The study of the autonomic nervous system (ANS) function has shown to provide useful indicators for risk stratification and early detection on a variety of cardiovascular pathologies. However, data gathered during different tests of the ANS are difficult to analyse, mainly due to the complex mechanisms involved in the autonomic regulation of the cardiovascular system (CVS). Although model-based analysis of ANS data has been already proposed as a way to cope with this complexity, only a few models coupling the main (...) elements involved have been presented in the literature. In this paper, a new model of the CVS, representing the ventricles, the circulatory system and the regulation of the CVS activity by the ANS, is presented. The models of the vascular system and the ventricular activity have been developed using the Bond Graph formalism, as it proposes a unified representation for all energetic domains, facilitating the integration of mechanic and hydraulic phenomena. In order to take into account the electro-mechanical behaviour of both ventricles, an electrophysiologic model of the cardiac action potential, represented by a set of ordinary differential equations, has been integrated. The short-term ANS regulation of heart rate, cardiac contractility and peripheral vasoconstriction is represented by means of continuous transfer functions. These models, represented in different continuous formalisms, are coupled by using a multi-formalism simulation library. Results are presented for two different autonomic tests, namely the Tilt Test and the Valsalva Manoeuvre, by comparing real and simulated signals. (shrink)

The subjective Everettian approach to quantum mechanics presented by Deutsch and Wallace fails to constitute an empirically viable theory of quantum phenomena. The decision theoretic implementation of the Born rule realized in this approach provides no basis for rejecting Everettian quantum mechanics in the face of empirical data that contradicts the Born rule. The approach of Greaves and Myrvold, which provides a subjective implementation of the Born rule as well but derives it from empirical data rather than decision theoretic arguments, (...) avoids the problem faced by Deutsch and Wallace and is empirically viable. Blindsight is the term commonly used to describe visually guided behaviour elicited by a stimulus falling within the scotoma (blind area) caused by a lesion of the striate cortex. Such is normally held to be unconscious and to be mediated by subcortical pathways involving the superior colliculus. Blindsight is of considerable theoretical importance since it suggests that destriate man is more like destriate monkey than had been previously believed and also because it supports the classical notion of two visual systems. It is also of potential clinical importance, since it has been claimed recently that systematic practice in blindsight can lead to the recovery of normal visual function in patients with cortical lesions. From a review of the literature it is concluded that all of the phenomena of blindsight can be attributed either to light scatter into unimpaired parts of the visual field or to residual vision resulting from spared striate cortex. The possible contribution f other factors is also considered. It is concluded that blindsight studies have generally failed to control for such nonblindsight interpretations partly because of poor methodology and partly because of difficulties in defining the term. The ‘right to the truth’ involves disclosing all the pertinent facts to a patient so that an informed decision can be made. However, this concept of a ‘right to the truth’ entails certain ambiguities, especially since it is difficult to apply the concept in medical practice based mainly on current evidence-based data that are probabilistic in nature. Furthermore, in some situations, the doctor is confronted with a moral dilemma, caught between the necessity to inform the patient (principle of autonomy) and (...) the desire to ensure the patient's well-being by minimising suffering (principle of beneficence). To comply with the principle of beneficence as well as the principle of non-maleficence ‘to do no harm’, the doctor may then feel obliged to turn to ‘therapeutic privilege’, using lies or deception to preserve the patient's hope, and psychological and moral integrity, as well as his self-image and dignity. There is no easy answer to such a moral dilemma. This article will propose a process that can fit into reflective practice, allowing the doctor to decide if the use of therapeutic privilege is justified when he is faced with these kinds of conflicting circumstances. We will present the conflict arising in practice in the context of the various theoretical orientations in ethics, and then we will suggest an approach for a ‘practice of truth’. Last, we will situate this reflective method in the broader clinical context of medical practice viewed as a dialogic process. (shrink)

The identification of additional subunits of the cGMP-gated cation channel suggests exciting questions about their regulatory roles and about structure/functional relationships. How do the different subunits interact? How is the complex assembled into the plasma membrane? Divalent cations have been implicated in the regulation of adaptation. One often overlooked cation is magnesium. Could this ion play a role in phototransduction?

We claim that, as it stands, the Deutsch–Wallace–Everett approach to quantum theory is conceptually incoherent. This charge is based upon the approach’s reliance upon decoherence arguments that conflict with its own fundamental precepts regarding probabilistic reasoning in two respects. This conceptual conflict obtains even if the decoherence arguments deployed are aimed merely towards the establishment of certain ‘emergent’ or ‘robust’ structures within the wave function: To be relevant to physical science notions such as robustness must be empirically grounded, and, on (...) our analysis, this grounding can only plausibly be done in precisely the probabilistic terms that lead to conceptual conflict. Thus, the incoherence problems presented necessitate either the provision of a new, non-probabilistic empirical grounding for the notions of robustness and emergence in the context of decoherence, or the abandonment of the Deutsch–Wallace–Everett programme for quantum theory. If you focus your charitable giving on global causes where it will do the most good, how should you feel about passing by the local soup kitchen? Would the ideally virtuous agent have their (local) empathy still activated, but simply overridden by the recognition that distant others are in even greater need, leaving the agent feeling torn? Or would their empathetic impulses be wholeheartedly redirected towards the greatest needs? Consequentialist views have traditionally taken a maximizing form, requiring agents to bring about the very best outcome that they can. But this maximizing function may be questioned. Satisficing views instead allow agents to bring about any outcome that exceeds a satisfactory threshold or qualifies as “good enough.” Scalar consequentialism, by contrast, eschews moral requirements altogether, instead evaluating acts in purely comparative terms, i.e., as better or worse than their alternatives. After surveying the main considerations for and against each of these (...) three views, I argue that the core insights of each are not (despite appearances) in conflict. Consequentialists should be deontic pluralists and accept a maximizing account of the ought of most reason, a satisficing account of obligation, and a scalar account of the weight of reasons. Relevance Theory (RT) argues that human language comprehension processes tend to maximize “relevance,” and postulates that there is a relevance-based procedure that a hearer follows when trying to understand an utterance. Despite being highly influential, RT has been criticized for its failure to explain how speaker-related information, either the speaker’s abilities or her/his preferences, is incorporated into the hearer’s inferential, pragmatic process. An alternative proposal is that speaker-related information gains prominence due to representation of the speaker within higher level goal-directed (...) schemata. Yet the goal-based account is still unable to explain clearly how cross-domain information, for example linguistic meaning and speaker-related knowledge, is integrated within a modular system. On the basis of RT’s cognitive requirements, together with contemporary cognitive theory, we argue that this integration is realized by utilizing working memory and that there exist conversational constraints with which the constructed utterance interpretation should be consistent. We provide an analysis of the empirical consequences of the AdS/CFT duality with reference to the application of the duality in a fundamental theory, effective theory and instrumental context. Analysis of the first two contexts is intended to serve as a guide to the potential empirical and ontological status of gauge/gravity dualities as descriptions of actual physics at the Planck scale. The third context is directly connected to the use of AdS/CFT to describe real quark-gluon plasmas. Richard Swinburne is one of the most important figures in philosophy of religion who took special interest in the soul. Also he is one of the most prominent defenders of dualism –also known as mind-body dualism or substance dualism– that regards humans as composed of two different substances called body and the soul. He defended the dualist view against contemporary problems of dualism and contributed to it with his three books, namely The Evolution of the Soul; Mind, Brain and (...) Free Will and Are We Bodies or Souls? alongside with other publications. In this paper, I will introduce Swinburne’s version of Cartesian dualism and critically investigate whether it provides us some reasons to believe in the soul. For this reason, I will examine the two principle arguments Swinburne offers in the explanations of dualism –one of them is known as the conceivability argument and the other is based on an imaginary scenario from transplants– along with their critics. To sum up, the main point drawn in this paper will be Swinburne’s arguments for dualism along with some of their critics. (shrink)

We study arithmetic and hyperarithmetic degrees of categoricity. We extend a result of E. Fokina, I. Kalimullin, and R. Miller to show that for every computable ordinal $\alpha$, $\mathbf{0}^{}$ is the degree of categoricity of some computable structure $\mathcal{A}$. We show additionally that for $\alpha$ a computable successor ordinal, every degree $2$-c.e. in and above $\mathbf{0}^{}$ is a degree of categoricity. We further prove that every degree of categoricity is hyperarithmetic and show that the index set of structures with degrees (...) of categoricity is $\Pi_{1}^{1}$-complete. (shrink)

BackgroundIn the ruling inY[2018], the UK Supreme Court has confirmed that there is no general requirement for the courts in England and Wales to authorise the withdrawal of clinically assisted nutrition and hydration from patients with prolonged disorders of consciousness. The perceived requirement, which originated in a court ruling in 1993, encompassed those in the vegetative state and those in the minimally conscious state. The ruling inYconfirms that the court may still be approached to decide difficult or contested cases, but (...) there is otherwise no routine requirement that the judges be approached.Main bodyThere is much to welcome in this ruling, particularly as it means that these decisions for these patients are no longer (unusually) singled out for a judicial decision, with all the financial and emotional costs that court proceedings can entail. However, there is also a risk that the ruling might have unwelcome consequences. First, there is the possibility that patients might die too soon, particularly if doctors should now adopt the courts’ previous reasoning, which has suggested that patients in the vegetative state lack interests, so treatment may – perhaps must – be withdrawn. Secondly, there is the converse possibility that patients might live too long, since empirical research suggests that – whether intentionally or not – patients’ families, clinicians, and the health system appear to promote treatment-by-default.ConclusionRather than adopt general positions, which may be contestable and potentially risky, this article argues, on a pluralistic basis, that the individual patient should be the focus of any decision made in his or her ‘best interests’. The existing legal framework in England and Wales, which is provided by the Mental Capacity Act 2005, already points in this direction, although more efforts may be needed to ensure that those involved in making these decisions are suitably educated and supported. Fortunately, new guidance from the British Medical Association could help clinicians and families to make decisions in the future, which are appropriate for the incapacitated individual patient in question. When constrained by limited resources, how do we choose axioms of rationality? The target article relies on Bayesian reasoning that encounter serioustractabilityproblems. We propose another axiomatic foundation: quantum probability theory, which provides for less complex and more comprehensive descriptions. More generally, defining rationality in terms of axiomatic systems misses a key issue: rationality must be defined by humans facing vague information.
