Since 1989, clinical ethics consultation in form of hospital ethics committees was established in most of the transition countries of Central and Eastern Europe. Up to now, the similarities and differences between HECs in Central and Eastern Europe and their counterparts in the U.S. and Western Europe have not been determined. Through search in literature databases, we have identified studies that document the implementation of clinical ethics consultation in Central and Eastern Europe. These studies have been analyzed under the following (...) aspects: mode of establishment of HECs, character of consultation they provide, and their composition. The results show that HECs in the transition countries of Central and Eastern Europe differ from their western-European or U.S. counterparts with regard to these three aspects. HECs were established because of centrally imposed legal regulations. Little initiatives in this area were taken by medical professionals interested in resolving emerging ethical issues. HECs in the transition countries concentrate mostly on review of research protocols or resolution of administrative conflicts in healthcare institutions. Moreover, integration of non-professional third parties in the workings of HECs is often neglected. We argue that these differences can be attributed to the historical background and the role of medicine in these countries under the communist regime. Political and organizational structures of healthcare as well as education of healthcare staff during this period influenced current functioning of clinical ethics consultation in the transition countries. Worin gründet die Wirkmacht der Poesie, Freiräume des Denkens zu eröffnen, Lesern Grenzüberschreitungen der eigenen Zeit, der eigenen Identität, der eigenen Kultur zu ermöglichen und damit das Bewusstsein für gänzlich andere Vermessungen der Welt? Wie können Literaturwissenschaftler die Freiheit der Kunst und damit die Komplexität ästhetischer Erfahrungen verteidigen, angesichts der Reduzierung von Freiheitsgraden in den aktuellen Kulturkämpfen? Wie gelingt es zeitgenössischer politischer Dichtung, denen Gehör (...) zu verschaffen, deren Unfreiheit nur die Konsequenz des rücksichtslosen Freiheitsanspruchs privilegierter Individuen und Nationen ist? Purpose: The method of diagnosing diabetic retinopathy (DR) through artificial intelligence (AI)-based systems has been commercially available since 2018. This introduces new ethical challenges with regard to obtaining informed consent from patients. The purpose of this work is to develop a checklist of items to be disclosed when diagnosing DR with AI systems in a primary care setting. -/- Methods: Two systematic literature searches were conducted in PubMed and Web of Science databases: a narrow search focusing on DR and a (...) broad search on general issues of AI-based diagnosis. An ethics content analysis was conducted inductively to extract two features of included publications: (1) novel information content for AI-aided diagnosis and (2) the ethical justification for its disclosure. -/- Results: The narrow search yielded n = 537 records of which n = 4 met the inclusion criteria. The information process was scarcely addressed for primary care setting. The broad search yielded n = 60 records of which n = 11 were included. In total, eight novel elements were identified to be included in the information process for ethical reasons, all of which stem from the technical specifics of medical AI. -/- Conclusions: Implications for the general practitioner are two-fold: First, doctors need to be better informed about the ethical implications of novel technologies and must understand them to properly inform patients. Second, patient's overconfidence or fears can be countered by communicating the risks, limitations, and potential benefits of diagnostic AI systems. If patients accept and are aware of the limitations of AI-aided diagnosis, they increase their chances of being diagnosed and treated in time. (shrink)

This chapter reflects on why and how the forces of globalization have altered the conventional political belief systems codified by social power elites since the French Revolution. In order to explain these dramatic transformations, the chapter discusses at some length the crucial relationship between two ‘social imaginaries’—the national and the global—that underpin the articulation of political ideologies. The chapter suggests a new typology of three contemporary ‘globalisms’ based on the disaggregation of new ideational clusters not merely into core concepts, but, (...) perhaps more dynamically, into various sets of central ideological claims that play crucial semantic and political roles. These three globalisms—market globalism, justice globalism, and religious globalism—represent a set of political ideas and beliefs coherent and conceptually thick enough to warrant the status of mature ideologies. Examining the nature of Václav Havel's 'postmodernism,' we suggest that his use of this ambiguous label can be best understood if interpreted outside the conventional binary framework of modern/postmodern philosophy, which does not sufficiently answer to the lingering crisis of foundational certainty in political theory. In our view, the Czech playwright-turned-politician offers not merely a less confining sense of what it means to be 'postmodern,' but his commitment to moral political action also lends itself to overcoming some of the limitations (...) of contemporary postmodern theorizing by reentering the depreciated terrain of ontology. Specifically, we argue that an assessment of Havel's brand of postmodernism yields a rich interpretive harvest when read through ontological lenses. Indeed, Havel's theoretical posture serves as an instructive example of what political theorist Stephen K. White has called a 'felicitous weak ontology.'. (shrink)

Activation of gene transcription in vivo is accompanied by an alteration of chromatin structure. The specific binding of transcriptional activators disrupts nucleosomal arrays, suggesting that the primary steps leading to transcriptional initiation involve interactions between activators and chromatin. The affinity of transcription factors for nucleosomal DNA is determined by the location of recognition sequences within nucleosomes, and by the cooperative interactions of multiple proteins targeting binding sites contained within the same nucleosomes. In addition, two distinct types of enzymatic complexes facilitate (...) binding of transcription factors to nucleosomal DNA. These include type A histone acetyltransferases (e.g. GCN5/ADA transcriptional adaptor complex) and ATP‐driven molecular machines that disrupt histone‐DNA interactions (e.g. SWI/SNF and NURF complexes). These observations raise the important question of what happens to the histones during chromatin remodeling. We discuss evidence supporting the retention of histones at transcription factorbound sequences as well as two alternative pathways of histone loss from gene control elements upon transcription factor binding: histone octamer sliding and histone dissociation. Employee shareholder associations (ESAs) have emerged as a novel, and widely underestimated actor in the European corporate arena, established to collect and pool the shares and voting power held by a company's employees. As such, they parallel existing institutions for employee representation, potentially empowering employees in their role as shareholders and possibly even providing a counterweight to traditional company owners. Unfortunately, we know little about the actual functioning, the inner workings, and, particularly, the ESAs' contributions to date. To address these (...) shortcomings this paper explores the limitations but also the potential of ESAs in large, German listed companies to contribute to employee share ownership (ESO), to organizational democracy (OD), and to corporate sustainability (CS). Our findings show that, as far as ESOs and OD are concerned, in the specific German context, ESAs usually do not offer alternatives to (or even to compete with) existing employee representation but are instead rather dependent on cooperation with them. Regarding CS, any contribution here is closely linked to the ESAs' own principles and depends on the extent to which the ESA management takes them seriously and prioritizes them over other objectives. Arguing that today’s burgeoning globalization literature still neglects the investigation of powerful subjective dynamics of growing social interconnectivity, this article explores how various ideological articulations of globalization have shaped its material designs and in­stantiations. The thickening of global consciousness can be conceptualized along the three interrelated dimensions or layers of ideology, imaginary, and ontology. Each of these three layers of subjective globalization is constituted in practice at an ever-greater generality, durability, and depth.
