Publisher Summary This chapter describes the different aspects of the theory of mind and autism. Difficulty in understanding other minds is a core cognitive feature of autism spectrum conditions. It is found that normal 3- to 4-year-olds already know that the brain has a set of mental functions, such as dreaming, wanting, thinking, and keeping secrets. In contrast, children with autism appear to know about the physical functions, but typically fail to mention any mental function of the brain. Children with (...) autism, when studied under experimental conditions, have been shown to have difficulties both in the production of deception and in understanding when someone else is deceiving them. People with autism-spectrum conditions are clearly having mentalizing difficulties for reasons different from those seen in people with learning difficulties or those who are blind or deaf, since a deficit can be revealed even in the highest functioning individuals with an autism-spectrum condition in whom general comprehension problems can be ruled out. Data access committees (DAC) gatekeep access to secured genomic and related health datasets yet are challenged to keep pace with the rising volume and complexity of data generation. Automated decision support (ADS) systems have been shown to support consistency, compliance, and coordination of data access review decisions. However, we lack understanding of how DAC members perceive the value add of ADS, if any, on the quality and effectiveness of their reviews. In this qualitative study, we report findings from 13 semi-structured (...) interviews with DAC members from around the world to identify relevant barriers and facilitators to implementing ADS for genomic data access management. Participants generally supported pilot studies that test ADS performance, for example in cataloging data types, verifying user credentials and tagging datasets for use terms. Concerns related to over-automation, lack of human oversight, low prioritization, and misalignment with institutional missions tempered enthusiasm for ADS among the DAC members we engaged. Tensions for change in institutional settings within which DACs operated was a powerful motivator for why DAC members considered the implementation of ADS into their access workflows, as well as perceptions of the relative advantage of ADS over the status quo. Future research is needed to build the evidence base around the comparative effectiveness and decisional outcomes of institutions that do/not use ADS into their workflows. (shrink)

Autism and Asperger syndrome are psychiatric conditions diagnosed primarily on the basis of deficits and problems in social behaviour; interaction and communication. At present the explanation of these behavioural features is dominated by three cognitive models. However, it is a characteristic of each of these models that they only explain a sub-set of the overall features.The aim of this paper is to suggest an alternative conceptual theory of autism and Asperger syndrome that unites the current three models. Thus, the aim (...) is to situate the existing models as special cases of the one being proposed here. This alternative conceptualisation draws heavily on distinctions and ideas present in the philosophy of science, most especially in the area of critical realism. Central to the theory is the idea that the core “problem” in autism and Asperger syndrome concerns ontological depth. More specifically, that people with these conditions find it difficult to cope with phenomena characterised by depth , open systems and high internal relationality. (shrink)

After 40 years of attributing high rates of physician career dissatisfaction, attrition, alcoholism, divorce and suicide to ‘burnout’, there is growing recognition that these outcomes may instead be caused by moral injury. This has led to a debate about the relative diagnostic merits of these two terms, a recognition that interventions designed to treat burnout may be ineffective, and much perplexity about how—if at all—this changes anything. The current research seeks to develop the construct of moral injury outside military contexts, (...) generate more robust validity tests and more fully describe and measure the experiences of persons exposed to moral harms. Absent from the literature is a mechanism through which to move from the collective moral injury experience of physicians to a systematic change in the structure of medical practice. To address this, after providing a brief history, definitions and contrasts between burnout, moral distress and moral injury, we review the interplay of moral and ethical codes in the context of moral injury. We conclude by suggesting that professional associations can potentially prevent moral injury by providing protections for physicians within their codes of ethics. There are no data in this work. In this article we give a general categorical construction via reflection functors for various completions of T0-spaces subordinate to sobrification, with a particular emphasis on what we call the -completion, a type of directed completion introduced by Wyler [O. Wyler, Dedekind complete posets and Scott topologies, in: B. Banaschewski, R.-E. Hoffmann , Continuous Lattices Proceedings, Bremen 1979, in: Lecture Notes in Mathematics, vol. 871, Springer Verlag, 1981, pp. 384–389]. A key result is that all completions of a certain type are (...) universal, hence unique . We give a direct definition of the -completion and develop its theory by introducing a variant of the Scott topology, which we call the d-topology. For partially ordered sets the -completion turns out to be a natural dcpo-completion that generalizes the rounded ideal completion. In the latter part of the paper we consider settings in which the -completion agrees with the sobrification respectively the closed ideal completion. (shrink)

It examines the effect on the educational pattern of such major cultural upheavals as the Renaissance; it looks at the different parts played by church and state, and the influence of new social and educational philosophies.

Social prescriptions are one term commonly used to describe non-pharmaceutical approaches to healthcare and are gaining popularity in the community, with evidence highlighting psychological benefits of reduced anxiety, depression and improved mood and physiological benefits of reduced risk of cardiovascular disease and reduced hypertension. The relationship between human health benefits and planetary health benefits is also noted. There are, however, numerous barriers, such as duration and frequencies to participate in activities, access, suitability, volition and a range of unpredictable variables impeding (...) a comprehensive approach to their use on a wider scale. From a multidisciplinary perspective, this commentary incorporates a salutogenic and nature-based approach to health, we also provide a range of recommendations that can be undertaken at the patient level to assist in shifting the acknowledged systemic barriers currently occurring. These include using simple language to explain the purpose of health empowerment scripts, ensuing personal commitment to a minimum timeframe, enabling ease of access, co-designing a script program, providing ongoing motivational support and incorporating mindfulness to counter unexpected disruptions.
