The emphasis on evidence based medicine (EBM) has placed increased focus on finding timely answers to clinical questions in presence of patients. Using a combination of natural language processing for the generation of clinical excerpts and information theoretic distance based clustering, we evaluated multiple approaches for the efficient presentation of context-sensitive EBM excerpts.

This article provides a critical comparative analysis of the substantive and procedural values and ethical concepts articulated in guidelines for allocating scarce resources in the COVID-19 pandemic. We identified 21 local and national guidelines written in English, Spanish, German and French; applicable to specific and identifiable jurisdictions; and providing guidance to clinicians for decision making when allocating critical care resources during the COVID-19 pandemic. US guidelines were not included, as these had recently been reviewed elsewhere. Information was extracted from each (...) guideline on: 1) the development process; 2) the presence and nature of ethical, medical and social criteria for allocating critical care resources; and 3) the membership of and decision-making procedure of any triage committees. Results of our analysis show the majority appealed primarily to consequentialist reasoning in making allocation decisions, tempered by a largely pluralistic approach to other substantive and procedural values and ethical concepts. Medical and social criteria included medical need, co-morbidities, prognosis, age, disability and other factors, with a focus on seemingly objective medical criteria. There was little or no guidance on how to reconcile competing criteria, and little attention to internal contradictions within individual guidelines. Our analysis reveals the challenges in developing sound ethical guidance for allocating scarce medical resources, highlighting problems in operationalising ethical concepts and principles, divergence between guidelines, unresolved contradictions within the same guideline, and use of naïve objectivism in employing widely used medical criteria for allocating ICU resources. Background There is a growing concern about artificial intelligence (AI) applications in healthcare that can disadvantage already under-represented and marginalised groups (eg, based on gender or race). Objectives Our objectives are to canvas the range of strategies stakeholders endorse in attempting to mitigate algorithmic bias, and to consider the ethical question of responsibility for algorithmic bias. Methodology The study involves in-depth, semistructured interviews with healthcare workers, screening programme managers, consumer health representatives, regulators, data scientists and developers. Results Findings reveal considerable (...) divergent views on three key issues. First, views on whether bias is a problem in healthcare AI varied, with most participants agreeing bias is a problem (which we call the bias-critical view), a small number believing the opposite (the bias-denial view), and some arguing that the benefits of AI outweigh any harms or wrongs arising from the bias problem (the bias-apologist view). Second, there was a disagreement on the strategies to mitigate bias, and who is responsible for such strategies. Finally, there were divergent views on whether to include or exclude sociocultural identifiers (eg, race, ethnicity or gender-diverse identities) in the development of AI as a way to mitigate bias. Conclusion/significance Based on the views of participants, we set out responses that stakeholders might pursue, including greater interdisciplinary collaboration, tailored stakeholder engagement activities, empirical studies to understand algorithmic bias and strategies to modify dominant approaches in AI development such as the use of participatory methods, and increased diversity and inclusion in research teams and research participant recruitment and selection. Recent events and popularized stereotypes call into question the ethics of salesperson behaviors. Although prior research demonstrates that salespeople’s emotional exhaustion can have negative consequences for several job outcomes, little is known about the factors that can mitigate such relationships—particularly the relationship between emotional exhaustion and ethical behavior. To remedy this knowledge gap, we draw from self-control theory to propose a novel theoretical framework and develop hypotheses. These hypotheses are tested on a unique dataset consisting of survey data collected from (...) 123 matched business-to-business salesperson–manager dyads. The findings reveal that emotional exhaustion is negatively associated with sales performance, emotional exhaustion is negatively associated with ethical behaviors, ethical behaviors are positively associated with sales performance, ethical behaviors mediate emotional exhaustion’s negative effect on sales performance, perceived supervisor support attenuates the negative association between emotional exhaustion and ethical behaviors, and contrary to expectations, grit strengthens the negative association between emotional exhaustion and ethical behaviors. As we show here, perceived supervisor support may attenuate the undesirable effects of emotional exhaustion on ethical behaviors and sales performance. The article’s broader contribution thus lies in its suggestion that managers pay special attention to these factors. Moreover, factors such as grit can have unexpected and undesirable influences; therefore, we draw attention to the importance of scrutinizing these interactions, even when the factors involved are almost universally touted as beneficial. Τήν άνθιση πού παρατηρούμε στην παραγωγή καί εξαγωγή του κρητικού κρασιού, μαρτυρούν επίσης καί οί αρχαίες πηγές. Έκτος από μιά πρώτη τυπολογία θά βρούμε επίσης καί μιά λεπτομερή περιγραφή των εργαστηριακών εγκαταστάσεων καί τής κεραμεικής παραγωγής πού βρέθηκαν κατά τή διάρκεια τής ανασκαφής στό Καστέλλι Κισσάμου.

We report the identification of a recurrent, 520-kb 16p12.1 microdeletion associated with childhood developmental delay. The microdeletion was detected in 20 of 11,873 cases compared with 2 of 8,540 controls and replicated in a second series of 22 of 9,254 cases compared with 6 of 6,299 controls. Most deletions were inherited, with carrier parents likely to manifest neuropsychiatric phenotypes compared to non-carrier parents. Probands were more likely to carry an additional large copy-number variant when compared to matched controls. The clinical (...) features of individuals with two mutations were distinct from and/or more severe than those of individuals carrying only the co-occurring mutation. Our data support a two-hit model in which the 16p12.1 microdeletion both predisposes to neuropsychiatric phenotypes as a single event and exacerbates neurodevelopmental phenotypes in association with other large deletions or duplications. Analysis of other microdeletions with variable expressivity indicates that this two-hit model might be more generally applicable to neuropsychiatric disease. © 2010 Nature America, Inc. Mais c’est au début du xxe siècle qu’elle devient une personnification de la résistance pacifique au nom des droits de la conscience et de la tolérance et qu'elle accède à un statut d'héroïne. Cela permet (...) aussi à la Réforme un renouveau moral et spirituel. La référence à Marie Durand s'accentue en 1945 et culmine lors des cérémonies de 1968. Faisant suite à celles réalisées en Crète occidentale (BCH 113 [1989], p. 551-580), l'Éphorie des Antiquités d'Hérakleion et l'École française d'Athènes ont mené en collaboration en 1989 des recherches sur les ateliers de fabrication d'amphores de Crète centrale, dont l'activité s'échelonne entre l'époque hellénistique et l'époque impériale. On a ainsi localisé de nouveaux ateliers et réexaminé des ateliers déjà connus à Hérakleion, Chersonisos, Tsoutsouros (Inatos), Kératokambos et Dermatos. L'étude typologique permet d'affirmer que différents types d'amphores pouvaient être fabriqués dans le (...) même atelier. On a identifié trois nouveaux types, qui ont coexisté en Crète centrale avec tous les autres types déjà connus. L'étude concerne également d'autres productions des mêmes ateliers (vases, tuiles, etc.) et aborde quelques aspects techniques (supports de vases, ratés de cuisson, parois vitrifiées de fours). Yves Grandjean, Tony Kozelj and François Salviat The Zeus Gate on Thasos p. 175-268 The Gate of Zeus and Hera, offered by Pythippos son of Paiestratos, is one of the most remarkable features of the Thasian enceinte: to the sculptured reliefs that adorned it was added a particular architectural embellishment which has never been analysed in its entirety. The numerous blocks preserved in situ make it possible to reconstruct the elevation of the gate, which was in fact a gate (...) tower, and to detail the appearance of the latter with its architectural facing of superimposed orders — Doric and Ionic — on the town side and its Doric frieze integrated into the walls of the tower on the side facing the country. The influences of Macedonian, funerary and palatial architecture are manifest. According to the lists of theoroi and the amphora stamps discovered in the levels preceding its construction, the gate tower was erected in around 300 BC. It replaced an original opening going back to the time when the enceinte visible today was built, in other words after the raid on Thasos by Histiaios of Miletus in 494; the old excavations like the recent investigations confirm the account by Herodotus (VI 46), which there is no reason to doubt. Dans une première partie sont présentés les résultats d'une troisième et dernière campagne de prospection en Crète, consacrée à la partie orientale de l'île : deux ateliers ampho- riques ont été découverts, à Trypitos et à Lagada, et l'on dispose d'éléments suffisants pour localiser d'autres ateliers à Makry Ghialos, Aghios Nikolaos et Hiérapétra. Dans une seconde partie, nous essaierons de tirer quelques enseignements à l'issue de ces prospections qui ont permis de découvrir une vingtaine d'ateliers dans l'ensemble de l'île, avec (...) huit types amphoriques assurément crétois. Ces ateliers domaniaux sont liés à des villas qui exploitent la vigne à des fins d'exportations massives. Situé près d'un lieu d'embarquement qui permette un chargement facile des innombrables amphores destinées au commerce lointain, l'atelier répond aussi aux besoins de la villa en céramique commune et fine ainsi qu'en éléments architectoniques (briques, tuiles, hypocaustes, σφήνες). On en retire l'impression d'une vitalité économique à partir du Ier s. av. n. è., au moment où le vignoble italien semble connaître une certaine stagnation au profit des crus provinciaux. (shrink)

Stakeholder theory advocates that firms bear responsibility for the implications of their actions. However, while a firm affects or can affect stakeholders, stakeholders can also affect the corporation. Previous stakeholder theorising has neglected the reciprocal nature of responsibility. The question can be asked whether—in a spirit of reciprocity, loyalty and fairness—stakeholders should treat the corporation in a fair and responsible way. This study based on different definitions of stakeholders argues that various stakeholder attributes differ for different categories of stakeholders. This (...) analysis presumes that the attribute of stakeholder reciprocity can probably be restricted to real stakeholders, labelled stakeowners: genuine stakeholders with a legitimate stake, the loyal partners who strive for mutual benefits. Stakeowners own and deserve a stake in the firm. Stakeholder reciprocity could be an innovative criterion in the corporate governance debate as to who should be accorded representation on the board. Corporate social responsibility should imply corporate stakeholder responsibility. (shrink)

Recent academic articles point to an increased vagueness and overlapping of the concepts around business ethics and corporate responsibility. However, the perception of these notions in the entrepreneurial world can differ from the original academic definitions. The aim of this exploratory study is to uncover how the small business entrepreneur understands these various concepts. For this analysis, the Repertory Grid Technique (RGT) is used, a method with limited applications in the business and society field. Our findings partially reject the confusion (...) in terminology in academic literature. Despite the recent increase in interest in corporate social responsibility and the propagation of corporate governance in both business and academic literature, from observations of actual practice, the author has seen at all company levels, in everyday operations, instances of non-ethical behaviour vis-à-vis the whole gamut of stakeholders. This state of affairs is linked with: pressure from stakeholders, short-term tactics, hegemony of financial considerations, ‘juridisation’ of business, the tyranny of communications and the media and the difficulties in translating strategy into (...) practical implementation. The paper scrutinises the motivation and the psychology of entrepreneurs and business people, and their behaviour is compared to other professional groups and confronted with the decline in ethics in society as described by some important observers. The conclusion drawn is that the recent positive evolutions do not provide sufficient ethical guidelines for the day-to-day activities of middle managers and entrepreneurs in the present competitive environment. Managers will always be confronted by difficult choices with ethical dilemmas. There will always be a tension between theory and pragmatism, but progress can be made with the effective implementation of the ethical discourse in companies. To achieve this, ethical management should not be confined to the large strategic issues but also applied to the small practical matters of everyday business life. Ethics in business and entrepreneurship requires more than corporate governance and corporate social responsibility (CSR). (shrink)

Stakeholder literature has acknowledged the need to complement the extant theory on stakeholder management by more dynamic perspectives. This article makes use of the recent terminology of stakewatcher and stakeseeker to illustrate the dynamic aspect of stakeholder theory transposed in the graphical representation of Freeman’s stakeholder model. Presenting a few selected case studies, it applies the scheme on the concept of value responsibility chain; it exemplifies the role of stakeseekers in various forms of activism, from shareholders, NGOs and government, in (...) the stakeholder mobilisation process. This article clarifies how stakewatchers and stakeseekers can profoundly affect stakeholder salience, especially in crises. The transposition and integration of the dynamic aspect of stakeholder theory into the graphical representation strengthen the forceful pedagogical value of the Freeman’s stakeholder graphical model. The object of this paper is two-fold: first, to show that contrary to what seem to have become a widely accepted view among historians of biology, the famous 1953 first Nature paper of Watson and Crick on the structure of DNA was widely cited — as compared to the average paper of the time — on a continuous basis from the very year of its publication and over the period 1953–1970 and that the citations came from a wide array of (...) scientific journals. A systematic analysis of the bibliometric data thus shows that Watson's and Crick's paper did in fact have immediate and long term impact if we define "impact" in terms of comparative citations with other papers of the time. In this precise sense it did not fall into "relative oblivion" in the scientific community. The second aim of this paper is to show, using the case of the reception of the Watson—Crick and Jacob—Monod papers as concrete examples, how large scale bibliometric data can be used in a sophisticated manner to provide information about the dynamic of the scientific field as a whole instead of limiting the analysis to a few major actors and generalizing the result to the whole community without further ado. This paper makes an analysis of the recent collapse of Fortis, a major European bank, from the perspective of shareholder activism and social movement theory to illustrate the limits of corporate governance. Some specific issues of the Fortis affair – and more especially the chaotic events at the general assembly - illustrate the application of social movement theory to shareholder activism, besides agency theory problems and conflicts of interests. Paradoxes between legal constraints and principles of good corporate governance are presented. (...) The Fortis case illustrates the limits of the juridical bureaucracy in situations of urgency. The case illustrates the limits of corporate governance. Governance systems need to find appropriate mechanisms to align the voting rights with the long-term interests of the company. We argue for new criteria that will restrict representation to loyal shareholders with a long-term commitment. Good governance mechanisms should prevent opportunistic behaviour and encourage a responsible attitude. (shrink)

This article discusses plant-closing decisions by multinational enterprises applying a stakeholder theory approach. In particular, we focus on the emergence of “intra-stakeholder alliances,” that is, alliances among the various stakeholder groups of a specific corporation. We analyze the emergence of stakeholder alliances in reaction to MNEs' decisions to terminate production locally and discuss their influence on the outcomes of such decisions. Our research is inspired by two exceptional case studies of two multinational breweries that announced their decisions to close niche (...) breweries in small towns in Italy and Belgium. In both cases, the initial decision was ultimately reversed through the actions of intra-stakeholder alliances. We combine insights from stakeholder theory and the social movement literature to analyze the action and influence of intra-stakeholder alliances in seven cases of plant-closing decisions. We conclude by formulating four general propositions that can provide guidance to MNE management in plant-closing decisions. Our findings extend managerial stakeholder theory, show how this approach can improve strategic management analysis, emphasize the importance of the relationships among stakeholders in the value-creation process, and shed light on the collective action and influence of intra-stakeholder alliances. Pathologizing ugliness refers to the use of disease language and medical processes to foster and support the claim that undesirable features are pathological conditions requiring medical or surgical intervention. Primarily situated in cosmetic surgery, the practice appeals to the concept of “aesthetic pathology”, which is a medical designation for features that deviate from some designated aesthetic norms. This article offers a two-pronged conceptual analysis of aesthetic pathology. First, I argue that three sets of claims, derived from normativist and naturalistic accounts (...) of disease, inform the framing of ugliness as a disease. These claims concern: (1) aesthetic harms, (2) aesthetic dysfunction, and (3) aesthetic deviation. Second, I introduce the notion of a hybridization loop in medicine, which merges the naturalist and normative understanding of the disease that potentially enables pathologizing practices. In the context of cosmetic surgery, the loop simultaneously promotes the framing of beauty ideals as normal biological attributes and the framing of normal appearance as an aesthetic ideal to legitimize the need for cosmetic interventions. The article thus offers an original discussion of the conceptual problems arising from a specific practice in cosmetic surgery that depicts ugliness as the disease. (shrink)

Invités à se prononcer sur cette question, anthropologues, philosophes, politicologues, sociologues et théologiens livrent ici leurs propres analyses de cette problématique par un riche panorama des réflexions sur la sexualité, la mort, la mode, l'art, les jeunes de la rue, les activités extrêmes, la religion et la pop psycho. Sans épuiser la problématique de la transcendance contemporaine, cet ouvrage stimulera la curiosité de tous les lecteurs intéressés aux nouvelles valeurs culturelles ou aux nouveaux comportements sociétaux. Pathologizing ugliness refers to the framing of unattractive features as a type of disease or deformity. By framing ugliness as pathology, cosmetic procedures are reframed as therapy rather than enhancement, thereby potentially avoiding ethical critiques regularly levelled against cosmetic surgery. As such, the practice of pathologizing ugliness and the ensuing therapeuticalization of cosmetic procedures require an ethical analysis that goes beyond that offered by current enhancement critiques. In this article, I propose using a thick description of the goals of medicine (...) as an ethical framework for evaluating problematic medical practices. I first describe the goals of medicine based on Daniel Callahan's account. I then propose that the goals work best in conjunction with ancillary ethical concepts, namely medical knowledge and skills, standards of practice and medical duties and virtues. Next, I apply the thick description of the goals of medicine in critiquing the practice of framing ugliness as disease. Here, I demonstrate ethical conflicts between aesthetic judgments that underpin the practice of pathologizing ugliness and medical judgments that inform ethical medical practices. In particular, the thick description of the goals of medicine helps reveal ethical conflicts in at least three key domains common to clinical practices, which include (a) disease determination, (b) diagnostic evaluation and (c) establishing clinical indications. My analysis offers a novel way of critiquing the practice of pathologizing ugliness in cosmetic surgery, which tends to be neglected by enhancement critiques. (shrink)

For Yves R. Simon, philosophy has an affinity to science, not in the sense that philosophy is a mere metascience, a commentary on the sciences, but rather because it shares the same aim as science: the search for explanation. The philosophy Simon espouses is philosophical realism which, following Jacques Maritain, he prefers to call critical realism. Against the prejudice that only some version of philosophical idealism, be it critical or absolute, is capable of understanding positive science. Simon, in Foresight (...) and Knowledge, develops a philosophy of science form a realistic perspective. Philosophy of science or the critique of science, as it was known in France, is according to Simon, metahphysics in the exercise of its critical function. Simon selects as the central focus of the treatise the problem of determinism, causality, and chance. Simon shows that the concept “determinism” must be understood in different conceptual systems, such as a philosophy of nature and physics; in the latter, determinism is conceived as a possibility of certain and exact prediction.
