However, the definition and measurement of the therapeutic misconception is a subject of continuing debate. Our qualitative pilot study of persons enrolled in a phase I trial of gene transfer for Parkinson disease suggests potential avenues for both measuring and preventing the therapeutic misconception. Building on earlier literature on the topic, we developed and (...) tested an interview guide that focuses on how the subjects decided to participate, emphasizing the integration of subjects’ various statements that are relevant to assessing the therapeutic misconception, rather than evaluating them as isolated statements. Robotic or automatic milking systems are novel technologies that take over the labor of dairy farming and reduce the need for human–animal interactions. Because robotic milking involves the replacement of ‘conventional’ twice-a-day milking managed by people with a system that supposedly allows cows the freedom to be milked automatically whenever they choose, some claim robotic milking has health and welfare benefits for cows, increases productivity, and has lifestyle advantages for dairy farmers. This paper examines how established ethical relations on dairy (...) farms are unsettled by the intervention of a radically different technology such as AMS. The renegotiation of ethical relationships is thus an important dimension of how the actors involved are re-assembled around a new technology. The paper draws on in-depth research on UK dairy farms comparing those using conventional milking technologies with those using AMS. We explore the situated ethical relations that are negotiated in practice, focusing on the contingent and complex nature of human–animal–technology interactions. We show that ethical relations are situated and emergent, and that as the identities, roles, and subjectivities of humans and animals are unsettled through the intervention of a new technology, the ethical relations also shift. On the one hand, freedom is said to be the property of a subject. On the other, freedom only happens in the space of being-in-common. Freedom, then, is the place of a conflict between the “self” and the “with,” between independence or autonomy and dependence or sharing. Resolving this apparent antinomy requires showing how the with ontologically constitutes the self. This article focuses on emergency medical care in black urban populations, suggesting that the classification of a "community" within clinical trial language is problematic. The article references a cultural history of black Americans with pre-hospital emergency medical treatment as relevant to contemporary emergency medicine paradigms. Part I explores a relationship between "autonomy" and "community." The idea of community emerges as a displacement for the ethical principle of autonomy precisely at the moment that institutionalized medicine focuses on diversity. Part II examines (...) a clinical trial for the blood substitute PolyHeme® (Northfield Laboratories, Inc., Evanston, IL). It illustrates the ways in which bias in research paradigms and Institutional Review Board decisions attach to the notion and utility of the language of "community." The conclusion's contemporary anecdote makes apparent the vitality of the issues of prehospital emergency medical care and the ways in which decisions and practices fall too easily into a narrative of culturally biased treatment. In recent decades, academic science has increasingly been directed toward commercializable ends by neoliberal governments. In this article, I outline a concern that academic scientists have not been consulted about the transformation of science, but nevertheless, in some ways accept commercialization as the way things are done. I focus on the ways in which academic scientists attempt to exercise agency, albeit within the parameters of the neoliberal knowledge economy. In this economy, scientific inquiry has transformed to be focused more on (...) producing marketable products. In order to explore the parameters of scientists’ agency in the context of that transformation, I first elaborate on the idea of agency’s “parameters” and argue that the literature on commercialization lacks attention to how researchers’ agency is encouraged and discouraged in the context of academic research in the United States and Canada; second, I make a case for using the concept of hegemony to understand the ideas and practices of contemporary science; third, I propose a methodological direction that can attend to researchers’ agency in the contemporary context of the neoliberal knowledge economy. This paper explores the analytical relevance of Foucault's notion of biopower in the context of regulating and managing non-human lives and populations, specifically those animals that are the focus of livestock breeding based on genetic techniques. The concept of biopower is seen as offering theoretical possibilities precisely because it is concerned with the regulation of life and of populations. The paper approaches the task of testing the 'analytic mettle' of biopower through an analysis of four policy documents concerned with farm (...) animal genetics: the UK's National Scrapie Plan (2003); the UK National Action Plan on Farm Animal Genetic Resources (2006); the Agriculture and Environment Biotechnology Committee's report on Animals and Biotechnology (2002); and the Farm Animal Welfare Council's report on the Welfare Implications of Animal Breeding and Breeding Technologies in Commercial Agriculture (2004). Of interest is whether and how the four policy case studies articulate a form of biopower in relation to human-livestock animal relations in the context of genetic approaches to livestock breeding, and how biopower is variably expressed in relation to the different policy issues addressed. In concluding, the paper considers the overall applicability and relevance of biopower in the context of regulating animal lives within livestock breeding, highlighting both possibilities and limitations, and offers suggestions for taking forward research on livestock populations from a neo-Foucaultian perspective. This article is concerned with ethical issues that have to be considered when under taking qualitative research. Some of the issues - such as informed consent, the dignity and privacy of the research subjects, voluntary participation and protection from harm - are the same as in other types of research and have their basis in moral and ethical principles. Qualitative research, however, generates specific ethical problems because of the close relationship that researchers form with participants. Qualitative research with patients is (...) especially difficult because of their vulnerability and lack of power in the clinical situation. Therefore the potential conflict between the dual role of the nurse - the professional and the research roles - has to be solved. Researchers also learn how to cope with the tension of subjective and objective elements of the research. Nurses who attempt qualitative research have to consider a variety of complex ethical issues, which are addressed in this paper. (shrink)

Bioethicists have long been concerned that seriously ill patients entering early phase (‘phase I’) treatment trials are motivated by therapeutic benefit even though the likelihood of benefit is low. In spite of these concerns, consent forms for phase I studies involving seriously ill patients generally employ indeterminate benefit statements rather than unambiguous statements of unlikely benefit. This seeming mismatch between attitudes and actions suggests a need to better understand research ethics committee members’ attitudes toward communication of potential benefits and risks (...) of early phase studies to potential subjects. We surveyed the members of two U.S. research ethics committees using a phase I gene transfer study scenario, and compared the results to a previous survey of potential subjects’ perceptions and attitudes toward benefit and risk for the same protocol. The results show that there is indeed a gap between the subjects’ perceptions and the committee members’ views on what is appropriate to be communicated to research subjects. This discrepancy is the product of both the commonly assumed optimism of the subjects and to a “protective pessimism” of the research ethics committee members. We discuss this discrepancy using “frameworks of trust” and demonstrate the need to incorporate these frameworks into the existing model of informed consent. (shrink)

In 300 BCE, the tutor of the heir-apparent to the Chu throne was laid to rest in a tomb at Jingmen, Hubei province in central China. A corpus of bamboo-strip texts that recorded the philosophical teachings of an era was buried with him. The tomb was sealed, and China quickly became the theater of the Qin conquest, an event that proved to be one of the most significant in ancient history. For over two millennia, the texts were forgotten. But in (...) October 1993, they were unearthed. The discovery of the Guodian texts, together with other recently discovered Warring States manuscripts, has revolutionized the study of early Chinese intellectual history. Kenneth Holloway argues that the Guodian corpus puts forth a political philosophy based on the harmonious interconnection of individuals engaged in moral self-cultivation. This unique worldview, says Holloway, cannot meaningfully be categorized as "Confucian" or "Daoist," because it shares important concepts and vocabulary with a number of different textual traditions that have anachronistically been characterized as competing or incompatible "schools" of thought. He finds that within the Guodian corpus familiar philosophical concepts and texts are applied in distinctive ways, presenting a worldview that is quite different from the received textual traditions. In the corpus, the most important function of government is to assist in the harmonization of state and family relations. It sees the relationship between these two entities - the family and the collection of families that ultimately constitute the state - as being inherently conflicting social groupings. The texts posit an interesting solution: State and family disharmony can be overcome by developing a hybrid government that employs both meritocratic and aristocratic methods. Without knowledge of the emphasis on hybridization found in the Guodian texts, however, scholars were unable to understand the interrelationships between these two methods of government. This new understanding illuminates central issues of government, religion, and philosophy in early China that were overlooked in received texts. As part of the contribution to our understanding of this particular body of texts, Holloway proposes a methodology for assessing a corpus of texts without relying on assumptions and definitions that derive from two thousand years of scholarship. The Guodian corpus, and Holloway's analysis of it, are now absolutely indispensable to any student or scholar of ancient Chinese intellectual history. (shrink)

During the set-up phase of an international study of genetic influences on outcomes from sepsis, we aimed to characterise potential differences in ethics approval processes and outcomes in participating European countries. Between 2005 and 2007 of the FP6-funded international Genetics Of Sepsis and Septic Shock project, we asked national coordinators to complete a structured survey of research ethic committee approval structures and processes in their countries, and linked these data to outcomes. Survey findings were reconfirmed or modified in 2017. Eighteen (...) countries participated in the study, recruiting 2257 patients from 160 ICUs. National practices differed widely in terms of composition of RECs, procedures and duration of the ethics approval process. Eight countries used a single centralised process for approval, seven required approval by an ethics committee in each participating hospital, and three required both. Outcomes of the application process differed widely between countries because of differences in national legislation, and differed within countries because of interpretation of the ethics of conducting research in patients lacking capacity. The RECs in four countries had no lay representation. The median time from submission to final decision was 1.5 months; in nine approval was received within 1 month; six took over 6 months, and in one 24 months; had all countries been able to match the most efficient approvals processes, an additional 74 months of country or institution-level recruitment would have been available. In three countries, rejection of the application by some local RECs resulted in loss of centres; and one country rejected the application outright. The potential benefits of the single application portal offered by the European Clinical Trials Regulation will not be realised without harmonisation of research ethics committee practices as well as national legislation. Design features for language and stone toolmaking (not tool use) involve similar if not homologous cognitive processes. Both are arbitrary transformations of internal symbolization, whereas non-human tool using is mostly an iconic transformation. The major discontinuity between humans and non-humans (chimpanzees) is language. The presence of stone tools made to standardized patterns suggests communicative and social control skills that involved language.

This paper describes neoliberalism and summarizes new works on democracy in Continental philosophy. Unlike laissez-faire or liberal economic theory—a “leave us alone” strategy in which the state does not interfere with private enterprise—neoliberal governments use the resources of the state to assist the market directly and employ the market to direct or oversee the resources of the state. Alongside neoliberal government, and in its wake, is a society in which the guiding axioms for each human being are self-entrepreneurship and competition. (...) Over the last decade, however, a new body of philosophical work has been dissociating democracy from neoliberal government, critiquing a failed system of political representation, and considering to what extent democracy must take place beyond or outside of the current state. Of equal concern to these philosophers is how to take flight from a way of life that is characterized by self-entrepreneurship and competition. For some, the start of a political future beyond neoliberalism hinges upon a recent distinction between constituent and destituent forms of power. Whereas constituent power attempts to reform one’s government through demonstrations in public space, destituent power abandons the project of reforming one’s government momentarily or even completely in order to experience another form of life entirely. Epigenetic research has brought several important technological achievements, including identifying epigenetic clocks and signatures, and developing epigenetic editing. The potential military applications of such technologies we discuss are stratifying soldiers’ health, exposure to trauma using epigenetic testing, information about biological clocks, confirming child soldiers’ minor status using epigenetic clocks, and inducing epigenetic modifications in soldiers. These uses could become a reality. This article presents a comprehensive literature review, and analysis by interdisciplinary experts of the scientific, legal, ethical, and societal issues (...) surrounding epigenetics and the military. Notwithstanding the potential benefit from these applications, our findings indicate that the current lack of scientific validation for epigenetic technologies suggests a careful scientific review and the establishment of a robust governance framework before consideration for use in the military. In this article, we highlight general concerns about the application of epigenetic technologies in the military context, especially discrimination and data privacy issues if soldiers are used as research subjects. We also highlight the potential of epigenetic clocks to support child soldiers’ rights and ethical questions about using epigenetic engineering for soldiers’ enhancement and conclude with considerations for an ethical framework for epigenetic applications in the military, defense, and security contexts. Interventional clinical studies of convalescent plasma to treat COVID-19 were predominantly funded and led by public sector actors, including blood services operators. We aimed to analyze the processes of clinical studies of convalescent plasma to understand alternatives to pharmaceutical industry biopharmaceutical research and development, particularly where public sector actors play a dominant role. We conducted a qualitative, critical case study of purposively sampled prominent and impactful clinical studies of convalescent plasma during 2020-2021.

This paper presents a novel perspective on an evolving policy area. The UK’s withdrawal from the EU has led to the creation of a new Agriculture Act and proposals for significant changes to the way farming subsidies are structured in England. Underpinned by a ‘public money for public goods’ approach, where public goods are those outputs from the farm system which are not rewarded by markets, yet which provide benefits to many members of society. New schemes include the Animal Health (...) and Welfare Pathway, where certain aspects of farm animal health and welfare (FAHW) will be subsidised through government support, raising a much-debated issue in the literature regarding the representation of FAHW as a public good. For policy to be responsive to societal demands and accountable to citizens, understanding public attitudes and preferences towards FAHW as a public good, and how the public might prioritise this in relation to a wider suite of environmental public goods from farming, is important. An online survey of 521 members of the UK public was conducted and analysed with descriptive statistics and ordered logistic regression. Findings reveal low awareness of the changing agricultural policy context, but strong support for public money being used to provide public goods, particularly for FAHW. Findings also indicate a need for more effective public communication of farming and FAHW issues from farming stakeholders to ensure public policy in this domain is responsive and accountable to its citizens. Further work is needed to inform future debates and engagement surrounding FAHW, including through which combination of funding mechanisms (public or private) it is provided. (shrink)

Malignant tumours are often characterised by significant rearrangement of the genome. This may be visible in the form of a deranged karyotype with both loss and gain of DNA sequences extending from chromosomal regions to whole chromosomes. In several tumour types, however, gross genomic derangements are minimal, and tumour cells contain one or more additional (supernumerary) chromosomes that may be unrecognisable in terms of a single origin. In this review we term such chromosomes cancer‐associated neochromosomes (CaNCs). In the absence of (...) other identified genomic abnormalities, and because the CaNC is a common feature of the cancer type, it is hypothesised that the genetic alterations required for cell transformation are contained within its structure. In this review, we discuss the potential impact of modern genomic technologies on our understanding of the nature and causes of CaNC formation, which is central to several cancer types, exemplified here by well‐differentiated liposarcoma.
