Gender Dysphoria in Children and Controversies about its Treatment: Two Different Conceptions of Gender Identity Disforia de gênero em crianças e controvérsias em seu tratamento: duas concepções diferentes sobre a identidade de gênero Gender dysphoria is a marked mismatch between the sex felt or expressed by an individual and that assigned to them at birth. It is associated with clinically significant discomfort or social impairment and can occur in children, adolescents, and adults. The literature mainly describes three approaches for preadolescents: (...) therapeutic model, watchful waiting, and gender affirmative model. However, there is currently no consensus on its treatment nor any evidence that favors one approach over another. This paper provides a brief review of the existing literature on gender dysphoria, explains various approaches for children, and discusses the anthropological views behind them. We argue that at least two ways of understanding gender identity have given rise to disagreements over the proposed treatments. It is quite important to comprehend the anthropological assumptions on which these models are based since they underlie different conceptions of the person’s sex-gender relationship and, therefore, the role of corporality and self-awareness in gender identity. Los textos hipocráticos y su vigencia: elementos para la formación humanística del médico en el siglo XXI Os textos hipocráticos e sua validade: elementos para a formação humanística do médico no século XXI By way of notes, comments are made on the validity of some Hippocratic works from the perspective of the learning and practice of medicine in the first decades of the 21st century, amidst a expanding technical-scientific and computer civilization. The paper highlights the affirmation of perennial truths —valid (...) at all times— extracted from a selection of Hippocratic writings, which express the spirit of classic Greek medicine understood as tekhne iatrike. It was originally conceived as an activity of a rational, human, philanthropic and scientific nature aimed at serving the good of patients, following an ethos and some ethical principles and values of universal range, validity and application. It is proposed that taking in these concepts provides the medicine student with humanity, dignity, moral and intellectual growth, and improves their attitudes and aptitudes, skills and abilities to understand and face multiple existential situations that permeate daily clinical practice. This not only requires professional technical skills, but also humane attributes for better approaching interpersonal relationships with and attitudes towards the patient and their families. These ideas are introduced as a contribution of bioethics, characterized by its humanities-science-technology integration and educational mission of training critical consciences within the techno-scientific civilization, in order to foster respect for and openness towards diverse aspects of reality. Voluntades anticipadas: desafíos éticos en el cuidado del paciente Diretivas antecipadas de vontade: desafios éticos no cuidado do paciente Advance healthcare directives are intended for the individual to personally express their will and preferences about healthcare and treatment ahead of time. This literature review aims to describe the concept and structure of advance directives and the ethical aspects involved in patient care. Using the keywords “Advance Healthcare Directive” AND “Ethical Implication” and its Spanish equivalents, five databases were accessed: ProQuest, Philosophy, (...) PubMed, Web of Science and Scielo. Publications were narrowed down to the 2010-2018 period. Thirty-one articles were obtained and read critically. Results of this review were grouped into the following categories: concept, structure, clinical situations in which advance directives apply, strengths and limitations, as well as the ethical aspects involved. End of life-related experiences in which a person loses their ability to make decisions and cannot express their wishes, so it is impossible to know their will, are more common nowadays. Most health workers are not appropriately trained in the preparation and application of advance directives, which becomes an opportunity to research and delve deeper into the subject. Advance directives are a tool that gives health workers reliable information on a patient’s values and wishes, so it is vital to train them to provide respectful quality care. Clinical and Care Bioethics Committees at Public and Private Healthcare Institutions of Medium and High Complexity Levels in Cities of the Atlantic Coast of Colombia Comitês de Bioética clínico-assistencial nas instituições de saúde públicas e privadas dos níveis de média e alta complexidade das cidades da Costa Atlântica da Colômbia Care bioethics committees are interdisciplinary groups engaged in providing education to members of said committees, all the staff in the hospital —including professors and students in their internship—, and members of (...) the community. The objective of this study is to determine the situation of clinical and care bioethics committees at healthcare institutions in the Atlantic Coast of Colombia between 2010-2016. The study is cross-sectional, descriptive, and retro-prospective. Twenty-six health institutions that met the inclusion criteria were selected and administered a semi-structured survey. The statistical software SPSS version 22 was used to analyze the information. This study adhered to ethical research guidelines. The name most commonly given to committees is Hospital Ethics Committee. Only half have staff trained in ethics or bioethics and the most outstanding function is education in ethics; however, in the same percentage there are committees whose opinion is divided as their operation is unclear. Committees should follow existing ethical and bioethical models, and bills should be brought forward to help committees at healthcare institutions become more effective as regards their structure and operation. The author sums up her anthropological conception, through an accessible language intended to a general public, examining topics such as the specific trait of our mentality; the distinction between signs and symbols, as well as between the domains of the real and the possible; symbolic transformation; the essence of language; the importance of non-discursive articulate forms for human expression and cognition; the relationship between reason and lunacy. In addition to that synthesis, Langer relates those aspects, in an insightful and opportune way, to the war period when this article was written, observing, as a symptom of the cultural crisis of that time, the loss of symbolic references and the substitution of the old symbols (mythical and religious) for the one of nationalism. Background Moral courage is a recognized virtue. In the context of the COVID-19 pandemic, the master’s students of nursing (MSNs) in China have shown tenacious moral courage. Objective This study elaborates on the moral courage of Chinese MSNs through their experiences of volunteering during the pandemic. Research Design Descriptive qualitative, interview-based. Participants and Research Context Participants were nursing postgraduate students who participated in the prevention and control of the COVID-19 pandemic selected by purposeful sampling. The sample size was determined by (...) data saturation, which was reached with 10 participants. Data were analyzed using a deductive method of content analysis. Because of the isolation policy, telephone interviews were adopted. Ethical considerations After obtaining the approval of the ethical institution of the author’s school (No. 138, 30 August 2021), verbal consent was obtained before the interview with the participants. All data were processed anonymously and confidentially. In addition, we recruited participants through MSNs’ counselors, and obtained their phone numbers with their permission. Results Data analysis resulted in 15 subcategories that were subsequently grouped into 3 major categories including proceed without hesitation, the outcome of practicing moral courage, and develop and maintain moral courage. Conclusion This qualitative study is based on the special background of the COVID-19 pandemic, and the MSNs in China have shown tenacious moral courage in the work of epidemic prevention and control. Five factors led them to take action without hesitation, and six possible outcomes followed. Lastly, this study provides some suggestions for nurses and nursing students to enhance their moral courage. To better develop and support moral courage in the future, it is necessary to use different methods and multidisciplinary approaches to study moral courage. (shrink)

ObjectiveTo elucidate the clinical, radiologic characteristics of Leber’s hereditary optic neuropathy (LHON) associated with the other diseases.Materials and methodsClinical data were retrospectively collected from hospitalized patients with LHON associated with the other diseases at the Neuro-Ophthalmology Department at the Chinese People’s Liberation Army General Hospital (PLAGH) from December 2014 to October 2018.ResultsA total of 13 patients, 24 eyes (10 men and 3 women; mean age, 30.69 ± 12.76 years) with LHON mitochondrial DNA (mtDNA) mutations, were included in the cohort. 14502(5)11778(4)11778 (...) &11696(1)12811(1)11696(1)3460(1). One patient was positive for aquaporin-4 antibody (AQP4-Ab), and two were positive for myelin oligodendrocyte glycoprotein antibody (MOG-Ab). Three patients were associated with idiopathic optic neuritis (ON). Two patients were with compression optic neuropathy. Three patients were with the central nervous system (CNS) diseases. One patient was with proliferative diabetic retinopathy (PDR) and one with idiopathic orbital inflammatory syndrome (IOIS). At the onset, visual acuity (VA) in eighteen eyes was below 0.1, one eye was 0.5, five eyes were above 0.5, while VA in sixteen eyes was below a 0.1 outcome, three eyes experienced moderate vision loss. MRI images showed T2 lesions and enhancement in nine patients who received corticosteroids treatment; additional immune modulators treatment was performed on two patients. None of the patients had relapse during the follow-up time.ConclusionLeber’s hereditary optic neuropathy can be accompanied with multiple-related diseases, especially different subtypes of ON, which were also exhibited with IOIS and compression optic neuropathy for the first time in this cohort. This condition may be a distinct entity with an unusual clinical and therapeutic profile. (shrink)

Resumo: Propomos neste artigo uma reflexão sobre a relação entre a educação e a sociedade partindo de Émile Durkheim. Analisamos as prescrições que o sociólogo francês nos fornece sobre a educação moral e sua importância no combate à anomia social. Da mesma forma, ressaltamos como sua reflexão sobre a educação nos permite pensar um circuito de afetos inerente aos processos de socialização. Todos esses elementos nos direcionam à questão central do artigo, qual seja, se teria sentido retomarmos a educação moral (...) tal como nos propõe Durkheim. A fim de respondermos essa questão, analisaremos a maneira como os processos de socialização se efetivam na contemporaneidade e como a educação se insere nesse processo. Essa reflexão nos leva a pensar se devemos ou não nos adequarmos aos valores sociais vigentes, ou se devemos pensar a escola como lugar de resistência. Thoughts on school according to Durkheim's moral education Abstract: In this paper, we propose a discussion on the relationship between education and society based on Émile Durkheim. We consider the prescriptions that the French sociologist provides regarding moral education and its significance in addressing social anomie. Furthermore, we highlight how his thoughts on education allow us to contemplate a circuit of affections inherent to the socialization processes. All these elements lead us to the central question of the paper: whether it would make sense to revisit moral education as proposed by Durkheim. To answer this question, we will examine how socialization processes are achieved in contemporary society and how education is involved in this process. This reflection will prompt us to consider whether we should adapt to current social values or understand the school as a place of resistance. This underscores the importance of Durkheim's thinking for contemporary education: prompting us to reflect on its purpose. Keywords: Socialization; Morality; Education; Educational Purpose. Reflexiones sobre la escuela a partir de la educación moral de Durkheim Resumen: En este artículo proponemos una reflexión sobre la relación entre la educación y la sociedad a partir de Émile Durkheim. Del mismo modo, destacamos cómo su reflexión sobre la educación nos permite pensar en un circuito de afectos inherentes a los procesos de socialización. Todos estos elementos nos dirigen a la pregunta central de este artículo, que es si tendría sentido volver a la educación moral tal como la presenta Durkheim. The paths and the ways: an insight into transdisciplinarity: This is a short study of how the notion of thinking that Heidegger developed in his writing, in the Conversation on a Country Path about Thinking, can be read through a consideration of a Chinese Taoist text Tao Te Ching and the Confucian, though a Taoist inspired text, by Zisi, the Zhongyong, to illuminate the essential nature of openness in transdisciplinarity and the restrictions of disciplinarity of knowing taken as the ritual (...) and rules of methodology. This approach offers a way to understand unconcealment in the onto-cosmology of the harmony of all Being and of personal cultivation which is essential to the ontology of Heidegger. È mettendo in primo piano l’idea di filosofia pratica universale, già presente nella filosofia dell’Aufklärung, che Fichte è riuscito a dare un nuovo volto alla speculazione trascendentale postkantiana. Il saggio tenta innanzitutto di ricostruire la filosofia pratica abbozzata negli anni 1793-1794. Viene proposto un breve confronto con Wolf. In seguito, lo studio rivolge la sua attenzione ad uno dei contributi della filosofia pratica universale fichtiana: aver trattato il sentimento come nozione pratica fondamentale, presente nella conoscenza e nell’azione. Infine, si tratta (...) di mostrare che Fichte propose, nella prima Dottrina della Scienza, di fare della filosofia pratica universale una philosophia prima che fosse chiamata a prendere il posto della ontologia dogmatica e della dottrina degli elementi critica. It has long been known that in April 1343 gems from the Byzantine imperial crown were pledged with the Venetians as collateral for a loan; the sum secured by this transaction was the substantial one of 30,000 ducats. The action had been taken by the empress Anna of Savoy, who, after the death of her husband Andronikos III in 1341, had ensured that her young son, born in June 1332, was crowned by the patriarch in November of the same year (...) as the emperor John V Palaiologos. She negotiated the loan when desperately short of funds while acting as his regent during the ensuing civil war of 1341-1347; under the agreed terms the loan would be repaid at the rate of 10,000 ducats per year over three years, plus interest at 5%. The timescale of the agreement implies that she must have initially hoped that the jewels would be recovered for her emperor son, then only eleven years old, when he reached the age of fourteen. Study explores the core areas that enhance the teaching ability of Chinese teachers in the Chinese+Medical learning program. The participants in the study were 150 students of Dali University and questionnaires were used to collect data from the participants. Structural Equation Modelling (SEM), Pearson correlation analysis, and ANOVA test were conducted to determine the relationship of these factors. The results of the Pearson correlation are the following findings: teaching experience (TE) has a small positive correlation with teaching ability (r = (...) 0.40,p = 0.05) while professional development training (PDT) has a strong positive correlation with the teaching ability, (r = 0.52,p = 0.01) and the pedagogical strategies (PS) (r = 0.47, p = 0.03) have the strongest positive connections with teaching ability, teacher collaboration (TC) (r = 0.30,p = 0.10 ) and educational resources (ER) (r = 0.35,p = 0.07) exhibit weaker non-significant associations. The SEM analysis findings indicate that the highest path coefficients about teaching ability areTE (C = 0.30,p = 0.02), TC (C = 0.40,p = 0.01), PDT (C = 0.45,p = 0.01),PS (C = 0.35,p = 0.01), and ER(C = 0.25,p = 0.03). The findings of the ANOVA also indicate high variation in teaching competency regarding different PDT (p = 0.03), TE (p = 0.02), ER (p = 0.01), PS (p = 0.04), and TC (p = 0.02). The results altogether suggest that the most significantkey elements that can be employed to improve the teaching ability within Chinese+Medical Education are PDT and TC. (shrink)

Mặc dù Việt Nam cùng với 146 quốc gia trên thế giới đã có cam kết mạnh mẽ nhất trong việc giảm phát thải vào năm 2050, tuy nhiên việc cụ thể hóa cam kết thông qua các giải pháp sáng tạo là rất quan trọng. Fourteen healthy children (13.8±2.2 years, range 10 to 16; M:F=5:9) received 30 Hz intermittent theta burst transcranial magnetic stimulation (iTBS) with a stimulation intensity of 70% of resting motor threshold (RMT) with a total of 300 (iTBS300) pulses. All volunteers were free of neurologic, psychiatric and serious medical illnesses, not taking any neuropsychiatric medications, and did not have any contraindications to Transcranial Magnetic Stimulation. Changes in the mean amplitudes of motor-evoked potentials from baseline following iTBS were expressed as a ratio (...) and assessed from 1 to 10 minutes (BLOCK1) and 1 to 30 minutes (BLOCK2) using repeated-measures analysis of variance. All 14 subjects completed iTBS300 over the dominant primary motor cortex (M1) without any clinically reported adverse events. ITBS300 produced significant M1 facilitation (F5,65=3.165, p=0.01) at BLOCK1 and trend level M1 facilitation at BLOCK2 (F10,129=1.69, p=0.089). Although iTBS300 (stimulation duration of 92 seconds at 70% RMT) delivered over M1 in typically developed children was well-tolerated and produced on average significant facilitatory changes in cortical excitability, the post-iTBS300 neurophysiologic response was variable in our small sample. ITBS300-induced changes may represent a potential neuroplastic biomarker in healthy children and those with neuro-genetic or neuro-psychiatric disorders. However, a larger sample size is needed to address safety and concerns of response variability. (shrink)

Resumen La argumentación es una forma de discurso en la que un hablante defiende una postura en una discusión. El presente trabajo se enfoca en la complejidad argumentativa y los recursos evaluativos y evidenciales que caracterizan las disputas de niños de diverso nivel socioeconómico. Se analizaron 94 disputas entre niños de 4 años y otros niños (5 a 10 años) en situaciones de juego espontáneo registradas en hogares de sectores socioeconómicos medio y bajo. El análisis identificó diferencias marginales en la (...) complejidad argumentativa entre los grupos. Por su parte se hallaron diferencias significativas en recursos de evaluativos de emocionalidad y en los evidenciales deónticos y exclamativos a favor de los sectores de nivel socioeconómico bajo. Los sectores medios registraron un mayor empleo de expresiones de causalidad y conectores. Los resultados se discuten a la luz de los hallazgos de investigaciones previas. Abstract Argumentation is a type of discourse in which a speaker defends a claim in a discussion. The present work focuses on the argumentative complexity and the evaluative and evidential resources that characterize the disputes of children of different socioeconomic levels. We analyzed 94 disputes between 4-year-old children and other children (5 to 10 years old) in situations of spontaneous play registered in homes of middle and low socioeconomic status (SES). The analysis identified marginal differences in argumentative complexity between the groups. On the other hand, significant differences were found in evaluative expressions of emotion and in deontic and exclamatory evidential markers, more frequently used by the low SES group. The mid SES group tends to use a higher number of expressions of causality and connectors. The results are discussed with reference to the findings of previous research. (shrink)

En este artículo se examina la situación de los fonemas africados /ʈ͡ʂ/ y /t͡ʃ/ del mapudungun hablado por niños y niñas bilingües de la zona cordillerana de Alto Bío-Bío. En concreto se describe la realización de estas consonantes, comparándolas con las descripciones que se han realizado para los adultos de la zona y se identifican los principales rasgos de variación fonético-fonológica. Los colaboradores fueron 30 niños bilingües mapudungun-castellano, pertenecientes a comunidades del valle del Queuco y del valle del Bío-Bío, cuyas (...) edades fluctúan entre los 10 y 14 años. Según los datos analizados, los niños realizan principalmente variantes tradicionales de estos segmentos; sin embargo, en algunas comunidades se observan rasgos de variación asociados al fonema africado retroflejo áfono que podrían atribuirse al contacto con el castellano. Ethical Aspects Related to Nursing Care for Persons with Disabilities: A Qualitative Analysis Aspectos éticos relacionados com a atenção de enfermagem em pessoas com deficiência: uma análise qualitativa The purpose of the study is to describe ethical aspects related to nursing care for persons with disabilities; a population considered socially vulnerable and in conditions of inequality. It corresponds to the first phase of a primary study conducted in Atacama, Chile using a qualitative methodology and content analysis. Nursing graduates are interviewed (...) respecting Ezekiel Emanuel’s ethical criteria. The main findings account for ontological categories such as moral reasoning, value system, nurse-patient relationship model, consequences of the ethical nursing action, strategies to overcome moral conflicts, and barriers in care. It is concluded that caring for people with disabilities is a source of dilemma and moral anguish for nurses, especially when it comes to individuals with sensitive or cognitive disabilities. The paternalistic medical model predominates in the clinical context, limiting the right to autonomy of persons with disabilities. Instrumental values such as efficiency are added to traditional moral values promoted by nurses. Civic values considered fundamental to the challenge of human diversity and life in a democracy are not mentioned. Science fiction cinema proposal for Bioethic education Proposta do cinema de ficção científica para educar em Bioética Posthumanism state that technologies will eliminate aging, improve cognitive, physical and psychological skills and keep alive human minds by uploading them in machines as artificial intelligence. These beliefs, as constant features in science fiction films, could be useful to educate medical students in Bioethics. We propose some of these science fiction films related with technological changes of humans and the noxious consequences: Being turned (...) into a humanoid created by genetic engineering; a slave or a merchandize without rights; discrimination because of eugenesic genetic manipulation; and/or the creation of cyborgs of mass destruction. Also, the movies can show the reader issues of ethnic supremacy dilemmas, identity impersonate and ambition, impersonal intelligence and conscience and identity transpass after death. These films will help teachers to analyze with students the moral limits of science and technologies. This book is the first to approach Jacques Rancière’s work from a legal perspective. A former student of Louis Althusser, Rancière is one of the most important contemporary French philosophers of recent decades: offering an original and path-breaking way to think politics, democracy and aesthetics. Rancière’s work has received wide and increasing critical attention, but no study exists so far that reflects on the wider implications of Rancière for law and for socio-legal studies. Although Rancière does not pay much specific (...) attention to law—and there is a strong temptation to identify law with what he terms the "police order"—much of Rancière’s historical work highlights the creative potential of law and legal language. Indeed, he repeatedly offers powerful instances of what Robert Cover labelled _jurisgenesis, _of norm-generation, with important legal implications and ramifications. So, rather than excavate the Rancièrean corpus for isolated statements about the law, this volume reverses such a method and asks: what would a Rancière-inspired legal theory look like? Bringing together specialists and scholars in different areas of law, critical theory and philosophy, this rethinking of law and socio-legal studies _through_ Rancière provides an original and important engagement with a range of contemporary legal topics, including constituent power, constitutional interpretation, the judicialization of politics, legal subjectivity, conflicts, human rights, the nomos of modernity, globalization, and the sensory configurations of law. The present article presents the results of a literature review that show diverse positions, while delimitating the subject. Cuidados paliativos: solución ética acorde con la dignidad humana al final de la vida Cuidados paliativos: solução ética de acordo com a dignidade humana no final da vida It is said that passing laws to legalize euthanasia is absolutely necessary but, in my opinion, it is more pressing to enact a law for the patient to access a universal palliative care service at the end of their life than a law on euthanasia. This article discusses the difference between euthanasia and (...) palliative care. It is affirmed that being in favor of the latter does not mean wanting the patient to be in pain and to suffer, nor extending their life unreasonably. However, opting for palliative care is, on the one hand, what the physician can ethically do as a health worker and, on the other, the only morally acceptable alternative to the natural end of human beings that is appropriate to their dignity. Therefore, palliative care immerses us in an anthropological reflection about our own limitation and fragility, since respect for human life and palliative care dignify the process of dying.
