In this article I attempt to transcend the mainstream conception of health care ethics, including nursing ethics, by bringing into the foreground a tension between a sense of life and an industrial-bureaucratic style of health care, with its emphasis on the systematic and procedural work culture necessary for mass production. I use the concept of ‘a sense of life’ to draw attention to the wisdom, sensitivity and responsibility that is necessary for the authentic care of others (...) to be given a chance in the development of modern health care. I emphasize the mindfulness that the professional requires for genuine care, and how the systematic organization of modern health care, on the whole, ignores, obstructs and even suppresses such mindfulness. The paper examines the dialectics of the sense of life and death. In the analysis three mutually dependent points of view are applied: existential , moral and psychological ones. Although the self is distinct from identity, this essay offers some insight into how identity is maintained—in the processes by which a self is formed, and through the actual content of the schemata that compose the self-concept. The author explains how in the 1920s utopian representations of a “new man” indicate the blank space where an aesthetic for the self can appear long before a theory of choice and commitment can exist in reality. Far from being a dramaturgical plan of (...) action, such utopian representations disorient the elements of discourse in order to prepare the way for the appearance of something inconceivable or inexpressible within this discourse. The theatricality produced in this oppositional public sphere—like that of Max Reinhardt, Bertolt Brecht, Ernst Toller, and later by Jean-Paul Sartre and Albert Camus—can thus be read as expressions of unfulfilled desire resisting the limitations of the present system and breaking beyond with self-defined narratives not yet realized in our everyday lives. The aim of this study was to assess attitudes of intensive care nurses to selected ethical issues related to end-of-life decisions in paediatric intensive care units. A self-administered questionnaire was distributed in 2005 to intensive care nurses at two different scientific occasions in Turkey. Of the 155 intensive care nurse participants, 98% were women. Fifty-three percent of these had intensive care experience of more than four years. Most of the nurses failed to agree about withholding (65%) or withdrawing (60%) (...) futile treatment. In addition, 68% agreed that intravenous nutrition must continue at all costs. In futile treatment cases, the nurses tended to leave the decision to parents or act maternalistically. The results showed that intensive care nurses could ignore essential ethical duties in end-of-life care. We suggest that it is necessary to educate Turkish intensive care nurses about ethical issues at the end of life. (shrink)

Debussy, Cooper succeeds in conveying some deep and difficult philosophical ideas about the meaning of life in an engaging manner, and in showing how those ideas bear upon the practical question of how we should relate to our world and live our lives. This essay is a discussion of Aquinas’s argument “from motion” to the existence of God as the argument is found in his Summa Contra Gentiles. The aimof the essay is to suggest an approach to Aquinas’s argument that emphasizes its particular context, where “context” signifies not so much the assumed Aristotelian physics as Aquinas’s larger project of carrying out “the office of a wise man,” namely, “to order things.” Construing the relevant “ordering” as a making sense of things—indeed of (...) “the whole of things”—the argument from motion is thus seen as part of an attempt to make sense of what, following Aristotle, can be called “the whole of life,” that whole within which any one of us must live out his or her particular life. Several ideas found in Wittgenstein’s Tractatus Logico-Philosophicus are introduced in the conviction that they may help at least some of us to see the “strangeness” of the conclusion of Aquinas’s argument, the conclusion, namely, that the first principle of the whole of being is an “unmoved mover”—the strangeness of which conclusion, it is argued, is essential to its significance. This paper is concerned with some limitations of the vignette methodology used in contemporary appraisal research and their implications for appraisal theory. We focus on two recent studies in which emotional manipulations were achieved using textual materials, and criticise the investigators' apparent implicit assumption that participation in everyday social reality is somehow comparable to reading a story. We take issue with three related aspects of this cognitive analogy between life and its narrative representation, by arguing that emotional reactions in (...) real life are not necessarily mediated by symbolic processes, that people are involved participants of real life rather than neutral observers, and that in real life people's evaluations and emotions are typically part of an ongoing dialogue rather than the expression of a soliloquy. Results from these studies of emotional vignettes therefore tend to overestimate the importance of constructive, abstract, and individualistic processes in the everyday causation of social emotions. In real life, people do not necessarily have to calculate, transform, or internally represent the meaning of the dynamic situation in order to make emotional sense of what is happening to them in the social world. It then examines some contemporary arguments for an extended evolutionary synthesis from the perspective of this philosophy of nature. I argue that Merleau-Ponty's philosophy of nature may provide a productive ontological grounding to the extended evolutionary synthesis.

ObjectivesThe breast cancer stigma affects Health-related quality of life, while general resilience resources, namely, sense of coherence, social support, and coping skills, are thought to alleviate this effect. The study aimed to explore the mediating/moderation role of GRRs in the relationship between stigma and HRQoL and its dimensions in Iranian patients with breast cancer.MethodsIn this cross-sectional study, Stigma Scale for Chronic Illness 8-item version, SOC-13, Medical Outcome Survey- Social Support Scale, Brief COPE, and Functional Assessment of Cancer Therapy-Breast (...) were investigated in a convenience sample of Iranian women with confirmed non-metastatic breast cancer. Following the establishment of correlations using Pearson’s correlation, single and parallel mediation analysis and moderation analysis were conducted to determine the extent to which each GRR might be impacted by stigma or decrease the adverse impact of stigma on HRQoL.ResultsAn analysis of 221 women with the mean age of 47.14 showed that stigma was negatively correlated to all HRQoL’s dimensions, SOC, social support, and the bulk of coping skills. In the single mediation analysis, stigma affected all facets of SOC, all subscales of social support, and positive reframing, which partially reduced breast cancer HRQoL. Stigma affects general HRQoL through damaging meaningfulness, social support, and positive reframing. Meaningfulness was marked as the most impacted GRR in terms of all domains of HRQoL. In parallel mediation, reduced meaningfulness, total social support, and positive reframing were highlighted as the pathways of diminished breast cancer HRQoL. Moderation analysis indicated the higher levels of humor, behavioral disengagement, and use of instrumental support behaviors to be functional in protecting different dimensions of HRQoL, while the results were mixed for venting, especially in patients with mastectomy surgery.ConclusionWhile GRRs may be impacted by stigma, they exert a relatively small protective effect against the impact of stigma on HRQoL. This study provides some novel findings, but longitudinal studies are needed to further verify these before any causal conclusion or recommendations for health policy can be drawn. (shrink)

The entry‐point for ethical reflection in ancient ethics is the question of how I ought to live. This develops into thoughts about my final end and the formal conditions that have to hold of it. The assumption that this is happiness raises issues about similarities and differences between modern notions of happiness and the ancient concept of happiness or eudaimonia.

A positive attitude toward later life is crucial for wellbeing among older adults. Maintaining a healthy relationship with adult children can help reduce older parents’ sense of loneliness and nurture a positive life attitude. This study aimed to investigate the associations between multidimensional intergenerational relationship quality and attitudes toward later life among aging Chinese adults in Hong Kong and examine the mediating effects of a sense of loneliness. Representative survey data were collected from 801 participants (...) with at least one adult child. Multiple linear regression was employed to investigate the associations between overall intergenerational relationship quality with a sense of loneliness as well as the attitude toward later life. To examine the mediating effects of a sense of loneliness, causal mediational analyses were performed. Results demonstrated that overall intergenerational relationship quality was positively associated with aging parents’ attitude toward later life, and this relationship could be partially mediated by a sense of loneliness. Among the four subdomains of intergenerational relationship quality, the influences of structural-associational solidarity and intergenerational conflict on attitude toward later life were almost fully mediated by a sense of loneliness, whereas the influences of consensual-normative solidarity and affectual closeness were partially mediated. These findings contributed to an improved understanding of the relationship between intergenerational relationship quality, sense of loneliness, and attitude toward later life, and could inform future policies and service programs that promote aging adults’ social integration and positive aging. Anna-Teresa Tymieniecka expands the phenomenological study of meanings (sense-bestowal) into an onto-genetic inquiry by grounding it in a phenomenology of life, including the emotional dimension. This phenomenology of life is informed by the empirical sciences and its doctrines parallel the new scientific paradigm of open dynamic systems. Embedded in the dynamics of the real individuation of life forms, human consciousness emerges at a unique station in the evolutionary process. Tymieniecka treats the constitution of sense as (...) a function of life, and thus the transcendental constitutive function of the cognitive, objectifying intentionality of consciousness, the purview of classical phenomenology, is viewed as a project that is limited in its scope. According to the phenomenology of life, meaning-genesis is exhibited throughout the various stages of structurization in the evolution of life. This paper highlights the transformative function of the “Imaginatio Creatrix,” which is the dynamic process at the human station of evolution that accounts for humanity’s inventive capacities necessary for the construction of a human world. The Imaginatio Creatrix transforms the more primitive stirrings of the human “soul” into subliminal passions of human existential significance. The enactive theory of the mind corroborates Tymieniecka’s rejection of Husserl’s doctrine of passive synthesis. However, Tymieniecka’s study of the creative function offers a key for further advancement in enactive theory. Three sense-bestowing functions of the Imaginatio Creatrix account for the human expansion of life: the Aesthetic/Poetic sense, the Objectifying Sense, and the Moral Sense. The Moral Sense, for Tymieniecka, is not the product of reasoning powers, but rather the fruit of subliminal passions that acquire their moral aspect through trans-actions guided by the “benevolent sentiment.”. Background: Relatives of intensive care unit patients who lack or have reduced capacity to consent are entitled to information and participation in decision-making together with the patient. Practice varies with legislation in different countries. In Norway, crucial decisions such as withdrawing treatment are made by clinicians, usually morally justified to relatives with reference to the principle of non-maleficence. The relatives should, however, be consulted about whether they know what the patient would have wished in the situation. Research objectives: To examine (...) and describe relatives’ experiences of responsibility in the intensive care unit decision-making process. Research design: A secondary analysis of interviews with bereaved relatives of intensive care unit patients was performed, using a narrative analytical approach. Participants and research context: In all, 27 relatives of 21 deceased intensive care unit patients were interviewed about their experiences from the end-of-life decision-making process. Most interviews took place in the participants’ homes, 3–12 months after the patient’s death. Ethical considerations: Based on informed consent, the study was approved by the Data Protection Official of the Norwegian Social Science Data Services and by the Regional Committee for Medical and Health Research Ethics. Findings: The results show that intensive care unit relatives experienced a sense of responsibility in the decision-making process, independently of clinicians’ intention of sparing them. Some found this troublesome. Three different variants of participation were revealed, ranging from paternalism to a more active role for relatives. Discussion: For the study participants, the sense of responsibility reflects the fact that ethics and responsibility are grounded in the individual’s relationship to other people. Relatives need to be included in a continuous dialogue over time to understand decisions and responsibility. Conclusion: Nurses and physicians should acknowledge and address relatives’ sense of responsibility, include them in regular dialogue and help them separate their responsibility from that of the clinicians. Firms operating in developing economies are increasingly expected to implement CSR practices aligned with recognized global standards. Drawing on extensive field study data in four Latin American countries, we contribute to business ethics scholarship by making visible and explaining firm shortcomings across social, environmental, and governance goals and activities of CSR. Building on and extending sensemaking literature, we find that leaders and managers responsible for their firms’ CSR activities make sense of and justify CSR shortcomings. We specify that justification (...) based on individual, organizational/industry, and macro-environmental influences serves as a mechanism that decouples firms’ actual CSR practices from recognized CSR standards. Further, we propose that detachment, involving disconnection from the local operating environment, underlies the CSR shortcomings of firms. We develop a theoretical multi-level model that specifies justification as a mechanism and detachment as an underlying driver of CSR shortcomings. (shrink)

This paper critically engages with the work of John Harris. Its central focus is his 1985 book, The Value of Life: a foundational text in philosophical bioethics, whose relevance and resonance continue firmly to endure. My aim is to examine what it says—and omits to say—about political authority. Through analysis of apparent and substantive contradictions, and of John’s core focus on moral reasons rather than a basic moral theory, I argue that John says too little about the founding of (...) political obligation. This is so even while he sees political obligation as morally required. I argue that the framings he gives in favor of moral requirements to accept political obligations are particularly significant because they indicate problems in the fundamentality and import of the idea of respect for persons as it features in The Value of Life.
