Since Vatican II a new breed of Thomism has emerged on the scene, in effect superseding the two streams of neo-Thomistic thought which blossomed in the wake of Leo XIII’s encyclical Aeterni Patris, namely, Aristolelian Thomism, a movement which remains within the conceptual horizon of form and matter, and Existential Thomism, which insists upon pushing beyond hylomorphism to the ontological depth dimension of the actus essendi, the nonformal act responsible for suffusing the composite being with real existence. Taking its inspiration (...) from Kant, the ascendancy of Transcendental Thomism seemed to be the logical response to the inadequacies of neo-Thomism, mired as it was in naive realism and furthermore incapable of rising to the Infinite Being on the basis of a scrutiny of finite sensible composites. But this transformation of the Thomistic landscape, Knasas warns us, “is a disaster for Thomism itself”. We would be better off to return to the Existential Thomism proffered by Gilson, Maritain, and Owens. (shrink)

Cet article interroge, à l’aide d’une trajectoire singulière issue de la clinique des auteurs, le lien entre la construction des imagos parentales chez une femme française issue de l’immigration maghrébine et son adhésion à l’idéologie religieuse radicale de Daech. Il s’agit plus précisément d’entrevoir les échecs et les impasses de l’institution familiale et sociale et la manière dont cette jeune femme s’est saisie de la Umma (la communauté des musulmans dans le monde) – dont la racine Um signifie en arabe (...) « mère » – pour rompre avec la France et construire une nouvelle famille au sein de l’organisation terroriste de Daech. Cette analyse permettra de faire des hypothèses cliniques quant à la défaillance des fonctions maternelles et paternelles dans la construction d’un devenir femme et de mettre en lumière l’absence ou l’exclusion du tiers et la tentative de séparation avortée menant vers des positions radicales. Aim: Patients with advanced cancer need information about end-of-life treatment options in order to make informed decisions. Clinicians vary in the frequency with which they initiate these discussions.Patients and methods: As part of a long-term longitudinal study, patients with an expected 2-year survival of less than 50% who had advanced gastrointestinal or lung cancer or amyotrophic lateral sclerosis were interviewed. Each patient’s medical record was reviewed at enrollment and at 3 months for evidence of the discussion of patient wishes concerning (...) ventilator support, artificial nutrition and hydration , resuscitation and hospice care. A Kaplan–Meier analysis was also performed and 2-year survival calculated.Results: 60 cancer and 32 ALS patients were enrolled. ALS patients were more likely than cancer patients to have evidence of discussion about their wishes for ventilator support , ANH , DNR and hospice care . At 6 months, 91% of ALS patients were alive compared with 62% of cancer patients; at 2 years, 63% of ALS patients were alive compared with 23% of cancer patients .Conclusions: Cancer patients were less likely than ALS patients to have had documented advanced care planning discussions despite worse survival. This may reflect perceptions that ALS has a more predictable course, that advanced cancer has a greater number of treatment options, or differing views about hope. Nevertheless, cancer patients may be less adequately prepared for end-of-life decision-making. (shrink)

The author has recently shown that a mathematical question regarding the fundamental constituents of hardrons cannot be resolved unless the classical axioms of nonfinite mathematics are revised in such a way as to produce a new theory of particle motion in continuous space-time. Under this new theory, the instantaneous position of a moving object has a magnitude that is increasing as the object's velocity. The purpose of this paper is to show that, quite apart from the question of Cantorian axiomatics, (...) the new theory of motion is more consistent with what physicists really believe than the traditional theory of motion. It is argued that organ donation from a patient to the patient's physician is ethically dubious because donation decisions will be inappropriately influenced and the negative public perceptions will result in more harm than good. It is suggested that to protect the perception of the physician–patient relationship, avoid cynicism about medicine’s attitude to patient welfare and maintain trust in the medical profession, a new professional boundary should be established to prevent physicians from receiving organs for transplantation donated by their patients.

The following article is a response to the position paper of the Hastings Center, “Ethical Challenges of Chronic Illness”, a product of their three year project on Ethics and Chronic Care. The authors of this paper, three prominent bioethicists, Daniel Callahan, Arthur Caplan, and Bruce Jennings, argue that there should be a different ethic for acute and chronic care. In pressing this distinction they provide philosophical grounds for limiting medical care for the elderly and chronically ill. We give a critical (...) survey of their position and reject it as well as any attempt to characterize the physician-patient relationship as a commercial contract. We emphasize, as central features of good medical practice, a commitment to be the patient's agent and a determination to acquire and be guided by knowledge. These commitments may sometimes conflict with efforts to have the physician serve as an instrument of social and economic policies limiting medical care. (shrink)
