ABSTRACT Medical AID organisations such as Médecins Sans Frontières receive several requests from individuals and international academic institutions to conduct research at their implementation sites in Africa. Do AID agencies have an ethical duty to comply with research requests? In this paper we respond to the views and constructed theories (albeit unfounded) of one such researcher, whose request to conduct research at one of our sites in the Democratic Republic of Congo was turned down.

The purpose of this study is to determine whether there is any unified theory of mental phenomena in the writings of Charles Sanders Peirce . Our thesis is that we find in Peirce's semiotic approach to human consciousness a remarkably unified perspective. ;In order to understand Peirce's reflections on the nature of mind, it is necessary first to situate them in the larger context of his philosophical system. Thus, in the first chapter, we present an overview of his system of (...) thought. In the second chapter, we examine Peirce's concept of sign-activity or what he called semiosis, since this constitutes for him the perspective from which all manifestations of mind must be interpreted. In order to be in a position to grasp Peirce's semiotic approach to mind, it is essential to have a basic understanding of his concept of semiosis. ;In the third chapter, we consider the way in which Peirce's critique of intuitionism cleared the way for his semiotic interpretation of mental phenomena. In the fourth chapter, we examine the particulars of Peirce's attempt to interpret the various modifications of human consciousness as instances of semiosis or sign-activity. ;This semiotic interpretation of mental phenomena is found in Peirce's early writings. But, as a result of later refinements and developments in his understanding of semiosis, Peirce's theory of mind undergoes corresponding refinements and developments. In the fifth and final chapter, we examine Peirce's mature formulation of his semiotic theory, a formulation which gives a prominent place to both habits and self-control. (shrink)

: The essay explores how C. S. Peirce, especially in his mature thought, addressed the question of meaning. It underscores how he not only took meaning to be at bottom a function of our habits but also how he conceived these habits themselves to be functions of the histories in which they originate and operate. Hence, what I propose here is this: One of the most fruitful ways to interpret Peirce's own contribution to this question is to see his efforts (...) as carrying forward the impetus intensified by Hegel and Darwin's emphasis on temporality and historicity.In Part One of this paper , I suggest how Peirce approaches the question of meaning primarily in reference to what I call the drama of self-correction. Moreover, his approach is explicitly related to his pragmatism, semeiotic , and phenomenological doctrine of categories. Finally, the historical development of his own reflections on this question is taken to provide a basis for an historicist interpretation of his mature position. In Part Two , I propose that the metaphor of a route provides a corrective to the excessively formal and indeed formalistic interpretations of Peirce's theory of signs still dominating the discussion of this theory. Routes are understood here not as antecedently fixed paths, but as historically emergent and alterable trajectories defined by the movements of travelers. In conjunction with this emphasis I explore several distinct senses in which both beliefs and signs are what agents go on. Such reflections are articulated for the purpose of facilitating a deeper understand of not only what Peirce meant by meaning but also what we might mean by this elusive term.Key-words: Meaning. This open access book looks at how a democracy can devolve into a post-factual state. The media is being flooded by populist narratives, fake news, conspiracy theories and make-believe. Misinformation is turning into a challenge for all of us, whether politicians, journalists, or citizens. In the age of information, attention is a prime asset and may be converted into money, power, and influence – sometimes at the cost of facts. The point is to obtain exposure on the air and in (...) print media, and to generate traffic on social media platforms. With information in abundance and attention scarce, the competition is ever fiercer with truth all too often becoming the first victim. Reality Lost: Markets of Attention, Misinformation and Manipulation is an analysis by philosophers Vincent F. Hendricks and Mads Vestergaard of the nuts and bolts of the information market, the attention economy and media eco-system which may pave way to postfactual democracy. Here misleading narratives become the basis for political opinion formation, debate, and legislation. To curb this development and the threat it poses to democratic deliberation, political self-determination and freedom, it is necessary that we first grasp the mechanisms and structural conditions that cause it. Background: Moral case deliberation is increasingly becoming part of various Dutch healthcare organizations. Although some evaluation studies of moral case deliberation have been carried out, research into the results of moral case deliberation within aged care is scarce. Research questions: How did participants evaluate moral case deliberation? What has moral case deliberation brought to them? What has moral case deliberation contributed to care practice? Should moral case deliberation be further implemented and, if so, how? Research design: Quantitative analysis of a (...) questionnaire study among participants of moral case deliberation, both caregivers and team leaders. Qualitative analysis of written answers to open questions, interview study and focus group meetings among caregivers and team leaders. Participants and research context: Caregivers and team leaders in a large organization for aged care in the Netherlands. A total of 61 moral case deliberation sessions, carried out on 16 care locations belonging to the organization, were evaluated and perceived results were assessed. Ethical considerations: Participants gave informed consent and anonymity was guaranteed. In the Netherlands, the law does not prescribe independent ethical review by an Institutional Review Board for this kind of research among healthcare professionals. Findings: Moral case deliberation was evaluated positively by the participants. Content and atmosphere of moral case deliberation received high scores, while organizational issues regarding the moral case deliberation sessions scored lower and merit further attention. Respondents indicated that moral case deliberation has the potential to contribute to care practice as relationships among team members improve, more openness is experienced and more understanding for different perspectives is fostered. If moral case deliberation is to be successfully implemented, top-down approaches should go hand in hand with bottom-up approaches. Conclusion: The relevance of moral case deliberation for care practice received wide acknowledgement from the respondents. It can contribute to the team’s cohesion as mutual understanding for one another’s views is fostered. If implemented well, moral case deliberation has the potential to improve care, according to the respondents. (shrink)

It is unclear what proper remuneration for surrogacy is, since countries disagree and both commercial and altruistic surrogacy have ethical drawbacks. In the presence of cross-border surrogacy, these ethical drawbacks are exacerbated. In this article, we explore what would be ethical remuneration for surrogacy, and suggest regulations for how to ensure this in the international context. A normative ethical analysis of commercial surrogacy is conducted. Various arguments against commercial surrogacy are explored, such as exploitation and commodification of surrogates, reproductive capacities, (...) and the child. We argue that, although commodification and exploitation can occur, these problems are not specific to surrogacy but should be understood in the broader context of an unequal world. Moreover, at least some of these arguments are based on symbolic rhetoric or they lack knowledge of real-world experiences. In line with this critique we argue that commercial surrogacy can be justified, but how and under what circumstances depends on the context. Surrogates should be paid a sufficient amount and regulations should be in order. In this article, the Netherlands and India are case examples of contexts that differ in many respects. In both contexts, surrogacy can be seen as a legitimate form of work, which requires the same wage and safety standards as other forms of labor. Payments for surrogacy need to be high enough to avoid exploitation by underpayment, which can be established by the mechanisms of either minimum wage, or Fair-Trade guidelines. An international treaty governing commercial surrogacy should be in place, and local professional bodies to protect the interests of surrogates should be required. Commercial surrogacy should be permitted across the globe, which would also reduce the need for intended parents to seek surrogacy services abroad. (shrink)

Broad genome‐wide testing is increasingly finding its way to the public through the online direct‐to‐consumer marketing of so‐called personal genome tests. Personal genome tests estimate genetic susceptibilities to multiple diseases and other phenotypic traits simultaneously. Providers commonly make use of Terms of Service agreements rather than informed consent procedures. However, to protect consumers from the potential physical, psychological and social harms associated with personal genome testing and to promote autonomous decision‐making with regard to the testing offer, we argue that current (...) practices of information provision are insufficient and that there is a place – and a need – for informed consent in personal genome testing, also when it is offered commercially. The increasing quantity, complexity and diversity of most testing offers, however, pose challenges for information provision and informed consent. Both specific and generic models for informed consent fail to meet its moral aims when applied to personal genome testing. Consumers should be enabled to know the limitations, risks and implications of personal genome testing and should be given control over the genetic information they do or do not wish to obtain. We present the outline of a new model for informed consent which can meet both the norm of providing sufficient information and the norm of providing understandable information. The model can be used for personal genome testing, but will also be applicable to other, future forms of broad genetic testing or screening in commercial and clinical settings. (shrink)

The aim of this research was to gain insight into the experiences and perspectives of individual members of a Medical Research Ethics Committee regarding their individual roles and possible tensions within and between these roles. We conducted a qualitative interview study among members of a large MREC, supplemented by a focus group meeting. Respondents distinguish five roles: protector, facilitator, educator, advisor and assessor. Central to the role of protector is securing valid informed consent and a proper risk-benefit analysis. The role (...) of facilitator implies that respondents want to think along with and assist researchers in order to help medical science progress. As educators, the respondents want to raise ethical and methodological awareness of researchers. The role of advisor implies that respondents bring in their own expertise. The role of assessor points to contributing to the overall evaluation of the research proposal. Various tensions were identified within and between roles. Within the role of protector, a tension is experienced between paternalism and autonomy. Between the role of protector and facilitator tensions occur when the value of a study is questioned while risks and burdens for the subjects are negligible. Within the role of assessor, a tension is felt between the implicit nature of judgments and the need for more explicit formulations. Awareness of various roles and responsibilities may prevent one-sided views on MREC work, not only by members themselves, but also by researchers. Tensions within and between the roles require reflection by MREC members. (shrink)

What are the moral implications of cancel culture? If it is viewed as a means to achieve social justice, we might be more inclined to say that cancel culture is morally good. However, one could argue that cancel culture has too harsh consequences or involves immoral – even hateful – behaviour. We propose that cancel culture is used as an umbrella term for (at least) two different kinds of ‘cancelling’. Cancelling is often seen in public debate as punishment. Following Radzik’s (...) objections to social punishment we argue that this kind of cancelling is morally reprehensible. However, cancel culture as an umbrella term also includes other kinds of cancelling. Many also refer to cancel culture as a phenomenon when someone is being called out or held accountable for their supposedly problematic behaviour. Such cancelling does not need to be punishment, but is often rather an attempt to remove privileged access to the public sphere. In this way, cancelling is used as a tool for redistributing attention: it can (re)claim attention and recognition for marginalized perspectives, by a radical attempt to deny a privileged person access to the public sphere. We conclude that cancel culture as a whole cannot be seen as either morally good or bad, because ‘cancelling’ can be used as a tool for both punishment and redistribution. Each have their own moral implications: cancelling as punishment is reprehensible, but cancelling to redistribute attention might be less of a problem. (shrink)

This chapter discusses how a comparative philosophical model can contribute to both substantive and procedural values in energy policy. We discuss the substantive values in the mainstream light-emitting diodes debate and Taylor Stone’s alternative plea for darkness. We also explore Value Sensitive Design as a procedural approach. We conclude that the comparative philosophical model of Ulrich Libbrecht can appropriately broaden the set of substantive values used in VSD. We discuss the values of ‘by-itself-so’ and ‘alter-intentionality’, which come with the unforeseen (...) necessity of accepting elements from other worldviews and of normativity in the procedural VSD approach. This article examines and assesses Pierre Manent’s critique of the European political project and his concomitant endorsement of the nation-state. It first presents Manent’s basic arguments against the European Union, focusing on his Aristotelian understanding of political forms and his notion of the body politic. Subsequently, it argues that Manent’s position, in part due to its Aristotelian underpinnings, is deeply problematic, in that it disregards the contingency and the element of representation that are necessarily inherent in the foundation of every (...) body politic. As a result, Manent’s critique tends to downplay the contingency of the nation-state and to overplay the contingency of the European Union. (shrink)

Based on the brain in a vat thought experiment, skeptics argue that we cannot have certain knowledge. At the same time, we do have the intuition that we know some things with certainty. A way to justify this intuition is given by semantic contextualists who argue that the word “knows” is context sensitive. However, many have objected to the intelligibility of this claim. In response, another approach called “moderate pragmatic contextualism” was invoked, which claims that “knows” itself is not context (...) sensitive, but knowledge assertions are. I show, however, that to refute skepticism, moderate pragmatic contextualism rests on unjustified and implausible assumptions as well. Since no form of contextualism works as a response to skepticism, I argue that we should simply accept skepticism. However, I argue that skepticism is not a problem because skeptic pragmatic contextualism can offer a plausible explanation of why we have the intuition that our ordinary knowledge claims are true, even though they are not. I conclude that skeptic pragmatic contextualism offers the most plausible response to the brain in a vat thought experiment. (shrink)

This contribution is a report of a two months' participant observation in a Dutch hospice. The goal of the observation was to gain an overview of moral decisions in a hospice in which euthanasia, a tolerated practice in the Netherlands, is not accepted as an option. In an introduction, the development of palliative care in the Netherlands will be briefly presented. Subsequently, various moral decisions that were taken during the participant observation are presented and analysed by means of case reports. (...) Attention is especially drawn to decisions that directly or indirectly relate to euthanasia. These moral decisions will be clarified in the light of the philosophy behind the concept of palliative care as it has evolved since the foundation of St Christopher's Hospice, London in 1967. The last four sections of the first book of Abū al-Barakāt al-Baghdādī’s summa, entitled Kitāb al-Mu‘tabar, deal with dialectics, sophistical refutations, rhetoric, and poetics in full line with Aristotle’s Organon. However, they are not so much based on Aristotle’s works, but on a work of the young Ibn Sīnā, namely al-Ḥikma al-‘Arūdiyya. Both texts have much in common not only in their structure, but also in their very wording. The article presents a basic survey of the correspondences for all four (...) sections and also highlights the most significant differences. However, important part of these differences has a counterpart in an logical text, which is present in the manuscript Nuruosmaniye 4894 and which is, in turn, very close to, albeit not identical, with Ibn Sīnā’s al-Ḥikma al-‘Arūdiyya as conserved in the unique manuscript Uppsala 364. Therefore, it is obvious that Abū al-Barakāt takes over many formulations and, at once, many ideas from Ibn Sīnā. However, he adds clearly personal elements that often seem to have been religiously inspired and, on occasion, consciously returns to Aristotle’s wording. (shrink)

Detection of genetic aberrations in prenatal samples, obtained through amniocentesis or chorion villus biopsy, is increasingly performed using chromosomal microarray (CMA), a technique that can uncover both aneuploidies and copy number variants throughout the genome. Despite the obvious benefits of CMA, the decision on implementing the technology is complicated by ethical issues concerning variant interpretation and reporting. In Belgium, uniform guidelines were composed and a shared database for prenatal CMA findings was established. This Belgian approach sparks discussion: it is evidence-based, (...) prevents inconsistencies and avoids parental anxiety, but can be considered paternalistic. Here, we reflect on the cultural and moral bases of the Belgian reporting system of prenatally detected variants. (shrink)

The main premise of the Royal Dutch Medical Association's (RDMA) guideline on palliative sedation is that palliative sedation, contrary to euthanasia, is normal medical practice. Although we do not deny the ethical distinctions between euthanasia and palliative sedation, we will critically analyse the guideline's argumentation strategy with which euthanasia is demarcated from palliative sedation. First, we will analyse the guideline's main premise, which entails that palliative sedation is normal medical treatment. After this, we will critically discuss three crucial propositions of (...) the guideline that are used to support this premise: (1) the patient's life expectancy should not exceed 2 weeks; (2) the aim of the physician should be to relieve suffering and (3) expert consultation is optional. We will conclude that, if inherent problematic aspects of palliative sedation are taken seriously, palliative sedation is less normal than it is now depicted in the guideline. (shrink)

A controversy over the pudgala or “person” raged among Indian Buddhists for more than a millennium. Their polemics were at least as much a matter of canonical exegesis as of reasoning and argument, for the “mainstream” Buddhist doctors had to account for—and explain away—the numerous places in scripture where the Buddha speaks of the “person.” The Bhārahārasūtra or “sūtra on the bearer of the burden” was one of the scriptures most frequently quoted and discussed in this connection. The present paper (...) is aimed at presenting a reconstructed Sanskrit version of this sūtra and providing an overview of the doctrinal and argumentative uses that were made of it in the context of the debates about the status of the “person.”. This work of remarkable depth takes up intensely debated topics, exhibiting in their treatment of them what Petrilli admires—creativity and imagination. Petrilli presents a careful integration of divergent thinkers and diverse perspectives. While (...) she abandons hope of attaining a final synthesis or an unqualifiedly comprehensive outlook, there remains a drive for coherence and detailed integration. The theory of identity being advocated in this book will provide the reader with an aid to appreciating the identity of the theorizing undertaken by Petrilli in her confrontation with an array of topics. Her theory differentiates itself from other offerings and, at the same time, is envisioned as a process of self-differentiation. Petrilli's contribution is at once historical and theoretical. It is historical in its recovery of major figures of language; it is theoretical in its articulation of a comprehensive framework. In the present paper it is shown that al-Ämidî has tried to integrate somehow pilosophy into theology in way that reveals closer to al-Ghazâlî's than to F.D. al-Râzî's way of combining both currents of thought. Based on an analysis of several ideas taken from different works it is shown that al-Ämidî not only accepts the logic of the philosophers, but also their views that may be considered as scientifically established beyond any reasonable doubt.

Given the two important functions of the media, offering a world image and setting the public agenda, the main question in this article is : "What is the image of society offered by the media?" For that purpose three years of the radio programme "BRT-Aktueel" are analyzed on the basis of a printed index. A distinction is made between home news, news on states, news on groups of countries and international organizations. The main conclusions are :1. Most of the coverage (...) concerns foreign issues ;2. Within each category there is a clear influence of temporary events. This influence seems more important for foreign topics ;3. The news coverage is strongly concentrated on a relatively small number of issues. This conclusion applies on all categories ;4. Social and domestic policy dominate the home news ;5. The coverage of international issues deals mostly with Europe, then with the USSR, and the United States. Other countries are part of the periphery and get attention when a major crisis occurs.The question rises which factors determinate the selection of issues. Within the limits of this research a few factors can be mentioned : geographical proximity, the structure of the media, the role of press agencies, the international status of a country, the selection by the journalist. It is quite clear that this problem of objectivity ofthe media is a major problem which is linked to the role of the media in a democratie society. (shrink)

The 17 essays he has collected here discuss such aspects as his heritage in the Islamic world and the Latin West, the problem of human freedom, al-Gazzali and his use of Avicennian texts, and some elements of Avicennian influence on Henry of Ghent's psychology. Nella seconda parte del _Libro della condanna dello status e dell’ostentazione_ il ventottesimo libro della sua opera principale, _La rinascita delle scienze religiose_ – al-Ghazālī trae molta ispirazione da al-Muhāsibī, soprattutto dalla sua opera _al-Ri’āya al-huqūq Allāh _(_L’osservanza dei diritti di Dio_). Il fatto che al-Ghazālī menziona esplicitamente il nome di al-Muhāsibī non meno di quattro volte in questa sezione testimonia la sua ammirazione profonda per questo grande mistico delle origini. Un attento esame di questi riferimenti e del loro contesto (...) rivela una grande familiarità e un ampio uso della _Ri’āya_, così come un accesso diretto a un’altra grande opera di al-Muhāsibī, cioè il _Kitāb al-Wasāya_ (_Il Libro dei Comandamenti_). Inoltre, la scelta di al-Ghazālī di specifici versetti coranici e delle Tradizioni, sia profetiche che di storie, è molto debitrice della _Ri’āya_, come diventa chiaro sulla base di un esame del primo capitolo di questa seconda parte che consiste interamente in citazioni di versetti coranici e delle Tradizioni riguardanti la questione dell’ostentazione. The very fact that al-Ghazālī explicitly mentions al-Muhāsibī’s name no less than four times throughout this section clearly testifies to his profound admiration for this great early mystic. A close examination of these references and their context reveals a great familiarity with and an extensive use of the _Ri’āya_, as well as a direct access to another major work of al-Muhāsibī, i.e., _Kitāb al-Wasāya_ (_The Book of Commandments_). Moreover, al-Ghazālī’s choice of specific Qur’anic verses and of Traditions, both prophetic and stories, is much indebted to the _Ri’āya_, as becomes clear on the basis of an examination of the first chapter of this second part which consisting entirely of quotations of Qur’anic verses and Traditions concerning the issue of ostentation. As to the second chapter, which focuses on the ways used to draw people’s attention to oneself both in religious as well as in wordly acts, it shares many features with al-Muhāsibī’s exposition in the _Ri’āya_, including the almost verbatim copying of some passages. Finally, al-Ghazālī’s use of the _Ri’āya_ in each of the other chapters is briefly indicated. This study shows that al-Muhāsibī was not just a major source for al-Ghazālī – a fact already known for nearly a century – but _the_ major source, at least for this section of his _Revival_. BackgroundCarrier screening is generally performed with the aim of identifying healthy couples at risk of having a child affected with a monogenic disorder to provide them with reproductive options. Expanded carrier screening, which provides the opportunity for multiple conditions to be screened in one test, offers a more cost-effective and comprehensive option than screening for single disorders. However, implementation of ECS at a population level would have implications for genetic counseling practice.MethodsWe conducted semi-structured interviews with sixteen European clinical and molecular (...) geneticists with expertise in carrier screening to explore their views on the implementation of ECS in the clinical setting.ResultsUsing inductive content analysis, we identified content categories relevant to the pre- and post-test settings. Participants believed ECS would ideally be targeted at couples before pregnancy. There was some disagreement regarding the acceptability of performing ECS in individuals, with several participants actively opposing individual-based screening. In addition, participants discussed the importance of ensuring informed and voluntary participation in ECS, recommending measures to minimize external pressure on prospective parents to undergo testing. A need for adequate counseling to foster informed, autonomous reproductive decision-making and provide support for couples found to be at risk was emphasized.ConclusionsPractical challenges in optimizing pre-test education and post-test counseling should not be underestimated and they should be carefully addressed before implementing ECS in the clinical setting.
