Six individuals with experience of psychosis were interviewed about their psychotic experiences. The material was analyzed using the empirical phenomenological psychological method. The results consist of a whole meaning structure, a gestalt, entailing the following characteristics: The feeling of estrangement in relationship to the world; the dissolution of time; the loss of intuitive social knowledge; the alienation of oneself, and finally; the loss of intentionality/loss of agency. In brief, the results show that an altered perception of the self and the (...) world was an essential part of the psychotic experience where subjects described themselves as changed; something was sensed as being wrong as psychosis is perceptible but hard to communicate. The normal life-world experience was altered and reality seemed strange. Time perception seemed to be changed as temporality appeared dissolved and the experience of time was focused on the current moment excluding the future. The subjects described loss of intentionality, they were no longer agents in their actions but partly steered by others and they could feel as if their experiences were not theirs. The patients also describe problems regarding their ability to socialize and communicate with others. They seem to lose their intuitive social capacity and were prone to suspiciousness. (shrink)

We write U≼V if there exists a countable subset C of equation image such that U is contained in the subsemigroup generated by V and C. We give several results about the structure of the preorder ≼. In particular, we show that a certain statement about this preorder is equivalent to the Continuum Hypothesis.The preorder ≼ (...) is analogous to one introduced by Bergman and Shelah on subgroups of the symmetric group on equation image. The results in this paper suggest that the preorder on subsemigroups of equation image is much more complicated than that on subgroups of the symmetric group. (shrink)

Paternalism in the medical care of children is appropriate and ethically justifiable. However, dilemmatic disagreement by paternalistic agents as to which clinical choice is in the child's best interest may occur because of the underlying conflict between two rival standards for the moral value of life: longevity versus quality. Neither standard is unreasonable. Either could be the basis for choice of medical care by the parents or by the pediatrician. Having the child choose between options disputed by his parents and (...) the pediatrician is unlikely to resolve their conflict. Exercise of informed consent by the adolescent requires agreement by his parents to relinquish their paternalistic veto. The probable best-interest choice by the child when he has matured could be reasonably made from either standard. Therefore, the longevity/quality of life question ought not ordinarily to be foreclosed by paternalistic authority which opts for one standard to the exclusion of the other. Medical interventions, paternalistically determined, are justified in the face of deteriorating quality, but only as long as the interventions themselves do not cause deterioration. Application of this limitation of paternalism to the zone of agreement between the rival life standards is made to clinical case examples. Multiple extrinsic criteria may measure the quality of life. Three quality factors, sensation of pain, capacity to communicate and physical functioning are considered. The extent of the zone of agreement between the two life standards varies because quality of life is a relative good, contingent both upon which extrinsic criteria are selected to assess it and upon the priorities which are set among these criteria. I believe the time is come for a re-estimation of the status of minds in the universe. I use the word mind quite naïvely at first, in the belief that it has a nucleus of meaning that is sufficiently clear. I do not wish its meaning to be restricted to the phenomena of clear consciousness, still less of self-consciousness.

'Uncertainty and chance' is a subject with a broad span, in that there is no academic discipline or walk of life that is not beset by uncertainty and chance. In this book a range of approaches is represented by authors from varied disciplines: natural sciences, mathematics, social sciences and medical sciences. At one extreme, this volume is concerned with the foundations of probability. At the other extreme, we have scholars who acknowledge the concept of chance and uncertainty but do not (...) cope with it by means of systematic measurement or quantative analysis. (shrink)

In an era dominated by information and communication technologies, when science continues to amaze us with its discoveries and technological applications, and when social media are controlling part of our lives and social relationships, does philosophy still make sense? Is humanistic knowledge still necessary today as an indispensable part of our human world?

Objectives: To investigate the current situation of completing the informed consent for do-not-resuscitate (DNR) orders among the competent patients with terminal illness and the ethical dilemmas related to it. Participants: This study enrolled 152 competent patients with terminal cancer, who were involved in the initial consultations for hospice care. Analysis: Comparisons of means, analyses of variance, Student’s t test, χ2 test and multiple logistic regression models. Results: After the consultations, 117 (77.0%) of the 152 patients provided informed consent for hospice (...) care and DNR orders. These included 21 patients (17.9%) who signed the consent by themselves, and 96 (82.1%) whose consent sheet was signed only by family members. The reasons why patients were not involved in the discussions toward the consent (n = 82) included poor physical or psychological condition (44.9%), concerns of the consultant hospice team (37.2%), and the family’s refusal (28.2%). On a multivariate analysis, patients’ awareness of their poor prognosis (odds ratio = 4.07, 95% confidence interval = 2.05 to 8.07) and their understanding of hospice care (2.27, 1.33 to 3.89) were two independent factors (p<0.01) that influenced their participation in the discussions or their personal signature in the informed consent. Conclusion: The family-oriented culture in Asian countries may violate the principles of the Patient Self-Determination Act and the requirements of the Hospice Care Law in Taiwan, which inevitably poses an ethical dilemma. Earlier truth-telling and continuing education of the public by hospice care workers will be helpful in solving such ethical dilemmas. (shrink)

Nature has been viewed as a curative for the problems of urbanized society, and primitivism has been forwarded as a viable alternative to modification of the industrial world. Critical theory maintains that the current phase of social development tends toward 'total administration', and that escapes to nature are them selves administered and controlled by the larger society. Back-to-nature is critiqued as an unproductive strategy whose middle-class origins render it élitist.
