An emerging approach to complement randomised controlled trial (RCT) data in the development of radiotherapy treatments is to use routinely collected ‘real-world’ data (RWD). RWD is the data collected as standard-of-care about all patients during their usual cancer care pathway. Given the nature of this data, important questions remain about the permissibility and acceptability of using RWD in routine practice. We involved and engaged with patients, carers and the public in a two-day citizens’ jury to understand their views and obtain (...) decisions regarding two key issues: (1) preferred approaches to consent for the use of RWD within the context of patients receiving radiotherapy for lung cancer in RAPID-RT and (2) how RWD use should be best communicated to patients. Individual views were polled using questionnaires at various stages of the jury, whilst group discussion activities prompted further dialogue about the rationale behind choices of consent. Key decisions obtained from the jury include: (1) an opt-out approach to consent for the use of RWD; (2) the opt-out approach to consent should be informed. Furthermore, it was advised that information and communication regarding the consent process and use of RWD should be accessible, clear and available in a variety of formats. It is important that the consent process for patient data use is underpinned by principles of autonomy and transparency with clear channels of communication between those asking for and giving consent. Moreover, the process of seeking consent from patients should be proportionate to the risks presented from their participation. (shrink)

This chapter is an edited transcript of a panel discussion at the Truth 20|20 Conference. The discussion centers around a discussion between P.F. Strawson and Gareth Evans recorded for the Open University in 1973. In the ensuing discussion, Strawson’s and Evans’ comments on truth are compared both to Ramsey’s work on truth just before his death, and also to contemporary pluralist accounts. One of the major themes of the discussion is the distinction, suggested by Strawson and Evans, between a (...) ‘thin’ notion of truth suitable for application across a variety of domains, and a more substantive notion appropriate for certain domains but not others. The panelists consider whether these two notions could be made to fit into a coherent overarching account, and whether the theory of truth is the right place to locate this ‘thin’/‘substantive’ distinction. Is non-cognitivism compatible with minimalism about truth? A contemporary argument claims not, and therefore that moral realists, for example, should take heart from the popularity of semantic minimalism. The same is said to apply to cognitivism about other topics—conditionals, for example—for the argument depends only on the fact that ordinary usage applies the notions of truth and falsity to utterances of the kind in question. Given this much, minimalism about truth is said to leave no room for the view that (...) the utterances concerned are non-cognitive in nature. (shrink)

A number of writers have been attracted to the idea that some of the peculiarities of quantum theory might be manifestations of 'backward' or 'retro' causality, underlying the quantum description. This idea has been explored in the literature in two main ways: firstly in a variety of explicit models of quantum systems, and secondly at a conceptual level. This note introduces a third approach, intended to complement the other two. It describes a simple toy model, which, under a natural interpretation, (...) shows how retrocausality can emerge from simple global constraints. The model is also useful in permitting a clear distinction between the kind of retrocausality likely to be of interest in QM, and a different kind of reverse causality, with which it is liable to be confused. The model is proposed in the hope that future elaborations might throw light on the potential of retrocausality to account for quantum phenomena. (shrink)

The Free Discussion between Richard Price and Joseph Priestley (1778) originated as a correspondence between the two after the publication of Priestley's Disquisitions on Matter and Spirit, his most important philosophical work (1777). At the time it was thought remarkable that a controversey such as this could be conducted so amicably, but then the two were close friends. Nevertheless their philosophical, as opposed to their oft mentioned political views, were at opposite ends of a spectrum.

Epistemologists have not usually had much to say about believing ‘in’, though ever since Plato's time they have been interested in believing ‘that’. Students of religion, on the other hand, have been greatly concerned with belief ‘in’, and many of them, I think, would maintain that it is something quite different from belief ‘that’. Surely belief ‘in’ is an attitude to a person, whether human or divine, while belief ‘that’ is just an attitude to a proposition? Could any difference be (...) more obvious than this? And if we over-look it, shall we not be led into a quite mistaken analysis of religious belief, at any rate if it is religious belief of the theistic sort? On this view belief ‘in’ is not a propositional attitude at all. (shrink)

In the first chapter of From Metaphysics to Ethics, Frank Jackson begins, as he puts it, ‘by explaining how serious metaphysics by its very nature raises the location problem.’ (1998, p. 1) He gives us two examples of location problems. The first concerns semantic properties, such as truth and reference: Some physical structures are true. For example, if I were to utter a token of the type ‘Grass is green’, the structure I would thereby bring into existence would be true (...) ... How are the semantic properties of the sentence related to the non-semantic properties of the sentence? Where, if anywhere, are the semantic properties of truth, content and reference to be found in the non-semantic, physical or naturalistic account of the sentence?’ (1998, p. 2) Jackson notes two possible answers to questions of this kind. The purpose of this paper is to sound two notes of caution about a beguiling argument for the negative answer: for the Humean view that desires cannot be beliefs, or cognitive states more generally.

In this paper, I try to develop an account of functions which might be of use to a biologist engaged in classifying and explaining natural phenomena. The most pressing difficulty facing such an account is the need to reconcile the normativity of function statements with their explanatory force. I consider two familiar accounts of function statements, offered by Andrew Woodfield and Larry Wright . I examine both accounts in search of the strongest possible formulation of each type of theory. I (...) fail to find a formulation of the ACT which is completely immune from counterexamples, but I do find a satisfactory formulation of the HT. In particular, I argue that the HT should incorporate a point central to the ACT that functions involve a means/end relationship between two devices. I then consider functional explanations: I argue that an ACT which holds that function statements are intrinsically explanatory cannot offer a satisfactory account. The account offered by Woodfield, who rejects this assumption, is adequate, though less attractive than the account suggested by the HT. I conclude by considering how the HT can account for functional norms. (shrink)

Objectives - To explore the wvay ethical principles develop during a medical education course for three groups of medical students - in their first year, at the beginning of their penultimate (fifth) year and towards the end of their final (sixth) year. Design - Survey questionnaire administered to medical students in their first, fifth and final (sixth) year. Setting - A large medical school in Queensland, Australia. Survey sample - Approximately half the students in each of three years (first, fifth (...) and sixth) provided data on a voluntary basis, a total of 385 students. Results - At the point of entry, minor differences were found between medical students and first year law and psychology students. More striking were differences between male and female medical students, suggesting early socialization had a substantial impact here. Conclusions - Results indicate that substantial changes in attitude have developed by the beginning of fifth year with little change thereafter. Gender difference persisted. Some difference in ethical attitudes were found when groups of different ethnic backgrounds were compared. The impact of a move to a graduate medical course, which gives high priority to ethics within a professional development domain, can now be evaluated. (shrink)

This essay reviews a collection of thirteen critical essays on the work of Ruth Millikan. The collection covers a broad range of her work, focusing in particular on her account of simple intentionality, her theory of concepts and her metaphysical views. I highlight and briefly discuss three issues that crop up repeatedly though the collection: (1) Millikan’s externalism (and in particular, her emphasis on how intentional states are used, rather than how they are produced); (2) the nature of intentional explanation; (...) and (3) the normativity of meaning. William James said that sometimes detailed philosophical argument is irrelevant. Once a current of thought is really under way, trying to oppose it with argument is like planting a stick in a river to try to alter its course: “round your obstacle flows the water and ‘gets there just the same’”. He thought pragmatism was such a river. There is a contemporary river that sometimes calls itself pragmatism, although other titles are probably better. At any rate it is the denial (...) of differences, the celebration of the seamless web of language, the soothing away of distinctions, whether of primary versus secondary, fact versus value, description versus expression, or of any other significant kind. What is left is a smooth, undifferentiated view of language, sometimes a nuanced kind of anthropomorphism or “internal” realism, sometimes the view that no view is possible: minimalism, deflationism, quietism. Wittgenstein is often admired as a high priest of the movement. Planting a stick in this water is probably futile, but having done it before I shall do it again, and—who knows?—enough sticks may make a dam, and the waters of error may subside. Context The attitudes of medical professionals towards physician assisted dying have been widely discussed. Less explored is the level of agreement among physicians on the possibility of ‘rational suicide’—a considered suicide act made by a sound mind and a precondition of assisted dying legislation. Objective To assess attitudes towards rational suicide in a representative sample of senior doctors in England and Wales. Methods A postal survey was conducted of 1000 consultants and general practitioners randomly selected from a commercially available database. (...) The main outcome of interest was level of agreement with a statement about rational suicide. Results The corrected participation rate was 50%; 363 questionnaires were analysed. Overall 72% of doctors agreed with the possibility of rational suicide, 17% disagreed, and 11% were neutral. Doctors who identified themselves as being more religious were more likely to disagree. Some doctors who disagreed with legalisation of physician assisted suicide nevertheless agreed with the concept of rational suicide. Conclusions Most senior doctors in England and Wales feel that rational suicide is possible. There was no association with specialty. Strong religious belief was associated with disagreement, although levels of agreement were still high in people reporting the strongest religious belief. Most doctors who were opposed to physician assisted suicide believed that rational suicide was possible, suggesting that some medical opposition is best explained by other factors such as concerns of assessment and protection of vulnerable patients. This report, the first of the project, presents original research evidence based on 1,516 face-to-face interviews with young Syrian international protection beneficiaries and applicants, 18-32 years old, which were conducted in the UK, Lebanon and Greece, between April and October 2017. Key findings from this comparative analysis inform our policy recommendations concerning the settlement, training and skills provision for young forced migrants in the UK. Key Findings: - Young Syrian refugees in the UK have the highest levels of skills and (...) training, and are most eager to remain and contribute to the host country, compared with those in Greece and Lebanon. - Young Syrian refugees are faced with higher levels of unemployment in the UK than citizens, while many of them who are in employment are doing jobs for which they are over-qualified. - Refugees in the UK receive better support and have an overall more positive experience and evaluation of actors compared to those in Greece and Lebanon, but access to key provisions designed to enhance labour market participation remains patchy. - Syrian refugees who have been resettled to the UK report overall more positive experiences than those coming through the asylum route, despite higher levels of employment among the latter and the government supposedly taking the more vulnerable among the former. - Young Syrians in Scotland are better supported, and more positive about their engagement with people and institutions, although they are currently more distanced from re-integration into the labour market compared to those settled in England. Ismael traces our sense that the past is fixed and the future open to what she calls ‘the practical arrow’ – ‘the sense that we can affect the future but not the past.’ In this piece I draw a sharper distinction than Ismael herself does between agents and mere observers, even self-referential observers; and I use it to argue that Ismael’s explanation of the practical arrow is incomplete. To explain our inability to affect the past we need to appeal to (...) our own temporal orientation as agents, and not merely to the ingredients from physics that allow us to predict the consequences of our actions. I once stood staring at a map in a large US airport, looking for an ATM. Next to me a couple also stared at the map, trying to figure out where in the airport they were. “Sheesh!” said the male at last, pointing to the red dot and the words ‘You are here’ in the key beside the map: “We’re way over here, right off the map!” Jenann Ismael’s understanding of red dots lies very much at the other extreme, but (...) self-location – the task that couple were engaged in, however haplessly – is the unifying theme of The Situated Self. In Ismael’s hands, it is a thread that turns out to tie together a remarkable range of topics. Imagine being taken on a tour of your own home town, weaving in and out through various neighbourhoods, familiar and less so, and ducking between them via surprising paths and shortcuts. (“This next to that? Who knew!”) The Situated Self is that kind of book – an exhilarating and often demanding excursion, startling in its breadth and the ways it relates seemingly unconnected topics. Although the route of Ismael's tour is often highly original, the vehicle itself has a familiar shape, at least in outline. Ismael builds a chassis to specifications due especially to John Perry, fits it out to her own requirements, and then shows us where it can take us. Mostly, I found the excursion convincing, as well as fascinating. In what follows, I shall take up one case in which I was not convinced, and then raise an issue about another destination – somewhere in the same vicinity, apparently – and ask whether Ismael’s bus takes us there, too. First, though, to the fundamentals, as Ismael develops them. (shrink)

This article examines soldier performance optimisation, enhancement, and augmentation across the three dimensions of physical performance, cognitive performance, and socio-cultural understanding. Optimisation refers to combatants attaining their maximum biological potential. Enhancement refers to combatants achieving a level of performance beyond their biological potential through drugs, surgical procedures, or even gene editing. Augmentation refers to a blending of organic and biomechatronic body parts such as electronic or mechanical implants, prosthetics, and brain–machine interfaces. This article identifies that soldier optimisation is a necessity (...) to protect individual combatants and to give them the ability to make legal and morally justifiable decisions in battle. While enhancement and augmentation of military personnel can lead to accelerated and more destructive warfare, it can also be argued that there is an ethical and moral responsibility to provide combatants the best opportunity for survivability, and that better functioning, less fatigued, and better informed military personnel can make better decisions in battle. There is also a moral responsibility of the state for the combatants themselves, and short-term military success must be balanced against the short- and long-term health and wellbeing of the personnel. This article concludes that it is both the intent and the degree that decide the acceptability. (shrink)

In his influential book 'Making Things Happen' and in other places, Jim Woodward has noted some affinities between his own account of causation and that of Menzies and Price, but argued that the latter view is implausibly ‘subjective’. In this piece I discuss Woodward’s criticisms. I argue that the Menzies and Price view is not as different from Woodward’s own account as he believes, and that in so far as it is different, it has some advantages whose importance (...) Woodward misses; but also that the Menzies and Price view lacks some elements whose importance Woodward rightly stresses. When properly characterized, however, the ‘subjectivity’ survives unscathed. ABSTRACT Thematic analysis is the most widely used method for analysing qualitative data. Recent debates, highlighting the binary distinctions between reflexive TA grounded within the qualitative paradigm and codebook TA with neo-positivist orientations, have emphasized the existence of numerous tensions that researchers must navigate to produce coherent and rigorous research. This article attempts to resolve some of these tensions through developing an approach to TA underpinned by realist philosophy of science. Focusing on interview data, we propose the use of three (...) types of themes and the use of corresponding validity indicators. Using an illustrative example, we outline the conceptual foundations of a realist approach to TA and present recommendations for conducting it in practice. This approach, we claim, reconciles several existing binaries between distinctive types of TA by incorporating the contributions of both for the development of different types of themes. (shrink)

Seneca's meteorological theme raises our gaze from a terrestrial level to a higher, more intuitive plane - a conceptual climb by which Seneca promotes a change of perspective in his readership towards the cosmic viewpoint.
