Cost containment by means of prospective payment and other mechanisms is widely seen as a challenge to modern medicine; but the challenge is seldom articulated clearly in terms of core professional values and the moral content of a claim to professionalism. Medical ethics, as it has evolved as a field of study in the past twenty years, has contributed little to the concept of professionalism in medicine. For an investigation of professionalism in the face of cost containment to (...) evolve fruitfully, several things must occur. The true nature of today's challenge to medicine must be appreciated. The balance between tradition and response to social change in defining a professional value system must be appreciated. Physician income must be explicitly addressed as a moral issue. And practical strategies for investigating and affirming core professional values must be developed for health delivery systems and medical education settings. (shrink)

This squib investigates a paradox that arises from the interaction of two well-studied domains of grammar: antecedent-contained deletion and the licensing of negative polarity items. The conflict arises from a simple set of facts that have been overlooked in the literature, given in (1).

Machery & Mallon [The moral psychology handbook (pp. 3–47). New York, NY: Oxford University Press, 2010] argue that existing evidence does not support the claim that moral cognition, understood as a specific form of normative cognition, is a product of evolution. Instead, they suggest that the evidence only supports the more modest claim that a general capacity for normative cognition evolved. They argue that if this is the case then the prospects for evolutionary debunking arguments are bleak. A debunking argument (...) which relied on the fact that normative cognition in general evolved might threaten all areas of normative belief, including the epistemic norms upon which the argument relies. For the sake of argument, we accept their claim that specifically moral cognition did not evolve. However, we reject their contention that this critically undermines evolutionary debunking arguments of morality. A number of strategies are available to solve what we call the “containment problem,” or how to effectively debunk morality without thereby debunking normative cognition tout court. Furthermore, debunking arguments need not rely on the claim that normative cognition in general evolved. So long as at least some aspects of moral cognition have evolved, this may be sufficient to support an evolutionary debunking argument against many of our moral beliefs. Thus, even if Machery & Mallon are right that specifically moral cognition did not evolve, research in evolutionary psychology may have radical implications for moral philosophy. (shrink)

In response to rapidly rising health care costs in the United States, federal and state governments and private industry are instituting numerous and diverse cost-containment plans. As devices for coping with a scarcity of resources, such plans present serious challenges to physicians' traditional single-minded devotion to patient welfare. Those which contain costs by directly limiting medical options or by controlling physicians' daily clinical decisions can threaten the quality of medical care by allowing economic authorities to make essentially medical judgments. (...) In contrast, other plans coax compliance by arranging incentives, e.g., offering financial rewards for successful cost containment. While they allow for clinical freedom, these plans create conflicts between physicians' fiduciary obligations to their patients and the competing interests of the payers. Such conflicts arise as physicians try to work within governmental or corporate cost containment policies, and also as they attempt to streamline clinical efficiency. Throughout, issues of justice emerge as physicians seek to reconcile their own patients' claims upon limited common resources with others' equally legitimate claims. (shrink)

Containment and support have traditionally been assumed to represent universal conceptual foundations for spatial terms. This assumption can be challenged, however: English in and on are applied across a surprisingly broad range of exemplars, and comparable terms in other languages show significant variation in their application. We propose that the broad domains of both containment and support have internal structure that reflects different subtypes, that this structure is reflected in basic spatial term usage across languages, and that it (...) constrains children's spatial term learning. Using a newly developed battery, we asked how adults and 4-year-old children speaking English or Greek distribute basic spatial terms across subtypes of containment and support. We found that containment showed similar distributions of basic terms across subtypes among all groups while support showed such similarity only among adults, with striking differences between children learning English versus Greek. We conclude that the two domains differ considerably in the learning problems they present, and that learning in and on is remarkably complex. Together, our results point to the need for a more nuanced view of spatial term learning. (shrink)

Winnicotts Konzept des »Holding« bezieht sich auf das komplexe psychische Tun und Wirken der Mutter für ihr neugeborenes Kind, also darauf, dass sie sowohl als »Umwelt-Mutter« als auch als »Objekt-Mutter« fungieren muss. Während das Holding mit der Umwelt-Mutter assoziiert ist, möchte ich das »Containing« hier der Objekt-Mutter zuordnen. Es soll gezeigt werden, dass Winnicott diese Konzepte in Anlehnung an Freuds frühe Beobachtungen der für die emotionale Entwicklung des Kindes entscheidenden Rolle der Mutter erarbeitete. Obwohl die Rolle des Vaters ebenso wichtig (...) sei, betonte Winnicott, es liege in der Hand der Mutter, der Psyche des Kindes die Bedeutung des Vaters nahezubringen – oder nicht. Anknüpfend an Winnicotts späte Formulierungen aus »Objektverwendung und Identifizierung« wird vorgeschlagen, dass das psychische Überleben des Objekts die entscheidende interpsychische Dynamik im Kern der Eltern-Kind-Beziehung ist. Das intrapsychisch entstehende subjektiv überlebende Objekt gewinnt im Kontext der analytischen Situation potenziell an Stärke, wenn der Analytiker imstande ist, sein Überleben zu unterstützen. Contemporary cost containment measures ignore patients' need for privacy, destroy long-term doctor-patient relationships, and demand ethical and standard of care compromises.Economic considerations have distracted the physician and he/she no longer focuses primarily on the patient's welfare. The superficiality of the doctor-patient relationship and the cost-cutting efforts have jointly contributed to the deterioration of the quality of medical care.

This article concerns a new constraint on the interaction of quantifier phrases and epistemic modals. It is argued that QPs cannot bind their traces across an epistemic modal, though it is shown that scoping mechanisms of a differentnature are permitted to cross epistemic modals. The nature and source of this constraint are investigated.

The Logic of Port-Royal, first published in 1662 by the Jansenists Antoine Arnauld and Pierre Nicole, is a work that underlines the inadequacies of the traditional logic. Traditional logic, which included the texts of Aristotle's Organon and the works of the scholastics, was experiencing a mild renaissance in the seventeenth century following its outright and brutal discrediting by the humanists of the previous two centuries. Arnauld and Nicole introduce a fairly original system of logic that attempts to remedy the shortcomings (...) of the revived tradition; they dismiss the Aristotelian "Categories", "Predicables", and "Topics", ignore the scholastic "Doctrine of Terms and Consequence", and suggest a single "General Principle" by means of which the validity of all arguments can be decided. The Principle is based on a notion of containment, divided into entendue and comprehension , which applies to all terms. Because the notion of containment is linked to the objective reality of Cartesian ideas, the authors can claim that their system of logic properly describes reality. The General Principle also suggests that terminist logic is logically and epistemologically prior to sentential logic because, Arnauld and Nicole claim, it governs the Dictum de Omni et Nullo and the Nota Notae. The self-evidence of the two dicta confirmed the validity of all syllogisms of the first figure; furthermore, all syllogisms of other figures could be reduced to one of the first figure. With the General Principle, however, there is no need for such reduction to the first figure. This additional result makes the theorem of sentential logic, "$\supset$r)$\approx$$\supset$-q)", which was needed for the reductions of Baroco and Bocardo, unnecessary. The fact that no theorem of sentential logic is needed to underline term logic guarantees the autonomy of the latter. (shrink)

In a well-known passage from the Introduction to Kant’s Critique of Judgment, Kant defines the power or faculty of judgment [Urteilskraft] as "the capacity to think the particular as contained under the universal" (Introduction IV, 5:179).1 He then distinguishes two ways in which this faculty can be exercised, namely as determining or as reflecting. These two ways are defined as follows: "If the universal (the rule, the principle, the law) is given, then judgment, which subsumes the particular under it... is (...) determining. But if merely the particular is given, for which the universal is to be found, then judgment is merely reflecting" (ibid.) As Kant goes on to make clear, the Critique of Judgment is particularly concerned with judgment in its capacity as reflecting rather than determining. It is concerned, that is, with how we are to find universals (which he glosses as rules, principles, or laws) for given particulars. Despite the fact that the term "concept" does not appear in this set of definitions, Kant’s discussions of judgment elsewhere make it clear that this faculty can be identified at least in part with our capacity to think particular objects under concepts, in particular empirical concepts.2 The.. STS research has devoted relatively little attention to the promotion and reception of science and technology by non-scientific actors and institutions. One consequence is that the relationship of science and technology to political power has tended to remain undertheorized. This article aims to fill that gap by introducing the concept of sociotechnical imaginaries. Through a comparative examination of the development and regulation of nuclear power in the US and South Korea, the article demonstrates the analytic potential of the imaginaries concept. (...) Although nuclear power and nationhood have long been imagined together in both countries, the nature of those imaginations has remained strikingly different. In the US, the state’s central move was to present itself as a responsible regulator of a potentially runaway technology that demands effective containment. In South Korea, the dominant imaginary was of atoms for development which the state not only imported but incorporated into its scientific, technological and political practices. In turn, these disparate imaginaries have underwritten very different responses to a variety of nuclear shocks and challenges, such as Three Mile Island (TMI), Chernobyl, and the spread of the anti-nuclear movement. In 1977, R. B. Angell presented a logic for analytic containment, a notion of relevant implication stronger than Anderson and Belnap's entailment. In this paper I provide for the first time the logic of first degree analytic containment, as presented in [2] and [3], with a semantical characterization—leaving higher degree systems for future investigations. The semantical framework I introduce for this purpose involves a special sort of truth-predicates, which apply to pairs of collections of formulas instead of individual (...) formulas, and which behave in some respects like Gentzen's sequents. This semantics captures very general properties of the truth-functional connectives, and for that reason it may be used to model a vast range of logics. We propose an approach to natural language inference based on a model of natural logic, which identifies valid inferences by their lexical and syntactic features, without full semantic interpretation. We greatly extend past work in natural logic, which has focused solely on semantic containment and monotonicity, to incorporate both semantic exclusion and implicativity. Our system decomposes an inference problem into a sequence of atomic edits linking premise to hypothesis; predicts a lexical entailment relation for each edit using a statistical (...) classifier; propagates these relations upward through a syntax tree according to semantic properties of intermediate nodes; and composes the resulting entailment relations across the edit sequence. We evaluate our system on the FraCaS test suite, and achieve a 27% reduction in error from previous work. We also show that hybridizing an existing RTE system with our natural logic system yields significant gains on the RTE3 test suite. (shrink)

Ärztliches Handeln unterliegt gesellschaftspolitischem Wandel. Ziel der hier vorgelegten qualitativen Fallstudie war es, Folgen gesellschaftlicher Konflikte auf ärztliches Handeln zu untersuchen. Zu diesem Zweck wurden Gruppendiskussionen mit Ärzten zu ihren Erfahrungen mit Geflüchteten 2015/16 in Sachsen und Bayern durchgeführt und tiefenhermeneutisch ausgewertet. An den Ergebnissen wird gezeigt, dass sowohl die Verwaltung als auch die politische Kultur und das gesellschaftliche Umfeld eine Funktion für Ärzte übernehmen, die als »Containment« begriffen werden kann. Zudem ließ sich so herausarbeiten, welche Konsequenzen es für (...) das ärztliche Handeln haben kann, wenn diese »Container«-Funktion im Zuge gesellschaftlicher Veränderungen bedroht ist. Während in zwei sächsischen Gruppen der Zusammenbruch des »Containments« den Rückgriff auf primäre Abwehrmechanismen notwendig machte, konnten die Ärzte in der bayrischen Gruppe ihr »Containment« aufrechterhalten und sich weiterhin als selbstwirksam erleben. The federal government's introduction in 1983 of DRG‐based reimbursement for Medicare patients shook the entire health care industry into the vigorous and dramatic cost containment efforts which today are reshaping health care in America.

The paper focuses on Richard Moran's account (inAuthority and Estrangement) of the distinction between attitudes that meet, and alternatively fail to meet, his transparency criterion for what he calls rational health, and compare this with the psychoanalytic distinction between contained and uncontained states of mind. On the face of it, Moran's distinction appears to be a useful theoretical deepening of the psychoanalytic distinction. On closer examination, however, it appears that (a) rational health is a more demanding standard than containment, (...) so the rationally unhealthy contains much that is mentally (quite) healthy; and (b) more seriously, some states that meet the “transparency” condition are manifestly mentally unhealthy. So transparency needs to be supplemented by an independently understood notion of containment to approach a realistic norm of mental health/ill health. (shrink)

Abstract. On the Direct Reference thesis, proper names are what I call ‘genuine terms’, terms whose sole semantic contributions to the propositions expressed by their use are the terms’ semantic referents. But unless qualified, this thesis implies the false consequence that sentences containing names that fail to refer can never express true or false propositions. (Consider ‘The ancient Greeks worshipped Zeus’, for instance.) I suggest that while names are typically and fundamentally used as genuine terms, there is a small class (...) of names, which I call ‘descriptive names’, whose reference is fixed by commonly associated definite descriptions, and I also suggest that there is an idiom of natural language on which such names can be used as abbreviated definite descriptions in a limited set of sentential contexts, including (1) positive and negative existentials, (2) cognitive ascriptions, and (3) uses of names to talk about myth. Uses of empty descriptive names in such contexts can then be either true or false. Relying on Gregory Currie’s theory of truth in fiction, I also propose an idiom on which fictional names can be used as short for a certain type of description in talk about fiction. Along the way, I provide arguments that names are used as short for descriptions in substantive existential statements as well as in both metamythic and metafictive contexts. I also discuss and criticize alternative views of these matters, including the views of David Braun, Saul Kripke, Peter van Inwagen, and others. (shrink)

Neuroses are secondary mental illnesses, it being their purpose to contain psychoses.

Flavin‐containing monooxygenases (FMOs), traditionally known for detoxifying xenobiotics, are now recognized for their involvement in endogenous metabolism. We recently discovered that an isoform of FMO, fmo‐2 in Caenorhabditis elegans, alters endogenous metabolism to impact longevity and stress tolerance. Increased expression of fmo‐2 in C. elegans modifies the flux through the key pathway known as One Carbon Metabolism (OCM). This modified flux results in a decrease in the ratio of S‐adenosyl‐methionine (SAM) to S‐adenosyl‐homocysteine (SAH), consequently diminishing methylation capacity. Here we discuss (...) how FMO‐2‐mediated formate production during tryptophan metabolism may serve as a trigger for changing the flux in OCM. We suggest formate bridges tryptophan and OCM, altering metabolic flux away from methylation during fmo‐2 overexpression. Additionally, we highlight how these metabolic results intersect with the mTOR and AMPK pathways, in addition to mitochondrial metabolism. In conclusion, the goal of this essay is to bring attention to the central role of FMO enzymes but lack of understanding of their mechanisms. We justify a call for a deeper understanding of FMO enzyme's role in metabolic rewiring through tryptophan/formate or other yet unidentified substrates. Additionally, we emphasize the identification of novel drugs and microbes to induce FMO activity and extend lifespan. (shrink)

Olaf Müller’s book develops a new case for underdetermination, and, as he is focusing on theories of a ‘limited domain’, this assumes the containability of the theories. First, the paper argues that Müller’s theory of darkness is fundamentally Newtonian, but for Newton’s optical theory the type of theoretical structure Müller adopts is problematic. Second, the paper discusses seventeenth-century challenges to Newton, changes in the proof-structure of Newton’s optical theory, and how these affect Müller’s reconstruction. Müller’s book provides empirically equivalent theories, (...) yet the historical theories were not empirically equivalent, and the same experiments were used to extract different bodies of evidence to rebut the opponent. Third, Goethe’s multi-layered critique of Newton’s experimental proof is investigated, including his developmental account of prismatic colours, the role of experimental series in rejecting Newton’s observations, and his incorporation of the ‘limited domain’ of prismatic colours in a broader framework. Two key elements of Goethe’s method, polarity and strengthening are discussed in contrast to Müller, who only utilises polarity in his account. Finally Neurath’s attempts to come to grips with the optical controversies and the prism-experiments with ‘blurred edges’ are recalled. Müller also discusses in detail some of these experiments and heavily draws on Quine. Neurath developed Duhem’s and Poincaré’s conventionalist insights and had good reasons to be pessimistic about theory-containment. Their differences provide some additions to the history of the Duhem–Quine thesis. (shrink)

ABSTRACTThis article provides the first comprehensive and chronological analysis of Carl Schmitt’s reception of Carl von Clausewitz. While earlier scholarship has mostly stressed Schmitt’s shift from Clausewitzian ‘instrumentality’ to an ‘existential’ view of war, I note some inherent difficulties in this dichotomy and instead promote the parallel distinction between two argument types: those of containment and intensification. Schmitt theorized both limited political war and the intensification of war out of traditional bounds, and focusing on one should not eclipse the (...) other. Further, both elements are identifiable already in Clausewitz. I analyse Schmitt’s oscillation between containment and intensification arguments chronologically from the mid-1920s to the 1960s. Despite sometimes nominally rejecting Clausewitz’s famous thesis of war as the continuation of politics, Schmitt nevertheless affirmed the idea of war’s political nature. I conclude that Schmitt’s view can be read as a radicalized version of the Clausewitzian political theory of war rather than a strict deviation from it. This becomes evident as soon as we place Schmitt’s partly incoherent observations on Clausewitz in their argumentative contexts. (shrink)

COVID-19 caused levels of household food insecurity to spike, but the precarity of so many people in wealthy countries is an outgrowth of decades of eroding public provisions and labour protections that once protected people from hunger, setting the stage for the virus’ unevenly-distributed harms. The prominence of corporate-sponsored foodbanking as a containment response to pandemic-aggravated food insecurity follows decades of replacing rights with charity. We review structural drivers of charity’s growth to prominence as a hunger solution in North (...) America, and of its spread to countries including the UK. By highlighting pre-pandemic pressures shaping foodbanking, including charities’ efforts to retool themselves as health providers, we ask whether anti-hunger efforts during the pandemic serve to contain ongoing socioeconomic crises and the unjust living conditions they cause, or contest them through transformative pathways to a just food system. We suggest that pandemic-driven philanthropic and state funding flows have bolstered foodbanking and the food system logics that support it. By contextualising the complex and variegated politics of foodbanking in broader movements, from community food security to food sovereignty, we reframe simplistic narratives of charity and highlight the need for justice-oriented structural changes in wealth redistribution and food system organisation if we are to prevent the kinds of emergency-within-emergency that we witnessed as COVID-19 revealed the proximity of many to hunger. This paper proposes a description of the moral obligations of economic agents. It will show that a threefold division should be adopted to distinguish moral obligations applying to their interactions in the market, obligations applying to their interactions inside business firms and obligations applying to their interactions with agents outside the market. Competition might be permissible in the first case since markets are special patterns of social interactions (called adversarial schemes). They produce their benefits when agents try to satisfy exclusive (...) preferences at the expense of others. However, moral obligations inside the firm and moral obligations outside the market are of a different nature. This argument will be developed in the two first parts of this paper. In the third part, it will outline the relevant strengths of that account in relation with two popular views of economic agents’ moral obligations: the shareholder primacy view and the stakeholder theory. In the article, we set out to develop the contours of a general containerology, interrogating containment as a technology and an ontological condition that shapes human and more-than-human relations. While exploring containment technologies as spatio-temporal devices of ordering vis-à-vis leakage, we nevertheless stress how containment inevitably remains contextual, provisional, and leaky at best. Addressing the selective permeability of containers, we also discuss the relation between contained interiorities and exteriorities as processes of articulations between milieus. We problematise the (...) opposition and incommensurability between containment and leakage, and instead insist on conceiving them as co-constitutive. Ultimately, we outline an affirmative, positive take on containment beyond exclusionary inclusion, in contrast to prevailing negative conceptions and practices. (shrink)

This series of articles by Charles Dougherty, Robert Berenson, and Kathleen Powderly and Elaine Smith, as well as “Cost Containment: Challenging Fidelity and Justice” by E. Haavi Morreim (Hastings Center Report, December 1988), result from a Hastings Center project, “Ethics and Prospective Payment Systems: DRGs.” The two-year project was jointly funded by The General Electric Foundation and the American Medical Association Education and Research Foundation.The project tried to gauge the systematic effects of the introduction of cost containment strategies, (...) prospective payment by Diagnostic Related Groups in particular, on the provision of health care and the ethical practice of medicine. Cost containment strategies, no matter how necessary in curbing spiraling medical costs, leave no sector or participants in the health care system untouched—hospitals and other health care institutions; doctors, nurses, social workers and other health care professionals; patients, their families, and the thirty-six to forty million medically indigent. The articles discuss the several and interrelated ethical issues raised by the DRG prospective payment system (Dougherty); its particular affect on the doctor-patient relationship and how it is transforming the ethical practice of medicine (Berenson and Morreim); and its specific impact on the professions and professional ethics of nursing and social work (Powderly and Smith). The overall message of the articles is that the health professions, their provision of care, and the ethics of their practices—as driven by economic considerations and realities—are in the midst of a more or less radical sea change. The purpose of this paper is to describe the nature of dental practice and to identify some recent innovations which have effect on cost containment in dentistry. The first of these innovations is dental insurance or prepaid dental services. Dental insurance has only recently emerged as a significant economic factor. The chronic and prevalent nature of dental disease mandates that the management of these programs incorporate insurance devices which limit demand and utilization. These devices amount to cost containment (...) measures. The second factor to be discussed is advertising. Advertising has been foisted on the dental profession by recent rulings of the Federal Trade Commission in an effort to promote competition. The Reagan administration, despite a market oriented approach to health care, had attempted to blunt some of the FTC actions. Advertising, nevertheless, has the potential to stimulate competition and restrain price escalation. The third factor includes a discussion of alternative delivery systems. These commercially oriented dental enterprises have the potential to compete with established practices as well as increase the demand for services. (shrink)

One of the central and most distinctive theses of Kant’s philosophy of mathematics is that mathematical knowledge is synthetic. In this context, synthetic judgments are defined in opposition to analytic ones, whose predicate concept is “contained in” the subject. Kant’s thesis has often been attacked as indefensible, but just as frequently critics have complained that the thesis itself, and even the analytic/synthetic distinction on which it rests, are simply unintelligible. Thus, even prior to questions of its correctness, the Kantian doctrine (...) has been rejected as unclear, based on general skepticism that there is a real distinction of “containment”—or indeed any tolerably clear distinction at all—between analytic and synthetic judgments. Cooper proved in [S.B. Cooper, Strong minimal covers for recursively enumerable degrees, Math. Logic Quart. 42 191–196] the existence of a c.e. degree with a strong minimal cover . So is the greastest c.e. degree below . Cooper and Yi pointed out in [S.B. Cooper, X. Yi, Isolated d.r.e. degrees, University of Leeds, Dept. of Pure Math., 1995. Preprint] that this strongly minimal cover cannot be d.c.e., and meanwhile, they proposed the notion of isolated degrees: a d.c.e. degree is isolated (...) by a c.e. degree if is the greatest c.e. degree below , and we also say that isolates . In [G. Wu, Bi-isolation in the d.c.e. degrees, J. Symbolic Logic 69 409–420], Wu extended Cooper–Yi’s notion and proved that there are intervals of d.c.e. degrees containing exactly one c.e. degree . Following Cooper and Yi’s notion, is called a bi-isolating degree. The bi-isolating degrees are dense in the high c.e. degrees. Arslanov asked whether the bi-isolating degrees occur in every jump class. In this paper, we prove that there are low bi-isolating degrees, providing a partial solution to Arslanov’s question.
