Metaphysical realism about mental disorder is the thesis that mental disorder exists mind-independently. There are two ways to challenge metaphysical realism about mental disorder. The first is by denying that mental disorder exists. The second is by denying that mental disorder exists mind-independently. Or, differently put, by arguing that mental disorder is mind-dependent. The aim of this paper is three-fold: (a) to examine three ways in which mental disorder can be said to be mind-dependent (namely, by being causally dependent on (...) the human mind, by being weakly dependent on human attitudes, and by being strongly dependent on human attitudes), (b) to clarify their differences, and (c) to discuss their implications regarding metaphysical realism about mental disorder. I argue that mental disorder being mind-dependent in the first two senses is compatible with metaphysical realism about mental disorder, whereas mental disorder being mind-dependent in the third sense is not. In this chapter, I first review and assess evidence regarding brain damage or neural abnormalities associated with some psychopathologies and cognitive deficits, such as hemispatial neglect, agnosias, amnesia, somatoparaphrenia, and others. It becomes clear just how closely normal mental functioning and consciousness depend upon normal brain functioning as well as how some very specific mental changes occur when, and only when, very specific brain damage occurs. I then explore the metaphysical implications of these results with respect to the nature of (...) mind and consciousness. In particular, I examine the plausibility of materialism, roughly the view that mental processes are brain processes, in light of the evidence discussed and in contrast to a dualist conception of the mind (whereby mental states are not physical in some sense). I also explore the prospects for a conscious afterlife based both on the brain damage evidence adduced and the metaphysical implications discussed. (shrink)

Since psychiatry remains a descriptive discipline, it is essential for its practitioners to understand how the language of psychiatry came to be formed. This important book, written by a psychiatrist-historian, traces the genesis of the descriptive categories of psychopathology and examines their interaction with the psychological and philosophical context within which they arose. The author explores particularly the language and ideas that have characterised descriptive psychopathology from the mid-nineteenth century to the present day. Tracing the evolution of concepts such as memory, consciousness, will and personality, and of symptoms ranging from catalepsy and aboulia to anxiety and self-harm, this book provides fascinating insights into the subjective nature of mental illness, and into the ideas of British, Continental and American authorities who sought to clarify and define it. This thesis investigates the nature of psychiatric disorders, and to what extent they can form a basis for classification, explanation, and treatment interventions. These questions are important in the light of the “crisis of validity” in psychiatry, according to which current diagnostic categories do not pick out real disorders. I address the questions by defending an account of psychiatric disorders that can better accommodate social aspects and non-epistemic values than the symptom-based model of the Diagnostic and Statistical Manual of Mental (...) Disorders (DSM) and the brain disease model of the biomedical approach, including the Research Domain Criteria (RDoC). I concentrate on three ways that psychiatric disorders can differ from prototypical natural kinds, such as biological species and chemical elements. First, the concept of psychiatric disorder or mental disorder is partly value-laden and cannot be defined based only on scientific facts. Second, the objects of psychiatric classifications are interactive human kinds because people with disorders respond to their classifications through looping effects. Third, sociocultural factors can shape disorders in complex ways, which is indicated by their cross-cultural variation. I argue that these challenges can be overcome with a pluralistic explanatory account that grants an explicit role to value-sensitive considerations and social scientific research. Based on this, I argue that particular disorders can in principle support inductive inferences. ------------------------------------------------------------------------------------------ My key argument is that although whether a condition is considered pathological is partly a value-laden question, this does not rule out a realist account of particular psychiatric disorders as homeostatic property cluster kinds (HPC). I assert that causal mechanisms responsible for psychiatric kinds can be understood non-metaphysically based on the contrastive-counterfactual and interventionist theories of explanation. An advantage of this account is that it can address the concern that psychiatric explanations require one to relate heterogeneous causal factors on different levels, such as genetic, neurological, psychological, and social. My novel argument is that mechanistic explanations of scientific kinds have applicability domains over which they account for specific aspects of kinds and warrant inductive inferences. That is, identifying the applicability domain of an explanation spells out the conditions under which the explanation is expected to be reliable and when it can break down. This helps to understand how different discipline approaches, including social and cultural ones, can be complementary and make an explanation of a psychiatric kind more reliable. On the other hand, the account also shows how the complex nature of psychiatric kinds can license re-classifications for different epistemic and non-epistemic purposes. Finally, in the light of the social implications of psychiatric classifications, I suggest a value-sensitive or ameliorative approach to engineering the concept of psychiatric disorder. In conclusion, my account shows that research on the social and cultural factors that shape psychiatric disorders, and weighing the non-epistemic values that influence classificatory practices, can and should inform classificatory choices as well as policy and treatment interventions. In this paper the author presents two different modes of relationship between phenomenological psychopathology and philosophy. The dominant mode conforms to the medical-psychiatric discourse which takes pathological time experiences as negative deviations from the ‘normal’ and ‘adequate’ equivalent. In this mode phenomenological description of ‘disturbed’ time experiences requires philosophy to provide an insight into the ‘essence’ of time and an essentially adequate experience of time. Only such a philosophical insight can deliver a valid reference point for investigating what is (...) really missing in pathological time experiences. This philosophical reference point can either consist in a metaphysical-normative concept of time and time experiences, or in a transcendental concept of how normal and abnormal time experiences are constituted in the inner consciousness of time. There is another mode of relationship which conforms to Freud’s discovery that pathological symptoms are not just negative alterations of the ‘normal’ but hide an own ‘meaning’. The author makes the case for the use of philosophy to unveil the specific kind of meaning in pathological experiences of time. She refers to existential philosophy in this respect, and in particular to its concept of Angst as a fundamental philosophical experience. This allows for a hermeneutic approach to ‘disturbed’ time experiences. What looks from a pure psychiatric perspective to be a mere ‘disorder’ of the ‘normal’, becomes understandable as a specific form of ‘suffering from one’s own temporality’. She argues that people with disturbed time experiences are especially sensitive to anxiety-laden experiences of what it means to be temporal, whilst ‘normal’ people can screen them out in favor of a ‘mentally undisturbed’ everyday life. Accounts of mental disorders focusing either on the brain as neurophysiological substrate or on systematic connections between symptoms are insufficient to account for the multifactorial nature of mental illnesses. Recently, multiplexes have been suggested to provide a holistic view of psychopathology that integrates data from different factors, at different scales, or across time. Intuitively, these multi-layered network structures present quite appealing models of mental disorders that can be constructed by powerful computational machinery based on increasing amounts of real-world data. (...) In this paper, I systematically examine what challenges psychopathology models face and to what extent different species of psychopathology models can address them. My analysis highlights that while multiplexes, as they are usually conceived, appear promising, they suffer from the same problems as other approaches. To remedy this, I suggest, we must go a step further and combine different kinds of multiplexes into 4D models. Once we embrace 4D multiplexes and identify appropriate ways to constrain them, we might unlock the true potential of multiplexes for making headway in psychopathology research. (shrink)

Philosophical psychopathology lies at the intersection of philosophy and psychiatry. The name is new. The field is not. This paper surveys work in the field since about 1980. Special attention is given to work on two topics: mental illness semantics and the metaphysics of disorders of self-consciousness.

The "width" of the mind is an important topic in contemporary philosophical psychology. Support for active externalism derives from theoretical, engineering, and observational perspectives. Given the history of psychology, psychopathology is notable in its absence from the list of avenues of support for the idea that some cognitive processes extend beyond the physical bounds of the organism in question. The current project is to defend the possibility, plausibility, and desirability of externalist psychopathology. Doing so both adds to the (...) case for externalism and suggests ways of improving our study of cognitive dysfunction. I establish the possibility of externalist psychopathology through the development of models of wide cognitive processing, and, by implication, failure of such processing, from the work of S.L. Hurley and Robert Wilson. The plausibility of wide conceptualization and explanation of cognitive disorders is shown through an examination of apraxia, disorders of learned, skilled movements. The desirability of externalist psychopathology is suggested through a look at theoretical and therapeutic virtues, again drawing on Wilson's work. (shrink)

Although delusion is one of the central concepts of psychopathology, it stills eludes precise conceptualization. In this paper, I present certain basic issues concerning the classification and definition of delusion, as well as its ontological status. By examining these issues, I aim to shed light on the ambiguity of the clinical term ‘delusion’ and its extension, as well as provide clues as to why philosophers are increasingly joining the ranks of psychiatrists, psychologists, and neuroscientists in the effort to come (...) to a comprehensive understanding of delusion. Cases like Femi’s (Rashed, 2010) illustrate how diagnosing and treating psychotic symptoms with religious content can perpetuate this injustice. Femi’s symptoms, which were very real, were interpreted solely as indicative of a psychotic episode, (...) without considering the possibility of a religious experience. Although hermeneutical injustice is often caused by negative stereotypes, lack of conceptual resources, implicit biases, practices of epistemic privileging, patronizing attitudes, and wrongful pathologization, I propose that interpretive exclusivism about religious psychopathology plays a pivotal role in causing hermeneutical injustice when attributing psychotic symptoms with religious content—that is, the view that an experience is either psychopathological or religious (Scrutton, 2023). Routinely, on attributing psychosis, the potential for the patient to have a genuine religious experience is disregarded and excluded from the therapeutic process and from the very realm of possibility. Why?In their thoughtful commentary, Rosa Ritunnano and Ian James Kidd do me the favor of not only giving careful attention to my argument, but also elucidating my position and providing avenues for further clarification. Although I have not gone much into what is behind interpretive exclusivism, they are right to point to unquestioning adherence to some form of naturalism. Contrary to what they intimate, however, I do not think we are speaking of different kinds of hermeneutical injustice. As I see it, we are speaking about the same kind of injustice, only at different levels of explanation. In my article, I rhetorically ask why [End Page 241] it is assumed that psychotic and religious experiences cannot coexist, and why it is assumed that the same experience cannot be both a genuine religious experience and an instance of psycho-pathology. Ritunnano and Kidd point in the right direction when they say, “Some interpretations of kinds of experience with religious content can be undermined if one is committed to a metaphysical naturalism” (2024, p. 235).Naturalism can be divided into two components: metaphysical and methodological (Papineau, 2020). The metaphysical component concerns the contents of reality, affirming the absence of “supernatural” or other metaphysically elusive entities. In contrast, the methodological component focuses on approaches to investigating reality and attributes a general authority to the scientific method. Therefore, metaphysical naturalism is the more comprehensive view, as it outright denies the existence of the supernatural. Meanwhile, methodological (or scientific) naturalism refers to a pragmatic approach within scientific inquiry, stating that the right methodology to investigate and research something (where this something might not be immediately obvious to be a scientific topic) is that of science, often particular of natural sciences.Although a commitment to metaphysical naturalism may at times be what lies behind interpretive exclusivism, we do not need to go that far, since a commitment to methodological naturalism is enough to breed unwarranted ontological exclusion in practice (Plantinga, 1997; Ratcliffe, 2003). This in turn leads to interpretive exclusivism which is, I argue, the main theoretical flaw behind the kind of hermeneutical injustice I am trying to flesh out. As Ritunnano and Kidd observe, either tacit or explicit adherence to such ontological exclusion causes individuals to struggle to view another person’s spiritual or religious interpretive framework as coherent or even intelligible. Note, however, that the opposite of interpretive exclusivism—that is, interpretive inclusivism—does not need to affirm that the experience is exclusively or even genuinely religious but rather keeps that question open while considering significant epistemic and meaning-making possibilities.Finally, in his commentary, Alasdair Coles (2024) proposes that I overlook the fact that some individuals undergo psychoses featuring deeply distressing and dark religious imagery, the dismissal of which can be therapeutically beneficial. He also contends that therapists often default to considering all religious content as inauthentic, contrasting this with my alleged stance, which he believes assumes the opposite extreme and may be equally or more harmful. The Viennese psychiatrist Rudolf Allers has been virtually relegated to oblivion, despite being quite relevant in the past. He developed a founding program in psychiatry, that of psychopathology, and characterology in Catholic anthropology, specifically Thomist. The article briefly discusses Allers’s main ideas on the distinction between person and character, and the relationship between character and values. The objective is to update his potential relevance for a critique of contemporary psychology, independent of any metaphysical reference.

The notion of causation that Mary Shepherd develops in her 1824 An Essay Upon the Relation of Cause and Effect (ERCE) has a number of surprising features that have only recently begun to be studied by scholars. This relation is synchronic, rather than diachronic (ERCE 49-50); it always involves a “mixture” of pre-existing objects (ERCE 46-7); and the effect must be “a new nature, capable of exhibiting qualities varying from those of either of the objects unconjoined” (ERCE 63). In this (...) essay I argue for an emergentist interpretation of Shepherd’s causal theory. On the reading I defend, all effects have qualities that metaphysically emerge from the complex interactions of their constituents. This reading explains the structure of Shepherd’s causal relation and clarifies the central aims of her philosophical project. In response to the problems raised by the science of her time, Shepherd developed a theory of emergence and published it during the period when the concept was first being shaped and adopted by prominent philosophers. This dissertation is a contribution to the contemporary field of phenomenological psychopathology, or the phenomenological study of psychiatric disorders. The work proceeds with two major aims. The first is to show how a phenomenological approach can clarify and illuminate the nature of psychopathology—specifically those conditions typically labeled as major depressive disorder and bipolar disorder. The second is to show how engaging with psychopathological conditions can challenge and undermine many phenomenological presuppositions, especially phenomenology’s status as a transcendental philosophy and (...) its corresponding anti-naturalistic outlook. In the opening chapter, I articulate the three layers of the subject matter of phenomenological research—what I refer to as “existentials,” “modes,” and “prejudices.” As I argue, while each layer contributes to what we might call the “structure” of human existence, they do not do so in the same way, or to the same degree. Because phenomenological psychopathology—and applied phenomenology in general—aims to characterize how the structure of human existence can change and alter, it is paramount that these layers be adequately delineated and defined before investigating these changes. In chapters two through five, I conduct hermeneutic and phenomenological investigations of psychopathological phenomena typically labeled as major depressive disorder or bipolar disorder. These investigations address the affective aspects of depression and mania, and the embodied aspects of depression. In addition to clearly articulating the nature of these phenomena, I show how certain psychopathological conditions involve changes in the deepest or most fundamental layer of human existence—what I refer to as existentials. As I argue, many of the classical phenomenologists (including Husserl and Heidegger) believed that these structural features were necessary, unchanging, and universal. However, this presupposition is challenged through the examination of psychopathological and neuropathological conditions, undermining the status of phenomenology as a transcendental philosophy. While this challenge to classical phenomenology is only sketched in the early chapters, in chapters six and seven I develop it in more detail in order to achieve two distinct ends. In chapter six I argue that psychopathology and neuropathology not only challenge phenomenology’s status as a transcendental philosophy, but also supply a key to developing a phenomenological naturalism (which I contrast with a naturalized phenomenology). Phenomenological naturalism, as I articulate it, is a position in which phenomenology is not subsumed by the metaphysical and methodological framework of the natural sciences, but nonetheless maintains the capacity to investigate how the natural world stands independent of human subjectivity (and how events in the natural world can bring about changes in the most fundamental structures of human existence). In the seventh chapter I argue that a phenomenology in which existentials are contingent and variable rather than necessary and unchanging allows phenomenologists to contribute to new dimensional approaches to psychiatric classification. Rather than begin from distinct categories of disorder, these approaches begin from distinct core features of human existence. These features, referred to as either dimensions or constructs, can vary in degree and are studied in both normal and pathological forms. (shrink)

Metaphysics is traditionally conceived as aiming at the truth — indeed, the most fundamental truths about the most general features of reality. Philosophical naturalists, urging that philosophical claims be grounded on science, have often assumed an eliminativist attitude towards metaphysics, consequently paying little attention to such a definition. In the more recent literature, however, naturalism has instead been taken to entail that the traditional conception of metaphysics can be accepted if and only if one is a scientific (...) realist (and puts the right constraints on acceptable metaphysical claims). Here, we want to suggest that naturalists can, and perhaps should, pick a third option, based on a significant yet acceptable revision of the established understanding of metaphysics. More particularly, we will claim that a fictionalist approach to metaphysics is compatible with both the idea that the discipline inquires into the fundamental features of reality and naturalistic methodology; at the same time, it meshes well with both scientific realism and instrumentalism. Book review: Julies Evola, Metafyzika války. Sol Noctis, 2021, 176 p.

Kant first publicized the concept of “metaphysics of morals” in the Critique of Pure Reason and further justified it in other works. However, we can state some distinctive features and perspectives of this project. There are reasons to single out three meanings of this concept: the entire project of pure moral philosophy (MM1), the late work with the same name Metaphysics of Morals (1797) (MM2), the chapter in Groundwork (1785) on “Transition from popular moral philosophy to the (...) class='Hi'>metaphysics of morals” (MM3). It is obvious that MM2 and MM3 belong to MM1 as a whole systematic project of the metaphysics of morals. Among Kant scholars was established the view that Kant’s project of “metaphysics of morals” was “completely new”, and the concept itself is Kant’s “creature” (G. Tonelli, R. Brandt). Instead, recent research shows that this project belongs to the modern tradition of universal ethics (Pufendorf, Wolff and his school). (shrink)

Realists about mental disorder have been hasty about dismissing social explanations of how mental disorder is constituted. However, many social ontologies are realist ontologies. In order to create a meaningful distinction between realism and social metaphysics about mental disorder, I propose that realism about mental disorder is best understood as Individual Trait Realism (ITR) about them. For ITR, mental disorders exist in virtue of traits. I defend the view that ITR is compatible with social metaphysics, arguing that, in (...) asking whether constituents in the social sphere figure the metaphysics of psychopathology, we are asking questions on three different strata of explanation: the strata of demarcation, instantiation, and individual traits. Distinguishing between these strata allows for nuanced realism that need not reject the social constitution of mental disorder. (shrink)

An under-explored intermediate position between traditional materialism and traditional idealism is the view that although the spatiotemporal world is purely material, minds nonetheless have a metaphysically special place in it. One way this can be is via a special role that subjects have in the metaphysics of material objects. Some metaphysical aspect of material objects might require the existence of subjects. This would support that minds must exist if material objects exist and thus that a mindless material world is (...) impossible. This view, labeled the subjectivity thesis by Anton Friedrich Koch, was defended by him with an intriguing, purely metaphysical argument connected to the individuation of material objects in space and time. The present paper hopes to make progress on assessing the viability of such a position. It starts by critically examining Koch's argument for the subjectivity thesis, as well as similar arguments that give minds a central place in the metaphysics of material objects via considerations about identity and difference. It then compares these ideas to similar ones in the philosophy of time, and concludes with an outlook on whether such a position is viable and what needs to be done to fill the gaps unearthed along the way. The present trend towards an atheoretical statistical method of psychiatric classification has prompted many psychiatrists to conceive of "mental disorder", or for that matter any other psychopathological designation, as an indexical cluster of properties and events more than a distinct psychological impairment. By employing different combinations of inclusion and exclusion criteria, the current American Psychiatric Association's scheme (called DSM-III) hopes to avoid the over-selectivity of more metaphysical systems and thereby provide the clinician with a flexible means of dealing with a (...) wide diversity of cases. In the hope of redirecting future inquiry, the paper will argue: (1) that this recent trend might appear to be clinically beneficial, but in point of fact it is riddled by unsound theoretical conclusions which leave the field without a deeply reaching base for understanding and treating mental disorder, and (2) that, a fortiori, "mental disorder" is best conceived as not a cluster of properties and events, nor a metaphorical reaction to a breakdown in social interpersonal relations, but as a deeply laid condition characterized by the absence of an imaginally integrated system. G. H. Mead developed an alternative "metaphysics" of selfhood and agency that underlies, but is seldom made explicit in discussions of, his social developmental psychology. This is an alternative metaphysics that rejects any pregiven, fixed foundations for being and knowing. It assumes the emergence of social psychological phenomena such as mind, self, and deliberative agency through the activity of human actors and interactors within their biophysical and sociocultural world. Of central importance to the emergence of self-consciousness and deliberative (...) forms of human agency is the ability of developing individuals to take the perspectives of others within their own conduct. However, precisely what is involved in this process, and whether or not it assumes the preexistence of exactly those forms of selfhood and agency, the emergence of which it seeks to explain, have been much disputed. In this essay, recent reinterpretations of Mead's "metaphysics" are used to argue that his overall theoretical program contains resources necessary to overcome charges of ontological and epistemological circularity. In particular, once his interactionism, perspectivism, emergentism, and compatibilism are explicated, it is possible to see that Mead's "taking the perspective of the other" is not primarily a process of internalization that requires a preexisting "internalizer," but a process of participation with others within conventionalized sequences of interactivity. This participatory process enables the development of prereflective forms of perspective taking and self-other differentiation that serve as necessary conditions for the further emergence of more reflective forms of perspective taking, self-consciousness, and deliberative agency. Compatibilist interpretations of deliberative agency, thus understood, also are discussed. (shrink)

How ironic that my application of pragmatism to psychopathology has been (...) charged with not being sufficiently pluralistic because it tries to fit all psychiatric disorders into the Procrustean bed of practical kinds. The Practical Kinds Model in General As a point of clarification, the practical kinds model is not a description of a type of category combined with an argument that all psychiatric disorders should be seen as belonging to that type. If it were that kind of model, I would fully agree with Haslam that practical kinds, fuzzy kinds, and discrete kinds are all potential types of psychiatric disorders. Like Nick Haslam, I believe that there are many different things in the DSM-IV and the ICD-10, and they cannot neatly be classified as the same type of thing.Rather than identifying a type of category, the practical kinds model is a general theory of scientific classification. It is a nonessentialistic theory. Essentialism is the view that things have real essences. In the Aristotelian version of essentialism, an essence is an internal structure, and it is the internal structure that makes anything be the kind of thing it really is. In this classical category model, category members either have the essence or they do not, and everything that has the appropriate essence is a member of that category. The world is supposed to therefore have a well-defined structure, and the job of the scientist is to discover that structure. Ian Hacking (1999) has termed this view of categorization inherent structurism.The practical kinds model does not deny that things have internal structures; it only denies that internal structure by itself determines category membership. Internal structure is an important consideration in identifying categories, but external criteria can always play a role. External or relational criteria play more important roles in some cases than they do in others, but they cannot be eliminated once and for all. Essentialistic theories, which favor an almost exclusive focus on inherent structures, have the advantage of being more parsimonious, but they do so by ignoring real complexity.The best way to illustrate this point is to examine a category whose identification is heavily weighted in terms of internal criteria, or an inherent structure, such as a chemical element. [End Page 219] For example, hydrogen is defined as the element that has an atomic number of 1. This means that it has one proton in its nucleus and normally one electron orbiting its nucleus. The atomic number reflects an internal property, and anything that has an atomic number of 1 is hydrogen. The same is true for the other elements. Anything that has an atomic number of 79 is gold; anything that has an atomic number of 19 is potassium, and so on. Also part of the essentialistic view is the notion that all the important properties of an element are systematically (or lawfully) related to its internal structure.It would be foolish to deny the practical utility of the atomic theory of chemistry. But even in this strongest of cases, external criteria can potentially play a role in how we decide to carve up the world and what we identify as the relevant categories. The point can be clearly illustrated with a Gedanken experiment based on the ideas of Donnellan (1983). Assume that we were to make contact with extraterrestrial life, and the result of that contact was our initiation into a larger galactic community like those portrayed on Star Trek or Babylon 5. Autism spectrum disorder (ASD) is a psychopathological condition characterized by persistent deficits in social interaction and communication, and restricted, repetitive patterns of behavior and interests. To build an ecological-enactive account of autism, I propose we should endorse the affordance-based approach of the skilled intentionality framework (SIF). In SIF, embodied cognition is understood as skilled engagement with affordances in the sociomaterial environment of the ecological niche by which an individual tends toward the optimal grip. The human econiche offers a whole landscape (...) of affordances, and situated individuals respond to a field of relevant affordances. An important part of SIF is an ecological-enactive interpretation of the free energy principle and predictive processing. Predictive processing accounts indicate that in ASD too much precision is assigned to prediction errors. Autistic persons depend heavily on current sensory information and less on prior beliefs and cannot attune to stable regularities. To reduce uncertainty, they over-rely on routines, strict habits, and a familiar environment—a predictable ecological niche they construct. I argue that skilled intentionality gives us the framework from which to analyze the autistic field of affordances. Autistic patterns of affordance-related bodily states of action readiness are only sensitive to very specific solicitations in the environment and achieve optimal grip in well-known situations. Autism is to be understood as a disorder of bodily normativity. Taking this approach helps us figure out what neurotypical people can do to attune their environment in order to scaffold the needs of autistic individuals by redesigning the landscape of affordances.
