I was tired and close to the end of an (...) overnight shift and thought he would be a quick turnaround, likely discharged, since rashes are typically benign. I walked over to his stretcher after a quick chart biopsy that highlighted his mildly increased heart [End Page 24] rate on arrival as well as his history of polysubstance use and homelessness."Hey doc, I don't feel so good.""I'm sorry to hear that. What's specifically bothering you?""I am itching all over with these bumps. My skin is red, and I also think I'm withdrawing since I have body aches and chills."The interview continued.I discovered his last heroin use was approximately eight hours prior to arrival. I thought he was an ideal candidate for buprenorphine since he was actively withdrawing. I was recently licensed to prescribe this medication and excited to use my license for the first time. He declined the suggestion, however, preferring methadone instead. Buprenorphine can precipitate withdrawal if not given long enough after opioid use or if an incorrect dose is given; thus, many patients are hesitant to take it. He also stated he was predominantly here for his rash and did not want rehabilitation placement. I ordered our initiation methadone dose as well as a screening COVID-19 test and a urine drug screen test, which are both needed for rehabilitation placement in case he changed his mind.His rash was highly suspicious for scabies. He was covered with red bumps that spared only his face, some in linear distributions, and was actively scratching his arms and legs during the interview. I prefer oral ivermectin over topical permethrin for the homeless population simply for ease of administration, lower cost, and increased compliance. He received ivermectin for his scabies and knew he could return to the emergency department for a repeat dose if he was unable to fill a prescription.What happened next is, in my experience, an all-too-common scenario.The patient told me that the methadone is "doing nothing." I explained he could no longer take buprenorphine since he already received methadone, which only made him more frustrated. Unfortunately, I am restricted with the amount of methadone I can give in the emergency department, largely due to the drug's long half-life, and thus I was unable to give him more, let alone attempt titrating to his tolerance.Opioid withdrawal is not a life-threatening diagnosis; these patients are not typically admitted to the hospital. The management consists of initiating either methadone or buprenorphine, symptomatic treatment and then outpatient referral or rehabilitation placement. I explained the options to him, and he requested to be discharged since he was deferring rehabilitation. He was discharged. I was able to treat his rash but his chronic illness, opioid use disorder, was not controlled.I discharge a patient in opioid withdrawal or who has opioid use disorder almost every day, and I think most emergency medicine physicians would echo that statement. The disposition is not wrong from the perspective of the individual physician, but it feels like the system has failed these patients when I think of the frequency that they return and their baffling risk of overdose. The latter can be reduced by giving patients Narcan, the drug to reverse opioid overdose, upon discharge but this is just placing a bandage over the problem and not all emergency departments can fund this risk-reduction tool.I saw my patient's COVID-19 result return as I wrapped up my shift. It was positive. And, of course, he was in our department's hallway the whole time, only intermittently using his mask. This book presents an exploration of the idea of the common or social good, extended so that alternatives with different populations can be ranked. The approach is, in the main, welfarist, basing rankings on the well-being, broadly conceived, of those who are alive. The axiomatic method is employed, and topics investigated include: the measurement of individual well-being, social attitudes toward inequality of well-being, the main classes of population principles, principles that provide incomplete rankings, principles that rank uncertain alternatives, best choices (...) from feasible sets, and applications. The chapters are divided, with mathematical arguments confined to the second part. The first part is intended to make the arguments accessible to a more general readership. This article examines several families of population principles in the light of a set of axioms. In addition to the critical-level utilitarian, number-sensitive critical-level utilitarian, and number-dampened utilitarian families and their generalized counterparts, we consider the restricted number-dampened family and introduce two new ones: the restricted critical-level and restricted number-dependent critical-level families. Subsets of the restricted families have non-negative critical levels, avoid the `repugnant conclusion' and satisfy the axiom priority for lives worth living, but violate an important independence condition.

In reflection concerning the study of literature, both categories, memory and forgiveness, are quite frequently contrasted. Literature, which activates memory, is perceived as a tool of settling the past. In this perspective, it takes the position opposite to that of forgiveness, which requires that 'one does not seek redress'. Using the example of the film bearing the title of The Tale, the author attempts to consider the conflict between a writer's duty to remember and the Christian appeal for forgiving. If (...) literature is the reservoir of memory, the very act of writing is purifying, and involves leaving the limits of own existence towards another human being, namely a reader. Reaching beyond the limits of own ego is not unlike the attitude of forgiveness, which requires reaching beyond the borders of personal pain. (shrink)

This article explores the critical history and reception of Charles Churchill, the mid-eighteenth century satirist, looking in particular at his skepticism towards all authority as a questioning of traditional ideas of `enlightenment', and at the reasons for the meteoric but short-lived nature of his poetic success.

Like most realistically oriented philosophers, I accept that the growth of explanatory power of cognitive system is a sign of the cognitive progress. In other words, I assumecertain version of the principle of „inference to the best explanation”. In the paper I propose a unified and more sophisticated criterion for the „betterness” of explanation, which is formulated in van Fraassen's terminology.

This two-volume critical edition of the Republic was another major contribution to the field. Though his preface claims 'an editor cannot pretend to have exhausted its significance by means of a commentary,' Adam's depth of knowledge and erudite analysis of the (...) Greek text ensured that his edition remained the standard reference for decades to follow, and it remains a thought-provoking evaluation of one of the great works of Western thought. This paper extends a notion of local grammars in formal language theory to autosegmental representations, in order to develop a sufficiently expressive yet computationally restrictive theory of well-formedness in natural language tone patterns. More specifically, it shows how to define a class ASL\ of stringsets using local grammars over autosegmental representations and a mapping g from strings to autosegmental structures. It then defines a particular class ASL\ using autosegmental representations specific to tone and compares its expressivity to established formal language (...) grammars that have been successfully applied to other areas of phonology. The principle of contradiction, or non-contradiction, is traditionally included as one of the three fundamental principles of logic, together with the principle of identity and the principle of excluded middle. There is a consensus now regarding the shape of the principle of contradiction in modern formal logic. However, a deeper look at the history of its formulation reveals a much more complicated picture. We trace some of such developments from the beginning of the twentieth century when all sorts of formalisms (...) were proposed, and even the name itself was up for debate. This paper offers a novel interpretation of the nature and role of logical argumentation in Aristotle’s natural philosophy. In contrast to the standard domain interpretation, which makes logikôs argumentation the contrary of phusikôs, relying on principles drawn from outside the domain of natural science, I propose that the essential or defining feature of logikôs argumentation is the use of principles that are general relative to the question under investigation. Much of contemporary metaethics revolves around the issue of “naturalism.” However, there is little agreement on what “naturalism” is or why it should be of significance. In this paper, I aim to rectify this situation by providing a set of necessary conditions on what positions ought to count as “naturalistic.” A metaethical view should count as an instance of naturalism only if it claims that there can be evidence for normative claims that is both public and spatiotemporal. I argue that, (...) unlike other characterizations of “naturalism,” this view shows a clear difference between many metaethical positions and the sciences. The view thereby renders debates about naturalism philosophically significant: the division between naturalists and non‐naturalists is that between philosophers who hold that ethics is relevantly similar to the sciences and those who deny this.
