Managed care has brought wide-ranging changes to the health care system. Some of these changes have been well publicized. Among them are the financial pressures that have resulted in numerous mergers of health care institutions, the restriction on the ability of patients to select their physician of choice, and the ever diminishing number of days that patients are permitted to stay in the hospital. Individual physicians, too, have been affected. For example, they are under pressure to see more patients per (...) unit time and to use fewer resources. Utilization review and payment methods may make them reluctant to undertake the full set of diagnostic tests and therapeutic interventions when they might otherwise have done so. (shrink)

In the contemporary ethical discourse, we constantly take recourse to human rights and dignity, and frequently rights are said to be founded on dignity. However, when we examine this concept, we begin to doubt that this recourse is adequate, because dignity manifests two features that are difficult to reconcile with liberal values on which our societies are established. First, and the appearances notwithstanding, dignity is not a universalist concept; and second, it possesses a perfectionist side. Historical aspects of the issue are also broached. Intuitions relative to self-organization can be found in the works of such key Western philosophical figures as Aristotle, Leibniz and Kant. Interacting with more recent authors and cybernetics, self-organization represents a notion in keeping with the modern world’s discovery of radical complexity. The themes of teleology and emergence are analyzed by philosophers of sciences with regards to the issues of modelization and scientific explanation. (publisher, edited).

Cette étude envisage les notions de sujet, d’individu et de personne dans une perspective d’anthropologie historique. La période considérée va de la disparition de l’Empire romain d’Occident à la mutation féodale, et son analyse est centrée sur la convergence de la culture savante – c’est-à-dire chrétienne – avec ce que nous pouvons savoir de la culture populaire à propos des trois notions précitées. Inaugurée par saint Augustin dans ses Confessions, l’émergence de la notion de personne coïncide avec l’oblitération du sujet (...) dans le De Trinitate de l’évêque d’Hippone. Cette conception rejoint celle du corps grotesque qui traverse la culture populaire de cette époque. Quant à l’individu, il n’apparaît durant toute la période considérée que sous la forme de l’individu particulier dans le cadre d’une individualité de types, et non sous celle de l’individu singulier propre à notre modernité.
