This book is a study of the second-edition version of the 'Transcendental Deduction', which is one of the most important and obscure sections of Kant's Critique of Pure Reason. By way of a close analysis of the B-Deduction, Adam Dickerson makes the distinctive claim that the Deduction is crucially concerned with the problem of making intelligible the unity possessed by complex representations - a problem that is the representationalist parallel of the semantic problem of the unity of the proposition. There is growing interest in clinical ethics. However, we still have sparse knowledge about what is actually going on in the everyday practice of clinical ethics consultations. This paper introduces a descriptive evaluation tool to present, discuss and compare how clinical ethics case consultations are actually carried out. The tool does not aim to define ‘best practice’. Rather, it facilitates concrete comparisons and evaluative discussions of the role, function, procedures and ideals inherent in clinical ethics case consultation practices. The tool (...) was developed during meetings of the European Clinical Ethics Network. Based on written reports and participation in the network meetings, the development and the content of the tool and the results of its application in presenting and discussing 10 case consultations are summarized. The tool facilitated understanding of the details of clinical ethics case consultations across individuals and institutions with various experiences and cultures, and comparison between various practices. Psychoactive drugs are being prescribed to millions of Americans at an increasing rate. In many cases these drugs are necessary in order to overcome debilitating emotional problems. Yet in other instances, these drugs are used to supplant, not supplement, interpersonal therapy. The process of overcoming emotional obstacles by introspection and the attainment of self knowledge is gradually being eroded via the gratuitous use of psychoactive medication in order to rapidly attain a release from the common problems that life inevitably presents (...) us with. In this paper, I argue that Kant’s formula of humanity, which maintains that persons ought never to treat others or themselves soley as a means to an end, proscribes this. Moreover, Kant argues that we have an imperfect duty of self development, and I argue that we fail to adhere to such a duty whenever we seek to evade the process of introspection and self knowledge in favour of the expedient results that drugs may provide us with as we attempt to overcome the emotional hurdles in our lives. This paper describes the perspectives of stakeholders within the North American dairy industry on key issues affecting the welfare of dairy cattle. Five heterogeneous focus groups were held during a dairy cattle welfare meeting in Guelph, Canada in October 2012. Each group contained between 7 and 10 participants and consisted of a mix of dairy producers, veterinarians, academics, students, and dairy industry specialists. The 1-h facilitated discussions were audio-recorded and transcribed verbatim. Content analysis of the resulting transcripts showed that participants (...) across all stakeholder categories identified lameness as the most important welfare issue facing dairy cattle. Other prominent issues of concern included cow comfort, disease, on-farm mortality, stockmanship, painful procedures, injuries, cull cow management, calf management, and restriction of behavioural freedoms. Participants typically gave several reasons for why they considered issues problematic. Underlying reasons were grouped according to animal-centered concerns [at both the individual and at the herd level ] and industry-centered concerns . This analysis identified areas of shared concern among diverse stakeholder groups, which should aid in the development of standards and policies that satisfy stakeholders within and external to the dairy industry. BackgroundThis study aimed to determine what, how, and under what circumstances individual-focused interventions improve well-being and decrease burnout for critical care healthcare professionals.MethodThis realist approach, expert opinion interview, was guided by the Realist And Meta-narrative Evidence Synthesis: Evolving Standards II guidelines. Semi-structured interviews with critical care experts were conducted to ascertain current and nuanced information on a set of pre-defined individual interventions summarized from a previous umbrella review. The data were appraised, and relationships between context, mechanisms, and outcomes were extracted, (...) which created theory prepositions that refined the initial program theory.ResultsA total of 21 critical care experts were individually interviewed. By understanding the complex interplay between organizational and personal factors that influenced intervention uptake, it was possible to decipher the most likely implementable intervention for critical care healthcare professionals. The expert recommendation suggested that interventions should be evidence-based, accessible, inclusive, and collaborative, and promote knowledge and skill development. Unique mechanisms were also required to achieve the positive effects of the intervention due to the presence of contextual factors within critical care settings. Mechanisms identified in this study included the facilitation of self-awareness, self-regulation, autonomy, collaboration, acceptance, and inclusion.ConclusionThis validation of a theoretical understanding of intervention that addressed well-being and burnout in critical care healthcare professionals by expert opinion demonstrated essential mechanisms and contextual factors to consider when designing and implementing interventions. Future research would benefit by piloting individual interventions and integrating these new theoretical findings to understand better their effectiveness for future translation into the “real-world” setting. During the past 2 decades, business has become increasingly active in the political process, and scholars continue to debate the extent to which this activity is organized. This fundamental issue is addressed by using multidimensional scaling to structurally analyze political action committee (PAC) campaign contributions within the context of resource dependence and class cohesion theories. Results indicate that resource dependence theory can better explain the forces that drive business participation in the U.S. public policy process. El artículo pone de manifiesto, en primer lugar, la escasa bibliografía que existe en español sobre el tema de la ecología en las obras y el pensamiento de san Agustín. Posteriormente, señala los tres peligros que existen al abordar este tema, a saber, el del anacronismo, el de la tergiversación de los textos agustinianos y el de la excesiva espiritualización del tema. Después hace una breve reseña de los aciertos de la obra de Scott Dunham The Trinity and the Creation (...) in Augustine. An ecological analysis, destacando que el Dios creador es el mismo que el Dios trino, y que en el pensamiento agustiniano sobre la creación no está ausente la cristología y la pneumatología. Se presentan a continuación las líneas ecológicas agustinianas, destacando, en primer lugar, la importancia de la Sagrada Escritura en el pensamiento del doctor de Hi¬pona. Luego se pone de manifiesto cómo la creación es comparable a un libro, en el que es preciso leer la presencia y la obra redentora de Dios. Se destaca que la naturaleza tiene un valor sacramental en un sentido lato y por ello su cuidado es necesario. Por otro lado, se muestra la figura del ser humano como un “villicus”, es decir, como el administrador o el guardián de la creación, prolongando en los escritos de san Agustín el mandato bíblico del libro del Génesis. Se señala asimismo la interrelacionalidad de todos los seres en el cosmos, la idea de una creación continuada y la importancia de la conversión ecológica. (shrink)

The leading ethical problem in medical practice for the coming decade is likely to be how to select patients for priority. Criteria for such decision-making in the case of cancer patients are discussed. Clinicians, ethicists and others need to agree guidelines on giving priority to one patient over another when resources are limited, and such criteria need to be approved by society at large. The public must accept that in a non-explicit rationing system, each individual competes with every other. In (...) the case of even more difficult ethical choices, a multidisciplinary national committee is required to advise on decision-making, and its views need to be debated by the general public. One of the more intriguing figures of the first period of the Successors is Nicanor, the lieutenant and admiral of Cassander. He came into prominence when he assumed command of the Macedonian garrison at Athens, late in 319 B.c. After distinguishing himself there he took a fleet to the Bosporus, where with Antigonus' collaboration he won a decisive victory over Polyperchon's royal navy. Subsequently his aspirations became sufficiently lofty to threaten his patron's security, and Cassander took elaborate precautions to ensure (...) his arrest and condemnation. Nicanor was clearly a figure of considerable importance; yet no source even hints at his origins and family background. Can conjecture go any way to filling the gaps? (shrink)

The randomised controlled trial (RCT) constitutes a quantitative, comparative, controlled study of a particular treatment, and provides invaluable evidence regarding its pharmacotherapeutic efficacy. These studies are generally predicated upon the ethical principle of clinical equipoise. However, this may be insufficient to justify withholding treatment from a control group while assessing drug therapy in a potentially fatal disease. Thus, the criteria for randomisation, informed consent methodology and timing, and consideration of treatment options in such a scenario remain the province of medical (...) ethics. This paper addresses the need for an RCT of ribavirin in the treatment of Crimean Congo haemorrhagic fever, and highlights underlying ethical concerns in light of the current medical, virological and ethical literature. Objectives: to assess physicians’ and patients’ views in Saudi Arabia towards involving the patient versus the family in the process of diagnosis disclosure and decision-making, and to compare them with views from the USA and Japan.Design: A self-completion questionnaire was translated to Arabic and validated.Participants: Physicians from different specialties and ranks and patients in a hospital or attending outpatient clinics from 6 different regions in KSA.Results: In the case of a patient with incurable cancer, 67% of doctors and 51% of (...) patients indicated that they would inform the patient in preference to the family of the diagnosis . Assuming the family already knew, 56% of doctors and 49% of patients would tell the patient even if family objected . However, in the case of HIV infection, 59% of physicians and 81% of patients would inform the family about HIV status without the patient’s consent . With regards to withholding ventilatory support, about 50% of doctors and over 60% of patients supported the use of mechanical ventilation in a patient with advanced cancer, regardless of the wishes of the patient or the family. Finally, the majority of doctors and patients were against assisted suicide.Conclusions: Although there was more recognition for a patient’s autonomy amongst physicians, most patients preferred a family centred model of care. Views towards information disclosure were midway between those of the USA and Japan. Distinctively, however, decisions regarding life prolonging therapy and assisted suicide were not influenced to a great extent by wishes of the patient or family, but more likely by religious beliefs. The liberal and democratic political order is underpinned by universal principles of justice. However, the universality of these principles is now being questioned and undermined by challenges from postmodernism, communitarianism, multiculturalism and other forms of anti-foundationalism. These challenges highlight the sheer diversity of cultures and values, treating liberal values and democratic political culture as one idea of social organization amongst many. While social and political orders are capable of almost endless variation, it may be that not every diverse order is (...) legitimate and we can justifiably regard some forms of social organization as beyond the pale and a 'thin' form of universalism has been developed to support this contention. The core idea behind 'thin universalism' is that it is possible to rehabilitate the universalist aspirations of normative theory while acknowledging the force of many antifoundationalist and multiculturalist criticisms of universalism. This book brings together prominent international political theorists to explain and discuss this contested concept and explore how we can stand up for and act upon principles in a world characterized by cultural difference, moral skepticism, and political division. The authors present a variety of perspectives: some chapters are sympathetic to the idea of a thin universalist account of justification, some are reluctant advocates and others are skeptical about its success or even desirability. Opening out core debates in contemporary normative theory, this volume will be of great interest to researchers and scholars working in the fields of contemporary political theory, political philosophy and IR theory.
