As an incapacitated patient is unable to make decisions regarding their care, physicians turn to next-of-kin when appointing a surrogate decision-maker in the absence of an advanced directive. With the increasing complexity of modern families, physicians are facing new ethical dilemmas when choosing the individual to make end-of-life decisions for their patients. Legal definitions and hierarchies are no longer adhering to the purpose of a surrogate-decision maker, which is to maintain a patient’s autonomy. Moral criteria for surrogates, which emphasize the (...) importance of making decisions that align with the patient’s desires and wishes and negate biological relationships over emotional connections, are becoming much more important. This paper explores a case study in which physicians must appoint a surrogate decision-maker for an incapacitated patient, forced to choose between a biological relationship and a strong emotional connection. BackgroundCommentators believe that the ethical decision-making climate is instrumental in enhancing interprofessional collaboration in intensive care units. Our aim was twofold: to determine the perception of the ethical climate, levels of moral distress, and intention to leave one's job among nurses and physicians, and between the different ICU types and determine the association between the ethical climate, moral distress, and intention to leave.MethodsWe performed a cross-sectional questionnaire study between May 2021 and August 2021 involving 206 nurses and physicians in a (...) large urban academic hospital. We used the validated Ethical Decision-Making Climate Questionnaire and the Measure of Moral Distress for Healthcare Professionals tools and asked respondents their intention to leave their jobs. We also made comparisons between the different ICU types. We used Pearson's correlation coefficient to identify statistically significant associations between the Ethical Climate, Moral Distress, and Intention to Leave.ResultsNurses perceived the ethical climate for decision-making as less favorable than physicians. They also had significantly greater levels of moral distress and higher intention to leave their job rates than physicians. Regarding the ICU types, the Neonatal/pediatric unit had a significantly higher overall ethical climate score than the Medical and Surgical units and also demonstrated lower moral distress scores and lower “intention to leave” scores compared with both the Medical and Surgical units. The ethical climate and moral distress scores were negatively correlated ; moral distress and "intention to leave" was positively correlated ; and ethical climate and “intention to leave” were negatively correlated.ConclusionsSignificant differences exist in the perception of the ethical climate, levels of moral distress, and intention to leave between nurses and physicians and between the different ICU types. Inspecting the individual factors of the ethical climate and moral distress tools can help hospital leadership target organizational factors that improve interprofessional collaboration, lessening moral distress, decreasing turnover, and improved patient care. The relation between the regimes of the accumulation of men and the accumulation of capital is problematised in the works of Michel Foucault. The paper challenges the prevailing wisdom that the relation between these regimes is contingent. The fundamental question of the conditions of the possibility of relation between the two regimes is raised. It is argued that both regimes are primordially related. Focusing on the Foucauldian analysis of the regime of the accumulation of men and its constituent elements an (...) effort is made to thematize the primordial relation between the two regimes. It is shown that freedom is the condition of the possibility of a primordial relation between the two regimes. It is explained why freedom plays such a fundamental role in making possible and sustaining a capitalist order. The dual role of freedom as a principle of diversity and a principle of management is stressed. It is argued that capitalism as an order is conditioned upon the production and reproduction of individuals and populations that are simultaneously useful and free. It is also the condition of such an order that docility is produced without hampering utility. Freedom makes possible the enhancement of utility without making it unmanageable. (shrink)

This essay introduces a special issue on the history of kāmaśāstra in medieval India. It briefly reviews the secondary scholarship on the subject from the publication of the first translations of the genre at the end of the nineteenth century. It highlights the relatively unexplored history of later kāmaśāstra, and stresses the need for contexualized and detailed studies of the many kāmaśāstra treatises produced in the second millennium CE. The introduction, and the essays that follow, also argue for an expanded (...) interpretive framework for the genre, moving beyond ‘sex’ and ‘sexuality,’ to a more widely defined notion of a ‘kāma world’, in which sensual pleasure is understood as being deeply enmeshed with aesthetic, ethical and cosmopolitan cultures. Spice is also peppered throughout the Old Testament.2 The Song of Solomon recites: “Thy plants are an orchard of pomegranates, with pleasant fruits; camphire, with spikearion 28.1 spring/summer 2020 2 a vexed pharmacopeia nard and saffron; calamus and cinnamon, with all trees of frankincense; myrrh and aloes, with all the chief spices.”3 Every one of these fruits, plants and spices remained popular in the West throughout antiquity, into the Middle Ages, and beyond. As early as 3000 bc, trade routes had connected the Mediterranean to the near east. Courtesy of this “Spice Road,” ancient Mediterranean civilizations had access to a wide range of horseradishes, mustards, peppers, gingers and other spices. The Greeks and Romans had a special relationship with black pepper—so much so that four out of five recipes in Apicius’s famous 1st century cookbook4 called for a sprinkling of this sometimes-costly commodity. It is perhaps not coincidental that black pepper contains heat-producing alkaloids very similar to those found in New World chili peppers. Much has been written about the important role of chilis in South and Central American culture—a region that, by the Middle Ages, was in many respects the equal of Europe in “artistic sensibility, social complexity, and political organization.”5 The habanero and its capsainoid cousins were at the heart of the culinary and ceremonial customs of Mesoamerica, their biting heat and florid livery an apt metaphor for the “extravagant feats of human butchery”6 —very much in the tradition of the spectacular venationes of ancient Rome—for which the Aztecs at least were well known. To what extent was hot spice also a significant part of the culinary, medical and cultural landscape of the Old World? At a conference hosted by the Institute for Digital Archaeology at the University of Oxford last December, experts from Europe and North America examined the literary, historical, archaeological and scientific evidence around these questions. Their inquiry received a boost from new and rapidly developing techniques for examining the physical evidence of ancient diet and food production—laboratory techniques not available even five years ago. There was also fresh medical evidence to consider: significant research is being done in the area of chemosensation in the context of spicy foods. This Roger Michel, Alexy Karenowska, George Altshuler, Matthew Cobb 3 includes trying to understand how different so-called “TRP channels” in the nervous system process spice information— in particular, the connection between spice and endorphin levels. It appears that many of the hot spices available in antiquity could potentially trigger the same TRP channels as the capsaicin-charged Mesoamerican peppers. This creates a nice connection between old and new world spice that helps to explain and contextualize both. However, in the end, firm conclusions at the Oxford conference were scarce. It quickly became clear that more work, a greater diversity of viewpoints, better refined questions, and a change of venue were required to make additional progress. The present research applies Michel Foucault's perspectives on discipline and punishment to Steve Hamilton's novel The Lock Artist, exploring the criminal subjectivity of the protagonist beyond the binary of good and evil. Through textual analysis, the study examines the norms, upbringing, and circumstances that shape the protagonist's identity as a deviant and criminal. The research also investigates the role of confession as a tool of the modern state in altering the subjectivity of its subjects. The findings suggest that deviance and (...) illegality are social constructs, created and maintained by power structures. The study highlights how society defines deviance and the state defines illegality, leading individuals down a path of criminalization. This research provides insight into the ways power operates to shape an individual’s subjectivity, particularly in relation to criminality, and offers a critical analysis of the ways the modern state exercises control over its subjects. For useful explanation and analysis of criminal subjectivity, Michel Foucault’s book ‘discipline and punishment’ has been referred. This article discusses the norms, upbringing, and circumstances of the protagonist and tries to explore his subjectivity as a criminal beyond the paradigm of good and evil. It elaborates those factors which constitute his identity as a deviant and then as a criminal. A despeito disso, poucos têm se dedicado a investigar suas cinco visitas a nosso país, datadas de 1965, 1973, 1974, 1975 e 1976. Sendo assim, a autora desenvolveu a pesquisa "Michel Foucault no Brasil: presença, efeitos e ressonâncias", cujos objetivos incluem o estabelecimento de uma "audiografia" da presença do Foucault-corpo no Brasil - análise do modo como ele aqui ocupou os espaços de fala -, bem (...) como de uma "geo-epistemologia" - busca das condições geopolíticas de produção do saber - e de uma "cronobibliografia" das ideias de Foucault entre nós - exame analítico-crítico das temporalidades associadas à primazia conferida a determinados procedimentos, categorias, problemáticas e conceitos pelos intelectuais e militantes brasileiros. Para tanto, foram utilizados documentos escritos e fonográficos relativos às visitas, artigos, coletâneas e livros brasileiros voltados à discussão das perspectivas de Foucault, e narrativas orais tanto dos que com ele conviveram nas passagens por São Paulo, Rio de Janeiro, Belo Horizonte, Salvador, Recife e Belém como daqueles que, atualmente, exibem efeitos e ressonâncias de seu pensamento nos âmbitos acadêmico, profissional e político. Bringing together the worlds of photography, geography, and philosophy to create an interdisciplinary theoretical understanding of place in relation to photography, Shobeiri has expertly unearthed a nuanced, exciting way of considering the photographic process and images.Shobeiri, an Assistant Professor of Photographic Studies and Visual Arts Theory at Leiden University, brings his knowledge and expertise of visual culture, photographic theory, human geography, and philosophy to the fore in his latest publication. His knowledge, passion, personal interest in photography, and his critical thought come through nicely in his writing, and it is clear that the development of Shobeiri’s ideas and framework have been given a lot of thought, time, and energy. This book meaningfully speaks to those interested in a discussion of place within the realm of photography, evidenced through the depth of knowledge and sheer detail explored within this book.Inspired by his own experience as a photographer and influenced by Edward Casey’s “delimited” definition of place, Shobeiri explores how place is never simply presented or fixed (11). It is here that he expertly uses photography to do this across six different yet clearly complimentary chapters. Critically, there is no way to give a succinct overview of the chapters of this book without losing meaning and diluting Shobeiri’s message; the book is rich, deeply detailed, embedded within philosophical thought, and is nuanced throughout, which is important to remember when reading this non- fiction piece of work.In the first chapter, Shobeiri provides a phenomenological exploration of the importance of the photographer’s body both as a lived experience and a physical entity. He then juxtaposes this duality to the lived [End Page 136] place of the photograph, returning to the notion of fixedness and using the idea of belonging to explore the photographer’s body and the photographic experience as a lived place. The next chapter discusses spatiality and time, and the way in which space is temporalized, using key influences such as Roland Barthes and Susan Collins to illustrate the argument. Central to this is the idea that a photograph captures time, but also aims at space, which raises questions about what this means for the space before the lens. In chapters 3 and 4, the author argues that the photograph itself travels through space, and highlights the importance of the photographic place, allowing us to “bestow meaning upon that space on the photograph’s surface” (67). Interestingly, Shobeiri points to philosophy to help the reader understand this, such as Jacques Derrida, and uses more familiar images and scholarly thought from NASA to discuss blind field and the liminality of photographic place. Chapter 5 moves away from location and journeys the reader to a discussion of places viewed as moment of encounter, specifically considering the spectator and the photograph. This evolves further in the sixth and final chapter insofar as it explores the transition and translation of the photographed place to the communication of the content of the image and the agency of the photograph. Shobeiri finishes with a simple yet engaging epilogue, several helpful notes, and a plethora of references which are useful for further reading.Place: Towards a Geophilosophy of Photography is genuinely interesting, centred within academic scholarship, and employs the thought of theorists that readers of Environment, Space, Place will undoubtably recognise. Scholarly figureheads such as Jacques Derrida, Alain Badiou, and Michel Foucault, space and place authorities such as Tim Cresswell, Tim Ingold, and Doreen Massey, and art aficionados such as Roland Barthes, Susan Collins, and John Berger frequent the pages. Familiar artistic names are also drawn upon, including Ansel Adams, Gert Jan Kocken, and Gerhard Richter, which soften the strong academic tones that this book voices. Estas noções consistem no que Foucault chamou de Racismo de Estado, e no que Agamben nomeou de Tanatopolítica. O contexto, e tambémo objeto, no qual se efetiva este intento é, em uma palavra, a política totalitária do Estado nazista.Ambos os autores compreendem os fenômenos (...) totalitários a partir do ponto de vista da biopolítica, entretanto, ao tratar do Estado Nacional-socialista, Foucault identifica nele um paradoxo, sem, contudo desenvolver com maior profundidade a imbricação que ali se deu. Agamben, por sua vez, concebe semelhante paradoxo como pertencendo a uma forma de fazer política que já traz, desde a sua origem, os fundamentos que permitiram a emergência do fenômeno da forma como surgiu e que fora identificada pelos ditos autores, ou seja, trazendo em si tanto elementos antigos, como a glória e os ideais de sangue, quanto modernos, como a eugenia. We examine whether mindfulness can neutralize the negative impact of COVID-19 stressors on employees’ sleep duration and work engagement. In Study 1, we conducted a field experiment in Wuhan, China during the lockdown between February 20, 2020, and March 2, 2020, in which we induced state mindfulness by randomly assigning participants to either a daily mindfulness practice or a daily mind-wandering practice. Results showed that the sleep duration of participants in the mindfulness condition, compared with the control condition, was less (...) impacted by COVID-19 stressors (i.e., the increase of infections in the community). In Study 2, in a 10-day daily diary study in the United Kingdom between June 8, 2020, and June 19, 2020, we replicate our results from Study 1 using a subjective measure of COVID-19 stressors and a daily measure of state mindfulness. In addition, we find that mindfulness buffers the negative effect of COVID-19 stressors on work engagement mediated by sleep duration. As the COVID-19 pandemic is ongoing and the number of reported cases continues to rise globally, our findings suggest that mindfulness is an evidence-based practice that can effectively neutralize the negative effect of COVID-19 stressors on sleep and work outcomes. The findings of the present study contribute to the employee stress and well-being literature as well as the emerging organizational research on mindfulness. (shrink)

Our shared program of research has led us to discover the potentials, complexities, and dilemmas associated with research assistantships where newcomers assist more experienced researchers to conduct research projects. We considered a wide range of proposals and papers addressing different aspects of research assistantships. The resulting collection includes self-studies and analyses of others, as well as policy reviews and recommendations. Some critical philosophers of race have argued that whiteness can be understood as a technology of affect and that white supremacy is comprised partly of unconscious habits that result in racialized perception. In an effort to deepen our understanding of the affective and bodily dimensions of white supremacy and the ways in which affective habits are socially produced, I look to insights from situated affectivity. Theorists in this field maintain that affective experience is not simply a matter of felt inner (...) states, but rather socially and environmentally embedded and fundamentally relational. Jan Slaby presents the concept of an ‘affective arrangement’ as a way to approach affectivity in terms of relational dynamics unfolding within a particular setting. Applying this concept to the societal level, Paul Schuetze introduces the notion of ‘affective milieu.’ I argue that these notions of ‘affective arrangement’ and ‘affective milieu,’ together with an organicist account of habit, can help to illuminate the workings of white supremacy in the United States. My proposed account highlights the extent to which white supremacy is an affective, bodily phenomenon and how racist habits are formed over the course of learning and ongoing affective engagement, in the context of various social settings. Crucially, these affective habits are fully bound up with habits of appraisal, interpretation, and judgment, and therefore inseparable from how subjects come to see and understand their world. Few theorists would challenge the idea that affect and emotion directly influence decision-making and moral judgment. There is good reason to think that they also significantly assist in decision-making and judgment, and in fact are necessary for fully effective moral cognition. However, they are not sufficient. Deliberation and more reflective thought processes likewise play a crucial role, and in fact are inseparable from affective processes. I will argue that while the dual-process account of moral judgment set forth by Craigie (2011) (...) has great merit, it fails to appreciate fully the extent to which affective and reflective processes are not only integrated, but also mutually interdependent. Evidence from psychopathy indicates that when reflective processes are not assisted adequately by what I will call ?affective framing?, and moral cognition is of the ?cooler,? less emotionally-informed variety, what results is not effective cognitive functioning, but rather psychopathology. My proposed account of affective framing aims to make sense of the way in which affect plays a strictly necessary and integral role not just in intuitive moral responses, but also in reflective moral judgments, so that moral cognition is accomplished by the joint operation of affective processes and reflective reasoning processes. 'Propositionalism' is the widely held view that all intentional mental relations-all intentional attitudes-are relations to propositions or something proposition-like. Paradigmatically, to think about the mountain is ipso facto to think that it is F, for some predicate 'F'. It seems, however, many intentional attitudes are not relations to propositions at all: Mary contemplates Jonah, adores New York, misses Athens, mourns her brother. Although mental health professionals traditionally have been viewed as sole experts and decision-makers, there is increasing awareness that the experiential knowledge of former patients can make an important contribution to mental health practices. I argue that current patients likewise possess a kind of expertise, and that including them as active participants in diagnosis and treatment can strengthen their autonomy and allow them to build up important habits and skills. To make sense of these agential benefits and describe how patients might (...) act as co-regulators of a therapeutic encounter, I look to the enactivist notion of “participatory sensemaking”. We assessed the attitudes, perceptions, and stereotypes toward Latino immigrants among 247 mental health professionals across 32 U.S. states. We also randomly presented two versions of an attitude measure that varied in their references to immigrants. Participants reported that they did not agree with the anti-immigration law Arizona SB 1070 and other similar bills. Also, greater multicultural awareness was related to positive attitudes and fewer stereotypes toward immigrants. Furthermore, participants who were asked to think about “undocumented immigrants” viewed Latino immigrants (...) more positively than those who were asked think about “illegal aliens.” Findings show the continued need for multicultural awareness and competence training for mental health professionals, which align with the American Psychological Association’s Ethical Principles of Psychologists and Code of Conduct. Franz Brentano’s theory of emotion is tightly bound up with many of his other central claims, in such a way that one has to work out how it relates to these other claims if one is to understand its distinctive character. There are two main axes of investigation. The first results from the fact that Brentano introduces his theory of emotion as part of his overall theory of mind, which consists of a number of closely interconnected theses concerning the nature (...) of mental phenomena, the nature of consciousness, and the classification of mental phenomena into fundamental kinds (Brentano 1874/1973a). The second derives from the fact that his theory of emotion also forms the foundation of his epistemology of objective value, which he elaborates in a framework constituted by an epistemic theory of truth and an extended analogy he draws between emotions and judgments (Brentano 1889/1969). In this consensus report by a diverse group of academics who conduct and/or are concerned about social and behavioral genomics (SBG) research, the authors recount the often‐ugly history of scientific attempts to understand the genetic contributions to human behaviors and social outcomes. They then describe what the current science—including genomewide association studies and polygenic indexes—can and cannot tell us, as well as its risks and potential benefits. They conclude with a discussion of responsible behavior in the context of SBG research. (...) SBG research that compares individuals within a group according to a “sensitive” phenotype requires extra attention to responsible conduct and to responsible communication about the research and its findings. SBG research (1) on sensitive phenotypes that (2) compares two or more groups defined by (a) race, (b) ethnicity, or (c) genetic ancestry (where genetic ancestry could easily be misunderstood as race or ethnicity) requires a compelling justification to be conducted, funded, or published. All authors agree that this justification at least requires a convincing argument that a study's design could yield scientifically valid results; some authors would additionally require the study to have a socially favorable risk‐benefit profile. Existing research on the “weapons effect” indicates that simply seeing a weapon can prime aggressive thoughts and appraisals and increase aggressive behavior. But how and why does this happen? I begin by discussing prevailing explanations of the weapons effect and propose that these accounts tend to be over-intellectualistic insofar as they downplay or overlook the important role played by affectivity. In my view, insights from the fields of situated affectivity and enactivism help us to understand how cognitive and affective processes (...) jointly contribute to the weapons effect. Insofar as the presence of weapons alters subject’s bodily-affective orientation and thereby brings about embodied mindshaping, it changes the way they engage with and understand their surroundings. To understand the weapons effect, we will need to examine the constitutive interdependency of appraisal and affectivity and the way in which they jointly motivate action. My proposed account emphasizes the role of affectivity in affordance perception and the way in which subjects gauge the meaning of an object according to its action-possibilities. (shrink)

Awe has been defined as an emotional response to perceptually vast stimuli that overwhelm current mental structures, yet facilitate attempts at accommodation. Four studies are presented showing the information-focused nature of awe elicitors, documenting the self-diminishing effects of awe experience, and exploring the effects of awe on the content of the self-concept. Study 1 documented the information-focused, asocial nature of awe elicitors in participant narratives. Study 2 contrasted the stimulus-focused, self-diminishing nature of appraisals and feelings associated with a prototypical awe (...) experience with the self-focused appraisals and feelings associated with pride. Study 3 found that dispositional awe-proneness, but not dispositional joy or pride, was associated with low Need for Cognitive Closure, and also documented a relationship between dispositional awe and increased emphasis on membership in “universal” categories in participants’ self-concepts. Study 4 replicated the self-concept finding from Study 3 using experimentally elicited awe. Implications for future work on awe are discussed. (shrink)

The author argues that well-known forms of relativism are unable to accommodate, at once, a set of three highly intuitive theses about the distinctive character of moral reasons. Yet the author argues it is possible to formulate a novel form of normative relativism that has the power to accommodate these claims. The proposed view combines the relativist idea that the normative facts are attitude-dependent with the insight that there are non-human agents to which it makes sense to attribute the kinds (...) of attitudes that give rise to normative reasons. Societies, too, can possess reasons to pursue their aims. What distinguishes moral reasons from reasons of practical rationality is that the former apply directly to societies in virtue of aims held by each society as a group, while the latter apply directly to persons in light of their own individual interests. The reasons for this are twofold. Firstly, the practice of storytelling is ancient, pre-dating not only the advent of writing, (...) but of agriculture and permanent settlement as well. Secondly, narrative is ultimately a product of the mind, which in turn is the product of a long history of evolution by natural selection. Thus, an understanding of why and how humans create and consume narrative requires an understanding of (1) features of ancestral environments and (2) features of the mind that made the emergence of this phenomenon possible.There can be little doubt that narrative emerged in human prehistory. Language, an obvious prerequisite for storytelling, is likely to have emerged by at least 50,000 and possibly 250,000 years ago, depending upon whether one places one's trust in archaeological or anatomical evidence. 1 The most reasonable estimate is offered by Geoffrey Miller, who points out that, given its universality, the language faculty must have emerged by the time ancestral Homo sapiens began migrating out of Africa approximately 100,000 years ago. 2 Although the oldest known written narrative (The Epic of Gilgamesh) dates back only 5,000 years, the written literary traditions of many ancient cultures are known to be rooted in longstanding oral traditions. 3 The fact that many modern foraging peoples have rich and complex oral traditions further suggests that the emergence of narrative is not linked to the development of agriculture 10,000 years ago. Moreover, other forms of symbolic expression, such as the cave paintings, Venus figurines, and engraved bone [End Page 233] and antler that have been found at various sites throughout Europe, date back approximately 30,000 years, and rock paintings in Australia may date back even farther. 4 Since humans were physiologically capable of speech long before they began producing these artifacts, storytelling is likely to be at least as ancient as these other representational forms. Indeed, one scholar situates the "dawn of the oral tradition" within this period (Pfeiffer, p. 189). Given, then, that modern humans (Homo sapiens sapiens) have been in existence for approximately 100,000 years 5 and are the only hominid species or subspecies known for certain to exhibit storytelling behavior, we can safely say that oral narrative is a product of our hunting-and-gathering past, likely to have emerged between 30,000 and 100,000 years ago.The universality of narrative is further testimony to its being an ancient cognitive phenomenon. Literate or not, all known cultures, past and present, practice storytelling. 6 Moreover, all normally developing humans acquire the ability to process and generate stories: studies of Western children indicate that the ability to tell stories emerges spontaneously between the ages of two-and-a-half and three, and children as young as thirty months can distinguish between narrative and non-narrative uses of language. 7 In contrast to reading, writing, and arithmetic, no special education is required for narrative competence to develop, nor is there any evidence that oral literacy is acquired through contact with other cultures; although subject matter is often exchanged between groups, the practice of storytelling itself arises independently among even the most isolated peoples. Michelle Kosch examines the conceptions of free will and the foundations of ethics in the work of Kant, Schelling, and Kierkegaard. She seeks to understand the history of German idealism better by looking at it through the lens of these issues, and to understand Kierkegaard better by placing his thought in this context. Kosch argues for a new interpretation of Kierkegaard's theory of agency, that Schelling was a major influence and Kant a major target of criticism, and that both (...) the theory and the criticisms are highly relevant to contemporary debates. In an earlier paper I argued that J.G. Fichte (rather than Kant or Hegel or some amalgam) was the primary historical model for the ethical standpoint described in Kierkegaard’s Either/Or II . There I offered a list of reasons for thinking that Hegel was less important than some believed and that Kierkegaard addressed Kantianism largely in its Fichtean form. In the interim I have discovered another reason to add to that list: as it happens, there was a quite general consensus (...) among philosophers in the 1830s, ’40s and ’50s not just that Fichte’s ethics was Kantian ethics in its most perfect form, but even that it was the best example available of normative ethics on a philosophical (rather than a religious) foundation. So Kierkegaard’s use of Fichte as a foil was the perfectly obvious choice in the context. That is a curious fact, given the complete obscurity of Fichte’s ethics today, and one whose interest should not be confined to Kierkegaard scholars. (shrink)

The use of preimplantation genetic diagnosis (PGD) has expanded both in number and scope over the past 2 decades. Initially carried out to avoid the birth of children with severe genetic disease, PGD is now used for a variety of medical and non-medical purposes. While some human studies have concluded that PGD is safe, animal studies and a recent human study suggest that the embryo biopsy procedure may result in neurological problems for the offspring. Given that the long-term safety of (...) PGD has not been clearly established in humans, this study sought to determine how PGD safety is presented to prospective patients by means of a detailed website analysis. The websites of 262 US fertility centres performing PGD were analysed and comments about safety and risk were catalogued. Results of the analysis demonstrated that 78.2% of centre websites did not mention safety or risk of PGD at all. Of the 21.8% of centres that did contain safety or risk information about PGD, 28.1% included statements highlighting the potential risks, 38.6% presented information touting the procedure as safe and 33.3% included statements highlighting potential risks and the overall safety of the procedure. Thus, 86.6% of PGD-performing centres state that PGD is safe and/or fail to disclose any risks on their websites despite the fact that the impact of the procedure on the long-term health of offspring is unproven. This lack of disclosure suggests that informed consent is inadequate; this study examines numerous factors that are likely to inhibit comprehensive discussions of safety. (shrink)
