This predominance makes it difficult for healthcare professionals to deal with the ethical dilemmas that emerge in this subspecialty. This paper is a first endeavor to empirically investigate the axiological foundations of the healthcare professionals in a cardiology hospital. Our pilot study selected, as the target population, cardiology personnel not only because of their difficult ethical deliberations but also because of the stringent conditions in which they (...) have to make them. Therefore, there is an urgent need to reconsider clinical ethics and Value-Based Medicine. This study proposes a qualitative analysis of the values and the virtues of healthcare professionals in a cardiology hospital in order to establish how the former impact upon the medical and ethical decisions made by the latter.ResultsWe point out the need for strengthening the roles of healthcare personnel as educators and guidance counselors in order to meet the ends of medicine, as well as the need for an ethical discernment that is compatible with our results, namely, that the ethical values developed by healthcare professionals stem from their life history as well as their professional education.ConclusionWe establish the kind of actions, communication skills and empathy that are required to build a stronger patient-healthcare professional relationship, which at the same time improves prognosis, treatment efficiency and therapeutic adhesion. ¿QUÉ HAY DE NUEVO? LITERATURA E INNOVACIÓN EN LOS '90: CÉSAR AIRA.

Higher education, a key driver of women’s empowerment, is still segregated by gender across the world. Agricultural higher education is a field that is male-dominated, even though internationally women play a large role in agricultural production. The purpose of this study was to understand the experience, including challenges and coping strategies, of women from 10 Latin American countries attending an agricultural university in Latin America. The participants were 28 women students with a mean age of 20.9 ± 1.8 years. Following (...) informed consent and assurance of confidentiality, four focus group sessions (one for each year of study with a mean duration of 81 min) were conducted in Spanish. The central question was, “what has been your experience at the university?” Sessions were recorded and transcribed. Thematic coding was performed independently by two teams of researchers (from Latin America and North America), with the resulting schemas combined through mutual discussion. Member checking, auditing, and reflexivity contributed to trustworthiness of the process. Students reported that the personal qualities needed for success included determination, persistence, and self-efficacy. Many described an empowerment process, including increased discipline and self-efficacy from the first to fourth year of study. University life encompassed six themes: university structure and discipline (part of the exosystem), two supportive microsystems (friends and classmates and institutional support) as well as three challenges (academics, peers, and machismo). Cultural influences instantiated in students’ daily experiences included familism, machismo, and religious faith. Students anticipated futures involving further education and contributions to society. We conclude that higher education in agriculture can serve as an effective means of empowering women to feed the world. (shrink)

Airway mucin secretion is important pathophysiologically and as a model of polarized epithelial regulated exocytosis. We find the trafficking protein, SNAP23, selectively expressed in secretory cells compared with ciliated and basal cells of airway epithelium by immunohistochemistry and FACS, suggesting that SNAP23 functions in regulated but not constitutive epithelial secretion. Heterozygous SNAP23 deletant mutant mice show spontaneous accumulation of intracellular mucin, indicating a defect in baseline secretion. However mucins are released from perfused tracheas of mutant and wild-type mice at the (...) same rate, suggesting that increased intracellular stores balance reduced release efficiency to yield a fully compensated baseline steady state. In contrast, acute stimulated release of intracellular mucin from mutant mice is impaired whether measured by a static imaging assay 5 min after exposure to the secretagogue ATP or by kinetic analysis of mucins released from perfused tracheas during the first 10 min of ATP exposure. Together, these data indicate that increased intracellular stores cannot fully compensate for the defect in release efficiency during intense stimulation. The lungs of mutant mice develop normally and clear bacteria and instilled polystyrene beads comparable to WT mice, consistent with these functions depending on baseline secretion that is fully compensated. I discuss the two different responses from the angiosperms to the specific molecular mechanisms of the tumor-inducing agent contained in the bacteriumAgrobacterium tumefaciens. This is done in terms of the collective variables for expressing genetic response to a continuously varying supply of energy from metabolic pathways. We are led to the conjecture that the expression of the recessive oncogenes may not be restricted to humans (retinoblastoma and osteosarcoma), but may also occur in plants (crown gall), and be expressed through a (...) heat-shock. We formulate the following hypothesis: Life's origin may have occurred during the lower Archaean at a time when the environmental temperature was higher than it is at present. Preliminary consequences of this hypothesis are studied from the point of view of molecular evolution. We restrict our attention to implications regarding the genetic code. We conclude that alternative assignment of termination codons may be understood in terms of: (a) the elevated temperatures to which the progenote may initially have been exposed; and (...) (b) the subsequent response of its genome to the opportunity provided by the eventual loss of hyperthermal genetic expression during a thermal transition (TT) period, which was triggered off by the evolution of the dynamic Earth. Two aspects of the chromatin repeat length (r t) are discussed: (i) Why is r t, longer for slowly dividing cells than in rapidly dividing cells?, and (ii) Why is the temporal evolution of r ta decreasing function of time (t) in mammalian cortical neurons, whereas it is an increasing function of t for granule cells around the time of birth? Possible etiological factors of congenital malformations as well as of human trisomies are considered in the framework of the repressor hypothesis. In this approach gene expression is envisaged from the point of view of the functional variations of the total activation energy for normal gene expression in homeostatic equilibrium. We restrict our attention to variations of the total activation energy under the effect of temperature gradients. We discuss the evidence that hyperthermia may be an etiological factor for trisomies in humans.

I entered the room where one of the residents was seeing a patient that we will call Patty, a 32-year-old long-standing epilepsy patient. Patty was sitting in the chair with her husband by her side. She started screaming at me, complaining that she was tired of feeling sick and unable to do anything. (...) She had been in and out of emergency rooms with a variety of complaints, from passing out to dizziness with nausea and vomiting. She was visibly angry and started cursing me with direct expletives and blaming me for all her ailments.Her husband was quiet and, at times, would cover his face as if he was embarrassed. I felt an initial burst of anger. I felt my face getting red. I made a great effort to stay quiet, leaned on the exam table facing them, lowered my voice, and braced to take the barrage of insults that were coming my way. I had to make a strong conscious effort to control my emotions and let her vent. I felt that if I responded in kind, I would inflame the situation. Moreover, a resident and a student working with me that day were also present in the room, and I felt it was my duty to behave professionally, so I sat and tried to do active listening with the idea of offering solutions to her complaints. I am not sure how long the event lasted, but it seemed like an eternity. As she slowed down, I apologized for making her feel not cared for, repeated some of her complaints, and tried to offer solutions to them, but she would not have it and did not accept anything I said. She wanted to get her prescriptions and leave the clinic. I offered to have one of my colleagues do the follow-up as she really did not want to see me anymore.Patty is one of those patients that when you see the name in the schedule, you cringe and tell yourself, it is going to be a long clinic day. She could be in the group of patients who are considered very difficult. Before me, she had been seen by several other neurologists. They likely also preferred not to see her on the schedule because she is one of the 20–30% of patients that have a type of functional psychogenic disorder that becomes more complicated because of the coexistence of epileptic seizures along with non-epileptic psychogenic events. The latter is a more problematic event because it does not respond to pharmacological treatment and causes a lot of disability. The best way to establish whether an event is a true epileptic event or a non-epileptic psychogenic event is to do continuous electroencephalographic monitoring to capture as many events as possible on video. I tried different combinations of medications to no avail and with significant side effects. I tried to set her up for electroencephalogram home monitoring to tease the epileptic and psychogenic events apart. In truth, her social circumstances, insurance coverage, and economic situation made it impossible to do so. Every time she came to the clinic, she had a multitude of complaints, and she would ask for a medication to treat the problem. Any mention of the potential psychogenic nature of her problems would result in a defensive attitude "so you are saying I am crazy." We discussed multiple times the need to be careful with the medications and the potential bad side effects. I wanted to help her, but with every visit, there seemed to be a barrier, and the visit would end on bad terms. At least, that is how I felt. I was left every time, with a feeling of frustration. I believe that she left the clinic feeling the same. That day, I left the clinic upset and depressed. I tried to convince myself I had done all I could to help her. I just could not understand why this was the result.Several years have passed since I have seen Patty in the clinic. She certainly has not... (shrink)

Due to impaired communication, people with aphasia are often in a vulnerable situation and face barriers in accessing health information. This article discusses the contributions ofaugmentative and alternative communication for people with aphasia in optimizing communication, improving language recovery, and mainly in providing education and increasing access to healthinformation. This can be translated into a positive impact on respect for autonomy right, well-being, quality of life, and health outcomes (further participation in the decision-making process, involvement,independence, and control of the rehabilitation (...) process). Health professionals, including nurses, must have communication skills to communicate effectively with this population and also be prepared to useaugmentative and alternative communication strategies. A leitura pode ser analisada sob vários ângulos: social, pedagógico, cultural, cognitivo, dentre outros. Contudo, abordar os aspectos mencionados, em conjunto ou sequencialmente, demanda uma verticalização prodigiosa da pesquisa; por isso este artigo busca apenas apresentar e discutir alguns aspectos cognitivos relevantes para a prática leitora, dando ênfase à memória e, por isso mesmo, ao conhecimento prévio. Acreditamos que, conhecendo melhor os tipos de memória e não ignorando o vínculo necessário entre memória e conhecimento prévio, se torne mais viável para (...) o professor mobilizar meios eficazes para propor atividades de leitura compreensiva e, também, mais importante ainda, para mostrar e explicar aos alunos como fazer uma leitura desse tipo. A concepção subjacente à presente escolha decorre do entendimento de que a leitura é um processo sociocognitivo complexo não redutível a seu resultado imediato, obtido em testes padronizados e avaliado em termos positivos ou negativos, de modo negáabsoluto. Na verdade, não nos interessa exclusivamente conferir ao leitor a condição de leitor proficiente ou não, desconhecendo as práticas leitoras com as quais o sujeito testado tem ou teve contato até o momento. O que nos instiga e motiva, de fato, é o ensino-aprendizagem da compreensão do texto escrito. Assim, abordaremos ao longo deste artigo questões atinentes ao processamento cognitivo de textos, com a finalidade de elucidar a sua importância decisiva para o ensino-aprendizagem da habilidade de compreensão leitora, no contexto escolar. Mostraremos que Mill si bien conoció las objeciones de Hume, no se refiere a ellas en gran medida porque para él la justificación de la inducción es posible. La hipótesis principal será probada por medio de dos tesis: la primera, alude a la evasión de la argumentación circular respecto de la justificación de la inferencia inductiva; y la segunda, refiere (...) al tipo de inducciones que fundamentan el principio de regularidad: inducciones por enumeración simple. Los servidores públicos no siempre actúan en función del interés de la comunidad a pesar de las amenazas y sanciones establecidas en el derecho; es necesario encontrar nuevas vías de actuación para resolver tan importante problema. En ese sentido, este artículo busca fundamentar los principios que deben regir la actuación de los mismos. Para ello, en primer lugar, se intenta establecer un método que permita la creación de estos principios, basándose en la teoría del valor de Luis Villoro y en (...) la ética de Immanuel Kant. Posteriormente, se identifica el interés público como aquel valor que se busca por él mismo sin necesidad de acudir a otros valores para cuyo fin esté dispuesto. A continuación, se derivan los valores de responsabilidad, objetividad, imparcialidad, razonabilidad, publicidad, solidaridad, prudencia e instrumentalidad, a partir de los cuáles se construyen principios éticos que puedan, posteriormente, ser positivizados en el derecho. Este estudo tem como objetivo analisar, de forma crítico-reflexiva, os processos que se mostram implicados na formação do professor, tendo em vista a materialização de práticas educativas inclusiva-anticapacitistas no contexto da EPT. Para essa finalidade, a metodologia empregada subsidiou-se em uma abordagem qualitativa; recorrendo, para tanto, a pesquisa bibliográfica, por meio de estudos e produções de autores que tratam sobre a educação especial inclusiva e a formação de professores na EPT, sempre de modo a articular a discussão destas temáticas com (...) o capacitismo. Como resultado da análise realizada, compreende-se a necessidade de instituir políticas públicas voltadas à formação docente, numa perspectiva anticapacitista na EPT, a fim de ressignificar a condição da deficiência, a partir de um enfoque despatologizante do indivíduo afetado pela lesão. Those who favor and those who oppose the interruption of pregnancy with anencephalic fetuses answer the question ‘what is the right to life?’ differently. Those in favor argue that life exists only when it is ‘viable’; that is to say, when cerebral activities occur or may occur. Those who oppose it argue that it is not possible to describe ‘life’ as residing in a particular quality, since life ‘exists from conception’. In fact, in both cases, the noun ‘life’ is being (...) defined by a particular quality, either as ‘viable’ or as ‘existing from the time of conception’. Also, simply saying that ‘there is life’ cannot count as a neutral answer since those who utter such a sentence employ an unspecified criterion to establish if there is life or not. There are two possible ways to investigate this controversial matter: either we look for a definition of ‘life’ which is neutral and objective and does not reside in a particular quality or we try to establish whether or not the search for a neutral point of view can lead to a satisfactory answer. In this article we explore the argument against the interruption of pregnancy – as defined above – in order to show 1) the impossibility of establishing a neutral point of view regarding knowledge; 2) the existence of a psychological motivation which justifies the longing for an absolute criterion for the evaluation of human actions. This psychological motivation is analyzed from a Nietzschean perspective. (shrink)

A reivindicação da cidadania religiosa de pessoas LGBTQIAPN+ dentro das instituições religiosas tem gerado grande sofrimento a quem se entende como parte de uma tradição religiosa historicamente hegemônica. Tais situações configuram formas de violência religiosa que ultrapassam o campo de instituições específicas e se manifestam na cultura como parte do que se tem chamado de LGBTfobia. É no contexto desses enfrentamentos que se situam aquilo que se está se propondo chamar “martírio_ queer”_. Nessa reflexão particular, o contexto privilegiado para a (...) identificação das fontes e análise do tema em discussão é a emergência e desenvolvimento do que se convencionou chamar de “Igrejas inclusivas” no contexto estadunidense. Como exemplo de mártires queer, serão apresentadas as trajetórias de duas personagens fundamentais para a história das “Igrejas inclusivas” no mundo – a transativista Sylvia Rivera e o Reverendo Troy Perry, das _Metropolitan Community Churches_. With the expansion of artificial intelligence and advanced technologies, theworld in the 21st century is rapidly changing and imposing new living dynamics. Althoughsuch changes affect all age groups, younger generations accept them faster andreact more positively. The new cohorts - Generation Z and Alpha - live in a digital worldthat affect their lifestyle, interpersonal relations, quality of mental health, psychologicalwell-being and everyday challenges. The presence of the so called “Frankenstein effect”in some adults provoked by the fast development of artificial intelligence (...) and robotics,reflects a “humans versus machines” position, viewing artificial intelligence as athreat to humanity. However, the reality is that digital and human world are not inconflict, since many people are already using artificial intelligence tools on daily basis.It is implemented in certain aspects of medicine, education, business, law, agriculture,industry, space technology and in many other fields. With this in mind, the aspect ofmorality pops up as a very important one. A frequently asked question is: Does artificialintelligence have the capacity to make moral decisions independently? Therefore, integratingmorality into AI algorithms is one of the priorities that interdisciplinary teamsfrom engineering and computer sciences, psychology, philosophy, sociology, law, etc.are intensively working on. This paper addresses this issue by presenting findings fromrelevant research which discusses the challenges and possibilities for integrating thedimensions of morality and introducing human values into autonomous systems thatperform complex tasks. Introducción: Los docentes universitarios requieren nuevos saberes para enfrentar los diversos retos que plantea la educación superior; dichos desafíos se relacionan con el dominio de saberes disciplinarios, profesionales, laborales, pedagógicos y didácticos e incluso axiológicos. Lo que aquí se presenta son los resultados de una investigación llevada a cabo en la Universidad Autónoma del Estado de México en el periodo de enero de 2014 a abril de 2015. El objetivo: fue determinar los saberes específicos, métodos de enseñanza y valores sociales (...) que deben poseer los profesores universitarios desde la perspectiva de los estudiantes para la identificación de aquellos principios que deben orientar la formación y actualización docente. Método: consideró el cualitativo y la técnica de la encuesta; fueron aplicados 119 cuestionarios diseñados en Escala de Likert, a igual número de estudiantes; el instrumento de 48 reactivos fue construido con base en tres grandes "baterías" o ejes: "Saberes de un buen docente", "Métodos o estrategias de enseñanza", y "Actitudes y valores". Resultados: se resaltan los saberes pedagógicos y didácticos como aquéllos que los estudiantes visualizan que les hacen falta a sus docentes; de éstos, son los métodos de enseñanza eminentemente prácticos, y aquellos vivenciales o más cercanos a la realidad los que se consideran como importantes. En términos de las actitudes y valores sociales que deben tener y demostrar los profesores, se encuentran la actitud profesional, la actitud de servicio, así como la responsabilidad y la ética. Conclusiones: se discute aquello que se considera deben saber, y lo que necesitan mejorar los docentes universitarios. Introduction: Professors require new knowledge to face the different challenges that higher education has. Those challenges are related with the mastery of disciplinary, professional, pedagogic, work-related, didactic and even axiological knowledge. The results of a research that was carried out at the Universidad Autónoma del Estado de México from January, 2014 to April, 2015; are presented in this article. Objective: To determine specific knowledge, teaching methods and social values that professors must have from the perspective of the students for identifying the principles which should lead teaching formation and updating. Method: The qualitative method and the survey as a procedure were considered. One hundred nineteen questionnaires that were designed in Likert scale were also applied to the same number of students; the instrument of 48 reagents was based on 3 axes: 'Knowledge of a good professor', 'Teaching methods or strategies' and 'Attitudes and values'. Results: Pedagogic and didactic knowledge is emphasized, for instance the one students consider their professors should have. However, the professors consider those teaching methods essentially practical, based on past experiences or closer to the reality as the most important ones. There are some attitudes and social values that professors must have and demonstrate, as well as the professional attitude, the service attitude, responsibility and ethics. Conclusions: the ideas that must be known and improved by the professors are discussed here. (shrink)
