The goal of this paper is to review and describe the characteristics and outcomes of ethics consultations on a gastrointestinal oncology service and to identify areas for systems improvement and staff education. This is a retrospective case series derived from a prospectively-maintained database of the ethics consultation service at Memorial Sloan Kettering Cancer Center. The study analyzed all ethics consultations requested for patients on the gastrointestinal medical oncology service from September 2007 to January 2016. A total of 64 patients were (...) identified. The most common primary ethical issue was the DNR order, followed by medical futility. The most common contextual issues were dispute/conflict between staff and family, dispute/conflict intra-family, and cultural/ethnic/religious issues. The majority of ethical issues leading to consultation were resolved ; i.e., the patient, surrogate, and/or healthcare team followed the recommendation of the ethics consultant. 22% had a DNR order prior to the ethics consult and 69% had a DNR order after the consult. In this population of patients on a gastrointestinal oncology service, ethics consultations are most often called regarding patients with advanced cancers and the most common ethical conflicts arose between families and the health care team over goals of care at the end of life, specifically related to the DNR order and perceived futility of continued/escalation of treatment. Ethics consultations assisted with conflict resolution. Conflicts might be reduced with improved communication about prognosis and earlier end of life care planning. (shrink)

This paper introduces the special issue of papers selected from those presented at the International Conference on Business Ethics in Transitional Economies, held March 20–22, 2002 in Celakovice and Prague, Czech Republic. A brief background on the conference is given, and a summary of the papers offered in this special issue is provided.

The idea of elegance in science is not necessarily a familiar one, but it is an important one. The use of the term is perhaps most clear-cut in mathematics - the elegant proof - and this is where Ian Glynn begins his exploration. Scientists often share a sense of admiration and excitement on hearing of an elegant solution to a problem, an elegant theory, or an elegant experiment. The idea of elegance may seem strange in a field of endeavour (...) that prides itself in its objectivity, but only if science is regarded as a dull, dry activity of counting and measuring. It is, of course, far more than that, and elegance is a fundamental aspect of the beauty and imagination involved in scientific activity. Ian Glynn, a distinguished scientist, selects historical examples from a range of sciences to draw out the principles of science, including Kepler's Laws, the experiments that demonstrated the nature of heat, and the action of nerves, and of course the several extraordinary episodes that led to Watson and Crick's discovery of the structure of DNA. With a highly readable selection of inspiring episodes highlighting the role of beauty and simplicity in the sciences, the book also relates to important philosophical issues of inference, and Glynn ends by warning us not to rely on beauty and simplicity alone - even the most elegant explanation can be wrong. This Element provides an accessible introduction to the contemporary philosophy of causation. It introduces the reader to central concepts and distinctions and to key tools drawn upon in the contemporary debate. The aim is to fuel the reader's interest in causation, and to equip them with the resources to contribute to the debate themselves. The discussion is historically informed and outward-looking. 'Historically informed' in that concise accounts of key historical contributions to the understanding of causation set the stage for an (...) examination of the latest research. 'Outward looking' in that illustrations are provided of how the philosophy of causation relates to issues in the sciences, law, and elsewhere. Feminized care work occupations have traditionally paid lower wages compared to non–care work occupations when controlling for human capital. However, when men enter feminized occupations, they often experience a “glass escalator,” leading to higher wages and career mobility as compared to their female counterparts. In this study, we examine whether men experience a “wage penalty” for performing care work in today’s economy, or whether the glass escalator helps to mitigate the devaluation of care work occupations. Using data from the Survey (...) of Income and Program Participation for the years 1996-2011, we examine the career patterns of low- and middle-skill men in health care occupations. We found that men in occupations that provide the most hands-on direct care did experience lower earnings compared to men in other occupations after controlling for demographic characteristics. However, men in more technical allied health occupations did not have significantly lower earnings, suggesting that these occupations may be part of the glass escalator for men in the health care sector. Minority men were significantly more likely than white men to be in direct care occupations, but not in frontline allied health occupations. Male direct care workers were less likely to transition to unemployment compared to men in other occupations. (shrink)

Much recent philosophical attention has been devoted to the prospects of the Best System Analysis of chance for yielding high-level chances, including statistical mechanical and special science chances. But a foundational worry about the BSA lurks: there don’t appear to be uniquely correct measures of the degree to which a system exhibits theoretical virtues, such as simplicity, strength, and fit. Nor does there appear to be a uniquely correct exchange rate at which the theoretical virtues trade off against one another (...) in the determination of an overall best system. I argue that there’s no robustly best system for our world – no system that comes out best under every reasonable measure of the theoretical virtues and exchange rate between them – but rather a set of ‘tied-for-best’ systems: a set of very good systems, none of which is robustly best. Among the tied-for-best systems are systems that entail differing high-level probabilities. I argue that the advocate of the BSA should conclude that the high-level chances for our world are imprecise. (shrink)

The role of the ‘ideal’ in political philosophy is currently much discussed. These debates cast useful light on Mill's self-designation as ‘under the general designation of Socialist’. Considering Mill's assessment of potential property-relations on the grounds of their desirability, feasibility and ‘accessibility’ (disambiguated as ‘immediate-availability’, ‘eventual-availability’ and ‘conceivable-availability’) shows us not only how desirable and feasible he thought ‘utopian’ socialist schemes were, but which options we should implement. This, coupled with Mill's belief that a socialist ideal should guide social reforms (...) (as the North Star guides mariners), reveals much more clearly the extent of his socialist commitments (even if he thought political economists would be concerned with forms of individual property for some time to come). Moreover, this framework for assessments of ‘ideal’ institutions makes a useful contribution to an ongoing contemporary debate. (shrink)

In The Political Economy of Progress, Joseph Persky argues for seeing John Stuart Mill as a consistent ‘radical’ with much to offer modern ‘radical’ political discourse. In this article, I further this claim with consideration of Mill's political philosophy, as well as his political economy. Exploring Mill's commitment to radical reordering of the economy, as well as emphasizing his commitment to egalitarianism; his historically nuanced view of ‘the progress of justice’; and his desire for a transformation of social relations allows (...) us to see more clearly how Mill's radicalism was a specific species of socialism. That is, Mill's early radical enthusiasm for the ideals of ‘liberty, equality and fraternity’ is also to be seen in his later socialism. Recognizing his ‘radicalism’ as a species of socialism allows greater understanding of the depth, importance and ‘radicalism’ of Mill's desired socialist reforms. (shrink)

Voltaire (1694-1778), novelist, dramatist, poet, philosopher, historian, and satirist, was one of the most renowned figures of the Age of Enlightenment. How does Plato view his philosophical antecedents? Plato and his Predecessors considers how Plato represents his philosophical predecessors in a late quartet of dialogues: the Theaetetus, the Sophist, the Politicus and the Philebus. Why is it that the sophist Protagoras, or the monist Parmenides, or the advocate of flux, Heraclitus, are so important in these dialogues? And why are they represented as such shadowy figures, barely present at their own refutations? The explanation, the author argues, is a complex one involving (...) both the reflective relation between Plato's dramatic technique and his philosophical purposes, and the very nature of his late philosophical views. For in these encounters with his predecessors we see Plato develop a new account of the principles of reason, against those who would deny them, and forge a fresh view of the best life - the life of the philosopher. A number of late Stoic sources describe either ethical concepts or a supposed universal belief in gods as being innate in the human animal. Though Chrysippus himself is known to have spoken of "implanted preconceptions" (ἔμφυτοι προλήψεις) of good and bad, scholars have typically argued that the notion of innate concepts of any kind would have been entirely incompatible with his theory of knowledge. Both Epictetus' notion of innate concepts of good and bad and the references to an innate belief (...) in gods by other philosophers of the Roman era are thus generally held to be later developments, probably owing to a Platonist-Stoic syncretism. Review of the evidence, however, shows that Chrysippus, like Epictetus, held ethical concepts to represent a special category of conception in that their formation was guaranteed by oikeiôsis. Unlike other concepts, that is, these represent a formal conceptualization of an innate tendency to distinguish between things fitting for one's constitution and things not fitting that all animals, according to the Stoics, bring to their empirical experiences. While the notion that human belief in gods is similarly innate does seem to have been a later development, it too was explained with reference to oikeiôsis rather than resulting from a simple "syncretism.". ABSTRACT This paper draws upon the already extensive epistemology of science, both to provide a yardstick of comparison for the emergent epistemology of design, and to establish some starting points from which we might begin to construct the epistemology of design. In approaching the problem of how designers design, the paper employs Heidegger's distinction between ‘know‐how’and ‘knowledge‐that’, popularised by Ryle, and shows it to be central to the distinction between the implicit processes of design employed by craft technologies, and the (...) explicit processes employed by modern scientific technology. Borrowing from recent work by post‐structuralists, it explores the nature of the phases of analysis or ‘deconstruction’, synthesis or construction, and evaluation, by which scientifically informed design is supposed to proceed, paying particular attention to the relationship between deconstructive analysis and constructive synthesis, in order to re‐construct the creative moment/movement by which designers ‘leap’ from problem to solution. Love, fear, hope, calculus, and game shows-how do all these spring from a few delicate pounds of meat? Neurophysiologist Ian Glynn lays the foundation for answering this question in his expansive An Anatomy of Thought, but stops short of committing to one particular theory. The book is a pleasant challenge, presenting the reader with the latest research and thinking about neuroscience and how it relates to various models of consciousness. Combining the aim of a textbook with the style of (...) a popularization, it provides all the lay reader needs to know to participate in the philosophical debate that is redefining our attitudes about our minds. Drawing on the rich history of neurological case studies, Glynn picks through the building blocks of our nervous system, examines our visual and linguistic systems, and probes deeply into our higher thought processes. The stories of great scientists, like Ramon y Cajal, and famous patients, like Sperry's split-brained epileptics, illuminate the scientific issues Glynn selects as essential for understanding consciousness. Some might argue that his lengthy explorations of natural selection overemphasize evolutionary explanations of psychological phenomena, but they must also agree that evolutionary psychology has distanced itself mightily from social Darwinism in recent years and merits a reappraisal. From Library Journal The nature of consciousness, which perennially troubles the minds of scientists and philosophers, is the subject of an ever-growing body of literature. Two of the latest entries approach the topic from different perspectives. Glynn, a professor of physiology and head of the Physiological Laboratory at Cambridge, offers a comprehensive summary of what we know about the brain-both its evolution and its mechanisms. Among the topics he covers are natural selection, molecular evolution, nerves and the nervous system, sensory perception, and the specific structures responsible for our intellect. Using the mechanisms involved in vision and speech as models, Glynn skillfully describes various neurological deficiencies that can lead to 'disordered seeing' and problems with the use of language. He carefully distinguishes what we know through experimental evidence from what we know through the observation of patients with neurological damage. He also describes some of the major theories that attempt to explain why these structures arose. While his book concentrates on the structures that make up the mind, Glynn is well aware that some physical events appear explicable only in terms of conscious mental events-a situation that conflicts with the laws of modern physics. Only briefly, however, does he consider the various approaches that have been taken to deal with the issues of mind/body and free will. In contrast, this is the primary focus of The Physics of Consciousness. After reviewing the fundamentals of classic physics, Walker (who has a Ph.D. in physics) summarizes elements of the new physics in which our knowledge of space, time, matter, and energy are all dependent on the moment of observation. Walker explores the meaning of consciousness as a characteristic of the observer. In this context both the observer and the act of measurement are critical. In essence, Walker leads his reader on a journey through his concept of a 'quantum mind,' which can both affect matter (including other minds) and can be affected by other distant matter/minds. To break up what would otherwise be an extremely dense text, Walker also relates the very touching story of the loss of his high-school sweetheart to leukemia. Indeed, it is his memory of their relationship that drives Walker to seek an understanding of ultimate reality. At times, he has a tendency to be dogmatic-as when he concludes, 'our consciousness, our mind, and the will of God are the same mind.' From Booklist The codiscoverers of natural selection-Charles Darwin and Alfred Wallace-disagreed over the possibility of finding an evolutionary explanation for the human mind. Glynn here argues Darwin's side of the debate, tracing an eons-long path of development starting from simple amino acids floating in primal seas and extending through the erect hominids in which the powers of a massive brain first manifest themselves. Patiently adducing evidence of an evolutionary origin for the underlying molecular machinery, Glynn dissects the nerve centers that make possible speech and hearing, sight, and reading. Pressing deeper, he lays bare the cortical foundations of personality. But those who deal with the mind must attend also to the arguments advanced by philosophers. And it is when he turns from dendrites to syllogisms (especially the vexing mind-body paradox) that Glynn's empirical reasoning fails him. In the end, he concedes his perplexity in trying to conceive of an evolutionary origin for human consciousness. Plato gives us two model philosophical figures, apparently in contrast with each other—one is the otherworldly philosopher who sees truth and reality outside the cave and has the knowledge to rule authoritatively within it; the other is the demotic figure of Socrates, who insists that he does not know but only asks questions. I consider Plato’s contrasting idioms of seeing and asking or talking, and argue that the rich account of perception that is represented in the Republic requires both idioms, (...) and both models, to explain the development of epistemic virtue. Furthermore, the conditions he places on the giving and taking of reasons show how Plato takes intellectual virtue to be inseparable from moral virtue. That integrated picture of virtue may—however disposed we may be towards the role of virtue in either ethics or epistemology—have something to say to us about how philosophy might best be carried on. Harriet Taylor Mill and Anna Wheeler are two nineteenth-century British feminists generally over-shadowed by the fame of the men with whom they co-authored. Yet both made important and interesting contributions to political thought, particularly regarding deconstruction of (i) the patriarchal institution of marriage; and (ii) the current property regime which, in dominating workers, unfairly distributing the product of labour, and encouraging ‘individualism’, they believed did little to maximize the general happiness. Both were feminists, utilitarians, and socialists. How they link these (...) elements is both interestingly similar, and interestingly different. This article has four aims. Firstly, to make a strong claim concerning their authorial hand in works often considered to be solely the work of their male co-author. Secondly, to sketch those co-authoring relationships, and consider whether Taylor and Mill may even have consciously constructed their early letters ‘On Marriage' based upon what they knew of Thompson and Wheeler’s relationship. Thirdly, to map out their shared (though not identical) claim that marriage was a form of slavery, and the proposals they offered to free women from the domination of patriarchal relationships. Fourthly, to explore the way in which both thought female emancipation would be most truly realized via cooperative socialism. (shrink)

Almost 40 years ago, scholars identified a “chilly climate” for women in college classrooms. To examine whether contemporary college classrooms remain “chilly,” we conducted quantitative and qualitative observations in nine classrooms across multiple disciplines at one elite institution. Based on these 95 hours of observation, we discuss three gendered classroom participation patterns. First, on average, men students occupy classroom sonic space 1.6 times as often as women. Men also speak out without raising hands, interrupt, and engage in prolonged conversations during (...) class more than women students. Second, style and tone also differ. Men’s language is assertive, whereas women’s is hesitant and apologetic. Third, professors’ interventions and different structures of classrooms can alter existing gender status hierarchies. Extending Ridgeway’s gender system framework to college classrooms, we discuss how these gendered classroom participation patterns perpetuate gender status hierarchies. We thus argue that the chilly climate is an underexplored mechanism for the stalled gender revolution. (shrink)

In his youth, John Stuart Mill followed his father’s philosophy of persuasion but, in 1830, Mill adopted a new philosophy of persuasion, trying to lead people incrementally towards the truth from their original stand-points rather than engage them antagonistically. Understanding this change helps us understand apparent contradictions in Mill’s cannon, as he disguises some of his more radical ideas in order to bring his audience to re-assess and authentically change their opinions. It also suggests a way of re-assessing the relationship (...) between Mill’s public and private works, to which we should look if we are attempting to understand his thought. (shrink)

Herbert McCabe, OP , was a significant theological figure in England in the last century. A scholar of Aquinas, he was also influenced by Wittgenstein and Marx, his reading of whom helped him articulate a distinctive Thomistic account of human embodiment that serves as a critique of other dominant approaches in ethics. This article shows McCabe's contribution to moral theology by placing his work in conversation with other important approaches, namely, situation ethics, proportionalism, and the New Natural Law (...) This article presents the findings of a study on homeschooling in Czechia. It focuses on the gender aspects of this uncommon educational decision. Based on forty-three individual interviews with homeschooling parents, the article’s unifying thread is interest in understanding how mothers are involved in the decision to homeschool, and how the practice is embedded in gendered relationships within the society. We explore the results from two perspectives. First, we consider how the choice to homeschool lies simultaneously in embracing and opposing (...) cultural imperatives of good mothering. Second, we explore the precarious status of homeschooling mothers in relation to economic independence. By shedding light on individual choice within social structures, we situate the practice of homeschooling within the gender inequalities of today’s society. (shrink)

Even those who are unfamiliar with Aquinas will find this book gripping. Published posthumously, this study is thoroughly rewarding and will increase McCabe's reputation as one of Britain's finest theologians of recent years.

The Dalai Lama once wrote that the object of human existence was to be happy. This sounds extremely glib as happiness in the popular imagination is a feeling and in the words of the song 'the greatest gift that we possess'. On the other hand, von Hugel wrote 'Religion has never made me happy;it's no use shutting your eyes to the fact that the deeper you go, the more alone you will find yourself' This small masterpiece by the late Fr (...) Herbert McCabe of the Dominican order steers a steady courss between these two extremes. We feels instinctively that human beings are designed to enjoy themselves and to be happy and yet we are told that suffering is good for the soul. But in the Catholic tradition the true object of human existence is the vision of God and nothing less than this will ever make us truly happy. But Fr McCabe explores much deeper issues. Is Happiness a pleasure or a pain? You hardly know. Certainly it is not a comfort for comfort spells seciurity and hapiness can take you out of yourself to a degree where all secutiry is left behind. Behind a feeling of exultation, you can sense the flame of incandescent terror. This short book is entirely original and will further enhance McCabe's posthumous reputation. (shrink)

We introduce a family of rules for adjusting one's credences in response to learning the credences of others. These rules have a number of desirable features. 1. They yield the posterior credences that would result from updating by standard Bayesian conditionalization on one's peers' reported credences if one's likelihood function takes a particular simple form. 2. In the simplest form, they are symmetric among the agents in the group. 3. They map neatly onto the familiar Condorcet voting results. 4. They (...) preserve shared agreement about independence in a wide range of cases. 5. They commute with conditionalization and with multiple peer updates. Importantly, these rules have a surprising property that we call synergy - peer testimony of credences can provide mutually supporting evidence raising an individual's credence higher than any peer's initial prior report. At first, this may seem to be a strike against them. We argue, however, that synergy is actually a desirable feature and the failure of other updating rules to yield synergy is a strike against them. Academic integrity policies at 200 institutions of higher education were examined for the presence of academic prohibitions against the nonmedical use of prescription stimulants or any other cognitive enhancing drug. Researchers used online search tools to locate policy handbooks in a stratified random sample of IHE’s drawn from the Integrated Postsecondary Education Data System database, searching for NMUPS/CED use as violations of either academic integrity or alcohol and other drug policies. Of 191 academic integrity policies found online, NMUPS/CED prohibitions were (...) present in only one. However, NMUPS was addressed in all but two of the 200 IHE AOD policies, often with language referencing IHE adherence to federal or state law. NMUPS/CED prohibitions are predominantly absent in IHE academic integrity policies, raising questions about whether colleges and universities are concerned about the use of enhancement drugs as a form of cheating. Implications for fairness, health promotion, and future research are discussed. (shrink)

Though almost forty years have elapsed since its first publication, it is a testament to the philosophical acumen of its author that 'The Matter of Chance' contains much that is of continued interest to the philosopher of science. Mellor advances a sophisticated propensity theory of chance, arguing that this theory makes better sense than its rivals (in particular subjectivist, frequentist, logical and classical theories) of ‘what professional usage shows to be thought true of chance’ (p. xi) – in particular ‘that (...) chance is objective, empirical and not relational, and that it applies to the single case’ (ibid.). The book is short and dense, with the serious philosophical content delivered thick and fast. There is little by way of road-mapping or summarising to assist the reader: the introduction is hardly expansive and the concluding paragraph positively perfunctory. The result is that the book is often difficult going, and the reader is made to work hard to ensure correct understanding of the views expressed. On the other hand, the author’s avoidance of unnecessary use of formalism and jargon ensures that the book is still reasonably accessible. In the following, I shall first summarise the key features of Mellor’s propensity theory, and then offer a few critical remarks. (shrink)

Of the many ideological blind spots that have afflicted US and, to a lesser extent, European, perceptions and analysis of the economic, political and social milieu, none have been more debilitating than the equation of democracy with political liberalism. Thus those who attempt to derive propaganda value from such an equation are vulnerable, as the US government has found, to the rhetorical counter attack that in opposing democratically elected governments, such as that of Hamas or Hugo Chavez, they are not (...) merely being anti-democratic, but are in illiberally opposition to human rights and civil liberties also; an argument quiteindependent of the same charges, emanating more legitimately, from their support of, for example, the Masharraf regime and the Saud dictatorship.Furthermore no less an august body than the Council of Europe has drawn attention to the US government’s inhumane, humiliating, degrading and cruel treatment, including torture, of prisoners, at Guantanamo, and, seemingly even more extreme treatment of prisoners in the supposedly secret or “black” prisons operated both by the CIA, and other countries, where the torture of prisoners, often illegally or extra judicially rendered to them, has been outsourced. In light of this the paper takes up a discussion of the nature of the relationship between Liberalism, Democracy and Torture as it is germane to the current legitimation crisisfacing liberal democracies. We investigate whether standard counterfactual analyses of causation imply that the outcomes of space-like separated measurements on entangled particles are causally related. Although it has sometimes been claimed that standard CACs imply such a causal relation, we argue that a careful examination of David Lewis’s influential counterfactual semantics casts doubt on this. We discuss ways in which Lewis’s semantics and standard CACs might be extended to the case of space-like correlations.

The complex way Meno's paradox is presented in the Meno forces reflection on both the external conditions on inquiry—its objects—and its internal conditions—the state of mind of the person who inquires. The theory of recollection does not fully account for the internal conditions—as Plato makes clear in the critique of Meno's puzzle to be found in the Euthydemus. I conclude that in the Euthydemus Plato is inviting us to reject the externalist account of knowledge urged on Socrates by the sophists (...) in favour of a more richly internalist epistemology. This paper examines texts written by, or in collaboration with, female ex-members of the Italian left-wing armed organization, the Red Brigades. The corpus differs from male-authored or male-centred texts in that issues relating to identity and selfhood lie at the very heart of the project of narrating the terrorist past; the primary concern of Italian women's post-terrorist narration is not to narrate the experience of belonging to an armed organization, but to construct a new identity distinct from a pre-existing self (...) identified exclusively with the transgressive experience of political violence. I consider the corpus in the light of a number of critical problems posed both by the specificity of female perpetration and by the dearth of theoretical writings on perpetrator trauma more generally. I identify in each text an acute anxiety about the very act of speech or narration and find that, in order to circumvent the perceived prohibition on speech, the women of the Red Brigades subtly insinuate into their life writing a discourse of alterity bordering on subalternity that obscures the boundary between victim and perpetrator. The unacknowledged slippage between discourses of perpetration and victimization is explored in relation to Ruth Leys’ critique of Cathy Caruth's formulation of trauma as the wound that cries out through the voice of the victim. The paper concludes by questioning whether perpetrator trauma can ever be articulated as such and by considering the implications of that question for traumatized perpetrator and victimized society alike. An influential tradition in the philosophy of causation has it that all token causal facts are, or are reducible to, facts about difference-making. Challenges to this tradition have typically focused on pre-emption cases, in which a cause apparently fails to make a difference to its effect. However, a novel challenge to the difference-making approach has recently been issued by Alyssa Ney. Ney defends causal foundationalism, which she characterizes as the thesis that facts about difference-making depend upon facts about physical causation. (...) She takes this to imply that causation is not fundamentally a matter of difference-making. In this paper, I defend the difference-making approach against Ney’s argument. I also offer some positive reasons for thinking, pace Ney, that causation is fundamentally a matter of difference-making. (shrink)

These three reflections look at the theological and ethical implications of sexual violence in light of the attention brought by #MeToo. The first explores ethnographic interviews which indicate that Church leaders, teachers, and parents contribute to rape culture by leaving sexual violence unaddressed in Christian sexual education, arguing that it must be reconstructed to eliminate the Church’s participation in a culture that promotes gender-based violence. The second notes that feminist scholarship has made the case that rape and “unjust sex” are (...) associated with what is considered acceptable heterosexuality, require the category of “cultural sin” to account for the social responsibility of persons. Finally, the third explores how a feminist political theological ethics of “dangerous memory” is required to critique of the structures and systems that violate women’s selves and bodies. (shrink)
