk factors are present. Collectively, these findings bring us one step closer to understanding who might be more vulnerable to violent extremism as well as how. Overall, the study suggests that preventing and countering violent extremism (P/CVE) programs must take account of the constellation of multiple factors that interact with (and sometimes enable or disable) one another and which can be targeted in preventions strategies. Goal-setting theory states that challenging, specific, and concrete goals (i.e., subordinate goals) are powerful motivators and boost performance in goal pursuit more than vague or abstract goals (i.e., superordinate goals). Goal-setting theory predominantly focuses on single, short-term goals and less on broad, long-term challenges. This review article extends goal-setting theory and argues that superordinate goals also fulfill a crucial role in motivating behavior, particularly when addressing broad, long-term challenges. The purpose of this article is to show theoretically that people pursue (...) long-term goals more successfully when they focus on subordinate and superordinate goals simultaneously than when they focus on either subordinate or superordinate goals alone. The term ‘locked-in’ syndrome (LIS) describes a medical condition in which persons concerned are severely paralyzed and at the same time fully conscious and awake. The resulting anarthria makes it impossible for these patients to naturally communicate, which results in diagnostic as well as serious practical and ethical problems. Therefore, developing alternative, muscle-independent communication means is of prime importance. Such communication means can be realized via brain–computer interfaces (BCIs) circumventing the muscular system by using brain signals associated with preserved cognitive, (...) sensory, and emotional brain functions. Primarily, BCIs based on electrophysiological measures have been developed and applied with remarkable success. Recently, also blood flow–based neuroimaging methods, such as functional magnetic resonance imaging (fMRI) and functional near-infrared spectroscopy (fNIRS), have been explored in this context. After reviewing recent literature on the development of especially hemodynamically based BCIs, we introduce a highly reliable and easy-to-apply communication procedure that enables untrained participants to motor-independently and relatively effortlessly answer multiple-choice questions based on intentionally generated single-trial fMRI signals that can be decoded online. Our technique takes advantage of the participants’ capability to voluntarily influence certain spatio-temporal aspects of the blood oxygenation level–dependent (BOLD) signal: source location (by using different mental tasks), signal onset and offset. We show that healthy participants are capable of hemodynamically encoding at least four distinct information units on a single-trial level without extensive pretraining and with little effort. Moreover, realtime data analysis based on simple multi-filter correlations allows for automated answer decoding with a high accuracy (94.9%) demonstrating the robustness of the presented method.. The differences between the "habits of the heart" in German and U.S.-American corporations can be described by analyzing the way corporations deal with norms and values within their organizations. Whereas many U.S. corporations have introduced formal business ethics programs, German companies are very reluctant to address normative questions publicly. This can be explained by the different cultural backgrounds in both countries. By defining these different "habits of the heart" underlying German and American business ethics it is possible to show the (...) problems and questions within the intercultural management of values, but also the possible solutions. In transparent orthographies, persistent reading fluency difficulties are a major cause of poor reading skills in primary school. The purpose of the present study was to investigate effects of a syllable-based reading intervention on word reading fluency and reading comprehension among German-speaking poor readers in Grade 4. The 16-session intervention was based on analyzing the syllabic structure of words to strengthen the mental representations of syllables and words that consist of these syllables. The training materials were designed using the 500 (...) most frequent syllables typically read by fourth graders. The 75 poor readers were randomly allocated to the treatment or the control group. Results indicate a significant and strong effect on the fluency of recognizing single words, whereas text-level reading comprehension was not significantly improved by the training. The specific treatment effect provides evidence that a short syllable-based approach works even in older poor readers at the end of primary school. (shrink)

School psychologists are asked to systematically evaluate the effects of their work to ensure quality standards. Given the different types of methods applied to different users of school psychology measuring the effects of school psychological services is a complex task. Thus, the focus of our scoping review was to systematically investigate the state of past research on the measurement of the effects of school psychological services published between 1998 and 2018 in eight major school psychological journals. Of the 5,048 peer-reviewed (...) articles published within this period, 623 were coded by two independent raters as explicitly refering to school psychology or counseling in the school context in their titles or abstracts. However, only 22 included definitions of effects of school psychological services or described outcomes used to evaluate school psychological services based on full text screening. These findings revealed that measurement of the effects of school psychological services has not been a focus of research despite its' relevance in guidelines of school psychological practice. Anti-materialist thought experiments as, e.g., zombie arguments, have posed some of the most vexing problems for materialist accounts of phenomenal consciousness. I doubt, however, that arguments of this kind can refute the core thesis of materialism. Although I do not question that there is something very special about an adequate explanation of phenomenal consciousness, and although I accept the epistemic irreducibility of phenomenal consciousness, I deny that modal arguments reach far enough to establish essentialism about consciousness. I will draw upon (...) a relativistic conception of modal space and suggest to strictly separate between varieties of metaphysical possibility - depending on which world is considered as actual, and depending on accessibility relations. It is shown that the modal argument cannot endanger the reductive explanation of the mind-brain relation if one distinguishes carefully between possibility according to primary intensions and possibility according to secondary intensions . Modal arguments are strong enough to make an epistemological point but not an ontological one. (shrink)
