Prohibition on sex selection may well be unnecessary and oppressive as well as posing risks to women’s lives The urge to select children’s sex is not new. The Babylonian Talmud, a Jewish text completed towards the end of the fifth century of the Christian era, advises couples on means to favour the birth of either a male or a female child.1 The development of amniocentesis alerted the public in the mid-1970s to the scientific potential for prenatal determination of fetal sex,2 (...) and progressive decriminalisation of abortion afforded choice about continuation of pregnancy. The more recent emergence of preimplantation genetic diagnosis (PGD) obviates resort to abortion, and improved techniques of sperm sorting and diagnosis permit creation of zygotes that will ensure the sex of a future child. Growth of biomedical means to select the sex of future children has been accompanied by fear that such means will be employed to favour births of sons, and so perpetuate devaluation of girl children and women’s inferior family and social status. A reaction to this fear has been the demand for legal and medical professional prohibition of sex selection techniques. For instance, the Council of Europe’s Convention on Human Rights and Biomedicine3 provides in article 14 that: The use of techniques of medically assisted procreation shall not be allowed for the purpose of choosing a future child’s sex, except where serious hereditary sex-related disease is to be avoided. Legislation has been enacted in a number of countries, and proposed in others, to prohibit sex selection on non-medical grounds, such as the Prenatal Diagnostic Techniques (Regulation and Prevention of Misuse) Act, 1994 in India. There can be no doubt that the process of differentiation of the sciences today is going forward with rising intensity. This process is expressed in two ways: in the appearance of new sciences and disciplines outside those already existing, and as the further subdivision of existing sciences into narrower and more fragmented disciplinary branches.

For the past two decades, matters biological have been given constant attention in our press - not only in specialized publications of natural science but in those aiming at the very broadest range of readers. After the meeting of the Lenin Academy of Agricultural Sciences in 1948, biological questions were arbitrarily converted into problems of the most purely political nature, and the struggle against dissenters was waged in this field with no less bitterness and with essentially the same means and (...) techniques as in the field of politics. There was the same attaching of derogatory labels for holding nonconforming views, the same reprisals against spokesmen for independent trends in science, the same persecution of honest people whose sole "guilt" lay in their disagreement with the views of Academician Lysenko and his followers, who called themselves "Michurinists." It sufficed merely to name a scientist - not even in the field of biology - a Morgan-Weismann-Mendelian, for the need to support the accusations against him with scientific argumentation automatically to disappear, while the individual who had been accused of the deadly sin of Morgan-Weismann-Mendelianism was immediately and just as automatically deprived of the opportunity to defend his views, prove them, or offer facts or theoretical arguments of whatever nature in support of them. In those melancholy times, it sufficed to label an honest man an enemy of the people to do away with him once and for all. The external surfaces of the human body, as well as its internal organs, constantly experience different kinds of mechanical stimulations. For example, tubular epithelial cells of the kidney are continuously exposed to a variety of mechanical forces, such as fluid flow shear stress within the lumen of th nephron. The majority of epithelial cells along the nephron, except intercalated cells, possess a primary cilium, an organelle projecting from the cell's apical surface into the luminal space. Despite its discovery over 100 (...) years ago, the primary cilium's function continued to elude researchers for many decades. However, recent studies indicate that renal cilia have a sensory function. Studies on polycystic kidney disease (PKD) have identified many of the molecular players, which should help solve the mystery of how the renal cilium senses fluid flow. In this review, we will summarize the recent breakthroughs in PKD research and discuss the role(s) of th polycystin signaling complex in mediating mechanosensory function by the primary cilium of renal epithelium as well as of the embryonic node. The discovery of spectrum analysis occupies an important place on the long and thorny path of the advance of human knowledge toward the atoms, the stars, and the mastery of their laws. This great discovery was made one hundred years ago . The decisive role in this achievement belongs by right to two outstanding German scientists, Robert Bunsen and Gustav Kirchhoff. This discovery simultaneously opened the path into the atomic world, making it possible to judge the internal structure of atoms (...) from their optical properties, and into the world of heavenly bodies, the chemical composition of whose substances was first determined by means of spectrum analysis. All this led to an unprecedented expansion of the field of scientific knowledge both in the direction of the investigation of microscopic objects in nature and in that of the study of macro-objects. But the most remarkable thing was that in their development both these trends in research proved to be interrelated. The penetration of science into the field of the macro-cosmos was a direct consequence of its more profound penetration into the field of the micro-cosmos. In a word, this was one of the boldest of scientific achievements, one that enabled human reason to penetrate into the depths of the substance of the earth and the cosmos, to know its properties, its composition, and its laws. The study of the problem of religious relations is of particular interest, since upon the annexation to Russia, during the years 1794-1796, and in some places until 1798, the almost complete elimination of the Uniate Church in Right-Bank Ukraine took place. This issue has been covered extensively in pre-revolutionary historiography, including at the local level. Thus, the history of the existence and liquidation of the Uniate Church in Podillya became the leading topic of research of the "Podolsky Diocesan Historical and (...) Statistical Committee". Another printed organ, where the church-historical information about Podolsk parishes, collected by priests, became Podilskie Eparchial Gazettes. The main idea behind the content of these studies was that Podillya was considered as originally Russian possession, and Orthodoxy was the only possible denomination of the land. (shrink)

The discoveries of Copernicus, Lavoisier, Darwin, and many others have opened such periods of change in natural science. In the UK, over 70% of AIDS, including new cases, is located in a few Districts in central London where the distribution of previously occurring and new cases is essentially confined to the original risk groups of homosexual/bisexual men, drug addicts of both sexes, and some of their sexual partners and consorts. But control policy is still based on the assumption that HIV has already spread from persons in these risk groups into the general population, and that it will spread (...) hereafter at an increased rate because of heterosexual transmission to cause a widespread epidemic of AIDS.The basis and implications of this policy were investigated in the South East region adjacent to London, Analysis of demographic and epidemiological data shows that, with one exception, there is very little extension of AIDS from affected Districts in London to the surrounding region or even to the suburban fringe. Where AIDS is prevalent, as in this exception, the distribution follows the same, original pattern which relates essentially to risks arising from life-styles. There is no significant association between the prevalence or spread of AIDS and conventional markers of deprivation, economic and social disadvantage.Allocations of personnel, services and expenditure, assessed from returns required under the AIDS Control Act of 1987 and official registration data, continue to follow the original policy assumptions. They are therefore unrelated to the numbers of existing and new cases, disproportionate and unrealistic. Claims that this widespread excess of effort is justified by the fact that AIDS has not spread to the general population are falsified by the continuation of cases almost exclusively in risk groups. There is nothing in the data required under the Act or in registrations in this main locus of AIDS in the UK to suggest any change in this or to justify continuation of current expenditure and redundant activities.There is no evidence in these data that ethnic variations in the resident populations of Districts are associated with variations in the prevalence of AIDS. However, the larger figures available in some of the national data do indicate a disproportionate increase in some minority ethnic groups. Further detail about risks factors in these groups are required and, meanwhile, effort and expenditure should be re-orientated toward treatment, contact tracing and other public health measures for more effective containment of the continuing spread of AIDS in all the high risk groups. A case study of Chinese grandparents’ transnational caregiving experiences in Canada highlights two issues that have received limited attention in the broader feminist care literature: (1) elderly persons are usually positioned as receivers rather than providers of care; and (2) transnational care studies focus on women migrating as part of “global care chains,” rather than on elderly family members migrating to meet the caring needs of adult kin who work in market economies that do not recognize caring responsibilities. Aim: Patients with advanced cancer need information about end-of-life treatment options in order to make informed decisions. Clinicians vary in the frequency with which they initiate these discussions.Patients and methods: As part of a long-term longitudinal study, patients with an expected 2-year survival of less than 50% who had advanced gastrointestinal or lung cancer or amyotrophic lateral sclerosis were interviewed. Each patient’s medical record was reviewed at enrollment and at 3 months for evidence of the discussion of patient wishes concerning (...) ventilator support, artificial nutrition and hydration , resuscitation and hospice care. A Kaplan–Meier analysis was also performed and 2-year survival calculated.Results: 60 cancer and 32 ALS patients were enrolled. ALS patients were more likely than cancer patients to have evidence of discussion about their wishes for ventilator support , ANH , DNR and hospice care . At 6 months, 91% of ALS patients were alive compared with 62% of cancer patients; at 2 years, 63% of ALS patients were alive compared with 23% of cancer patients .Conclusions: Cancer patients were less likely than ALS patients to have had documented advanced care planning discussions despite worse survival. This may reflect perceptions that ALS has a more predictable course, that advanced cancer has a greater number of treatment options, or differing views about hope. Nevertheless, cancer patients may be less adequately prepared for end-of-life decision-making. (shrink)

IntroductionBisphenol A is highly debated and studied in relation to a variety of health outcomes. This large variation in the literature makes BPA a topic that is prone to selective use of literature, in order to underpin one’s own findings and opinion. Over time, selective use of literature, by means of citations, can lead to a skewed knowledge development and a biased scientific consensus. In this study, we assess which factors drive citation and whether this results in the overrepresentation of (...) harmful health effects of BPA.MethodsA citation network analysis was performed to test various determinants of citation. A systematic search identified all relevant publications on the human health effect of BPA. Data were extracted on potential determinants of selective citation, such as study outcome, study design, sample size, journal impact factor, authority of the author, self-citation, and funding source. We applied random effect logistic regression to assess whether these determinants influence the likelihood of citation.ResultsOne hundred sixty-nine publications on BPA were identified, with 12,432 potential citation pathways of which 808 citations occurred. The network consisted of 63 cross-sectional studies, 34 cohort studies, 29 case-control studies, 35 narrative reviews, and 8 systematic reviews. Positive studies have a 1.5 times greater chance of being cited compared to negative studies. Additionally, the authority of the author and self-citation are consistently found to be positively associated with the likelihood of being cited. Overall, the network seems to be highly influenced by two highly cited publications, whereas 60 out of 169 publications received no citations.ConclusionIn the literature on BPA, citation is mostly driven by positive study outcome and author-related factors, such as high authority within the network. Interpreting the impact of these factors and the big influence of a few highly cited publications, it can be questioned to which extent the knowledge development in human literature on BPA is actually evidence-based.
