Introduction: Congenital myasthenic syndromes refer to a heterogenic group of neuromuscular transmission disorders. CMS-subtypes are diverse regarding exercise intolerance and muscular weakness, varying from mild symptoms to life-limiting forms with neonatal onset. Long-term follow-up studies on disease progression and treatment-response in pediatric patients are rare.Patients and Methods: We analyzed retrospective clinical and medication data in a cohort of 32 CMS-patients including the application of a standardized, not yet validated test to examine muscular strength and endurance in 21 patients at the (...) last follow-up. Findings obtained in our cohort were compared with long-term follow-up studies of CMS-cohorts from the literature by considering the underlying molecular mechanisms. Outcomes of CMS-ST were compared to results of normal clinical assessment.Results: Thirty-two pediatric patients with defects in eight different CMS-genes were followed by a median time of 12.8 years. Fifty-nine percentage of patients manifested with first symptoms as neonates, 35% as infants. While 53% of patients presented a reduced walking distance, 34% were wheelchair-bound. Even under adequate therapy with pyridostigmine and 3,4-diaminopyridine, CHAT-mutations led to the progression of muscular weakness partly in combination with persistent respiratory and bulbar symptoms. RAPSN, CHRND, and CHRNB1 patients with neonatal manifestation, early respiratory problems, and bulbar symptoms showed a good and maintained treatment response. CHAT and CHRNE patients required higher PS dosages, whereas RAPSN patients needed a lower mean dosage at the last follow-up. The benefits of short-term medication and long-term progression of symptoms were highly dependent on the specific genetic defect. CMS-ST was carried out in 17/21 patients, determined affected muscle groups including bulbar and ocular symptoms, some of which were not reported by the patients.Conclusions: Our findings and comparison with the literature- suggest a better treatment-response and less severe progression of symptoms present in patients suffering from mutations in CMS-genes directly associated with receptor deficiency, while patients with defects leading to synaptopathy and presynaptic defects tend to have worse outcomes. Assessment of affected muscular groups and clinical symptoms by CMS-ST may be a useful tool for optimal therapeutic management of the patients, especially for future clinical studies. (shrink)

Aus philosophischer Perspektive diskutiert Ulrike Kleemeiers Studie einer Reihe auch aktuelle wieder sehr brisanter Probleme. Analysiert werden der Begriff des Krieges ebenso wie die Ursachen von Kriegen und die Kriegsprävention. Dabei untersucht die Autorin auch den Zusammenhang von Krieg und Gerechtigkeit bzw. Krieg und Recht, das Verhältnis von Krieg und Politik, das Problem des Bürgerkriegs sowie die Bedeutung kriegerisch-militärischer Tugenden und Kompetenzen. Die Auseinandersetzung erfolgt in exemplarischer Form anhand der Theorien von Platon, Hobbes und Clausewitz. Der Arbeit liegt die (...) Annahme zugrunde, dass eine Theorie des Krieges sinnvoll nur in einem interdisziplinären Bezugsrahmen entwickelt werden kann. Deshalb wendet sich dieses Buch an Fachphilosophen genauso wie an alle anderen am Thema Krieg interessierten Forscher, etwas an Historiker, Politologen und Sozialwissenschaftler. (shrink)

It is an assumption common to many theories of rationality that all practical reasons are based on a person's given desires. I shall call any approach to practical reasons which accepts this assumption a "Humean approach". In spite of many criticisms, the Humean approach has numerous followers who take it to be the natural and inevitable view of practical reason. I will develop an argument against the Humean view aiming to explain its appeal, as well as to expose its mistake. (...) We examine in detail three classic reasoning fallacies, that is, supposedly ``incorrect'' forms of argument. These are the so-called argumentam ad ignorantiam, the circular argument or petitio principii, and the slippery slope argument. In each case, the argument type is shown to match structurally arguments which are widely accepted. This suggests that it is not the form of the arguments as such that is problematic but rather something about the content of those examples with which they are typically justified. This (...) leads to a Bayesian reanalysis of these classic argument forms and a reformulation of the conditions under which they do or do not constitute legitimate forms of argumentation. (shrink)

This article discusses the effects of two trends in contemporary biomedicine that have so far been largely addressed separately: the steering of fields through programmatic “buzzwords” and the projectified nature of contemporary health research, care, and promotion. Drawing on a case study of an Austrian diversity-sensitive health promotion project related to obesity prevention, we show how the articulation of these trends—governance by buzzwords and projectification—often leads to not unproblematic and often paradoxical outcomes. Buzzwords such as “diversity” become especially important in (...) an innovation-driven environment encouraging a promissory rhetoric. At the same time, the project form shapes and restricts how buzzwords are articulated and translated into a specific project design. In our case study of an obesity prevention program, the need to translate diversity into a “doable” project encouraged the identification of seemingly clearly delineated target groups and thus promoted a rather narrow understanding of diversity, which stands in tension with much more fluid and context-sensitive ways of performing “diversity.” We show how actors grapple with these paradoxes. This restricts the full power a buzzword such as diversity could achieve in terms of social justice. (shrink)

In this paper, I argue that a person can have a reason to do what she cannot do. In a nutshell, the argument is that a person can have derivate reasons relating to an action that she has a non-derivative reason to perform. There are clear examples of derivative reasons that a person has in cases where she cannot do what she (non-derivatively) has reason to do. She couldn’t have those derivative reasons, unless she also had the non-derivative reason to (...) do what she cannot do. I discuss a number of objections to this view, in particular two: (1) The objection that if there were reasons to do what one cannot do, many of those would be ‘crazy reasons’, and (2) the worry that if there were such reasons, then agents would have reasons to engage in futile deliberations and tryings. I develop an explanation of ‘crazy reasons’ that shows that not all reasons to do the impossible are crazy and only those that are need to be filtered out, and, regarding the second objecting, I show that the reasons for trying as well as for taking the means to doing something—instrumental reasons in a broad sense—are different from the reasons for performing the action in the first place. They are affected by impossibility, and we can explain why that is so. The view I argue for is that a person may have a reason to do what she cannot do, but she does not have a reason to try to do so or to take means to realizing the impossible. (shrink)

Background: Recent research has demonstrated that depressed individuals show impairments in inhibiting irrelevant emotional material, and that these impairments are linked to rumination. Cognitive inhibition, however, is not a unitary construct but consists of several components which operate at different stages of information processing. The present study was designed to assess two components of inhibition and examine their relation to depression and rumination in a sample of clinically depressed and healthy control participants. Methods: Twenty-two individuals diagnosed with a current depressive (...) episode and 27 never-disordered control participants completed an Emotional Flanker Task to assess individual differences in interference control and a modification of the Working Memory Selection Task to assess individual differences in the ability to discard no longer relevant emotional material from working memory. Participants completed self-report measures to assess depressive symptoms and rumination. Results: Clinically depressed compared to control participants showed significantly reduced interference control of irrelevant negative information. The groups, however, did not differ in their ability to discard no longer relevant negative information from working memory. In contrast, rumination was associated with difficulty removing no longer relevant negative material from working memory but not with deficits in interference control. Conclusions: Results highlight the importance of differentiating among components of inhibition to gain a better understanding of cognitive mechanisms underlying depression and rumination. (shrink)

In this paper, it is argued that the most fruitful approach to developing normative models of argument quality is one that combines the argumentation scheme approach with Bayesian argumentation. Three sample argumentation schemes from the literature are discussed: the argument from sign, the argument from expert opinion, and the appeal to popular opinion. Limitations of the scheme-based treatment of these argument forms are identified and it is shown how a Bayesian perspective may help to overcome these. At the same time, (...) the contributions of the standard scheme-based approach are highlighted, and it is argued that only a combination of the insights of different traditions will yield a complete normative theory of argument quality. Mit dieser Frage befasst sich der folgende Beitrag, wobei untersucht wird, wie die Heranwachsenden speziell in der Herkunftsfamilie als primärer Sozialisationsinstanz in den vereinsgebundenen Sport „sozial eingespurt“ werden. Auf der Grundlage des Zinneckerschen Konzepts der familialen Sozialisation wird vornehmlich ein Forschungsprogramm skizziert und Annahmen formuliert, die (...) exemplarisch anhand von empirischen Daten des Brandenburgischen Jugendsportsurveys 2002 geprüft werden. Rhythm and meter are central elements of music. From the very beginning, children are responsive to rhythms and acquire increasingly complex rhythmic skills over the course of development. Previous research has shown that the processing of musical rhythm is not only related to children’s music-specific responses but also to their cognitive abilities outside the domain of music. However, despite a lot of research on that topic, the connections and underlying mechanisms involved in such relation are still unclear in some respects. (...) In this article, we aim at analyzing the relation between rhythmic and cognitive-motor abilities during childhood and at providing a new hypothesis about this relation. We consider whether predictive processing may be involved in the relation between rhythmic and various cognitive abilities and hypothesize that prediction as a cross-domain process is a central mechanism building a bridge between rhythm processing and cognitive-motor abilities. Further empirical studies focusing on rhythm processing and cognitive-motor abilities are needed to precisely investigate the links between rhythmic, predictive, and cognitive processes. (shrink)

The legal doctrine of ‘common purpose’ in South African criminal law considers all parties liable who have been in implicit or explicit agreement to commit an unlawful act, and associated with each other for that purpose, even if the consequential act has been carried out by one of them. It relieves the prosecution of proving the causal link between the conduct of an individual member of a group acting in common purpose, and the ultimate consequence caused by the action of (...) the group as a whole. The National Prosecuting Authority’s controversial and vociferously challenged decision to charge 270 demonstrators at Lonmin Platinum Mine in Marikana with the murders of 34 colleagues under the ‘common purpose’ doctrine, implying liability by association or agreement, raises the question as to the constitution and characteristics of the crowd and of the public, respectively. This article outlines the history of the application of the common purpose rule in South Africa, to then examine ‘common purpose’ within the philosophical parameters of group psychology and collective intentionality. It argues for methodological individualism within a psychoanalytic theorisation of group dynamics, and a non-summative approach to collective intentionality, in addressing some problems in the conceptualisation of group formation. Is the wrongness of an action a reason not to perform it? Of course it is, you may answer. That an action is wrong both explains and justifies not doing it. Yet, there are doubts. Thinking that wrongness is a reason is confused, so an argument by Jonathan Dancy. There can’t be such a reason if ‘ϕ-ing is wrong’ is verdictive, and an all things considered judgment about what (not) to do in a certain situation. Such judgments are based on (...) all the relevant reasons for and against ϕ-ing. If that ϕ-ing is wrong, while being an all things considered verdict, would itself be a reason, it would upset the balance of reasons: it would be a further reason which has not yet been considered in reaching the verdict. Hence, the judgment wasn’t ‘all things considered' after all. I show that the argument against wrongness being a reason is unsuccessful, because its main assumption is false. Is main assumption is that a consideration which necessarily does not affect the balance of reasons is not a reason. I also argue that there can be no deontic buck-passing account. (shrink)

Possible measures to mitigate climate change require global collective actions whose impacts will be felt by many, if not all. Implementing such actions requires successful communication of the reasons for them, and hence the underlying climate science, to a degree that far exceeds typical scientific issues which do not require large-scale societal response. Empirical studies have identified factors, such as the perceived level of consensus in scientific opinion and the perceived reliability of scientists, that can limit people's trust in science (...) communicators and their subsequent acceptance of climate change claims. Little consideration has been given, however, to recent formal results within philosophy concerning the relationship between truth, the reliability of evidence sources, the coherence of multiple pieces of evidence/testimonies, and the impact of independence between sources of evidence. This study draws on these results to evaluate exactly what has been established in the empirical literature about the factors that bias the public's reception of scientific communications about climate change. However, real information on these possibilities or the direction of these changes does not exist. Into this gap, Hahn and colleagues bring the power of Condorcet's (1785) Jury Theorem to show that changes in our information networks have affected voter inter‐dependence so that it is likely that voters are now collectively more ignorant even if individual voter (...) competence remains unchanged. Our beliefs and opinions are shaped by others, making our social networks crucial in determining what we believe to be true. Sometimes this is for the good because our peers help us form a more accurate opinion. Sometimes it is for the worse because we are led astray. In this context, we address via agent-based computer simulations the extent to which patterns of connectivity within our social networks affect the likelihood that initially undecided agents in a network converge on a (...) true opinion following group deliberation. The model incorporates a fine-grained and realistic representation of belief and trust, and it allows agents to consult outside information sources. We study a wide range of network structures and provide a detailed statistical analysis concerning the exact contribution of various network metrics to collective competence. Our results highlight and explain the collective risks involved in an overly networked or partitioned society. Specifically, we find that 96% of the variation in collective competence across networks can be attributed to differences in amount of connectivity and clustering, which are negatively correlated with collective competence. A study of bandwagon or “group think” effects indicates that both connectivity and clustering increase the probability that the network, wholly or partly, locks into a false opinion. Our work is interestingly related to Gerhard Schurz’s work on meta-induction and can be seen as broadly addressing a practical limitation of his approach. (shrink)

Donald Davidson writes that “[r]easons for intending to do something are very much like reasons for action, indeed one might hold that they are exactly the same except for time.” That the reasons for forming an intention and the reasons for acting as intended are in some way related is a widely accepted claim. But it can take different forms: (1) the reasons may mirror each other so that there is a (derivative) reason to intend whenever there is a reason (...) to act; or (2) they may reduce to just one kind: perhaps all reasons for action are really reasons for forming intentions.3 Or the other way around: (3) all reasons for intentions are really reasons to act. The three versions are not equally strong contenders though. The third - that reasons to intend could reduce to reasons to act - seems unlikely. After all, there may be reasons to form future-directed intentions, in particular, independently of the reason to act as intended. The second suggestion falls prey to different considerations: reasons to act can, at least sometimes, be reasons to produce a certain outcome, quite independently of the intention with which the action is done, or whether it is done intentionally at all. In these cases, the reason to act is not (or not obviously) a reason to intend. Therefore, I don’t pursue the possibility of a reduction in this paper. My focus is on the first, non-reductive proposal. I will discuss various versions of it in some detail, but ultimately reject it. (shrink)
