OBJECTIVES: To study the resuscitation preferences, choice of decision-maker, views on the seeking of patients' wishes and determinants of these of elderly hospital in-patients. DESIGN: Questionnaire administered on admission and prior to discharge. SETTING: Two acute geriatric medicine units (Southampton and Poole). PARTICIPANTS: Two hundred and fourteen consecutive consenting mentally competent patients admitted to hospital as emergencies. RESULTS: Resuscitation was wanted by 60%, particularly married and functionally independent patients and those who had not already considered it. Not wanted resuscitation was (...) associated with lack of social contacts. Sixty-seven per cent welcomed enquiry about their preferences and 78% wanted participation in decision, 43% as sole decision-maker. Wishing to choose oneself was associated with not wanting resuscitation, prior knowledge of it, and lack of a spouse. Patients' opinions remained stable during their admission. CONCLUSIONS: Discussion of resuscitation is practical on hospital admission without causing distress and the views expressed endure through the period of hospitalisation. Elderly patients' attitudes depend partly on personal health and social circumstances. This may assist doctors when patients are unable to participate themselves. (shrink)

During the German-Soviet War and in the decades that followed, the Volyn Orthodox clergy functioned under a cohesive social ghetto, which was to ensure the gradual assimilation of the social group in accordance with the needs of the totalitarian regime. In this situation, the national self-awareness of a part of the sacred priests did not fit into the coordinates of the Nazi religious and ethnic politics and, later, into the process of creation of the Soviet people. The question of national (...) identity remains relevant, and now, when it is the basis of the newest Ukrainian state-making, it has caused division in the Orthodox field. The national consciousness of the Volyn clergy in the years of war and in the era of Soviet totalitarianism never became a topic of separate study. To date, scientists have only partially addressed the problem. Although in 1960, the head of the ROC Council, V. Kuroedov, said that the remnants of religious remnants contributed to preserving the bourgeois nationalist sentiment. The compilers of the first volume of documents and materials “Martyrology of the Ukrainian Churches” Osip Zinkevich and Alexander Voronin, analyzing the national composition of the bishopric of the Russian Church in Ukraine as of 1986, hoped that in case of revival of Ukrainian Orthodoxy, the origin of some of them role. Objectives: To examine the perspectives of journal editors and authors on overlapping and redundant publications in clinical research.Design: Pretested cross-sectional survey, containing both forced choice and open ended questions, administered by mail to the senior editors and one randomly selected author from all journals in the Abridged Index Medicus that published clinical research.Main measurements: The views of editors and authors about the extent of redundant publications, why they occur, how to prevent and respond to cases, and when the publication of (...) overlapping manuscripts is justified.Results: Seventy two per cent of editors and 65% of authors completed the survey. There was consensus between both groups that redundant publications occur because authors feel the pressure to publish and journals do not do enough to publicise, criticise, and punish cases, and that the publication of most types of overlapping articles is unacceptable. Sixty seven per cent of authors but only 31% of editors felt, however, that it was justified to publish an overlapping article in a non-peer reviewed symposium supplement, and 68% of editors but 39% of authors supported imposing restrictions on guilty authors’ future submissions. In written comments, 15% to 30% of both groups emphasised that it was justified to publish overlapping articles when there were different or non-English-speaking audiences, new data, strengthened methods, or disputed findings.Conclusions: Editors, authors, and other academic leaders should together develop explicit guidelines that clarify points of contention and ambiguity regarding overlapping manuscripts. The process of turning a myth into a fairy tale, its internal and external causes, patterns, consequences... It may be difficult to find a more traditional way of exploring a fairy tale than this one. We will not avoid it either, because whatever aspect of the fairy tale analysis we choose, it is impossible to bypass this side of its genesis. And the choice of the method of explication of this problem largely predetermines both the angle of her vision and (...) the means of solution. The same traditions have already been given peculiar power lines that urge more and more researchers to move in the area of ​​these lines and to cover more and more varieties of the same dimensions of the object: demythologization and desacralization of the hero, his actions, his environment; giving the myth an exoteric character, with the elements of fiction and irony that are inconceivable as a result; the shrinking of the scale of the hero's actions, and hence the replacement of the mystery of myth with mysteriousness, intrigue in the fairy tale, the decline of the mythological worldview. The topics of branching of the subjects through the myth to the fairy tale, finding and researching common for both phenomena of binary oppositions, etc., are densely addressed. (shrink)

If we approach the analysis of fairy tales from the point of view of revealing in them a metaphysical dimension of human intentions, then in their subjects one can identify several paradigms. The most important of these should include, in particular, the following: the combination of man with the deity ; the loss of God's person as a result of her violation of some conditions for coexistence with God; the search for the lost man of God and the rejoining of (...) him. These through-world ideological paradigms, embodied in specific themes, may be adjoined in the same tale, and may exist separately, encompassing all of its plot. Tоday there is a strong tendency to incorporate the bioethical principle of social justice in healthcare in cross-cultural communication. Considering cultural differences makes it possible to ensure that the human right to medical care and wellbeing is fully respected. Several types of most vulnerable populations were identified – immigrants and social minorities. When seeking medical care they face a number of problems such as culture and language barriers, lower socio-economic status, lack of literacy, which impede effective communication and care provision. (...) The most promising ways of coping with the problem are developing cultural competence and practicing a patient-centered approach. New curricula aiming at raising cultural awareness have been elaborated for practical use in medical schools. (shrink)

To base oneself upon a scientific philosophy is often of great importance in framing and solving major problems in natural science, including the science of the universe at large. Moreover, one's approach to the solution of specific problems arising in natural science depends to an extent upon one's philosophy. This situation points the way to the elimination of certain preconceived notions and erroneous convictions of researchers, that is, those due to an inadequate knowledge of philosophy or to the influence of (...) a weak philosophy. The spinor covariant derivative through which the equations of quantum fields are generalized to include gravitational coupling has a direct and simple geometric significance. The formula for the difference of two spinor covariant derivatives taken in different order is derived geometrically; and the geometric proof of the covariant constancy of the spin-1/2 γ-matrices in curved space is given.

It is argued that it is a misinterpretation of frege to construe his talk of the bedeutungen of sentences and of predicates in an ontologically innocent way.

Moral philosophers have often felt the need of a concept which would cover all those cases where we are prevented from achieving our ends through no fault of our own: a criterion for saying when failure is not blameworthy. The deontologists thought we were not to blame for actions done in genuine ignorance of the facts. Kant declared in a famous passage that we were not morally responsible for failures due to the “niggardliness of stepmother nature.” In this article I (...) want to introduce into moral philosophy a new concept for clarifying the failures for which we are not to blame: the notion of scarcity, taken from economic theory. Harry Frankfurt's famous examples of overdetermined moral agents who are nevertheless responsible for their actions and omissions have long been hailed as proofs that the ability and/or opportunity to do otherwise is not a necessary condition for moral responsibility. In this paper I use recent clarifications of some of these examples by Frankfurt himself to show that their force relies in part on tacit ceteris paribus assumptions concealing a reliance on PAP that concerns matters of fairness in assessing moral responsibility.
