Wakefield's (2000) responses to our paper herein (Murphy and Woolfolk 2000) are not only unsuccessful, they force him into a position that leaves him unable to preserve any distinction between disorders and other problems. They also conflate distinct scientific concepts of function. Further, Wakefield fails to show that ascriptions of human dysfunction do not ineliminably involve values. -/- We suggest Wakefield is analyzing a concept that plays a role in commonsense thought and arguing that the task of science is to (...) identify the natural processes that accord with that commonsense concept. We argue that this represents an attempt to use conceptual analysis to legislate what should be acceptable science. Some suppose that the origin of the practice was brought to the Americas by the Spanish, who received the custom from the Italians.1 Some say that the Italians, through Marco Polo, appropriated the ritual from the Chinese.2 Others (...) see the Mexican piñata as originating locally in an Aztec religious ritual.3 But looking at this festival activity through a Girardian lens we can see that wherever a custom came into being, it seems that its origins lie in the lethal practice of collective violence. The following description may serve as an exemplar of the tradition:In Mexico, a plain, round clay cooking pot called an olla is decorated and finally almost entirely concealed by ruffles and streamer coverings shaped into animals, stars, etc. It is filled with nuts, sugar cane, fruits and small toys and is hung overhead from a rope. One participant in the game is blindfolded and given a long stick with which he or she tries to hit the pot. Several people (usually children) take turns at being blindfolded and swinging the stick, and finally when someone hits the pot [End Page 209] hard enough to break it, the other players scramble on the ground for the scattered prizes. It is a kind of noisy and happy version of a grab-bag, and the game goes on for quite a few minutes, since a controlling cord is used to swing the pot out of reach of the batters. There are several sing-song chants which the children say as the game goes on with each new target.4While the clay pot is the most traditional, piñatas are also made from other materials, including papier-mâché and cane plants.5 Furthermore, the piñata has a place in many different festivals; it is especially associated with Christmas, Easter, and birthday parties all over the Americas where there is a Latin influence.While the ritual of the piñata contains no explicit violence, the ritual itself contains many of the elements of the scapegoat mechanism René Girard describes in Violence and the Sacred. What Girard describes here is the banding together of society as they focus their internal animosity onto an arbitrary third-party victim. The peace that arrives after this violence is so profound that it is instinctually associated with the divine. This peace inspires a reenactment of this violence as a religious ritual. Therefore, religion functions as the force that keeps internal violence from turning all members of a society against one another. This controlled violence is the safety valve that releases the violence that would otherwise rend the social fabric of a community.6 Religion also veils the violence, which grants peace. It is important that the violence be expressed, but also hidden. Girard says, "Men cannot confront the naked truth of their own violence without the risk of abandoning themselves to it entirely. They have never had a very clear idea of this violence, and it is possible that the survival of all human societies of the past was dependent on this fundamental lack of understanding."7 This is the significance of the ritualism of cultic sacrifice.This relates to our current question of the piñata in the sense that the practice is reminiscent of a sacrificial ritual, especially when the piñata takes the shape of a being like an animal or human person. First, destruction is the initial aim of the activity. The piñata, like the oblation, must be destroyed in some way. This is what grants the ritual its efficacy. Second, there is a need for rubrics, a carefully followed procedure for the desired effect to come about. In cultic sacrifice, this can take place in a variety of ways, from ritualistic washing of the priest and the victim to the exact procedure and order for dissecting the victim. This paper is a critical analysis of the concept of mental disorder recently advanced by Jerome Wakefield. Wakefield suggests that mental disorders are most aptly conceived as "harmful dysfunctions" involving two distinct and separable components: the failure of the mechanism in the person to perform a natural function for which the mechanism was designed by natural selection, and a value judgment that the dysfunction is undesirable.

This paper examines the metaphysical status of Ezumezu logic in the direction of logical realism. While presenting Ezumezu logic as a prototype of African logic, Chimakonam makes statements that somewhat entail logical monism. Using the method of critical analysis of related literature, the paper argues that presenting Ezumezu logic as one of the prototypes of African logic while at the same time making claims that elevate it to a hegemonic status, gives rise to what is regarded in the paper (...) as the monist-pluralist paradox. Drawing lessons from Abakedi’s ID paradox, the paper argues that a monist logical foundation for African logic cannot be consistently defended without paradoxes. The paper proposes that different African philosophies should be grounded by different African logics such that one need not be trained in one particular prototype such as Ezumezu logic or argue in conformity to it to qualify as an African professional or scholar. Decisions about allocation of limited healthcare resources are frequently controversial. These decisions are usually based on careful analysis of medical, scientific and health economic evidence. Yet, decisions are also necessarily based on value judgements. There may be differing views among health professionals about how to allocate resources or how to evaluate existing evidence. In specific cases, professionals may have strong personal views (contrary to professional or societal norms) that treatment should or should not be provided. Could these disagreements rise (...) to the level of a conscientious objection? If so, should conscientious objections to existing allocation decisions be accommodated? In the first part of this paper, I assess whether resource allocation could be a matter of conscience. I analyse conceptual and normative models of conscientious objection and argue that rationing could be a matter for conscience. I distinguish between negative and positive forms: conscientious non-treatment and conscientious treatment. In the second part of the paper, I identify distinctive challenges for conscientious objections to resource allocation. Such objections are almost always inappropriate. (shrink)

Scotus’ theory of identity and distinction is a unique and central aspect of his thought, as he applies it throughout his metaphysics. On Scotus’ account of identity, the indiscernibility of identicals fails – i.e. A and B can be identical but not share all the same properties. As Ockham objected, Scotus is now in the difficult position of needing to provide alternative necessary and sufficient conditions for being identical, rather than simply invoking indiscernibility. The secondary literature has argued that the (...) lack of actual or potential separation is both necessary and sufficient for identity. I argue that is incorrect and provide alternative necessary and sufficient conditions for identity and real distinction, along with an analysis of Scotus’ theory of identity and distinction more broadly. Scotus thinks instead that the lack of actual, potential, and proportional separation are necessary and sufficient conditions for identity. In many forms of severe acute brain injury there is an early phase when prognosis is uncertain, followed later by physiological recovery and the possibility of more certain predictions of future impairment. There may be a window of opportunity for withdrawal of life support early, but if decisions are delayed there is the risk that the patient will survive with severe impairment. In this paper I focus on the example of neonatal encephalopathy and the question of the timing of prognostic (...) tests and decisions to continue or to withdraw life-sustaining treatment. Should testing be performed early or later; and how should parents decide what to do given the conflicting values at stake? I apply decision theory to the problem, using sensitivity analysis to assess how different features of the tests or different values would affect a decision to perform early or late prognostic testing. I draw some general conclusions from this model for decisions about the timing of testing in neonatal encephalopathy. Finally I consider possible solutions to the problem posed by the window of opportunity. Decision theory highlights the costs of uncertainty. This may prompt further research into improving prognostic tests. But it may also prompt us to reconsider our current attitudes towards the palliative care of newborn infants predicted to be severely impaired. (shrink)

Clinical guidelines summarise available evidence on medical treatment, and provide recommendations about the most effective and cost-effective options for patients with a given condition. However, sometimes patients do not desire the best available treatment. Should doctors in a publicly-funded healthcare system ever provide sub-optimal medical treatment? On one view, it would be wrong to do so, since this would violate the ethical principle of beneficence, and predictably lead to harm for patients. It would also, potentially, be a misuse of finite (...) health resources. In this paper, we argue in favour of permitting sub-optimal choices on the basis of value pluralism, uncertainty, patient autonomy and responsibility. There are diverse views about how to evaluate treatment options, and patients’ right to self-determination and taking responsibility for their own lives should be respected. We introduce the concept of cost-equivalence, as a way of defining the boundaries of permissible pluralism in publicly-funded healthcare systems. As well as providing the most effective, available treatment for a given condition, publicly-funded healthcare systems should provide reasonable suboptimal medical treatments that are equivalent in cost to the optimal treatment. We identify four forms of cost-equivalence, and assess the implications of CE for decision-making. We evaluate and reject counterarguments to CE. Finally, we assess the relevance of CE for other treatment decisions including requests for potentially superior treatment. (shrink)

Many social institutions function with rivalry, whether it is the legal adversarial system, the electoral system, competitive sports or the market. The literature on adversarial ethics (with authors such as Arthur Applbaum, David Luban and Joseph Heath) attempts to clarify what is a good behavior in these situations, but this work does not examine if institutionalized rivalry is desirable given its good and bad aspects. According to Monroe Freedman, for instance, the confrontation between lawyers in a trial may help discover (...) important facts about a case. Most economists believe that competition in the market increases economic efficiency. But institutionalized rivalry can also lead to morally wrong acts such as violence, deception or coercion. The aim of this article is to identify the conditions under which rivalry may be more or less desirable in our social arrangements. First, it will be necessary to clarify what is institutionalized rivalry and what is an adversarial scheme. Then, this article will explain what are the generic advantages and problems of adversarial schemes. Finally, this analysis will be used to outline a series of minimal requirements to consider that an adversarial scheme is desirable. (shrink)

The main aim of this paper is to explore and discuss the interesting juxtaposition of patient involvement within a standardised Enhanced Recovery After Surgery care programme (ERAS). We address our aim by examining the work and strategies of nursing staff caring for patients during postoperative recovery from surgery, exploring how these two potentially competing priorities might effectively co‐exist within a hospital ward. This was a qualitative exploratory study, with data generated through 42 semi‐structured interviews with patients and nurses who had (...) taken part in an ERAS programme in one of three hospital wards in the United Kingdom, adopting a reflexive thematic approach to data analysis. We shine a light on the work undertaken by patients and nurses during the navigation of postoperative recovery, identifying strategies of collaboration and negotiation during this journey. Furthermore, we also identify and consider patients engaged in peer‐peer support during postoperative recovery. This paper adds to the existing literature and current ways of thinking in relation to the quest for standardised, clinically effective care balanced with the aspirations for individualised, patient‐centred care. This paper also helps inform thinking about the use of care pathways in relation to service delivery, considering how best to initiate and deploy best practice aimed at safe and effective postoperative recovery. Justice is a divine attribute to which the sacred texts of the Abrahamic religions attest frequently and to which people attach great importance. However, it is the express subject of comparatively few contemporary studies. It has been argued that this is symptomatic of a long-standing trend in Christian theology, which has tended to conceive justice narrowly, as retributive. This paper makes the case that, mediaeval theologians, from Anselm to Aquinas, address the divine attribute of justice in depth and with philosophical (...) sophistication, viewing it primarily as God’s merciful and gracious distribution of merits and goods. It seeks to identify Aquinas’s contribution to the mediaeval analysis of this divine attribute and assess what he may have to contribute to current philosophy of religion. In particular, pointing to natural teleology, he offers more fully worked out metaphysical reasons for calling God just and considering all his works just. The existence of creatures can only be explained as an act of divine mercy, with the result that, since existence is the fundamental gift, all God’s works are merciful and just. (shrink)

Despite downsides, it must, on balance, be good to reduce prejudice. Despite upsides, collective action can also have destructive outcomes. Improving intergroup relations requires multiple levels of analysis involving a broader approach to prejudice reduction, awareness of potential conflict escalation, development of intergroup understanding, and promotion of a wider human rights perspective.

One of the few points of agreement to be found in mainstream responses to the logical and semantic problems generated by vagueness is the view that if any modification of classical logic and semantics is required at all then it will only be such as to admit underdetermined reference and truth-value gaps. Logics of vagueness including many valued logics, fuzzy logics, and supervaluation logics all provide responses in accord with this view. The thought that an adequate response might require the (...) recognition of cases of overdetermination and truth value gluts has few supporters. This imbalance lacks justification. As it happens, Jaskowski's paraconsistent discussive logic-a logic which admits truth value gluts-can be defended by reflecting on similarities between it and the popular supervaluationist analysis of vagueness already in the philosophical literature. A simple dualisation of supervaluation semantics results in a paraconsistent logic of vagueness based on what has been termed subvaluational semantics. (shrink)

BackgroundThe experiences of UK ethnic minority healthcare workers are crucial to ameliorating the disproportionate COVID-19 infection rate and outcomes in the UKEM community. We conducted a qualitative study on UKEM healthcare workers’ perspectives on COVID-19 vaccine hesitancy in the UKEM community.MethodsParticipants were 15 UKEM healthcare workers. Data were collected using individual and joint interviews, and a focus group, and analyzed using thematic analysis.ResultsWe generated three themes: heterogeneity, mistrust, and mitigating. Therein, participants distinguished CVH in the UKEM community in educational (...) attainment and ethnicity. They pointed to the role of mistrust in CVH in the UKEM community. They opined that the mistrust underlying CVH in the UKEM community is rooted in history and religion, conspiracy theories, the speedy development and novelty of the vaccines, post-vaccination complications/side effects, false positive test results, and social media and social support/influence. Participants recommended that interventions targeted at mitigating CVH in the UKEM community need to, in a non-judgmental way, tackle dis/misinformation and provide education, and incorporate UKEM healthcare worker endorsement. They also suggested such interventions be community-oriented, enhance the convenience of vaccination centers and the possibility of vaccine choice, and appreciate that overcoming CVH and accepting vaccination is a gradual process involving personal assessment of risks and benefits.ConclusionCVH in the UKEM community is a multifaceted phenomenon requiring multicomponent interventions. Tolstoy's remarkably economical novella The Kreutzer Sonata manages to create one of the most intense, vivid, and thought-provoking portraits of jealousy in the canon, and is as disturbing to read today as it no doubt was in 1889. The rather unhinged protagonist, Pozdnyshev, explains to his traveling companion and narrator: “Of all the passions, it is sexual, carnal love that is the strongest, the most malignant and the most unyielding” (48). This article identifies not only the “bestial” element of human (...) sexuality in Tolstoy's story but also the array of animals which the author offers to ventriloquize a certain complex (and times confused) polemic about gender relations. In other words, this analysis offers an interpretation of The Kreutzer Sonata as bestiary; one which offers a moral taxonomy of creaturely life and creaturely love. From the green-eyed monster of jealousy itself, through the Venus fly-trap and porcine couple, right up to the wild murderous beast, Pozdnyshev's confession is read via this zoological trope in order to emphasize, and question, that mobile border which separates the human from the inhuman, the civilized from the uncivilized. And it does so in order to highlight the incoherencies of the anthropocentric discursive regulation of this very same borderline. Animality and animosity are thus presented as the twin avatars of Tolstoy's intense and challenging vision. (shrink)

Ethical analyses, professional guidelines and legal decisions support the equivalence thesis for life-sustaining treatment: if it is ethical to withhold treatment, it would be ethical to withdraw the same treatment. In this paper we explore reasons why the majority of medical professionals disagree with the conclusions of ethical analysis. Resource allocation is considered by clinicians to be a legitimate reason to withhold but not to withdraw intensive care treatment. We analyse five arguments in favour of non-equivalence, and find only (...) relatively weak reasons to restrict rationing to withholding treatment. On the contrary, resource allocation provides a strong argument in favour of equivalence: non-equivalence causes preventable death in critically ill patients. We outline two proposals for increasing equivalence in practice: (1) reduction of the mortality threshold for treatment withdrawal, (2) time-limited trials of intensive care. These strategies would help to move practice towards more rational treatment limitation decisions. (shrink)

Well-trained, competent therapists are crucial for safe and effective psychedelic-assisted therapy (PAT). The question whether PAT training programs should require aspiring therapists to undergo their own PAT—commonly referred to as “experiential training”—has received much attention within the field. In this article, we analyze the potential benefits of experiential training in PAT by applying the framework developed by Rolf Sandell et al. concerning the functions of any training therapy (the therapeutic, modeling, empathic, persuasive, and theoretical functions). We then explore six key (...) domains in which risks could arise through mandatory experiential training: physical and psychological risks; negative impact on therapeutic skill; justice, equity, diversity, and inclusion; dual relationships; privacy and confidentiality; and undue pressure. Ultimately, we argue that experiential training in PAT should not be mandatory. Because many PAT training programs already incorporate experiential training methods, our exploration of potential harms and benefits may be used to generate comprehensive risk-mitigation strategies. The recent legal dispute about medical treatment for a 19-year-old patient, Sudiksha Thirumalesh, (known initially by the Court of Protection as ‘ST’) in A NHS Trust versus ST & Ors (2023) raised several challenging ethical issues. While Sudiksha’s case bears similarities to other high-profile cases in England and Wales, there are key differences. Crucially, Sudiksha herself was part of the disagreement. She was alert, communicative and sought to advocate for herself. Furthermore, this case was framed in the courts as pivoting (...) not on considerations of best interests but on a determination of decisional capacity. Sudiksha was deemed to lack capacity because she did not believe her doctors’ view of her prognosis.While the legal questions in the case were central to a recent Court of Appeal decision (which overturned the original finding), in this commentary, we focus on the ethical questions therein. We start by describing Sudiksha’s court case and the initial judgment. We then offer an ethical analysis of the relationship between false beliefs, values and the ‘capacity’ to make decisions, arguing for a need for particular care when judging patients to lack capacity based purely on ‘false and fixed beliefs’. After briefly noting the legal basis for the appeal finding, we offer ethical implications for future cases. Although it appears that Sudiksha had decision-making capacity, this did not settle the ethical question of whether health professionals were obliged to continue treatment that they believed to have no prospect of success. In Heidegger’s Being and Time certain concepts are discussed which are central to the ontological constitution of Dasein. This paper demonstrates the interesting manner in which some of these concepts can be used in a reading of T.S. Eliot’s The Love Song of J. Alfred Prufrock. A comparative analysis is performed, explicating the relevant Heideggerian terms and then relating them to Eliot’s poem. In this way strong parallels are revealed between the two men’s respective thoughts and distinct modernist sensibilities. (...) This thesis explores, thematically and chronologically, the substantial concordance between the work of Martin Heidegger and T.S. Eliot. The introduction traces Eliot's ideas of the 'objective correlative' and 'situatedness' to a familiarity with German Idealism. Heidegger shared this familiarity, suggesting a reason for the similarity of their thought. Chapter one explores the 'authenticity' developed in Being and Time, as well as associated themes like temporality, the 'they' (Das Man), inauthenticity, idle talk and angst, and applies them to interpreting Eliot's poem, (...) 'The Love Song of J. Alfred Prufrock'. Both texts depict a bleak Modernist view of the early twentieth-century Western human condition, characterized as a dispiriting nihilism and homelessness. Chapter two traces the chronological development of Ereignis in Heidegger's thinking, showing the term's two discernible but related meanings: first our nature as the 'site of the open' where Being can manifest, and second individual 'Events' of 'appropriation and revelation'. The world is always happening as 'event', but only through our appropriation by the Ereignis event can we become aware of this. Heidegger finds poetry, the essential example of language as the 'house of Being', to be the purest manifestation of Ereignis, taking as his examples Hölderlin and Rilke. A detailed analysis of Eliot's late work Four Quartets reveals how Ereignis, both as an ineluctable and an epiphanic condition of human existence, is central to his poetry, confirming, in Heidegger's words, 'what poets are for in a destitute time', namely to re-found and restore the wonder of the world and existence itself. This restoration results from what Eliot calls 'raid[s] on the inarticulate', the poet's continual striving to enact that openness to Being through which human language and the human world continually come to be. The final chapter shows how both Eliot and Heidegger value a genuine relationship with place as enabling human flourishing. Both distrust technological materialism, which destroys our sense of the world as dwelling place, and both are essentially committed to a genuinely authentic life, not the angstful authenticity of Being and Time, but a richer belonging which affirms our relationship with the earth, each other and our gods. This article argues that Summa Theologiae 3a Q17 art. 2 is consistent with the attribution of a proper being to Christ's human nature. It proceeds in three stages. First, it examines the emergence of the problem of Christ's being through an analysis of the Chalcedonian Decree. This paper describes a structured approach to the selection of an analytic strategy for cross-case analysis and a method for data display. To this end, criteria will be developed addressing both, aspects of scientific rigor as well as the practicability of application and the application of the decision process will be demonstrated.

This essay challenges the relevance of the primary analogy in Ronit Stahl and Ezekiel Emanuel’s article “Physicians, Not Conscripts: Conscientious Objection in Health Care.” The author then proposes an alternative, classi­cally inspired model of conscience based on the work of E. Christian Brugger, Edmund Pellegrino, and Alasdair MacIntyre. This teleological model enables a more robust analysis of conscience claims than does Stahl and Emanuel’s social-constructivist framework.

Huang and colleagues provide some intriguing insights into the attitudes about end of life care of practising Taiwanese neonatal doctors and nurses.1 There are some similarities with surveys from other parts of the world. Most Taiwanese neonatologists and nurses agreed that it was potentially appropriate to withhold or limit treatment for infants who were dying. A very high proportion was opposed to active euthanasia of such infants. But there were also some striking differences. Only 21% of Taiwanese doctors ‘agreed’ with (...) withdrawal of mechanical ventilation for dying newborn infants. This proportion is lower than reported in any European country.2 More than 90% of neonatologists surveyed in the UK, the USA, The Netherlands and Sweden found withdrawal of mechanical ventilation acceptable.2 ,3 Taiwanese doctors also seemed reluctant to give analgesia or sedatives to such infants, with almost 60% disagreeing with their use in dying infants where there was a risk of hastening death.1 In contrast, two-thirds or more of European neonatologists thought this was acceptable .2The results of the survey of Huang et al highlight three beliefs about end-of-life care that are widespread, but all of which are seriously mistaken1. An examination of Ian Parker's definitions of discourse reveals them to be non-distinctive and of limited utility. It is argued that discourse analysis should be integrated with, rather than set against, social psychology. Discourse analysts should attend to the issues of the representativeness and generality of their evidence, should be wary of attributing causality to discourse, and should consider the advantages of systematically investigating, rather than asserting, the social consequences of the use of different discourses.

In this review, I say a few words about the analysis portion of Piketty’s book, but I will focus mostly on its solution portion. In the first section, I go over Piketty’s main argument and make two critical points: there is a lack of consideration for, first, human capital and, second, absolute levels of income and capital per capita. The second section of this essay focuses on the solution portion of the book. I also go over Piketty’s argument and (...) make five more substantial points: I wonder what Piketty means by social justice; I emphasize that raising taxes is desirable; I ask why it is capital that should be taxed, and how feasible a global tax on capital would be; and finally, I claim that the book could provide more guidance in terms of the policies that could be put in place right now. If the global taxation of capital is not politically feasible in the short to medium term, what should be done in the meantime? (shrink)

The paper develops a framework for discussing concepts of health and disease along two dimensions. The first is the role of values in our disease concepts, and the second is the relationship between science and folk psychology. This framework is then applied to the concept of mental disorder. I argue that existing treatments of the concept yield too much authority to common sense, which produces a tension within the program of finding a scientific basis for our ascriptions of mental disorder. This dissertation addresses the ontological significance of poetry in the thought of Martin Heidegger. It gives an account of both his earlier and later thinking. The central argument of the dissertation is that poetry, as conceptualised by Heidegger, is beneficial and necessary for the living of an authentic life. The poetry of T. S Eliot features as a sustaining voice throughout the dissertation to validate Heidegger's ideas and also to demonstrate some interesting similarities in their ideas. Chapter one demonstrates how (...) effectively certain concepts from Heidegger's 'Being and Time' can be applied in an analysis of T.S. Eliots celebrated poem 'The Love Song of J. Alfred Prufrock'. The reading involves concepts such as angst, authenticity, inauthenticity, the they and idle talk as they appear in 'Being and Time' and then relates these to aspects of T.S Eliot's poem. Chapter two is an exegesis of Heideggers essay 'The Origin of the Work of Art' in order to understand the meaning of poetry as he describes it. The essay centres on the interpretation of a painting by Vincent van Gogh, and what the experience of the painting reveals to someone authentically engaging with the artwork. Heidegger attempts to establish what the essence of a thing is (the artwork as thing), for the 'origin' of the artwork resides in its 'thingliness'. He creates an important distinction between equipment and the artwork, as well as earth and the world in order to justify the unique, originary position that the artwork occupies. This leads Heidegger to create a new understanding of poetry (which is expanded to encompass all art forms) and to emphasise the importance of the human agent in both the creation and preservation of the artwork. Chapter three is an exploration of language in both the Heidegger of 'Being and Time' and the later Heidegger's thought. The aim is to explore the ontological effect that Heidegger's conception of language has for our existence. He places language within a primordial role arguing that it is not us who speak language, but language that speaks us. This conception has important consequences for our relationship with Being, and the way in which we understand our existence. The final chapter unifies the various themes of the dissertation into a cohesive argument. The chapter begins with a discussion on the meaning of our existence following the later Heidegger. This is nothing less than the guardianship of Being which can only be understood in its relation to our dwelling within the fourfold. In conclusion it is through the measure of the language of poetry that we can realise the possibility of authentic dwelling. (shrink)

In this paper, I do not propose to discuss the ways of thinking about and coming to terms with our awareness of our coming death. I would like rather to discuss a more particular and perhaps unusual problem, that of the relation between our awareness of our death and our consciousness of ourselves. In the following I take four examples: Parmenides, Plato, Epicurus and Plotinus. I sketch the different ways in which these philosophers saw the relation between self-consciousness and death, (...) how they tried to dissipate what seemed to them to be a tension, even a contradiction, between these two parts of our existence. My discussion does not seek to contribute to the philological analysis of specific ancient texts, but proposes rather an attempt to reach an overall view, which might be of a broader interest. "Our governing doctrine," as Manent calls it, is chiefly characterized by the primacy (...) of right over law and the rejection of any objective principle by which actions and persons might be judged.1 It is "the plasticity of the human form" itself which demands respect.2 The only non-constructed and thus normative characteristic of equal human nature is the power of being an "individval-conatus," the power to be one's own rule and reason for acting.3 And the only generally recognized purpose of this "individual-conatus" is to satisfy the desire for power, which is the sole universal motive of human action.4 Such an individual can be legitimately ruled and governed only by himself, a claim reflected in the ideal of the autonomous subject and representative government.5 In sum, Manent characterizes this ideology in terms of a thoroughgoing [End Page 335] rejection of any objective rule, "any law of God or of nature," according to which man is defined and judged.6Manent warns that this ideology is opposed to the very wellsprings of action, since an operation is rightly called an action only when it is performed according to some deliberate choice, which requires some rule and reason. If all rules and reasons are entirely self-generated, then there is no principle according to which deliberation might occur. Such a so-called autonomous subject purports to command himself, but in fact he is more patient than agent: the subject is moved by desires and preferences which cannot be weighed according to any discriminatory rule or principle, since the desires and preferences themselves are the only rule the subject acknowledges. Aside from submitting to the movement of the inexplicably strongest preference, the only positive reason for acting that remains is to destroy any vestiges of objective rules or reasons for acting—that is, the law—that might hinder the realization of individual preference: "The acting animal is now the prisoner of the very audacious and ingenious arrangement that he once devised to escape the urgency and to avoid the difficulty of the practical question. Caught in the realm of inaction, he seeks out, in a kind of terminal fever, the last corners of social existence that still escape the laissez-faire idea and where the very idea of the law might suffer its final defeat."7What is a Christian to make of this practical ideology? What natural inclinations does it privilege and how might the goods it seeks be sought rightly? Manent argues that the sole universal motive for action explicitly recognized by this practical ideology is a certain kind of power: the power of self-determination. According to the governing doctrine, this power is the only universal human motive that can be relied upon to construct common life and political order. Manent's response to this ideology is therefore to propose a recovery of a natural law account of the heterogeneity of universal human motivations to include useful, pleasing, and honorable goods. In this article, I will argue that Manent's own account of the ideology he critiques suggests a more fundamental motive driving the ideology, namely, a desire for an unassailable esteem. And since it is a desire for esteem that best explains the attractiveness of the ideology of modern freedom, an alternative vision of practical life will need to address the desire for esteem and articulate its proper bases. In advancing this argument, I will first show that Manent's own analysis highlights the importance of the desire for esteem in motivating the ideology of modern freedom. To precisely define wisdom has been an ongoing task of philosophers for millennia. Investigations into the psychological dimensions of wisdom have revealed several features that make exemplary persons "wise." Contemporary bioethicists took up this concept as they retrieved and adapted Aristotle's intellectual virtue of phronesis for applications in medical contexts. In this article, we build on scholarship in both psychology and medical ethics by providing an account of clinical wisdom qua phronesis in the context of the practice of psychoanalysis and (...) psychodynamic psychotherapy. With the support of qualitative data, we argue that the concept of clinical wisdom in mental healthcare shares several of the key ethical dimensions offered by standard models of phronesis in medical ethics and serves as a useful, albeit overlooked, reference point for a broader development of virtue-based medical ethics. We propose that the features of clinical wisdom are pragmatic skills that include, but are not limited to, an awareness of balance, the acceptance of paradox, and a particular clinical manner that maintains a deep regard for the other. We offer several suggestions for refining training programs and redoubling efforts to provide long-term mentorship opportunities for trainees in clinical mental healthcare in order to cultivate clinical wisdom. If a doctor is trying to decide whether or not to provide a medical treatment, does it matter ethically whether that treatment has already been started? Health professionals sometimes find it harder to stop a treatment (withdraw) than to refrain from starting the treatment (withhold). But does that feeling correspond to an ethical difference? In this article, we defend equivalence—the view that withholding and withdrawal of treatment are ethically equivalent when all other factors are equal. We argue that preference for (...) withholding over withdrawal could represent a form of cognitive bias—withdrawal aversion. Nevertheless, we consider whether there could be circumstances in which there is a moral difference. We identify four examples of conditional nonequivalence. Finally, we reflect on the moral significance of diverging intuitions and the implications for policy. We propose a set of practical strategies for helping to reduce bias in end-of-life decision making, including the equivalence test. (shrink)

There is a tendency in decision‐making research to treat uncertainty only as a problem to be overcome. But it is also a feature that can be leveraged, particularly in social interaction. Comparing the behavior of profitable and unprofitable poker players, we reveal a strategic use of information processing that keeps decision makers unpredictable. To win at poker, a player must exploit public signals from others. But using public inputs makes it easier for an observer to reconstruct that player's strategy and (...) predict his or her behavior. How should players trade off between exploiting profitable opportunities and remaining unexploitable themselves? Using a recent multivariate approach to information theoretic data analysis and 1.75 million hands of online two‐player No‐Limit Texas Hold'em, we find that the important difference between winning and losing players is not in the amount of information they process, but how they process it. In particular, winning players are better at integrative information processing—creating new information from the interaction between their cards and their opponents’ signals. We argue that integrative information processing does not just produce better decisions, it makes decision‐making harder for others to reverse engineer, as an expert poker player's cards act like the private key in public‐key cryptography. Poker players encrypt their reasoning with the way they process information. The encryption function of integrative information processing makes it possible for players to exploit others while remaining unexploitable. By recognizing the act of information processing as a strategic behavior in its own right, we offer a detailed account of how experts use endemic uncertainty to conceal their intentions in high‐stakes competitive environments, and we highlight new opportunities between cognitive science, information theory, and game theory. Intense public interest in scientific claims about COVID-19, concerning its origins, modes of spread, evolution, and preventive and therapeutic strategies, has focused attention on the values to which scientists are assumed to be committed and the relationship between science and other public discourses. A much discussed claim, which has stimulated several inquiries and generated far-reaching political and economic consequences, has been that SARS-CoV-2 was deliberately engineered at the Wuhan Institute of Virology and then, either inadvertently or otherwise, released to the (...) public by a laboratory worker. This has been pursued despite a clear refutation, through comprehensive genomic analysis, of the hypothesis that the virus was deliberately engineered and the failure of detailed investigations to identify any evidence in support of a laboratory leak. At the same time a substantial, established body of knowledge about the many factors underlying the emergence of novel zoonotic diseases has been largely ignored—including climate change and other mechanisms of environmental destruction, tourism, patterns of trade, and cultural influences. The existence and conduct of these debates have raised questions about the vulnerability of science to manipulation for political purposes. Scientific discourses are vulnerable because: (i) claims can be made with no more than probabilistic force; (ii) alleged “facts” are always subject to interpretation, which depends on social, ethical, and epistemological assumptions; and (iii) science and scientists are not inherently committed to any single set of values and historically have served diverse, and sometimes perverse, social and political interests. In the face of this complexity, the COVID-19 experience highlights the need for processes of ethical scrutiny of the scientific enterprise and its strategic deployment. To ensure reliability of truth claims and protection from corrupting influences robust ethical discourses are required that are independent of, and at times even contrary to, those of science itself.
